“Southern Louisiana’s hospitals, already packed with coronavirus patients from a fourth surge of the virus, were dealing Sunday with another challenge — the howling Category 4 hurricane pounding the coast.
Lady of the Sea General Hospital in Lafourche Parish, near where Ida made landfall, reported extensive roof damage. “All patients and staff are fine at this time without injury; although, our hospital has sustained significant damage,” hospital CEO Karen Collins said in a message relayed via Facebook. The hospital’s phone system was down.
“Once it is safe to do so they will evacuate their small number of patients,” state health department spokeswoman Aly Neel said in an email. Details on the number of patients involved were not immediately available.
Another Lafourche Parish hospital, Thibodaux Regional Medical Center, reported a partial generator failure to the state. Christina Stephens, a spokesperson for Gov. John Bel Edwards, said the facility “had not lost all critical power.” She said some patients were moved to another part of the facility and the state health department was working with the hospital.
Ida struck as hospitals and their intensive care units were filled with patients from the fourth surge of the COVID-19 pandemic, sparked by the highly contagious delta variant and low vaccination rates across Louisiana.
The governor also said 22 nursing homes and 18 assisted living facilities have been evacuated, though evacuating the largest hospitals was not an option because there simply aren’t other places to send them. Anticipating that power could be out for weeks in places, Edwards said a big focus will be on making sure there is enough generator power and water at hospitals so they can keep up with vital patient needs such as providing oxygen or powering ventilators.

“Hospitals in New Orleans are bracing for public health emergencies on two fronts as Hurricane Ida threatens to strike at the same time as Louisiana is experiencing a surge in coronavirus cases.
New Orleans is in the midst of a “severe outbreak” with a seven-day average of 220 new infections, according to the city’s Covid-19 dashboard. Across the state, more than 3,400 new cases were confirmed as of Friday, according to the Louisiana Department of Health. At least 2,684 people are hospitalized in Louisiana with Covid-19.
Last month, Louisiana hit the biggest single-day increase of Covid-19 hospitalizations since the pandemic’s start with 6,800 new cases in a single day, the second-highest single-day case count since Jan. 6, 2021…
Health care systems across New Orleans are already putting contingency plans in place, including beefing up staffing, ordering a surplus of supplies and minimizing the amount of patients who are admitted to hospitals with non-emergencies.
“We definitely were in full disaster mode and getting ready for this storm,” said Warner Thomas, president and CEO of Ochsner Health.
Ochsner has ordered 10 days worth of food, medication and medical supplies, Thomas said…
At LCMC Health, the hospital will enter a “code gray lockdown” early Sunday morning and bring in additional staff to care for patients. Staff and patients will shelter in place but have not been ordered to evacuate yet, said Dr. Jeffrey Elder, medical director for emergency management.
Additionally, doctors and nurses have been told to prepare for a high volume of patients amid the Covid-19 surge.

“As Louisiana reels from a fourth COVID-19 wave — with the highest single-day cases since the pandemic began — hospitals in the state are preparing for yet another public health crisis with Hurricane Ida battering the coast. The Louisiana governor said this hurricane will be one of the strongest to hit the state in more than 150 years.
Gov. John Bel Edwards said evacuation of hospitals in threatened areas — something that would normally be considered — is impractical with COVID-19 patients.
“That isn’t possible. We don’t have any place to bring those patients. Not in state, not out of state,” Edwards said.
More than 2,600 COVID-19 patients are hospitalized across Louisiana, according to state data. The seven-day average has begun to decline in recent days, having reached nearly 2,700 hospitalizations — the peak from April 2020.
“We have been talking to hospitals to make sure that their generators are working, that they have way more water on hand than normal, that they have PPE on hand,” Edwards said.
Officials decided against evacuating New Orleans hospitals. There’s little room for their patients elsewhere, with hospitals from Texas to Florida already dealing with a spike in coronavirus patients, according to Dr. Jennifer Avengo, the city’s health director…
Some hospitals appeared to have evacuated their most critical patients ahead of the storm, as they prepared to lose power. According to The Advocate, the Ochsner Health System evacuated 17 of its most critically ill patients from three hospitals, with 100 patients remaining at those locations.” (D)

“Several hospitals in Florida, South Carolina, Texas and Louisiana are struggling with oxygen scarcity. Some are at risk of having to use their reserve supply or risk running out of oxygen imminently, according to state health officials and hospital consultants.
With the continued uptick in Covid-19 cases, there has been more demand on the oxygen supply, and hospitals cannot keep up the pace to meet those needs, Donna Cross, senior director of facilities and construction at Premier — a health care performance improvement company — told CNN.

“Normally, an oxygen tank would be about 90% full, and the suppliers would let them get down to a refill level of 30-40% left in their tank, giving them a three- to five-day cushion of supply,” said Cross. “What’s happening now is that hospitals are running down to about 10-20%, which is a one- to two-day supply on hand, before they’re getting backfilled.”
Even when they’re getting backfill, it’s only a partial supply of about 50%, Cross said. “It is very critical situation.”” (F)
“The Florida story is a cautionary tale for dealing with the current incarnation of the coronavirus. The United States has used the vaccines as its primary pandemic weapon. But Florida shows that even a state that made a major push for vaccinations — Florida ranks 21st among states and Washington, D.C., in giving people of all ages at least one shot — can be crushed by the Delta variant, reaching frightening levels of hospitalizations and deaths…
The best explanation of what has happened is that Florida’s vaccination rates were good, but not good enough for its demographics. It has so many older people that even vaccinating a vast majority of them left more than 800,000 unprotected. Vaccination rates among younger people were uneven, so clusters of people remained at risk. Previous virus waves, which were milder than in some other states, conferred only some natural immunity.

“On the ground floor of a parking garage at the University of Mississippi Medical Center, there are coronavirus patients where the cars should be — about 20 of them on any given day, laid up in air-conditioned tents and cared for by a team of medical personnel from a Christian charity group. Another garage nearby has been transformed into a staging area for a monoclonal antibody clinic for Covid-19 patients.
These scenes, unfolding in the heart of Mississippi’s capital city, are a clear indication that the health care system in the nation’s poorest state is close to buckling under the latest avalanche of cases triggered by the highly contagious Delta variant of the virus.
“We have reached a failure point,” LouAnn Woodward, the medical center’s top executive, said late last week. “The demand has exceeded our resources.”
The current coronavirus spike has hit the South hard, but a combination of poverty and politics made Mississippi uniquely unprepared to handle what is now the worst coronavirus outbreak in the nation. The state has fewer active physicians per capita than any other. Five rural hospitals have closed in the past decade, and 35 more are at imminent risk of closing, according to an assessment from a nonprofit health care quality agency. There are 2,000 fewer nurses in Mississippi today than there were at the beginning of the year, according to the state hospital association.
“If you look around, the state’s hospitals were in bad shape before there was such a thing as Covid,” said Marty Wiseman, an emeritus political science professor at Mississippi State University. “It was not a good time to layer a pandemic on top of that.” (H)

“At Children’s Hospital New Orleans, where the intensive care unit has been jammed with Covid-19 patients, scenes like this have played out unrelentingly over the past month. Nurses raced around monitoring one gut-wrenching case after another. One child was getting a complicated breathing treatment known as ECMO, a last resort after ventilators fail, which nurses said was almost unheard-of for pediatric cases. About a half-dozen others were in various stages of distress.
“We all thought, ‘Well, thankfully it’s not happening to the kids; none of us would be able to stomach that,’” said Mark Melancon, a longtime nurse at the hospital, recalling previous stages of the coronavirus pandemic. “Fast forward to now, and it’s happening with the kids.”
Mr. Melancon said that until this summer, the hospital had treated only one or two Covid-19 patients at a time. Now, a procession of patients require constant attention, their oxygen levels often dipping precariously low without maximum intervention.
As children’s hospitals in many parts of the United States admit more Covid-19 patients, a result of the highly contagious Delta variant, federal and state health officials are grappling with a sharp new concern: children not yet eligible for vaccination in places with substantial viral spread, now at higher risk of being infected than at any other time in the pandemic. Nowhere is that worry greater than in Louisiana, which has among the highest new daily case rates in the country and only 40 percent of people are fully vaccinated, putting children at particular risk as they return to school.
Most children with Covid-19 have only mild symptoms, and there is not enough evidence to conclude that Delta makes some of them sicker than other variants do, scientists say. Doctors and nurses at Children’s Hospital agreed with that assessment.
Theresa Sokol, Louisiana’s top epidemiologist, said that people younger than 18 had among the highest test positivity rates in the state and were responsible for a significant share of transmission, with many cases most likely undetected.
“So many days are filled with this puzzle of: We don’t have enough beds for this patient who wants to come, so how are we going to shuffle our children around to accommodate one more?” said Devon H. Relle, a pediatric nurse practitioner at Children’s Hospital New Orleans, where she worked the front desk of the 17-bed I.C.U. The hospital was also seeing an early, worrisome wave of respiratory syncytial virus, known as R.S.V., which can cause some of the same symptoms and was contributing to the overflow conditions.
The crush of Covid-19 at Children’s Hospital grew so intense this month that the state called in a federal “surge team” of emergency responders from the Department of Health and Human Services’s National Disaster Medical System. The group of about 14 included a physician, a nurse practitioner, nurses, paramedics, a respiratory therapist and a pharmacist.
The team was the first assigned to a children’s hospital during the pandemic.
“Covid-19 right now down here is so endemic that you don’t have to have a specific exposure, because it’s just out there,” said Anne Barylick, a nurse practitioner on the surge team who handled patient intake in the emergency department and Covid-19 units. “Statistically, you’re going to run into it.”” (J)

“The US Food and Drug Administration on Monday granted full approval to the Pfizer/BioNTech Covid-19 vaccine for people age 16 and older. This is the first coronavirus vaccine approved by the FDA, and is expected to open the door to more vaccine mandates.
The vaccine will be marketed as Comirnaty, the FDA said in its announcement on Monday. The Pfizer/BioNTech vaccine has been authorized for emergency use in the United States since mid-December for people age 16 and older, and in May, the authorization was extended to those 12 and older.
“The vaccine also continues to be available under emergency use authorization (EUA), including for individuals 12 through 15 years of age and for the administration of a third dose in certain immunocompromised individuals,” according to the FDA.
Out of more than 170 million people in the United States fully vaccinated against Covid-19, more than 92 million have received the Pfizer/BioNTech vaccine.
“While this and other vaccines have met the FDA’s rigorous scientific standards for emergency use authorization, as the first FDA approved Covid-19 vaccine, the public can be confident that this vaccine meets the FDA’s gold standard for safety, effectiveness and manufacturing quality that we require for an approved product,” FDA Acting Commissioner Dr. Janet Woodcock said during a briefing on Monday, calling the approval “a pivotal moment” for the United States’ fight against the coronavirus pandemic.” (A)

It’s now “very clear” that immunity starts to fall after the initial two doses, and with the dominance of the delta variant, “we are starting to see evidence of reduced protection against mild and moderate disease,” according to the statement signed by CDC Director Dr. Rochelle Walensky, acting FDA Commissioner Dr. Janet Woodcock, White House chief medical advisor Dr. Anthony Fauci and other U.S. health leaders.

The plan is subject to a formal recommendation from a CDC vaccine advisory committee and approval from the FDA, also a formality….
U.S. officials changed their message on boosters in recent days as cases continued to rise. Fauci said Thursday that everybody will “likely” need a booster shot at some point. On Friday, federal officials approved administering booster shots to Americans with weakened immune systems, which includes cancer and HIV patients and people who have had organ transplants….
During a press briefing Wednesday, Walensky said officials based their decision on studies that showed immunity from Pfizer and Moderna’s vaccines diminished over several months. One study in New York from May 3 through July 25 showed that the vaccine’s effectiveness in protection against infection dropped from around 92% to 80%. Another study by the Mayo Clinic showed that Pfizer’s vaccine efficacy fell from around 76% to 42% while Moderna’s declined from 86% to 76%.” (B)
“…if the FDA follows its usual protocol, the agency would first convene a meeting of its outside advisory committee, as it did when it originally granted emergency use authorization for each COVID-19 vaccine.
“We know in sort of the heat of the moment, and when we’re all dealing with a real public health emergency, it becomes almost doubly important that we continually reassess and have the normal processes in place,” says Dr. Jesse Goodman, a professor at Georgetown University’s School of Medicine who used to be the chief scientist at FDA.
Goodman says he worries that setting a start date for a booster program before the relevant data can be evaluated through the normal federal processes may put “the cart before the horse.”
The committee of outside experts typically hears presentations from the government, the product manufacturer and the public. Then, it often votes on whether the FDA should give a given product the green light. The agency doesn’t have to follow the committee’s advice, but it usually does.
Norman Baylor, a former director of the Food and Drug Administration’s Office of Vaccines Research and Review, says putting those committee meetings together in a month — as the administration’s Sept. 20 booster start date would imply — is “pushing it.”
The agency’s interim commissioner, Janet Woodcock, tweeted about the agency’s booster plans Wednesday afternoon, though she didn’t commit to convening the FDA’s Vaccines and Related Biological Products Advisory Committee.
“FDA supports the administration’s work to plan for the deployment of additional vaccine doses, or boosters, this fall,” she wrote. “FDA will undertake an independent scientific evaluation of the safety and efficacy of boosters for each vaccine.”
After an FDA authorization, the CDC’s independent vaccine advisory committee would also weigh the evidence and make a recommendation.” (C)
“The CDC has pushed back its advisory panel meeting to discuss COVID-19 booster shots for the general population by one week, as medical professionals are split over whether the shots are necessary, Bloomberg reported Aug. 19.
The CDC’s Advisory Committee for Immunization Practices originally was set to meet Aug. 24 to discuss the need for booster shots and possibly make a formal recommendation but will now convene over two days starting Aug. 30.” (D)
“Federal health officials are expected Wednesday to present evidence for why people are likely to need Covid-19 boosters eight months after their second doses of a vaccine, according to sources with the Centers for Disease Control and Prevention.
The eight-month time frame is most likely based on findings from both the U.S. and abroad looking at how the vaccines have held up over time — and whether they can stand up to the hypertransmissible delta variant of the coronavirus that has overtaken the country.
“Delta is forcing this discussion” on boosters, said Dr. Colleen Kraft, associate chief medical officer at Emory University Hospital in Atlanta.
The variant now accounts for nearly 100 percent of new Covid-19 cases in the U.S., according to the CDC. The seven-day average of Covid-19 cases has soared by 700 percent since the beginning of July, when delta became the dominant strain in the U.S.
Questions remain about how well the vaccines fare against the delta variant, as well as whether protection simply wanes over time. Indeed, experts said they are eager to see any new data that led to the government’s proposed timeline for booster shots…
That’s a concern, particularly with how contagious the delta variant is, said an expert in pulmonary and critical care, Dr. Benjamin Singer, an assistant professor of medicine at the Northwestern University Feinberg School of Medicine.
The variant has been associated with breakthrough infections, and while such infections are rare and generally much less severe, studies indicate that fully vaccinated people can spread the virus if infected.
As for why the booster shots would be recommended at eight months, Singer said, “it’s not entirely clear, but it’s probably a combination of waning immunity over time and the fact the current circulating variants are just that much more contagious and spread that much more easily.”…
Dr. Peter Hotez, dean of the National School of Tropical Medicine at Baylor College of Medicine and co-director of the Texas Children’s Hospital Center for Vaccine Development, said, “All we’re getting is snippets of data on the Israel Ministry of Health website.” He said he was curious whether, in addition to a decline in efficacy against mild infections, there was a significant drop in protection against hospitalizations for vaccinated people over time…
A recent study from the Mayo Clinic, however, also suggested waning immunity from the vaccines. The research, which hasn’t yet been published in a peer-reviewed journal, found that protection against infection during the height of the delta variant’s spread fell to 76 percent among those who had received the Moderna shots and 42 percent among those who got the Pfizer shots.” (E)
“A person’s immune system is a complicated puzzle, with antibodies in a person’s blood stream as only one piece.
Scientists said they are looking at other clues too, including closely watching COVID-related hospitalizations involving vaccinated people.
Pfizer and BioNTech, which partnered to develop the nation’s first vaccine, said it has early data suggesting that a booster dose anywhere from six to 12 months after the initial vaccination will help maintain a high level of protection. Their data have not been shared publicly yet.” (F)
“After all that, World Health Organization (WHO) chief scientist Soumya Swaminathan suggested that prioritizing booster shots in wealthier countries like the U.S. could lead to more variants, and other scientists have come out saying it’s still too soon. The onslaught of new information has caused some confusion. These are some of the most pressing questions.
Who will need a third shot and when?
First up for the third shots are a limited number of individuals the CDC considers moderate to severely immunocompromised. The third shots are available for these individuals today.
For the rest of the population, for whom the vaccine is gradually losing some efficacy, the Biden administration hopes to start offering the shots near the end of September, starting with the same groups that were vaccinated earliest during the first vaccine rollout: healthcare workers and people over the age of 65. You’ll become eligible for your third shot eight months after you received the second jab, so aside from the immunocompromised patients going first, it’ll look a lot like the previous rollout. The administration still needs an advisory committee from the CDC to review evidence and make recommendations and for the FDA to authorize the plan.
The immunocompromised patients are going first, explains Onisis Stefas, a chief pharmacy officer at Northwell Health in New York City, because they’re not losing immunity—many of them never mounted a strong immune response to begin with. Additionally, if you’re immunocompromised “you have less of a chance of fighting off an infection like the coronavirus.” meaning if you do get infected, you’ll likely fare worse than a someone with an intact immune system…
The August 18 announcement that everyone will need a third shot came on the heels of two main findings. The first is that immunity gained from the vaccines seems to wane over time. Data from Israel, which was the first to vaccinate most of its population with the Pfizer vaccine shows that more than half of infections are occurring in individuals who have been vaccinated…
The second major finding is that while the vaccines still protect most patients from hospitalization and death, the new Delta variant can infect vaccinated individuals. And those individuals can spread the disease to others, albeit for a shorter period than unvaccinated individuals who are infected.” (G)
“Several countries have decided to provide booster shots to older adults and people with weak immune systems. European Union officials said on Wednesday they do not yet see a need to give booster shots to the general population.
Other experts said the U.S. plan requires thorough vetting by the FDA and a panel of outside advisers to the CDC. A meeting of those advisers to discuss boosters set for Aug. 24 is being rescheduled, the CDC said on Thursday on its website.
The Department of Health and Human Services, which oversees the CDC and FDA, did not immediately respond to a request for comment.
Some experts questioned the focus on booster shots when around 30% of eligible Americans have yet to get even a first vaccine dose, despite new COVID-19 cases and deaths surging across the country.
“The more important thing, I think, at this point than boosters is making sure we get the vaccine in any arm that hasn’t had one as fast as we can,” said Dr. Dan McQuillen, an infectious disease specialist in Burlington, Massachusetts, and the incoming president of the Infectious Diseases Society of America.
All experts interviewed by Reuters also emphasized the need to inoculate the vast number of people around the world who have yet to access COVID-19 vaccines.
“You could end up in situation where you are chasing your tail, giving more and more boosters in the U.S. and Western Europe, while more dangerous variants are coming from other places,” said Dr. Isaac Weisfuse, epidemiologist and adjunct professor at Cornell University Public Health.
“In reality you should be vaccinating the rest of the world to avoid new variants.”” (H)
“Current data does not indicate that COVID-19 booster shots are needed, the World Health Organization (WHO) said on Wednesday, adding that the most vulnerable people worldwide should be fully vaccinated before high-income countries deploy a top-up.
The comments came just before the U.S. government said it planned to make the booster shots widely available to all Americans starting on Sept. 20 as infections from the Delta variant of the coronavirus rise. read more
WHO chief scientist Soumya Swaminathan, asked about the need for boosters to increase protection against the disease, told a Geneva news conference: “We believe clearly that the data today does not indicate that boosters are needed.”
WHO senior adviser Bruce Aylward, referring to booster shots being administered in high-income countries, told reporters: “There is enough vaccine around the world, but it is not going to the right places in the right order.”
Two doses should be given to the most vulnerable worldwide before boosters are administered to those fully-vaccinated, he said, adding: “We are a long, long way from that.”” (I)
“A spokesperson from the Department of Health and Human Services tells National Geographic that regulators are taking all data into consideration—including those from research labs, clinical trials, and pharmaceutical companies like Pfizer. “We appreciate the information they shared, and officials continue to engage in a science-based rigorous process to consider whether, when, or for whom a booster might be necessary.”
In fact, contrary to Pfizer’s study, new laboratory data has emerged suggesting that the Pfizer vaccine offers protection that could last for years. So what exactly is going on? Here’s a look at what the data shows about how long immunity lasts among the fully vaccinated—and what scientists want to know before they recommend giving anyone another dose…
While the existing data does offer reassurance that the vaccines remain protective—and booster shots aren’t needed—scientists and regulators alike point to the need for more academic studies to untangle exactly how the immune system is responding to the COVID-19 vaccines.
“I think what we’re going to see, over the next six months or so, a lot of studies outlining what those other components of the immune response look like in both healthy people and in some of our vulnerable populations,” Wherry says. “We really just need a lot more information on multiple layers of the immune response to vaccination.”
It’s also important to keep an eye on public-health data, particularly the rate of hospitalizations and deaths among vaccinated people. Wherry says that, ideally, states will be able to pinpoint when those who are infected were first vaccinated to help identify when immunity seems to be waning.
The HHS spokesperson says regulators are monitoring all this new data as well. “The administration is prepared for booster doses if and when the science demonstrates that they are needed, and any recommendation by CDC and FDA would come after their thorough review process.”
All the same, Wherry says it can’t hurt to be prepared for the time when boosters are needed. “Right now you can have confidence that, if you’re fully vaccinated, your chances of getting severely ill from COVID are extremely low in the U.S.”” (J)
“Meanwhile the Advisory Committee on Immunization Practices — which reviews data on vaccines and makes recommendations on their use to the Centers for Disease Control and Prevention — is set to meet next week to look at the data for a third dose. Before Wednesday’s announcement, there was no suggestion a vote might be taken next Tuesday; the group may need additional sessions to address the question.
Surgeon General Vivek Murthy insisted the booster dose program would only go ahead if sanctioned by the FDA and the ACIP, as the CDC advisory committee is known. But the fact that there is a fixed date for the start of the booster program led some to wonder if the outcome had already been decided.
“How did you pick the week of September 20?” asked Baylor. “Since this meeting hasn’t occurred and you don’t know what they’re going to say — this is an independent body — suppose they say: ‘We don’t think so.’ Then what are you going to do with that?”
Grace Lee, professor of pediatrics at Stanford University School of Medicine and ACIP chair, insisted the group will conduct the review the way it always does — in sessions that are open to the public.
“When we can review that data, when we can review it with the public, when we can deliberate on the decisions, we’ll provide our best scientific advice and opinions,” she told STAT. “We will give our best possible advice based on what we see.”
Jesse Goodman, a former chief scientist at the FDA who is now a professor of medicine at Georgetown University, was concerned about the administration’s decision to announce the plan before that process has played out. Goodman believes booster shots will eventually be needed, and that it is prudent to plan for that eventuality. But “I’m not sure we had enough [data] to pull the trigger right now,” he said.
“In the heat of the moment and when we’re all dealing with a real public health emergency, it becomes almost doubly important that we continually reassess and have the normal processes in place. And I do worry that we not put the cart before the horse,” said Goodman.” (K)
“In Atlanta, CDC scientists are scrambling to comprehend what havoc the variant strains are wreaking, how well the vaccines are working, and what forecast ought to end up on the president’s desk. For several months, the CDC has refereed a sort of artificial intelligence conference of modelers and forecasters from all over the United States, mostly academics. With a forecast so grim, it’s time for a Hail Mary move. And that would be the mass third-dose vaccination of every American over 60 years of age, coupled with a return to mask-wearing and social distancing and a massive escalation in genomic surveillance nationwide. “Some predicted that boosters could reduce serious infections among immunocompromised people or others at high risk for complications. But to make a real dent in reducing transmission — the key to ending the pandemic — those who haven’t gotten vaccinated at all will have to get their first doses, Singer said.
“The boosters probably have a limited impact on reducing the worst impacts, because it’s really unvaccinated people who are making up the supermajority of hospitalizations, ICU admissions and deaths,” Singer said. “If you have less infection, including breakthrough infections, you might break that train of transmission, but if you’re looking now, most of the transmission is occurring among people who have had zero vaccines.”” (M)
“Dr. Melanie Swift, who has been leading the vaccination program at Mayo Clinic in Rochester, Minnesota, says getting more shots into people who haven’t yet been vaccinated at all is “our best tool, not only to prevent hospitalization and mortality from the delta variant, but to stop transmission.” Every infection, she says, “gives the virus more chances to mutate into who knows what the next variant could be.”
“People who took the vaccine the first time are likely to line up and get their booster,” Swift says. “But it’s not going to achieve our goals overall if all their unvaccinated neighbors are not vaccinated.”” (N)

The Biden administration on Wednesday urged adult Americans who received two doses of messenger RNA vaccines from Pfizer or Moderna to get a third dose eight months later.
Pfizer and partner BioNTech SE Covid-19 vaccine sales are expected to reach $74 billion through next year, excluding Germany and Belgium, 17% more than previous estimates, according to Ronny Gal, a Sanford Bernstein pharmaceuticals analyst.
Meanwhile, the analyst projects Moderna will ring up $35 billion during the span, 25% more sales than previously forecast.
The sums are further evidence the coronavirus pandemic is proving to be a significant moneymaking opportunity for vaccines, a business many drugmakers had abandoned because it was seen as requiring heavy investment while offering limited growth and carrying legal risks.” (O)

“Since the first inoculations emerged about 1,000 years ago, vaccines have been saving millions of lives. For many years, we haven’t thought twice about them or the benefits they give us. As infants, we receive vaccines against polio; tetanus, diphtheria and pertussis; measles, mumps and rubella; hepatitis A and B; and more diseases that — as a result — don’t trouble us, or cause illness or death. The smallpox vaccine was used so effectively that the disease was eradicated…
And yet, the COVID-19 vaccines that are the culmination of years of research that we were able to apply to rapidly meet this global public health emergency, are greeted with skepticism. It’s become clear that fear of booster side-effects may also be emerging. However, research by Johns Hopkins and vaccine scientists at Moderna is indicating that booster-dose side effects will not be more severe than those caused by the original vaccine
The many 100-year history of positive benefits conferred by vaccines should at last signal to Americans that it is time to not only accept but also to normalize the COVID-19 vaccines. As COVID-19 mutates and new, more contagious variants emerge, the virus itself is showing us that we can’t be complacent, and the administration will be making the right decision when it calls for boosters. We need to do everything we can to fight COVID-19, funding and putting into place programs that develop and make the latest boosters widely available. We will need them.” (Q)

“Following the White House administration’s announcement to start booster COVID-19 vaccinations for American adults next month, experts weighed in on the evidence for choosing an 8-month cut-off, how breakthrough infections figure in, and why calling one mRNA vaccine better than the other could be misleading.
Timing came up more than once at the August 18 White House briefing announcing the booster plans. Reporters asked about the start time of September 20 and people waiting at least 8 months after their second mRNA vaccine dose to get a booster.
“Clinicians who administer COVID-19 vaccines off-label to children under 12 years would be violating their provider agreement, risking liability for adverse events and potentially forfeiting payment.
Health officials laid out the possible consequences after the Food and Drug Administration (FDA) granted licensure to the Pfizer-BioNTech vaccine Comirnaty on Monday.
“Ordinarily … when a medical product is approved, physicians often do use off-label prescribing. However, this is a different situation,” Acting FDA Commissioner Janet Woodcock, M.D., said at a press conference. “The vaccine is being distributed under a provider agreement by the U.S. government through the CDC (Centers for Disease Control and Prevention), and there are many considerations that would pertain to off-label prescribing for the recipient and so forth.”
Vaccine providers must adhere to requirements of the CDC, its vaccine committee and the FDA. The FDA licensed the vaccine as a two-dose series for people ages 16 years and older. It also can be used for adolescents as young as 12 years under emergency use authorization (EUA). Some people ages 12 years and older who are immunocompromised are eligible for a third dose.
If vaccine providers use the vaccine off-label, the CDC said they could lose immunity from claims, eligibility for federal compensation after an adverse event, eligibility to provide COVID-19 vaccines and payment.
The AAP also is discouraging off-label use of the vaccine as clinical trials are still underway in children under 12 years.
“We do not want individual physicians to be calculating doses and dosing schedules one-by-one for younger children based on the experience with the vaccine in older patients,” Yvonne A. Maldonado, M.D., FAAP, chair of the AAP Committee on Infectious Diseases, said in a statement. “We should do this based on all of the evidence for each age group, and for that we need the trials to be completed. I know parents are anxious to protect their children, but we want to make sure children have the full benefit of ongoing clinical trials.”” (T)

Across the country, the shortages are complicating efforts to treat hospitalized coronavirus patients, leading to longer emergency room waiting times and rushed or inadequate care as health workers struggle to treat patients who often require exacting, round-the-clock attention, according to interviews with hospital executives, state health officials and medical workers who have spent the past 17 months in the trenches.
The staffing shortages have a hospital-wide domino effect. When hospitals lack nurses to treat those who need less intensive care, emergency rooms and I.C.U.s are unable to move out patients, creating a traffic jam that limits their ability to admit new ones. One in five I.C.U.s are at least 95 percent capacity, according to an analysis by The New York Times, a level experts say makes it difficult to maintain standards of care for the very sick.
“When hospitals are understaffed, people die,” said Patrica Pittman, director of the Health Workforce Research Center at George Washington University.” (A)
“With hospitals in the U.S. again running out of ICU bed space, administrators are dealing with a shortage of nurses, scrambling to fill shifts and offering high wages to those who can help. The problem comes as the crushing workload of the coronavirus pandemic, coupled with external and on-the-job pressures, have led some nurses to simply step down altogether….
Last year, when the first wave of COVID-19 patients threatened U.S. hospitals, some sounded the alarm about a looming staffing shortage.
“Recent analyses have raised concerns about whether the United States has enough hospital capacity for a surge of patients needing care for COVID-19 infections,” reads a March 2020 blog post authored by the University of California-San Francisco’s Joanne Spetz and published by Health Affairs. “But even if we can double or triple the number of intensive care unit beds, we don’t have enough nurses to staff them.”…
During earlier waves, hospitals also sent some nurses into COVID-19 wards to help out even if they were trained for other specialties. But Stimpfel says asking a neonatal nurse to work in the ICU with a COVID-19 patient – a high-risk environment involving possible exposure to a potentially deadly contagion – was a big ask.
“Not all nurses are trained in critical care,” she says. “There are different specialties that nurses are prepared for, and you can’t prepare a nurse to take care of these intensely ill patients, and do that overnight. That was still such a leap for them to be put into that sort of situation.”
Meanwhile, colleges and nursing schools are urged to expand capacity to graduate more caregivers, with experts saying the more trained hands working in a COVID-19 ward, the better. In a recent speech to graduates, McCauley, the Emory nursing school dean, says she compared the class to soldiers on the front lines of a battle: “I said, ‘In ways, you remind me of my colleagues when I graduated: nurses that were serving in the Vietnam War.”
The new nurses “are walking into a workforce where everyone is exhausted,” McCauley says. “These new graduates can hold up that workforce just with their youth, their enthusiasm. They’re willing to go the extra mile.”
Still, McCauley acknowledges that in battling a virus that’s mutated to become more contagious, youthful enthusiasm isn’t a substitute for the “wisdom and professionalism” that comes with experience. And, she says, treating COVID-19 patients who got sick because they chose not to get vaccinated – and may have spread the virus to other people – can be disheartening…
But it could take years before staffing levels get to a better place.
“There are some experts that could probably model it better than I can, but I would say it’s going to be at least six to 10 more years before we get over this,” Stimpfel says. “And that’s because of factors that were already in play before COVID.” (B)
“The rapidly escalating surge in COVID-19 infections across the U.S. has caused a shortage of nurses and other front-line staff in virus hot spots that can no longer keep up with the flood of unvaccinated patients and are losing workers to burnout and lucrative out-of-state temporary gigs.
Florida, Louisiana, Arkansas and Oregon all have more people hospitalized with COVID-19 than at any other point in the pandemic, and nursing staffs are badly strained.
In Florida, virus cases have filled so many hospital beds that ambulance services and fire departments are straining to respond to emergencies. Some patients wait inside ambulances for up to an hour before hospitals in St. Petersburg, Florida, can admit them — a process that usually takes about 15 minutes, Pinellas County Administrator Barry Burton said.
One person who suffered a heart attack was bounced from six hospitals before finding an emergency room in New Orleans that could take him in, said Joe Kanter, Louisiana’s chief public health officer.
“It’s a real dire situation,” Kanter said. “There’s just not enough qualified staff in the state right now to care for all these patients.”..
Miami’s Jackson Memorial Health System, Florida’s largest medical provider, has been losing nurses to staffing agencies, other hospitals and pandemic burnout, Executive Vice President Julie Staub said. The hospital’s CEO says nurses are being lured away to jobs in other states at double and triple the salary.
Staub said system hospitals have started paying retention bonuses to nurses who agree to stay for a set period. To cover shortages, nurses who agree to work extra are getting the typical time-and-a-half for overtime plus $500 per additional 12-hour shift. Even with that, the hospital sometimes still has to turn to agencies to fill openings.
“You are seeing folks chase the dollars,” Staub said. “If they have the flexibility to pick up and go somewhere else and live for a week, months, whatever and make more money, it is a very enticing thing to do. I think every health care system is facing that.” (C)
“Under typical circumstances, an I.C.U. nurse might care for two patients.
But Covid patients can require more attention and tend to stay in the I.C.U. longer — a median of seven days instead of about four.
The virus’s rampage through the body can take unexpected turns, throwing a relatively stable patient into an urgent crisis with little warning. This and other complications sometimes lead I.C.U.s to dedicate individual nurses to certain Covid patients.
To help, hospitals can draw staff members from other parts of the hospital, who may lack specialized skills, or bring in short-term travel staffers. Where that’s not possible, or not enough, it can affect the number of patients cared for by each nurse.
“We’ve been swamped,” said Judy Carver, an intensive care unit nurse at Martin Luther King, Jr. Community Hospital in Los Angeles. “We were having to take three patients. It was really heavy, super heavy.”
In California, Gov. Gavin Newsom temporarily altered the rules to allow one I.C.U. nurse to care for three patients instead of the previous maximum of two. In some hospitals, the ratios have gone even higher.
Covid patients are often rolled onto their stomachs, called “proning,” which has been shown to increase oxygen flow. Turning someone over carefully can take several people, and some hospitals have created “proning teams.”
Once patients are face-down, even simple tasks such as bathing them become more challenging — and require more time than nurses might need for a non-proned patient.
The prone position can help patients breathe, but it requires more staff attention.
Proned patients must be watched carefully and moved regularly, so sores do not develop on their faces.
Many Covid patients are on ventilators, which need to be finely adjusted; some are on continuous dialysis machines; and all must be watched for blood clots, which present a greater risk with Covid patients.”” (D)

A 2020 study on sepsis, published in the American Journal of Infection Control, found that each “additional patient per nurse is associated with 12% higher odds of in-hospital mortality, 7% higher odds of 60-day mortality, 7% higher odds of 60-day readmission.” Hospital-acquired infections kill 98,000 Americans annually; bed sores in particular pose a threat, as they can lead to sepsis unless they are prevented and cared for with proper nursing.” (F)
“Emergency medical technicians (EMTs) and certified paramedics can now care for patients in Mississippi hospitals and emergency rooms under a new health office order issued by the Mississippi State Department of Health on Wednesday.
In an effort to assist with staffing shortages in the state’s hospitals, EMTs, certified paramedics and advanced emergency medical technicians can help care for patients, acting under medical direction, while not on duty with a licensed Emergency Medical Services agency. Jim Craig, Senior Deputy and Director of Health Protection for the Mississippi State Department of Health, announced the new orders during a press conference.
“That’s going to be a real benefit in the requests that we receive from a lot of hospitals, to allow EMTs and medics to extend some of the services into the hospital setting,” Craig said. “It is another tool in the toolbox to help us try to make health care resources available in the state of Mississippi.”” (G)
“Regulatory authority is among steps the state has taken to help address staffing needs, Governor Bill Lee signing Executive Order 83 last week. The order extends previous orders which allows for trained National Guard personnel to assist in tending to the sick, testing, logistics, and other areas where help may be needed. The order also allows for out-of-state licensed professionals to assist, a move the governor’s office also made during last year’s peak. One lever they don’t anticipate needing to pull is for alternate care sites which had previously been built in Memphis and Nashville but have since been deconstructed.” (H)
“Georgia’s health agency will more than double the number of temporary hospital staff to help cope with the current surge in COVID-19 patients, Gov. Brian Kemp announced Monday.
The Department of Community Health (DCH) will commit $125 million in addition to $500 million the state already is spending to increase state-supported hospital staff at 68 hospitals across Georgia from 1,300 to 2,800, Kemp said.
Specifically, 170 of the new staff will go to rural hospitals, the governor told reporters during a news conference at the state Capitol.
Commissioner of Community Health Caylee Noggle and her team have also identified 450 beds in nine regional coordinating hospitals that will be soon be available for the new staff being deployed to treat patients.
Kemp said his decision to increase hospital staff was based on input during the last week from hospital CEOs.
“Virtually every hospital’s most pressing issue was a lack of qualified staff to treat the patients coming thru their doors, nurses, respiratory therapists, ICU personnel, just to name a few,” he said.
The additional hospital staff will be provided through a continuation of the state’s no-bid contract with a private Alpharetta-based staffing firm, Jackson Healthcare, and a subsidiary. Georgia Health News reported recently that the contract, which began with the first wave of COVID-19 in Georgia last year, had brought Jackson $434 million as of July 23.” (I)
“Missouri will spend $15 million to provide temporary staffing for hospitals operating under the stress of treating thousands of COVID-19 patients, Gov. Mike Parson said Wednesday.
Parson also announced a $15 million program to establish five to eight monoclonal antibody infusion stations to operate for 30 days throughout the state. Monoclonal antibodies are proteins to help fight off COVID-19 and reduce the risk of severe disease and hospitalization. It’s typically administered to high-risk patients after diagnosis. The state estimates 2,000 patients will be treated by the project.
Both programs will be funded through the state’s portion of the federal Coronavirus Aid, Relief and Economic Security (CARES) Act.
“We know our hospital capacity is limited due to staffing shortages, not a lack of bed capacity,” Parson said during a Wednesday news conference. “Every critical- and acute-care licensed hospital will have access to this program. Staffing allocations depend on the number of licensed beds per hospital. It is our hope this program will decrease hospital capacity strain caused by staffing shortages and decrease the need for future alternative care sites.”…
Robert Knodell, the acting director of the department of health and senior services, said medical staff from outside Missouri will be brought in by a vendor once a contract is finished.
“The state has entered into discussions with potential contractors across the country that have traveling medical professionals that move around the country into surge situations or severe events, whether it’s an infectious disease, hurricane or other extreme event,” Knodell said. “We have been assured those individuals will be available if contracted by the state and our local health care partners.”
Parson said ongoing communication with health care systems in Springfield and around the state will help meet medical needs caused by the pandemic.
“I think all of the resources we have put in there – infusion centers, ambulance resources, respiratory guidelines that were waved to get people in to help – from the state level, it still comes down to what they need,” Parson said. “They’re going to have to work with these companies because they won’t just send 200 people. Hospitals will have to say what they need and it will be determined whether that’s available. But I think we can distribute this around the state of Missouri. There’s going to be an opportunity for hospitals in both rural or urban to take advantage of this program.”” (J)

Johnson said the health system now has close to 100 COVID-19 patients. To address the surge in patients, Johnson said they have closed down elective surgery and are deploying available nurses from surgical services to the emergency department.
Through the Federal Emergency Management Agency, Johnson said the health system “ordered” 140 nurses and respiratory therapists. She said they’ve also requested about 60 from a mainland agency.
But Ross said relying on travel nurses, who do not require benefits, is part of the problem. Usually, there is a contractual agreement that says Queen’s cannot have more than 30 travel nurses at a time — COVID-19 changed that.
Hawaiʻi Pacific Health said it too has made arrangements for additional staff and travel nurses for its hospitals, and is working with the Healthcare Association for additional staffing aid through FEMA.” (K)

When I was appointed President and CEO of Jersey City Medical Center in1989 in the peak of the HIV/ AIDS crisis, we had a dedicated, and always full, dedicated 60 bed AIDS unit, staffed by one full time nurse and numerous part-time and per diem nurses. Today, right now, hospitals caught in the Covid-19 surge, are forced to redeploy nursing staff, respiratory therapists and other clinicians, most without prior infectious diseases experience. Why are we tolerating the unvaccinated putting our hospital staffs at risk?
