“According to research published Friday in the journal Scientific Reports, vaccinated people — counterintuitively — play a key role in that risk.
The best way to stop coronavirus deaths and severe illness is to roll out vaccines quickly. However, the researchers concluded that the chance a vaccine-resistant strain will emerge is highest in a scenario that combines three conditions: First, a large portion of a population is vaccinated, but not everyone. Second, there’s a lot of virus circulating. And third, no measures are in place to curb potential viral transmission from vaccinated people. Sound familiar?
Before the rise of Delta, which is now responsible for more than 80% of US coronavirus cases, that situation wasn’t a concern because research suggested vaccinated people weren’t likely to transmit other versions of the virus. But according to a CDC study released Friday, vaccinated people may transmit the Delta variant just as easily as the unvaccinated…
The researchers created a mathematical model that predicted which conditions are associated with the highest risk of new variants emerging that can evade vaccines.
They found if a chunk of people are vaccinated but many unvaccinated people remain, a variant that can either evade or partially evade vaccine-induced immune defenses has a competitive advantage over other versions of the virus. So over time, those less fit strains — which can’t infect vaccinated hosts — die out, leaving vaccine-resistant ones to dominate the viral landscape. Then if viral transmission goes unchecked — lots of people partying maskless, say — those newly dominant variants can easily spread and further evolve…

“The amount of breakthrough cases we’re seeing actually suggest that the vaccines “are still working great,” but too much is being asked of them, said Dr. Drew Weissman, who spent nearly two decades conducting research that led to the development of the mRNA vaccines manufactured by Pfizer and Moderna to stop the coronavirus.
Mounting research suggests the variants are placing greater strain on the vaccines. When people are infected with the delta variant, Huffman said they produce a thousand times greater viral load than seen from the original COVID-19 strain. That means the delta variant is more efficient at spreading and every time an infected person coughs, sneezes or speaks, they potentially release much more of the virus — and opportunities to get someone else sick — than earlier in the pandemic.
With only about half of the nation’s population — 163.3 million people — fully vaccinated against the coronavirus, the vaccine’s advantages begin to erode, said Weissman, a physician and infectious disease expert at the University of Pennsylvania. And in some pockets of the country, fewer than a third of people have received full vaccine protection.

Appearing on CBS’s “Face the Nation,” Fauci stressed that the majority of breakthrough cases involving the delta variant saw “minimal symptoms or no symptoms at all.”…

“So we know now that vaccinated people who get breakthrough infections can spread the virus to other people,” Fauci said. “The fundamental basis for the CDC modifying their guidelines and saying now, ‘If you’re an area of a high or substantial trend of level of virus, namely a red or an orange zone, when you’re in an indoor public setting, you need to wear a mask.’ That’s the fundamental reason for that change.”” (H)
“At least 125,000 fully vaccinated Americans have tested positive for Covid and 1,400 of those have died, according to data collected by NBC News.
The 125,682 “breakthrough” cases in 38 states found by NBC News represent less than .08 percent of the 164.2 million-plus people who have been fully vaccinated since January, or about one in every 1,300. The number of cases and deaths among the vaccinated is very small compared to the number among the unvaccinated. A former Biden adviser on Covid estimated that 98 to 99 percent of deaths are among the unvaccinated.
But the total number of breakthrough cases is likely higher than 125,683, since nine states, including Pennsylvania and Missouri, did not provide any information, while 11, like Covid hotspot Florida, did not provide death and hospitalization totals. Four states gave death and hospitalization numbers, but not the full tally of cases.
And vaccinated adults who have breakthrough cases but show no symptoms could be missing from the data altogether, say officials….
Critics of the agency (CDC) like former FDA Commissioner Scott Gottlieb, said federal officials are failing to capture the overall scope of infection of Covid cases. Gottlieb told CNBC Friday, “I suspect probably one in 10 infections is actually getting reported.” (D)

The state is in “unchartered territory,” he explained, noting that it just recorded the highest number of cases in a 24-hour period since the pandemic began. More significantly, though, more than 10,000 people are currently hospitalized with an infection, which accounts for 83 percent of Florida’s all-time pandemic high. New York Times’ data shows hospitalizations are up 123 percent in the last 14 days, the fourth highest rate in the country.

““At Tampa General Hospital, the 90-plus patients hospitalized with COVID already exceeds the previous high of 86, said Dr. Seetha Lakshmi, medical director of its Global Emerging Diseases Institute. She said the hospital, like many, can’t hire enough staff and it is leaving those working exhausted.
“It feels like we are getting hit by a train, the pace is so fast and uncontrolled,” Laskshmi said. “I just don’t have any words anymore. This is awful, just awful and it is going to be awful.”
She said last year, her patients’ median age was in the 70s. Now, it is just over 50, with the younger patients getting sicker than in the past.
She pointed to a patient in his early 30s whose lungs “sound like Velcro” being pulled apart. A father of young children, he will likely have permanent damage and might need a transplant eventually, she said.

“The Delta variant is much more contagious, more likely to break through protections afforded by the vaccines and may cause more severe disease than all other known versions of the virus, according to an internal presentation circulated within the Centers for Disease Control and Prevention.
Dr. Rochelle P. Walensky, the director of the agency, acknowledged on Tuesday that vaccinated people with so-called breakthrough infections of the Delta variant carry just as much virus in the nose and throat as unvaccinated people, and may spread it just as readily, if less often.
But the internal document lays out a broader and even grimmer view of the variant.
The Delta variant is more transmissible than the viruses that cause MERS, SARS, Ebola, the common cold, the seasonal flu and smallpox, and it is as contagious as chickenpox, according to the document, a copy of which was obtained by The New York Times.
An internal document from the Centers for Disease Control and Prevention notes that the Delta variant is much more contagious than other known versions of the virus.

“There are no more beds left,” Chief Medical Officer Katy O’Neal said at a press conference on Monday. At that same press conference, Louisiana Gov. John Bel Edwards announced the state would bring back its mask mandate and encouraged residents to get vaccinated.
Representatives from other hospitals in the state said they were having staffing shortages or dealing with COVID-19 outbreaks among staff.

When I was appointed President and CEO of Jersey City Medical Center in1989 in the peak of the HIV/ AIDS crisis, we had a dedicated, and always full, dedicated 60 bed AIDS unit, staffed by one full time nurse and numerous part-time and per diem nurses. Today, right now, hospitals caught in the Covid-19 surge, are forced to redeploy nursing staff, respiratory therapists and other clinicians, most without prior infectious diseases experience. Why are we tolerating the unvaccinated putting our hospital staffs at risk?

Over my career I’ve seen my share of Rapid Response challenges –
While the Mount Sinai administrator at Elmhurst Hospital. On December 29, 1975, a bomb detonated near the TWA baggage reclaim terminal at LaGuardia Airport in New York City. The blast killed 11 people and seriously injured 74. The perpetrators were never identified. Elmurst Hospital was the epicenter of the response.
At JCMC. The 1993 first World Trade Center bombing was a terrorist attack on the World Trade Center in New York City, when a truck bomb detonated below the North Tower of the complex. JCMC EMS “flew” across the Hudson River to participate in the response.
At JCMC. On September 11th JCMC, under the radar, took care of more casualties than all but two NYC hospitals, as ferries brought thousands of refugees across the Hudson River to Jersey City.
And then there was the four alarm arson fire in the “old” hospital, where 300 inpatients were at risk with the JCFD getting it under control as we were evacuating the ICUs before trying to evacuate the inpatient floors.

“Florida added more than 16,000 new coronavirus infections on Wednesday, according to data from the Centers for Disease Control and Prevention. It is the single largest day increase in the pandemic since Jan. 15.” (A)
“Officials with Central Florida’s largest hospital system said there are about 1,000 patients hospitalized with COVID-19 as of Thursday morning.
AdventHealth officials provided the update as Florida sees an increase in COVID-19 cases and hospitalizations.
Dr. Vincent Hsu, the executive director of infection prevention and hospital epidemiologist, said the current wave the hospital system is seeing has passed numbers seen in January during peak hospitalizations.
“What’s extraordinary is the speed at which we are currently seeing new cases and unfortunately right now, the slope is pretty steep and we haven’t seen the end of it, this is still coming,” he said.
In response to whether there are any signs of a deceleration in the number of cases, Hsu said there isn’t as of now and they are continuing to see a “very significant surge.” He said among the number of hospitalized patients, about 93-95% are unvaccinated patients.
“We have to recognize that the delta variant behaves in ways that … were unlike the other variants and so this is a significant issue that we’re going to have to deal with,” Hsu said.” (B)
“During their daily morning call, University of Kansas Health System doctors were asked about any preparations for a hospital influx.
“We have more than 100 ICU beds… and most of our eyes see that they’re occupied because our overall census is so high,” Dr. Steven Stites, chief medical officer, said.
He said the issue is that once people are stabilized, they can be moved out of the ICU and onto other floors, but there isn’t a lot of room on other floors available for those COVID-19 patients.
Until there is more room in the ICU, they can’t put more people in the ICU. Right now, that health system is operating two COVID-19 ICUs.
“So where we are today, we can manage. The question is the following,” Stites said. He said current numbers could be where we crest and then numbers decline, or it could just be a plateau and numbers could climb.” (C)
For sure, where the trajectory is steep hospitals are redeploying staff! To care mostly for unvaccinated patients.
So it’s time for mandatory vaccinations! Let’s not wait until local health care systems implode.
“President Joe Biden will announce on Thursday a requirement that all federal employees and contractors be vaccinated against Covid-19, or be required to submit to regular testing and mitigation requirements, according to a source with direct knowledge of the matter.

In a blog post, Google CEO Sundar Pichai said the vaccine mandate would apply to its U.S. offices in the coming weeks and would be required eventually for other locations.
“Getting vaccinated is one of the most important ways to keep ourselves and our communities healthy in the months ahead,” Pichai wrote.
Shortly after Google’s announcement, Facebook said it too will require anyone coming to work at its U.S. offices to be vaccinated.

Today, right now, hospitals caught in the Covid-19 surge, are forced to redeploy nursing staff, respiratory therapists and other clinicians, most without prior infectious diseases experience. Why are we tolerating the unvaccinated putting our hospital staffs at risk?
“Beginning in mid-September, New York City will require all of its 340,000 municipal workers, including police, firefighters and teachers, to either be vaccinated against COVID-19 or tested weekly. The mayor’s office says the policy will be announced Monday.
Under the latest plan, all city workers must comply with the vaccination-or-test rule beginning on September 13th, which is also the first day of public school in New York City. However, for 45,000 city staffers or contractors who work in residential and congregate care settings, the deadline to comply will come earlier, on August 16th.
As part of the transition to spur more vaccination, the city will double down on its existing mask mandates for unvaccinated employees. All city workers must show proof of vaccination starting August 2nd or be forced to wear a mask at all times.
The new directives come less than a week after Mayor Bill de Blasio issued the “get vaccinated or get tested” rule for the city’s 42,000 public health workers. At the time, the mayor hinted that the requirement would eventually be expanded to more of the city’s workforce as part of an incremental or “climb up the ladder” approach.
About 54% of all New Yorkers have been fully inoculated to date, but the pace of vaccinations has been slowing, leading experts to worry that the more contagious delta variant will fuel a rapid surge in infections. Across the country, government officials and private businesses have increasingly debated whether to require vaccinations, as the highly contagious Delta variant of the virus surges in many regions. Still, moves to adopt such measures have been limited.

All employees will be required to prove they have received the vaccine by showing their vaccination cards or through a verification code provided by the California Department of Public Health.
Unvaccinated state employees will be required to get tested at least once a week and will be required to wear N95 masks at all times. Unvaccinated health care workers will have to get tested at least twice a week, health officials said…

“The Department of Veterans Affairs will require its front-line health care workers to be vaccinated against COVID-19, a move that comes as vaccine mandates from employers are on the rise.
“I am doing this because it’s the best way to keep our veterans safe, full stop,” Denis McDonough, the secretary of Veterans Affairs, told The New York Times in an interview.
The move will apply to 115,000 employees who have the most “patient-facing” jobs, the Times reported, and they will have eight weeks to get the shots if they have not already.

“The Pentagon is considering making COVID-19 vaccinations for troops mandatory once the Food and Drug Administration fully approves the shots, part of ongoing discussions about the next step in the agency’s response.
In a briefing with reporters Tuesday, Pentagon Press Secretary John Kirby said senior leaders within the Defense Department have had “some preliminary discussions” about what would happen if and when full FDA approval comes through.
“Should the FDA approve it, then I am certain that Pentagon leadership will take a look at what our options are going forward, including the potential option of making it mandatory,” Kirby said.

“A federal appeals court late on Friday reversed course and let stand a lower court order prohibiting the U.S. Centers for Disease Control and Prevention (CDC) from enforcing coronavirus-related cruise ship rules in Florida.
The decision is a win for Florida that had filed a lawsuit arguing the CDC curbs made it tough for the cruise industry to recover from the pandemic…
The CDC said that even though it cannot require cruise ships to abide by the sail order, it will enforce its separate transit mask requirements on cruise ships in Florida that opt not to follow the now voluntary program.
All cruise ships in Florida will still be required to report “individual cases of illness or death and ship inspections and sanitary measures to prevent the introduction, transmission, or spread of communicable diseases,” the CDC said late on Friday.
Cruise lines that ensure at least 95% of passengers and nearly all crew are vaccinated can bypass simulated voyages and move more quickly to resuming commercial trips and can allow vaccinated people to avoid masks in inside common spaces, according to the CDC’s conditional sail order.
The conditional sail order “represents the most effective way of continuing to protect the public’s health,” the CDC said.
The CDC “remains committed to working with the cruise industry and seaport partners” to ensure the safe resumption of operations, it added.” (I)
Dr. Anthony Fauci said Sunday that the Centers for Disease Control and Prevention is examining whether to reinstate mask guidance even for fully vaccinated people in public.
“This is under active consideration,” Fauci told CNN anchor Jake Tapper, when asked if masks should be brought back for vaccinated Americans.
Tapper had cited CNN’s reporting, which said that the Biden administration’s top health officials are holding “preliminary conversations” on whether to urge vaccinated people to wear masks again in public. The Washington Post also reported that the talks were in a “preliminary phase and their result could be as simple as new messaging from top White House officials.”…

The big picture: Infectious disease experts say the Olympics don’t have strong enough protocols for testing or ventilation, either in competition venues or in the Olympic village.
“What kind of ventilation systems do they have? The last check-in we had with them, in their Olympic Village apartments, there wasn’t the kind of adequate ventilation that would substantially reduce the spread of the virus,” said Michael Osterholm, an infectious disease expert at the University of Minnesota.
Osterholm and a group of other researchers published safety recommendations for the Olympics earlier this year, but with the opening ceremonies fast approaching, they say they’ve seen little measurable progress.
Testing is a core component of the International Olympic Committee’s playbook, but it’ll be too easy for cases to slip through the cracks and then spread, said Annie Sparrow, a professor at the Icahn School of Medicine at Mount Sinai Hospital.
While there are daily testing protocols for the Olympic teams, the workforce has a varying frequency of testing depending on their role.
“What about the workers, the volunteers, the bus drivers exposed for 14 to 16 hours a day who are going into the village and then going back home to their families?” said Sparrow, who helped advise the WNBA through the pandemic…
Some of the IOC’s suggestions — like suggesting that athletes open their windows every 30 minutes — aren’t supported by science, Osterholm said.
And while formal guidelines call for social distancing, Sparrow pointed to media reports that show some facilities, namely a cafeteria, set up for what looks like full capacity…
“My mind boggles when I think about, ‘What are the risks here?'” said Lisa Brosseau, a bioaerosol scientist and research consultant.
Context: Other sports organizations have navigated this successfully, but experts said the IOC’s precautions simply aren’t as strong as those employed last year by the NFL or the NBA.
“I think [the NFL] understood the concept of airborne transmission and what it meant and they planned accordingly,” said Osterholm, who helped advise the NFL’s safety precautions.
The bottom line: “We all want the Olympics to happen. The world needs good news right now,” Osterholm said. “But we want it to happen as safely as possible.”” (A)
“Some 15,000 athletes and 53,000 officials from 207 countries are expected to enter Japan over the next few weeks for the Tokyo Olympics that are now just a month away.
While some medical experts have warned the games could result in superspreader events, organizers and political leaders in Japan insist they are taking measures to ensure everyone would be “safe and secure.”
At the heart of the preparations is the Olympics village in the Harumi waterfront district of Tokyo — a 44-hectare complex of 21 buildings where athletes will eat, sleep and train in bubbles. They will be frequently tested for COVID-19 and have to obey strict movement and social distancing restrictions.
A clinic will be open around the clock to provide care for any athlete thought to be infected. While waiting for test results, the athlete will have to quarantine in a private room. From July 1, any overseas athlete who tests positive on arrival will be taken for a confirmatory PCR test. If the result is positive, the athlete will have to isolate in a facility set up by Tokyo 2020.
All athletes will have to take daily tests, with up to 18,000 saliva samples expected to be collected each day. Organizers are considering measures such as video recordings and spot checks to ensure rules are followed.
Some 45,000 meals a day will be provided in the 3,000-seat two-story main dining hall. But to avoid overcrowding, athletes can look to a screen to check the numbers in the dining hall. The screen is refreshed and updated every two minutes.
Another change is that the buffets are gone. This year, cafeteria staff will place food on trays for the athletes who will eat behind plastic screens and be encouraged to keep a distance of at least 2 meters from the next person.
Alcohol can be consumed but athletes must drink alone in their rooms. Condoms will be handed out at the point of departure so that athletes will not have to opportunity to use them while still in the Olympic Village.
Because of these measures and restrictions, this year’s Olympics have proved to be even more work than other years. But International Olympic Committee President Thomas Bach struck a bullish tone: “Here we go… We are ready,” he said at a virtual conference between organizers on Monday.” (B)
“Two South African soccer players became the first athletes inside the Olympic Village to test positive for COVID-19, and other cases connected to the Tokyo Games were also confirmed Sunday, highlighting the herculean task organizers face to keep the virus contained while the world’s biggest sports event plays out.
The positive tests came as some of the 11,000 athletes and thousands more team officials expected from across the globe began arriving, having traveled through a pandemic to get to Tokyo.
They’ll all now live in close quarters in the Olympic Village on Tokyo Bay over the next three weeks.
International Olympic Committee President Thomas Bach said last week there was “zero” risk of athletes passing on the virus to Japanese or other residents of the village. But that bold statement was already being tested…
The two soccer players and a team video analyst who also tested positive had been moved to “the Tokyo 2020 isolation facility,” the South African Olympic committee said. The rest of the squad members and officials had also been quarantined.
Those positive tests further stoked local fears, with the South African team scheduled to play against host nation Japan in its first game on Thursday.
There has already been consistent opposition from the Japanese public to holding the Olympics during the pandemic, with fears that it could become a super-spreader event and cause a spike in infections among Japanese people.
Bach and the IOC have insisted it will be safe and have forged ahead against most medical advice. The IOC says it sees the Games as a chance to foster international solidarity during difficult times, but the IOC would also lose billions of dollars in broadcast rights if the Games were to be canceled completely.
Also Sunday, Team South Africa confirmed the coach of its rugby sevens team also tested positive at a pre-Olympics training camp in the southern Japanese city of Kagoshima. He was also in isolation there and would miss the entire rugby competition, the team said.
And there were other Olympics-related positive tests. Olympic organizers said that another athlete had tested positive, although they were not residing in the Olympic Village. The athlete was not named and only identified as “non-Japanese.”
The first International Olympic Committee official was reported as positive. He recorded a positive test on Saturday when arriving at a Tokyo airport. The IOC confirmed the test and identified him as IOC member Ryu Seung-min of South Korea. He was reportedly being held in isolation, too….
The British Olympic Association said six athletes and two staff in the track and field squad are isolating at the team’s pre-Olympic base in Yokohama after being deemed close contacts of a person who tested positive following their flight to Japan. U.S. tennis player Coco Gauff didn’t travel to Japan after testing positive for the coronavirus.
Tokyo reported 1,008 new COVID-19 cases on Sunday, the 29th straight day that cases were higher than seven days previously. It was also the fifth straight day with more than 1,000 cases. The Olympics will open under a state of emergency in Tokyo and three neighboring prefectures.” (C)
Japan is in a tough position right now. Certainly, many businesses have been banking on these Olympics and therefore may be pushing for the Games to go on as planned. Many Olympic athletes probably don’t want to see their dreams further deferred either. At the same time, having people assemble from around the world is one of the worst things that can happen to a country that’s trying to prevent a massive surge. Ideally, you want to at least get the virus under better control first before even thinking about staging a massive international event. It’s not great for the world either. Whatever happens in Japan isn’t likely going to stay in Japan since all of the athletes and accompanying coaches and staff will be returning to their home countries.” (D)
