“Breakthrough infections — those occurring in people who have been vaccinated against COVID-19 — are happening. So far, though, they are largely occurring without major problems. Most people are asymptomatic, and their cases are discovered only during routine testing.
As variants of the novel coronavirus continue to spread and mutate, researchers are monitoring how the vaccines perform and whether booster shots will be needed to maintain meaningful immunity.
Right now, experts say it’s too early to speculate whether we’ll need booster shots like some routine vaccines.
Dr. Amesh Adalja, a senior scholar at Johns Hopkins Center for Health Security in Baltimore, says it’s premature to predict whether COVID-19 boosters will be needed and, if so, at what intervals.
“To me, the threshold for boosters would be to see fully vaccinated individuals getting breakthrough infection severe enough to land them in the hospital,” Adalja told Healthline. “We have not crossed that threshold.”…(L)
“Leading infectious disease expert Anthony Fauci on Wednesday said that he believes recipients of the coronavirus vaccine will need a booster shot, but he declined to say when.
“I don’t anticipate that the durability of the vaccine protection is going to be infinite,” Fauci told a Senate subcommittee. “It’s just not. So I would imagine we will need, at some time, a booster.”
He said the interval of protection from the vaccines is unknown but that it lasts at least six months and likely a year.
Fauci said that he is “not exactly sure when” a booster would be needed but that research into vaccine durability is ongoing.” (A)
“The U.S. government is planning for the potential need for Covid-19 vaccine booster shots “just in case,” the head of the Centers for Disease Control and Prevention told CNBC.
CDC Director Dr. Rochelle Walensky didn’t say what those plans were. However, should Americans require booster shots, the U.S. would likely need to make arrangements with drugmakers to supply additional doses and make plans for vaccine distribution.
Walensky told CNBC that U.S. health officials are “thinking ahead” in the event that vaccine-induced immunity against the coronavirus begins to wane. Officials still don’t know if that will happen or if booster shots will be needed, “but we are planning for it just in case,” she said.
“Right now, if you have two doses of the mRNA vaccines, you are protected,” Walensky said in comments that aired Tuesday during CNBC’s Healthy Returns Summit.
“What we’re talking about is thinking ahead,” she said in the interview with CNBC’s Meg Tirrell. “What happens if in a year from now or 18 months from now your immunity wanes? That’s really our job is to hope for the best and plan for what might happen if we need further boosters in the future, the way we get flu vaccine boosters every year.”
Walensky said scientists are currently looking at the immunity of a small population of people who were vaccinated months ago. However, she said, she doesn’t expect scientists will do mass testing looking for reduced immunity against the virus….
Walensky told CNBC scientists still don’t know whether the coronavirus is seasonal with new infections slowing down during warm summer months and speeding up in the winter.
Some scientists predict states will see repeated outbreaks when the U.S. enters its colder months.
“We have other coronaviruses be seasonal viruses and yet this coronavirus has not proven to be seasonal,” she said, adding that the U.S. witnessed a surge in cases last summer.” (B)
“Pfizer announced Monday morning that it had started testing a COVID-19 booster shot — a third dose the company’s CEO said fully vaccinated people will likely need in another 12 months.
The goal of the study is to see whether another vaccine can be taken along with a booster dose of Pfizer’s COVID-19 vaccine. In the study, participants will get both the booster shot and a pneumonia vaccine.
“To see if they’re able to tolerate it, see if that in any way adversely affects their development of antibodies against COVID-19 and see how the immune reaction is, you know, the side effect profile of the combination,” Joshi explained. “And if that looks good, maybe then consider combining it with seasonal flu vaccine as well.” (C)
“A large body of research on the coronavirus now indicates that both natural infection and vaccines stimulate the production of these long-lasting memory cells, which means that boosters should not be needed any time soon to conquer covid-19.
We already knew mRNA vaccines — like the Moderna and Pfizer vaccines being widely administered against the coronavirus — form memory B cells. That finding was bolstered in a big way Monday, when researchers reported finding memory B cells in the bone marrow of people who had experienced even mild cases of covid-19. Another study of recovered covid-19 patients found the development of memory T cells comparable to those generated by yellow fever vaccination, which usually needs just one dose over a person’s lifetime. And a 2020 study showed that T cells generated among patients who survived the SARS pandemic of 2003, which is caused by a similar virus, were still going strong 17 years later, indicating long-lasting immunity…
The talk of boosters is not only distracting from the brighter and brighter light we are seeing at the end of our covid-19 nightmare in the United States and the start of normal life, it also distracts us from the most pressing problem of our day, which is to achieve global vaccine equity. Unfortunately, pharmaceutical company executives have suggested that boosters could be necessary for coronavirus vaccines as soon as this fall. Pouring effort into developing boosters — Pfizer has begun studying a modified third booster shot to follow its initial two-shot regiment — threatens to distract from production of adequate supplies of vaccine for the world. Moreover, such messaging of perpetual boosters could also dissuade some people from getting the vaccine altogether.
The pandemic is not over until it is over for all of us. We also have a moral and ethical obligation based on our interconnectedness to vaccinate the planet. Let’s cease talk that boosters will be needed in the near future and get vaccines out to the world.” (D)
““People who were infected and get vaccinated really have a terrific response, a terrific set of antibodies, because they continue to evolve their antibodies,” Dr. Nussenzweig said. “I expect that they will last for a long time.”
The result may not apply to protection derived from vaccines alone, because immune memory is likely to be organized differently after immunization, compared with that following natural infection.
That means people who have not had Covid-19 and have been immunized may eventually need a booster shot, Dr. Nussenzweig said. “That’s the kind of thing that we will know very, very soon,” he said….
The results of Dr. Nussenzweig’s study suggest that people who have recovered from Covid-19 and who have later been vaccinated will continue to have extremely high levels of protection against emerging variants, even without receiving a vaccine booster down the line.
“It kind of looks exactly like what we would hope a good memory B cell response would look like,” said Marion Pepper, an immunologist at the University of Washington in Seattle who was not involved in the new research.
The experts all agreed that immunity is likely to play out very differently in people who have never had Covid-19. Fighting a live virus is different from responding to a single viral protein introduced by a vaccine. And in those who had Covid-19, the initial immune response had time to mature over six to 12 months before being challenged by the vaccine.
“Those kinetics are different than someone who got immunized and then gets immunized again three weeks later,” Dr. Pepper said. “That’s not to say that they might not have as broad a response, but it could be very different.” (E)

Nepal has recorded half a million Covid cases and 6,000 deaths, numbers that experts believe deeply undercount the toll. Testing remains limited. One figure could indicate the true severity: For weeks now, about 40 percent of the tests conducted have been positive.” (F)

Increased transmissibility is an exponential threat. If a virus that could previously infect three people on average can now infect four, it looks like a small increase. Yet if you start with just two infected people in both scenarios, just 10 iterations later, the former will have caused about 40,000 cases while the latter will be more than 524,000, a nearly 13-fold difference.
Morally and practically, this emergency demands immediate action: widespread vaccination of those most vulnerable where the threat is greatest…
Vaccine supplies need to be diverted now to where the crisis is the worst, if necessary away from the wealthy countries that have purchased most of the supply. It is, of course, understandable that every nation wants to vaccinate its own first, but a country with high levels of vaccination, especially among its more vulnerable populations, can hold things off, especially if they also had big outbreaks before. In addition, excess stockpiles can go where they are needed without even slowing down existing vaccination programs…
The kind of catastrophic outbreak like the one in India can cause many more needless deaths simply by overwhelming our resources. Already, there are reports that countries ranging from Nepal to the Philippines to South Africa to Nigeria may face supplemental oxygen shortages of the kind seen in India. This pathogen has one fatality rate when oxygen is available as a therapy and one when it is not, and it would be an unspeakable tragedy to suffer the latter in the second year of the pandemic.
Like all pandemics, this one will end either with millions — maybe billions — being infected or being vaccinated. This time, world leaders have a choice, but little time to make that choice before it is made for them.” (G)
“…Most low-income and middle-income countries (LMICs) face difficulties in accessing and delivering vaccines and therapeutics for COVID-19 to their populations.5 COVAX will require decisive action by Gavi, the Vaccine Alliance, WHO, and the Coalition for Epidemic Preparedness Innovations (CEPI), supported by the countries they serve and with financing for vaccine purchasing, to ensure people worldwide have equitable access to COVID-19 vaccines…8
Many LMICs do not have an established platform for vaccinating their adult populations.10 Although it is feasible to deliver COVID-19 vaccines to health-care and other front-line essential workers, in some LMICs it will be difficult to effectively reach and vaccinate with two doses all elderly populations and individuals with co-morbidities, given insufficient mechanisms to identify such groups. Governments and technical leaders will need to use transparent, accountable, and unbiased processes when they make and explain evidence-based vaccine prioritisation decisions, while also building confidence in COVID-19 vaccines and engaging with all the stakeholders.
The ultracold chain requirements of mRNA COVID-19 vaccines are likely to be an insurmountable hurdle in LMICs, outside of major cities. COVID-19 vaccine delivery will require considerable investment of resources, health-care staff, and careful planning to avoid opportunity costs, including a disruption of routine health services and a decline in essential childhood vaccination coverage, which could result in outbreaks of measles and other vaccine-preventable diseases…
There are further challenges. Governments in LMICs with strong private health sectors, as those in high-income countries, will need to manage the inherent potential for inequity, whereby the rich could access COVID-19 vaccines before individuals with less access to private care who may be at increased risk of severe disease and death, such as older people and those with comorbidities. LMICs affected by war, civil conflict, economic crises, or natural disasters, or with large refugee populations or populations with special needs or vulnerabilities need additional support for vaccines and vaccination under extremely difficult operational conditions.” (H)
“In January, the director-general of the World Health Organization (WHO), Tedros Adhanom Ghebreyesus, issued a blunt warning. The world was “on the brink of a catastrophic moral failure,” he said. Wealthy countries were buying up available COVID-19 vaccines, leaving tiny amounts for others—a replay of what happened during the 2009 influenza pandemic. “The price of this failure will be paid with lives and livelihoods in the world’s poorest countries,” Tedros said.
He was right. Today, some rich countries are vaccinating children as young as 12 years old, who are at extremely low risk of developing severe COVID-19, while poorer countries don’t even have enough shots for health care workers. Nearly 85% of the COVID-19 vaccine doses administered to date have gone to people in high-income and upper middle–income countries. The countries with the lowest gross domestic product per capita only have 0.3%.
Tedros lambasted the “scandalous inequity” again in his opening speech at the World Health Assembly on 24 May. By September, at least 10% of the population in every country should be vaccinated, he said.
Disparities in global health are nothing new. Lifesaving therapies such as monoclonal antibodies are unavailable in large parts of the world. Even vaccines and drugs that cost pennies to make don’t reach millions of people who need them. But the COVID-19 crisis has exposed the inequities in a distinct, acute way. As normality is returning to vaccine front-runners such as Israel, the United Kingdom, and the United States, India’s health system is buckling under soaring case numbers—and the world is still recording almost 5 million cases and more than 80,000 deaths every week.
The moral argument aside, there’s a very practical reason to try to distribute vaccines more equitably: No part of the world can feel safe if the pandemic rages on elsewhere, posing the risk of reintroduction and spawning potentially more dangerous viral mutants.” (I)

Using publicly available figures from Our World in Data, The Washington Post found that 45 percent of all vaccine doses administered so far have gone to just 16 percent of the world’s population in what the World Bank considers high-income countries.
Through the summer and fall of last year, wealthy nations cut deals directly with vaccine-makers, buying up a disproportionate share of early doses — and undermining a World Health Organization-backed effort, called Covax, to equitably distribute shots.
So now, in a small number of relatively wealthy nations, including the United States, doses are relatively plentiful and mass immunization campaigns are progressing apace. But much of the world is still struggling to secure enough supply. For many, herd immunity is many months — if not years — away, which could extend the crisis.
A team at Duke University’s Global Health Innovation Center recently found that high-income countries locked up 53 percent of near-term vaccine supply. They estimate that the world’s poorest 92 countries will not be able to reach a vaccination rate of 60 percent of their populations until 2023 or later.” (J)

“U.S. health officials and the State Department on Monday warned Americans against travel to Japan because of a surge in coronavirus cases in the country, which is preparing to host the Olympics in just two months.
The twin alerts don’t ban U.S. citizens from visiting the country, but they could have an impact on insurance rates for travelers and may factor into decisions by Olympic athletes and spectators on whether to compete in or attend the games, which are due to start in July. There was no immediate indication as to what effect the warnings might have on would-be Olympic-goers.
“Travelers should avoid all travel to Japan,” the Atlanta-based Centers for Disease Control and Prevention said in a new Covid-19 update. “Because of the current situation in Japan even fully vaccinated travelers may be at risk for getting and spreading COVID-19 variants and should avoid all travel to Japan.”
The State Department’s warning, which followed the CDC alert, was more blunt. “Do not travel to Japan due to Covid-19,” it said in the announcement, which raised the department’s travel alert from Level 3 — Reconsider travel — to Level 4 — Do not travel. The previous alert was issued on April 21.
The United States Olympic & Paralympic Committee said it still anticipates that American athletes will be able to safely compete at the Tokyo Games.
“We feel confident that the current mitigation practices in place for athletes and staff by both the USOPC and the Tokyo Organizing Committee, coupled with the testing before travel, on arrival in Japan, and during Games time, will allow for safe participation of Team USA athletes this summer,” the committee said in a statement Monday.” (A)

Most metro areas in Japan are under a state of emergency and expected to remain so through mid-June because of rising serious COVID-19 cases that are putting pressure on the country’s medical care systems. That raises concern about how the country could cope with the arrival of tens of thousands of Olympic participants if its hospitals remain stressed and little of its population is vaccinated.
Chief Cabinet Secretary Katsunobu Kato said at a news conference Tuesday that the warning does not prohibit essential travel and Japan believes the U.S. support for Tokyo’s effort to hold the Olympics is unchanged.
“We believe there is no change to the U.S. position supporting the Japanese government’s determination to achieve the games,” Kato said, adding that Washington has told Tokyo the travel warning is not related to the participation of the U.S. Olympic team…

“The International Olympic Committee this week unveiled updated plans for holding the Summer Games in Japan, meticulous “playbooks” that promise to ensure the safety not only of the athletes taking part but also of the Japanese public during a global pandemic.
They represent “very hard work” and “determination, a lot of determination” to stage the Games safely and successfully, the IOC’s sports director, Christophe Dubi, said at a news conference, and they are based on the latest scientific expertise as well as considerable experience with holding global sporting events during the pandemic. IOC President Thomas Bach said his confidence the Olympics can go ahead also is based on his admiration for the “great resilience and spirit” of the Japanese people…
The meticulous planning, meanwhile, has one potentially fatal flaw: Japan’s already overloaded health system can’t cope with the additional demands the Games will bring without putting more lives at risk, doctors and nurses say. The Olympics would involve more than 11,000 athletes plus tens of thousands of officials, coaches, media members and support staffers converging on Tokyo, which is closed to most foreign visitors.
“Most health workers say even thinking about the Olympics is just ridiculous,” said Kentaro Iwata, an infectious-disease expert and doctor at Kobe University Hospital, which is in a city in western Japan where 1,700 people with covid-19 need hospitalization but can’t get a bed.
“We are really fighting a life-and-death situation,” he said. “How the hell can you speak of a sports event gathering so many spectators, staff, volunteers, nurses and doctors? Who could enjoy the Games in this situation?”..
On April 23, just three months before the scheduled Opening Ceremonies, Japan’s government declared a third state of emergency for Tokyo and several other regions as it battles a surge in infections that is being supercharged by the rapid spread of coronavirus variants and shows no sign of abating.
Japan’s caseload is still a fraction of what many countries are experiencing, but the death toll has doubled in just three months and crossed 10,000 this week. The daily rate of new infections approached 6,000, the highest level in three months. On the day the Olympics were postponed, March 24, 2020, Japan recorded just 65 new infections…

Vaccines will not be mandatory, but most athletes and officials probably will be vaccinated. Yet within Japan, just 3.2 million vaccine doses have been delivered, the equivalent of two shots for just 1.3 percent of the population, reflecting a combination of the country’s slow efforts to develop its own vaccine or source one from others and skepticism among many citizens about foreign drugs.
The Games organizers have asked the Japanese Nursing Association for 500 nurses to help out during the 17-day event. On social media, nurses soon began tweeting under the hashtag “nurses opposed to being dispatched to the Olympics,” arguing they had to prioritize existing patients. The hashtag attracted nearly 200,000 retweets within days.
“We are not disposable pawns,” one nurse wrote on a placard that she held up in a tweet.
Others have taken to the streets, asking why IOC officials will get regular testing for the coronavirus while many health workers still can’t access tests.
Nurse Konoka Shibata, 25, said it was “unreasonable” to expect nurses to volunteer for the Games, putting themselves at additional risk when many hospitals are short-staffed and struggling to cope with the pandemic. Kobe University’s Iwata said he also is concerned that delegations from Brazil and India could bring more dangerous variants to Japan.” (C)
“A senior member of the International Olympic Committee has said the Tokyo 2020 Games can go ahead even if the host city is under a state of emergency because of the coronavirus.
“All the measures we are undertaking will ensure a safe Games regardless of whether there is a state of emergency or not,” John Coates, an IOC vice-president who is in charge of preparations, told reporters after a virtual meeting with organisers on Friday.
“Provided that we can protect the Japanese public, the most important thing is giving athletes a chance to compete.”
The IOC and Tokyo 2020 organisers have insisted the Games will go ahead, despite widespread public opposition in Japan and warnings from health experts that the arrival of tens of thousands of athletes, officials, journalists and support staff this summer risked spreading Covid-19.
Japanese doctors and medical workers have called for the Games to be cancelled to relieve pressure on overwhelmed health services, while a recent poll found that 83% of the public want the event to be either called off or postponed for a second time.
While most athletes are expected to be fully vaccinated by the time the Games open in two months’ time, only 4.1% of Japan’s population has received at least one jab – the lowest rate of any advanced economy – and only 30% of medical workers in Tokyo are fully protected, the Nikkei business newspaper said.
Seiko Hashimoto, the president of Tokyo 2020, said up to 230 doctors and 310 nurses would be needed each day during the Olympics and the Paralympics. The number of officials, journalists and other Games-related staff due to arrive in Tokyo has been cut from 180,000 to 78,000, she added.
“I can say it’s now clearer than ever that these Games would be safe for everyone participating and, importantly, safe for the people of Japan,” Coates said, adding that the IOC was working with organisers to send medical personnel to Tokyo. “After eight years of hard work and planning, the finish line is within touching distance.”…
On Friday, Shigeru Omi, who heads the government’s coronavirus advisory panel, told MPs that organisers must consider the impact the Games could have on Japan’s medical infrastructure before deciding whether to go ahead.” (D)

The snap decisions don’t always make sense and by their nature discriminate against someone.
The International Olympic Committee has asked all accredited personnel heading to the games to sign a waiver removing all liability from the IOC, the International Paralympic Committee, and the Tokyo organisers, despite them not yet being able to provide definitive countermeasures as the existing precautions may still change.
The Olympic and Paralympic Games Tokyo 2020 Addendum distributed this week states:
“To the fullest extent admissible under applicable laws, the Responsible Organisation agrees to irrevocably release Tokyo 2020, the IOC, the IPC (and their respective members, directors, officers, employees, volunteers, contractors or agents) from any liability for any kind of loss, injury or damage that the Responsible Organisation, including any Accredited Person under its responsibility, may suffer or be exposed to in connection with their participation in the Games.”
On top of this athletes must sign a hard copy of a ‘written pledge’ and present it to immigration officials on arrival guaranteeing they will follow all the countermeasures despite not knowing the final requirements…

“Organizers are relying on a series of six playbooks of rules that detail how participants of the Olympics and Paralympics can compete, move around and socialize, in order to manage the risks of what will be the world’s biggest COVID-19 bubble. While those involved in the games will be somewhat isolated from the Japanese public, Tokyo has ruled out using two core tenets of containment: quarantines and vaccinations. Without those, experts say infections could spread.
If it does, not only could the Olympics become the site of a sizable outbreak that spreads into Japan, it could become a cauldron of novel variants gathered from around the world. The risk is that athletes could bring them home, potentially fueling the pandemic.
While Japan has seen much lower transmission rates compared with other rich nations, its vaccination campaign is only just starting, months behind places like the United Kingdom, the United States and even other parts of Asia. There are still many unknowns, including exactly how many people will be coming into Japan, and organizers haven’t decided yet on how many domestic spectators will be allowed into venues.
Depending on how infection rates develop, there’s still a possibility that more stringent measures will be adopted, with the final version of the playbooks due in June.
“The situation surrounding the coronavirus is constantly changing, and it’s our hope that the efforts of the government, the city of Tokyo and other stakeholders will help to mitigate spread of infections,” Tokyo 2020 said in an emailed statement.
Sporting events have gone on around the world this past year — with both success stories and cautionary tales.
The Olympic organizers are set to emulate the NBA, which saw no infections during its three-month run in the summer and fall of 2020. But the NBA bubble saw just a few hundred athletes cordoned off together at the Walt Disney World Resort in Orlando, Florida. Including staff and coaches, there were fewer than 1,000 people involved…
The extent of the risk also varies depending on the sport. A study of the NFL’s latest season in the U.S. found that players didn’t transmit the virus during game play, while matches for a high school wrestling tournament turned into deadly superspreading events. The Olympics will feature 33 sports across 42 venues across Japan.
Further complicating the task is the social nature of the Olympics. Places such as the Athlete’s Village were designed to have people meet and socialize. Although long conversations and collective meals will be off limits, how those rules will be enforced is unclear. Some of the athletes are teenagers and the average age of an Olympian is usually in the 20s — groups where virus spread has been harder to control…
It’s not just the athletes. The games will require thousands of volunteers to help the events run smoothly, as well as local staff who will need to go in and out of the Olympic bubble regularly to do things such as cook meals, clean facilities and run the proceedings. It’s not clear how such staffers — which the Olympic Organising Committee and Tokyo Metropolitan Government say will probably number more than 150,000 — will be handled, and the playbooks don’t offer explicit instructions.
Even in normal times, disease outbreaks are common at the Olympics. During the Winter Games in Pyeongchang in 2018, around 200 athletes caught norovirus. Two years earlier, the Rio Summer Olympics were held amid the specter of Zika. More than 300 athletes caught respiratory illnesses out of 10,568 competing in the London Olympics in 2012.” (G)
“Is there a level playing field for all nations expected to attend? To date, more than 458 million vaccine doses have been administrated worldwide. And while this is good news, there is a lack of global equitable access to vaccines, and even within countries, not everyone is being included in national vaccination plans.
COVAX is currently the only initiative working to deliver vaccines equitably to 92 lower-income countries. The World Health Organization argues that there will be “no end to the pandemic without equitable distribution.”
According to the IOC playbooks, being vaccinated is not a requirement for anyone attending the Games. IOC member Dick Pound suggested athletes must be given priority access to the coronavirus vaccine. IOC President Thomas Bach, however, argued that the IOC is not in favour of athletes jumping the queue and it was to be determined by governments.
Some countries — like Lithuania, Hungary, Serbia, Israel, Mexico and Russia — have already announced commitments to allowing their athletes priority or voluntary access to the vaccine. Other countries — like Canada, Germany, Britain and Italy — have announced vaccines will be given to the most vulnerable populations first.
The Chinese Olympic Committee has even gone so far as to offering to vaccinate Tokyo 2020 and Beijing 2022 athletes in territories that have approved the Chinese vaccine for use. So access for athletes is unevenly distributed…
Money over health?
But Japan is currently behind on vaccinations compared to other countries since it only started in February 2021. According to survey by Kyodo News, 80 per cent of Japanese residents would rather see the Games cancelled or further delayed.
Last April, the IOC argued that the Games could not be delayed to 2022 as the Japanese partners and the organizing committee could not manage it. This was due to the availability of the Olympic Village, sports venues and the need for people to carry on working.
The decision to continue with the Games during a global pandemic makes it clear that finances are driving decisions, not the health of those involved.
The Olympic Charter proclaims that the Games are about “promoting a peaceful society concerned with the preservation of human dignity.” The question becomes: who will be the voice of reason?” (H)
Japan wants to try to create a sort of athlete “bubble.” Is that enough?
Compared to last year when the Tokyo Games were postponed, the world knows a lot more about how to mitigate the spread of Covid-19. Large sporting events have since gone forward, from the NBA to the Australian Open to the Super Bowl.
But the Olympics are another feat entirely. As Lee Igel, a clinical professor at the NYU Tisch Institute for Global Sport, said, they’re really more a festival than an athletic competition — which means they have different challenges, and a lot more people.
Mass gatherings like the Olympics often present public health challenges (see: free condoms in the Olympic Villages) simply because you’re bringing a lot of people from around the world and packing them all in together.
Some of the festival atmosphere isn’t going to exist with Covid-19. But even without the influx of foreign fans this year, thousands of athletes and all their coaches and support staff, and global media and all their crews, will be arriving in Tokyo. They’ll be flying in from all over the world, some from places experiencing severe outbreaks. And they’ll arrive in Japan where, right now, cases are close to their January peak and the country’s fully vaccinated rate is around 1 percent.
“Is that a set of circumstances where you open a country to travelers bringing new variants from, well, everywhere?” Amir Attaran, professor of law, epidemiology, and public health at the University of Ottawa, wrote in an email.
Once the games are over, all those people will be flying back home, too. If there’s a coronavirus outbreak among athletes, or exposure, they could bring those cases back home. In other words, it could become a superspreader event….
The NBA bubble was successful, with zero positive Covid-19 tests. But the Olympics bubble can’t replicate that experience exactly. For one, NBA players and coaches were basically full-time quarantined in the Walt Disney World Resort in Orlando.
According to Bloomberg, just about 1,000 people were involved in that bubble, compared to thousands and thousands more — from all over the world — expected to attend the Olympics. Plus, the Olympics are not going to mandate that everyone quarantine, and though athletes will be discouraged from leaving the campus, they won’t be walled off from the rest of Tokyo.
The Olympics also won’t mandate that athletes be vaccinated to participate in the games, which raises the issue of whether athletes can or should get a vaccine before the Olympics.
Athletes, coaches, and media in places like the United States, which is approaching a vaccine glut, will likely have no trouble getting a shot. The US Olympic and Paralympic Committee is encouraging all of its athletes to get vaccinated, though it is not mandating it.
Other countries, like Australia and Italy, are also vaccinating their Olympic teams. Pfizer and BioNTech announced earlier this month that they reached an agreement with the IOC to distribute vaccine doses to athletes and delegates from participating countries, starting in late May to allow the two-dose regimen to be completed before the games.
But it’s unlikely everyone making the journey will be vaccinated. And there is a global shortage of vaccines, with lower-income countries lacking enough doses to inoculate their health care workers or their vulnerable older populations.
“We’re still in a global pandemic with cases higher than they’ve ever been,” said Krutika Kuppalli, an assistant professor with the division of infectious diseases at the Medical University of South Carolina. “Is it appropriate for people from Brazil to go and compete in the Olympics when you have thousands of people dying?”
Kuppalli added that she understood that so many of these athletes have a life-long dream at stake, but that the world is still very much facing a global emergency. Putting resources and investment into an athletic competition, as exciting and important as it is, seems a bit misplaced when countries in need don’t have enough vaccines or oxygen concentrators.
It makes the Olympics, a celebration of international sport and cooperation, seem a bit hollow, whether or not they go forward.” (H)
“The time has come to press pause and reimagine the Olympics. It might even be time, I’ve come to believe, for the entire endeavor to close down for good.
What say you?
First, consider the near term.
In July, yet another wildly overbudget Summer Games, originally slated for 2020 but postponed because of the pandemic, will begin in Tokyo.
The timing remains awful.
Japan has worked hard to tamp down the coronavirus, but now cases are creeping up, and the nation’s vaccination rate is lagging. Organizers just rerouted the torch relay planned this week to reach the streets of Osaka, where one health official said the spread of new variants had pushed the medical system to “the verge of collapse.”
Into this troubled environment, 11,000 athletes from all corners of the globe will descend, along with coaches, officials, Olympic support staff, media workers and more. The Tokyo Games could end up being a three-week superspreader event that leads to death and illness across Japan and far beyond.
The Japanese public has grown wise to the health risk. It is also aware of the estimates that show the cost for the Games has swollen to a record $15.4 billion, up $3 billion in the last year alone. Recent surveys show close to 80 percent of Japanese say the Games should be postponed again or canceled.” (I)
“One of the latest warnings came Tuesday in an article published by The New England Journal of Medicine, in which a group of public health specialists harshly criticized the International Olympic Committee’s so-called playbooks — the colorfully illustrated packets detailing countermeasures designed to keep athletes, other Olympic visitors and the broader Japanese population safe from the virus.
In the article, the specialists suggested the playbooks — created by the I.O.C. in consultation with the World Health Organization — lacked transparency, were insufficiently detailed, and focused at times on science that was outdated or irrelevant to ensuring the event could be safe. The authors stopped short of arguing for a cancellation of the Games, but said the risk was too high to ignore.
“All along the way there’s been an ignorance of science,” Dr. Annie Sparrow, a global health specialist and the lead author of the article, said of the I.O.C. in an interview. “It’s not rocket science to hold a safe Olympics. It’s basic medical science. But that’s what the I.O.C. has ignored, and I don’t know if they’re going to start paying attention now.”…
The I.O.C., which will publish the third and final iteration of its playbook next month, has trumpeted the partnership it established last year with the W.H.O. in an attempt to build confidence in the viability of its health protocols.
But Sparrow, a former adviser to Dr. Tedros Adhanom Ghebreyesus, the director-general of the W.H.O., and her three co-authors urged the health organization’s membership to convene emergency committee meetings before the Tokyo Games, just as it did before the 2016 Rio Olympics amid widespread outbreaks of the Zika virus…
Among other things, they criticized the playbooks’ focus on countermeasures like disinfecting surfaces and temperature checks, which they called ineffective; the lack of specialized guidelines for different competition venues, common spaces and sports; the use of contact-tracing and health-reporting apps, which they called inferior to wearable trackers; and the possibility that athletes were at a higher risk of infection by sharing rooms in the athletes’ village.
The authors also laid out in stark terms how little the landscape had improved in the past year.
“When the I.O.C. postponed the Tokyo Olympics in March 2020, Japan had 865 active cases of Covid-19 against a global backdrop of 385,000 active cases,” the article said, noting that many people had assumed the pandemic would be under control by 2021. “Fourteen months later, Japan is in a state of emergency, with 70,000 active cases. Globally, there are 19 million active cases.”
Such troubling numbers were behind the State Department’s decision on Monday to warn Americans not to travel to Japan. (The recommendation was mostly symbolic; nonresident foreigners have largely been banned from entering Japan since the middle of last year.)
Still, the alarming advisory — Level 4 is the highest possible warning — did not immediately change the United States Olympic and Paralympic Committee’s outlook on attending the Games.

The state of play: Even in health care settings, vaccine mandates raise questions of ethics and workers’ rights. Health care facilities have long required workers to get other vaccines, including flu shots.
In December, the Equal Employment Opportunity Commission said mandating coronavirus vaccines would not violate federal disability law or civil rights statutes on discrimination as long as there are options to apply for a religious or medical exemption.
Still, other health systems have decided not to mandate the vaccine until a shot is approved by the FDA.
“What we’re hearing from many hospitals is that they will likely make determination of requirement of the COVID-19 vaccine for their own employees at the time the vaccines receive full approval from the FDA, which has not happened yet, but will likely happen soon,” Nancy Foster, the American Hospital Association’s vice president of quality and patient safety policy, tells Axios.
What to watch: Those who do require shots risk having to dismiss those who won’t comply when health care staffing for many systems is tight.” (A)
“One of New Jersey’s largest private employers, the RWJBarnabas Health system, will require supervisors and higher-level staff to be vaccinated for COVID-19 by the end of June — a decision it said Thursday will eventually be extended to all 35,000 members of its staff.
The 11-hospital system is “committed to providing a culture of safety,” said Barry Ostrowsky, its president and CEO. “We have an obligation to do all we can to protect our patients and the communities we serve.”
About 500 members of the supervisory staff are unvaccinated, an RWJBarnabas spokeswoman said. The policy for those who do not comply “is being finalized over the next several days,” she said.
The health system appears to be the first major employer in New Jersey to require at least some of its staff to be vaccinated. No other major New Jersey company has made such a requirement, said Tom Bracken, CEO of the New Jersey Chamber of Commerce.
“Our position is if employers want to do it, they can do it,” he said. “We’re very much in favor of getting the workforce vaccinated. It creates a safer work environment. It helps open the economy up and gets us back to where we want to be.” ..
A majority of the RWJBarnabas Health staff already are fully vaccinated, and vaccinations will be available throughout the system to any who need them, the company said…
Other health systems said they weren’t planning to mandate staff vaccines at this time.
Hackensack Meridian Health, with three teaching hospitals and nine community hospitals, said through a spokesman that its vaccine rate “is approaching 70% and is 89% for our leaders.
“We continue to carefully evaluate our options,” said Benjamin Goldstein, director of public relations.
Bergen New Bridge Medical Center, the county-owned facility in Paramus, said that a mandate wasn’t necessary, because 82% of its staff has been vaccinated. It does, however, require all new employees, volunteers, students and interns to get the shots.
St. Joseph’s Health, with hospitals in Paterson and Wayne, is not considering a mandate at this time, said Pam Garretson, a spokeswoman. Currently, 72% of its staff are vaccinated. Holy Name Medical Center in Teaneck is not mandating COVID vaccines because they are still administered under “emergency use authorization” from the FDA rather than full approval, a spokeswoman said.
Virtua Health, with hospitals in Camden, Mount Holly, Marlton, Voorhees and Wiilingboro, does not mandate the vaccines, but is “strongly encouraging and promoting the vaccine among our workforce,” said a spokesman. Slightly fewer than 70% of its staff are vaccinated.
And Atlantic Health System, which includes Morristown Medical Center, Chilton Medical Center in Pompton Plains, Overlook Medical Center in Summit, Newton Medical Center and Hackettstown Medical Center, does not mandate vaccines. About 70% of its employees are vaccinated, said Luke Margolis, a spokesman.” (B)

Penn Health officials decided the only issue that really matters is protecting people.
“We recognize that not everyone will agree with this decision,” said a Wednesday memo to UPHS employees. “As an industry grounded in the science and art of healthcare, we believe it is imperative for the UPHS to take the lead in requiring vaccinations to protect our patients and to set an example for those who remain hesitant both within our institution and in the broader community. The safety of our patients and staff are our highest priorities. The threat of further harm and the possibility of more dangerous viral mutants emerging before we reach herd immunity compels us to take this step.”…
New UPHS hires will have to provide proof of vaccination, or be fully immunized two weeks before starting work. Religious or medical exemptions may be granted, just as with the seasonal flu shot policy in place for more than a decade. But non-exempted workers who remain unvaccinated will be subject to disciplinary actions, including firing, the memo said.
In a statement, UPHS said all employees and clinicians have been offered the vaccine, and more than 33,000 — nearly 70% of the 47,000-member workforce — are now fully immunized.
“Ultimately, people do have a choice,” Patrick J. Brennan, chief medical officer and senior vice president of UPHS, said in an interview. “But there is evidence that unvaccinated workers are still getting sick and spreading it in nursing homes and other health-care settings. And staff members are getting sick and dying. So it’s a double-edged sword. No one is forced to be vaccinated, but it is a choice that affects employment” at UPHS.
Ezekiel J. Emanuel, a Penn professor of medical ethics and health policy, echoed that sentiment in a recent pro-mandate opinion piece in the New York Times: “Health care workers are professionals whose primary obligation is to their patients’ health and well-being.”” (D)

The FDA has approved vaccines manufactured by Pfizer-BioNTech, Moderna and Johnson & Johnson for emergency use. However, the federal government has not given the vaccines full approval.
The FDA’s emergency use authorizations specify that people must give consent to being vaccinated, Lawrence Gostin, a professor at Georgetown University Law Center in Washington, D.C., with expertise in health law, told The Washington Post in an April 5 article. Therefore, he told the newspaper, mandating a vaccine only authorized for emergency use could raise legal questions.
The CDC has said the decision by employers about mandates is a matter of state or other applicable law, and if employers require proof from workers that they have been vaccinated, the employer can’t require the employee to provide medical information as part of the proof. The CDC said employers may implement religious and medical exemptions.
And the Equal Employment Opportunity Commission has released guidance on the issue. The commission told Becker’s April 22 that for employers requiring vaccination be obtained in the community as an employment condition, the Americans with Disabilities Act may require reasonable accommodation [unless there is significant difficulty or expense for the employer] for those who cannot be vaccinated because of disability. Similarly, according to the commission, Title VII of the Civil Rights Act of 1964 may require reasonable accommodation of employees’ religious beliefs or practices related to vaccines, unless the accommodation constitutes undue hardship on the employer’s operations under the law.
“Whether there are feasible accommodations that do not pose an undue hardship will depend on many facts specific to each workplace and the duties of the individual,” a commission spokesperson said… “ (E)

POST 163. May 23, 2021. CORONAVIRUS. RWJBarnabas Health in New Jersey announced a mandate…saying supervisors and those of higher rank must get the vaccine by June 30. They will eventually require the system’s 35,000 employees to do the same.
“New Jersey Gov. Phil Murphy is refusing to lift his indoor mask mandate for vaccinated residents, putting the Democrat at odds with guidance from the Centers for Disease Control and Prevention and creating a fresh opening for Republicans looking to knock him off his perch as he campaigns for reelection.
With the state’s infection rate plummeting and Covid-19 hospitalizations a small fraction of where they stood last spring, New Jersey will soon be one of the only states — and alone among its neighbors in the Northeast — to require face coverings at restaurants, retailers and indoor venues.
New York Gov. Andrew Cuomo and Connecticut’s Ned Lamont will suspend mask mandates for vaccinated residents on Wednesday, the same day indoor capacity restrictions across the tri-state region are scheduled to be lifted. Pennsylvania has already shifted its guidance to match the CDC’s. Even Delaware, a frequent punching bag of New Jersey’s official Twitter account, plans to lift both indoor and outdoor mask mandates later this week.
Leaders across the region have largely moved in unison on major Covid-19 announcements, and Murphy’s delay isolates the self-proclaimed progressive on a critical facet of the reopening as he preps for a reelection campaign that will focus heavily on his management of the pandemic.
His decision to continue indoor masking puts him in the uncomfortable position of having to play defense against GOP attacks that utilize the same expert guidance he’d previously used to justify economic lockdowns.
“I don’t want to get burned. I don’t want to go back. We’re the only state in America that has not gone back once and i don’t want to start now,” Murphy said Monday during his regular Covid-19 briefing in Trenton. “I can’t speak for our neighbors, they’ve been great partners, but on this one we feel quite strongly.”
Murphy declined during his briefing to provide an exact timeline for when he might lift the mask restriction, but said during an appearance on CNN later in the day that it could come with “a couple of more weeks on the clock.”..
Like most public officials, Murphy had been caught off-guard when the CDC announced Thursday it was easing its mask guidance.
The governor suggested Monday he thought epidemiologists would continue to recommend face coverings for at least the next few months and, responding to a question from POLITICO, said he’d previously expected the next round of CDC guidelines to relax rules relating to social distancing, a move that would allow New Jersey restaurants and businesses to further expand capacity.
Though Murphy has relaxed several aspects of the state’s lockdown — schools will be back for full-time, in-person learning in the fall, travel advisories are being lifted, the public health state of emergency is set to expire and outdoor masking rules, in accordance with CDC guidelines, have been shucked — “we are not out of the woods yet,” he said.
“The majority of New Jerseyans are still unvaccinated and we’re not checking anyone vaccine’s status at the door,” he said. “I don’t know how we can expect workers to be able tell who’s vaccinated and who isn’t, and it is unfair to put the burden on businesses and frontline employees to police every patron.”
That former point is particularly true in communities that bore the brunt of infections during the pandemic’s first wave.
While New Jersey’s public health is improving — nearly 70 percent of its adult population has obtained at least one vaccine dose — uptake has been slow in several of the state’s largest cities. Fewer than a third of residents in cities like Camden, Elizabeth and Trenton are fully vaccinated. In Newark, the state’s largest city and home to a large Black and Latino population, less than half of all adults ages 18 and up have gotten their first dose…
Murphy said Monday the decision to hold off was “to protect the folks who are not yet vaccinated.” (A)

More than 3.7 million people who live, work, or study in the Garden State have now been fully vaccinated at New Jersey sites, while another 165,700 residents have been vaccinated in other states.
The state has set a goal of having 70% of New Jersey’s adults vaccinated by the end of June. More than 54% of the state’s 6.9 million adults have been fully vaccinated so far, while kids as young as 12 are now eligible to receive shots.
About 40% of the state’s 9.2 million total population has been vaccinated, while about 4.6 million have received at least one dose at state sites.
By comparison, about 124 million Americans have been vaccinated — which is about 38% of the country’s 274 million total population.
But officials are still seeking to ramp up vaccinations in New Jersey as demand has decreased and many large municipalities have vaccination rates under 40%.
Murphy said keeping the mask mandate “is overwhelmingly an incentive to get vaccinated.”
NUMBERS ARE IMPROVING, BUT …
New Jersey has made “tremendous progress” in recent weeks against the virus, Murphy said Monday during his latest coronavirus briefing in Trenton.
The state’s seven-day average for new confirmed positive COVID-19 tests is now 711 — down 31% from a week ago and 76% from a month ago.
There were 810 COVID-19 patients hospitalized across the state as of Monday night — the lowest number since Oct. 19. Overall, coronavirus hospitalizations are down 79% since the state’s second-wave peak of 3,873 patients on Dec. 22.
But Murphy said it’s too soon to drop masks because “we need to know unequivocally that doing so will not lead to a backslide in our progress.”
“We think we have this thing on the run,” he said of the pandemic. “But when you’ve got it going in the right direction as we have it going now, the more time you can put on the clock, the higher chances are that you’re gonna drive this thing permanently into the ground.”
Persichilli also noted the state’s daily COVID-19 deaths are still in double digits. An early coronavirus hotspots, New Jersey has the most coronavirus deaths per capita among American states, largely driven by the early days of the pandemic, when the virus spread rapidly through the region.
The health commissioner also noted that the state has been seeing about 1,000 cases a day, though the number has been lower in recent days.
“We know we can drive these numbers down even further,” Perischilli said.
VARIANTS STILL SPREADING
Persichilli also noted there are still COVID-19 variants spreading and information on that is still “evolving.”
And officials said hospitalizations, while much lower, are still double what they were last August, when there was no vaccine.
“We do have reasons to remain vigilant,” Persichilli said. “This is a serious and unrelenting virus.”..
NO WAY OF PROVING VACCINATIONS
The CDC did not provide guidance on how people would prove they are vaccinated. It appears to be based on the honor system.
Murphy argued that it would be on businesses to determine who is vaccinated and not, and that’s not plausible or fair.
“We’re not checking anyone’s vaccine status at the door when you go to the supermarket or to a hardware store, for instance,” the governor said. “I don’t know how we can expect workers to tell who is vaccinated from who isn’t, and it is unfair to put the burden on business owners and frontline employees to police every patron.” (B)
“As of Wednesday, fully vaccinated residents in New York and Connecticut are no longer required to wear masks or stay 6 feet away from others in most settings, as a growing number of states follow the Centers for Disease Control and Prevention’s more relaxed mask guidelines.
But that decision is putting people at risk, warns Dr. Natasha Kathuria, an emergency medicine physician, who says there is no efficient way to verify who is vaccinated and who is not.
“We’re really depending on an honor system,” Kathuria told Yahoo Finance Live. “There’s no way to ensure that the people removing their masks are vaccinated or not.”
“We really want people to still be cautious. This is not the time to completely let your guard down while things are improving and looking great,” added Kathuria.
Many states immediately relaxed mask mandates following the CDC’s updated guidance, while others, including California, Maine and Massachusetts said they plan to rescind their face covering orders soon.
However, states still have the authority to uphold existing mask mandates if they choose and, so far, New Jersey is the only state with no plans to lift its mandate for fully vaccinated people indoors.”” (C)
“State lawmakers on Tuesday began advancing a plan that would end New Jersey’s public-health emergency over the coronavirus but allow Gov. Phil Murphy’s administration to retain many powers to keep battling the pandemic through the end of this year — including overseeing masking and social distancing rules…
Murphy said he would allow the order to end next month if the Legislature passed a bill allowing his administration to retain some “tools” to keep protecting the state and rolling out vaccinations “in order to continue on the path to normalcy.”
The public-health emergency — which Murphy has extended each month since last March — has allowed the governor to issue dozens of executive orders, including capacity restrictions on businesses and masking mandates, without input from lawmakers…
Murphy’s “most recent” orders on masking and social distancing. The governor can ease these orders but tighten them only if the state’s rate of transmission reaches 1 or higher, hospitalizations increase, or statewide test positivity increases. Currently, the state has lifted its outdoor mandate but is still requiring masks indoors in public, though Murphy said that could change in the coming weeks.
But it doesn’t immediately wipe away all the restrictions the governor put in place. Instead, it keeps 15 of the orders until Jan. 1, 2022. Those include:
A moratorium on evictions during the crisis.
A moratorium on insurance companies canceling policies for those who don’t pay.
A moratorium on utility shutoffs for those who don’t pay.
Expanded outdoor dining at restaurants.
A requirement that health-care facilities give the state daily data reports.
Workplace safety protocols, including temperature checks and masking rules.
Protocols at summer youth camps.
Shielding federal stimulus payments from wage garnishment.
Murphy’s administration also keeps powers over vaccination distribution, testing, and data collection.
The governor can revoke or alter any of the 15 remaining orders before Jan. 1…

“California says it isn’t ready to follow the federal lead and unmask, at least for another month. The state’s health director announced Monday that California will require people to keep wearing masks and practice social distancing indoors until June 15, when Governor Gavin Newsom had already said nearly all COVID-19 restrictions will be dropped if cases remain low.
The nation’s most populous state needs more time before it will follow the lead of many other states and allow fully vaccinated people to skip face coverings and socially distancing in virtually all situations, officials said.
People and businesses must adjust to the changes while the state continues its “relentless focus on delivering vaccines, particularly in underserved communities and those that were hard hit throughout this pandemic,” Health and Human Services Secretary Dr. Mark Ghaly said Monday…
The shifting ground of mask-wearing advice has confused and frustrated some Californians who only a few weeks ago were told that that people who are fully vaccinated don’t need to wear masks outdoors unless they’re at crowded gatherings…
Most of the 58 counties in California – including gigantic Los Angeles, Orange and San Diego – are heeding the state’s masking schedule.
L.A. County Supervisor Hilda Solis said the county will use the extra month to fully vaccinate more people. “More than half of our adult population remains vulnerable,” she said.
Both the CDC and state plan to keep the mask requirement in place for students for the rest of the calendar year…
Businesses are expected to adhere to the state’s guidelines, Ghaly said, but some trade group leaders feared the differing federal and state mask requirements will sow confusion.
It is difficult already for business owners to play “mask cop,” said John Kabateck, director of the California chapter of the National Federation of Independent Businesses. “We hope that they will not be vulnerable to penalties and scrutiny by state regulators or plaintiffs’ attorneys because they’re trying to make sense of this labyrinth.”
Still, Kabateck said he was generally supportive of the state’s decision.
“If wearing masks for a little bit longer is an ounce of prevention that’s going to let mom-and-pops reopen their doors and get people back to work, that’s a step in the right direction,” he said.
After becoming the epicenter for the virus in the U.S. at the start of the year, California has seen a precipitous decline. The state positivity rate among those tested has fallen below 1%, officials said Monday, and more than 34.5 million doses of vaccine have been administered as the state approaches the full reopening of its economy…
Forty-seven counties, which account for most of the population, are in the orange and yellow tiers of California’s COVID-19 system, which are the least restrictive. Orange County could move into the yellow tier on Wednesday, which would increase customer capacity for gyms and some other businesses and allow bars to reopen indoors.
Eleven counties remain in the second-highest red tier, where COVID-19 risk is considered substantial, but they represent only around 11% of the population.
The tiers are based on cases, positivity rate and on vaccination figures. Newsom has indicated California will do away with the system in mid-June.” (E)
“Republican Texas Gov. Greg Abbott issued an executive order Tuesday prohibiting state governmental entities such as counties, public school districts, public health authorities and government officials from requiring mask wearing.
“Texans, not government, should decide their best health practices, which is why masks will not be mandated by public school districts or government entities. We can continue to mitigate COVID-19 while defending Texans’ liberty to choose whether or not they mask up,” Abbott said in a news release.
The executive order allows public schools to continue current mask-wearing guidelines through June 4; however, after June 4, no student, teacher, parent or staff member can be required to wear a mask on school grounds, according to the order.
Local governments or officials attempting to impose a mask mandate can be subject to a fine of up to $1,000, beginning May 21, according to the news release.
The order, however, exempts state-supported living centers, government-owned or operated hospitals, state department of justice facilities and county and municipal jails, according to the release.
The governor’s order comes less than a week after the US Centers for Disease Control and Prevention released updated guidance allowing vaccinated people to be both outdoors and indoors without masks in most cases. Other states have moved to roll back mask mandates in recent weeks, with New York, California and Michigan announcing that they would lift such requirements.
Texas lifted its statewide mask mandate on March 2. A week later, Texas businesses of all types were allowed to open to 100% capacity.” (F)
