With coronavirus, the hospital to which you are admitted is most likely the hospital where you will stay.
We don’t have “emerging virus” hospital designations like we do for trauma centers, cardiac surgery and newborn nurseries.
As hospitals exceed their surge capacities, your ambulance could be diverted from one hospital to another. Perhaps from a regional center to a community hospital.
While hospitals are adding ICU beds, they might be staffed by redeployed nurses rather than ICU nurses, or “travel” nurses.
ICU nurses might each be taking care of more patients then “best practices” standards of care.
The ICU might not have a fulltime physician intensivist.
While there appears to be a sufficient number of ventilators, there is a shortage of respiratory therapists and infection protectionists (formerly infection control nurses).
While there are new therapeutics, the hospital’s supply chain might determine its availability.
The hospital might be part of a tertiary care network or affiliated with an academic medical center, where innovation moves quickly. Or it might not be.
The hospital might not be connected to a Coronavirus Recovery Center.
You might be transferred to a coronavirus field hospital.
You could be admitted to a children’s hospital. Can staff pivot to treating adults?
Some hospitals have stopped elective surgery, some have not. Does that make a difference?
Clinical staff might be working too many hours due to a full house and/or short staffing due to symptomatic coronavirus staff, or staff in quarantine due to community spread. Or internal hospital staff spread, for example in break rooms.
You might be treated by an asymptomatic tested-positive nursing staff “ordered” back to work in dedicated coronavirus unit.
There is an ongoing surge in hospitals of candida auris, a fungal infection highly resistant to antibiotics.
Since you don’t necessarily pick the hospital to which you are admitted or diverted, you may get cared for and billed by out-of-network physicians. If you are transferred by air ambulance due to lack of capacity, not for clinical reasons, that expense might not be covered by your insurance.
Bottom line is that where a coronavirus patient gets treated should not be a random event. Hospitals need to be designated for different levels of care, and patients, where necessary and appropriate, should be transferred to a hospital with the best capability for a successful outcome.
I am not a clinician! Clinicians should develop a continuum of hospital capabilities so patients are treated where they are most likely to recover. And health departments should develop capacity metrics to make sure hospitals do not overreach.
I am not a logistics expert! Just a retired hospital CEO tracking the pandemic with the expectation that others will jump in and develop a hierarchy of coronavirus hospitals.
So every hospital and system in Hudson County and New Jersey needs to monitor ever-changing “best practices,” identify its gaps and take steps to be up-to-date to maximize successful clinical results.
Gov. Murphy and Commissioner of Health Persichilli, national leaders in evidenced-based Coronavirus Rapid Response, should consider developing and sharing hospital coronavirus “report cards.”

“Experts who have spent the last year forecasting Covid-19 transmission across the US are now considering scenarios for the spread of new, more infectious strains of the coronavirus.
At the same time, the US continues to lag in surveillance for coronavirus variants, despite having among the most well-developed genomic sequencing infrastructure in the world.
The warnings come as the US appears to have crested a devastating winter wave of infections, which at one time saw more than 300,000 new infections and 4,000 deaths a day. Even though daily infections have more than halved from the peak, with death rates expected to drop soon also, the threat of the more infectious variants has some considering the possibility of a fresh surge.
“It’s a grim projection, unfortunately,” said Ali Mokdad, professor at the Institute for Health Metrics and Evaluation (IHME) at the University of Washington, one of the leading academic forecasters of Covid-19. “I’m concerned about a spike due to the new variant and the relaxation of social distancing,” he said. “People are tired. People are very tired.”
Forecasters still do not consider this the most likely scenario, nor do they see it as the worst-case scenario. But the addition of the model is a recognition of how dangerous new variants can be, even in an environment where hospitalization and death rates are expected to decline.” (A)
“Since May 2020, Jeffrey Milbrandt has had his systems fine-tuned to sequence 1000 coronavirus samples a week. The director of a major sequencing center at Washington University in St. Louis (WashU), Milbrandt knew months ago that the United States urgently needed to identify and track emerging variants of SARS-CoV-2, the pandemic coronavirus already spreading across the nation.
But to date, fewer than 100 coronavirus samples have made it to his sequencers at the McDonnell Genome Institute, and the United States remains nearly blind to several coronavirus strains that have recently upended the course of the pandemic. “We have it all worked out but there’s not a lot of takers,” Milbrandt says of his center’s sequencing abilities. “We are getting more inquiries from the press than from people who need the information. … Some of us have pipelines available—they are just not being utilized.”
The Centers for Disease Control and Prevention (CDC) plans to rapidly change that by coordinating a national surveillance system for coronavirus variants. Scientists and public health experts welcome the plan.
“The hardware is set up, the people are trained, the infrastructure is in place,” says genome scientist Chris Mason of Weill Cornell Medicine. “It’s just a big question: Who’s gonna pay for it?”…
CDC has also begun to require that state and local public health labs submit, collectively, 750 sequences each week, Walensky noted in late January. But such labs have been both underfunded and swamped during the pandemic. At the Utah Public Health Laboratory—one of the most sophisticated and best equipped of the state labs—a shortage of pipette tips drove sequencing rates down from 600 per week in mid-January to 100 last week, says Kelly Oakeson, the lab’s chief sequencing scientist.
Other scientists say CDC’s goal is too low. “Seven hundred and fifty surveillance strains a week is nothing,” says Lea Starita, a genome scientist at the University of Washington, Seattle, and the Seattle Flu Study. Combined with the 6000 viral genomes CDC expects to receive weekly from the companies, that would allow sequencing of just 0.9% of the current weekly average of nearly 800,000 COVID-19 cases.” (B)
“The coronavirus variant first detected in the United Kingdom is rapidly becoming the dominant strain in several countries and is doubling every 10 days in the United States, according to new data…
The researchers also found that the transmission rate in the United States of the variant, labeled B.1.1.7, is 30% to 40% higher than that of more common lineages.
The findings lend credence to modelling predictions the CDC released in January. The agency said at the time that the new strain could cause more than half of new infections in this country by March, even as the U.S. races to deploy vaccines.
In the new study, while clinical outcomes initially were thought to be similar to those of other coronavirus variants, early reports suggest that infection with the B.1.1.7 variant may increase death risk by about 30%.
A coauthor of the current work, Kristian Andersen, told The New York Times, “Nothing in this paper is surprising, but people need to see it.”
Andersen, a virologist at the Scripps Research Institute in La Jolla, CA, said, “We should probably prepare for this being the predominant lineage in most places in the United States by March.”
“Our study shows that the US is on a similar trajectory as other countries where B.1.1.7 rapidly became the dominant SARS-CoV-2 variant, requiring immediate and decisive action to minimize COVID-19 morbidity and mortality,” the researchers write.
The authors point out that the B.1.1.7 variant became the dominant SARS-CoV-2 strain in the United Kingdom within a couple of months of its detection.
“Since then, the variant has been increasingly observed across many European countries, including Portugal and Ireland, which, like the UK, observed devastating waves of COVID-19 after B.1.1.7 became dominant,” the authors write.
The B.1.1.7 variant has likely been spreading between US states since at least December 2020, they write…
“Because laboratories in the US are only sequencing a small subset of SARS-CoV-2 samples, the true sequence diversity of SARS-CoV-2 in this country is still unknown,” they note.
Michael Osterholm, director of the Center for Infectious Disease Research and Policy at the University of Minnesota, said the U.S. is facing a “Category 5” storm with the spread of the B.1.1.7 variant as well as the variants first identified in South Africa and Brazil.
“We are going to see something like we have not seen yet in this country,” Osterholm said recently on NBC’s Meet the Press.” (C)
“TWO new COVID-19 variants, one of which has been classified as a “concern”, have been identified in England with some similarities to the South African and Brazilian variants, a government advisory scientific committee said on Tuesday.
One of the new variants, first identified in Bristol, has been designated a “Variant of Concern”, by the New and Emerging Respiratory Virus Threats Advisory Group.
The other, first identified in Liverpool, has been designated as a “Variant under Investigation” by the group.” (D)
“South Africa has put its rollout of the Oxford-AstraZeneca vaccine on hold after a study showed “disappointing” results against its new Covid variant.
Scientists say the variant accounts for 90% of new Covid cases in South Africa.
The trial, involving some 2,000 people, found that the vaccine offered “minimal protection” against mild and moderate cases.
But experts are hopeful that the vaccine will still be effective at preventing severe cases.
South Africa has recorded almost 1.5 million coronavirus cases and more than 46,000 deaths since the pandemic began – a higher toll than any other country on the continent.
The country has received one million doses of the Oxford-AstraZeneca jab and was preparing to start vaccinating people.
Dr Katherine O’Brien, the WHO’s director of immunisation, said it was very plausible that the Oxford-AstraZeneca vaccine would still have a meaningful impact on the South African variant, especially when it came to preventing hospitalisations and death.” (E)
“The coronavirus variant that emerged from the United Kingdom is spreading quicker in Florida than in any other state.
Florida currently has 201 recorded cases Monday, with that number expected to rise, so we looked at how that moves our herd immunity goal posts.
“Cases have continued to decline over the last four weeks,” said Dr. Rochelle Walensky publicly, the new director of the CDC under the Biden administration. “New COVID-19 hospitalizations continue to decline.”
“Florida’s growing pretty significantly right now,” said Dr. Scott Gotlieb during an interview on Face The Nation, the former FDA commissioner. “Between [5%] and 10% of the infections in Florida, B.1.1.7, that UK variant, the more contagious variant and that’s centered in Southern Florida.”
Florida has more cases of the UK variant than anywhere else in the United States at this time, and research finds these cases can double in about a week, spilling out beyond our borders sooner rather than later.
“The projection is that the 117 lineage will likely become dominant in the United States by the end of March,” Dr. Anthony Fauci said.
That’s only in reference to the UK variant.
Epidemiologist Dr. Stanley Weiss, a professor at Rutgers School of Public Health, says there’s already community spread of the South African and the Brazilian variants.
“Unless you have virtually everyone in the population get the vaccine or have gotten infected already, you’re not going to get to herd immunity,” Weiss told WINK News. “A survey last week said that still 24% of us adults didn’t want to get the vaccine. Well, if they don’t, we can’t reach herd immunity. We might have been close before we had this UK variant.” (F)
“A lab study has found the vaccine effective against UK and South Africa variants. German Health Minister Jens Spahn has voiced his confidence in the EU’s three approved vaccines.
The peer review has confirmed that the BioNTech-Pfizer vaccine is effective against two variants of the coronavirus, the companies announced on Monday.
The review, which was published in the journal Nature Medicine, backed the results of a study completed by Pfizer and the University of Texas in late January.
When the study was originally released, BioNTech and Pfizer said its finding suggested that no new vaccine would be necessary to fight coronavirus mutations first discovered in the United Kingdom and South Africa.
Nevertheless, the continuous transformation of the deadly virus makes clinical data and constant observation imperative. Experts say that it is not a foregone conclusion that vaccines currently in use will remain effective against possible new variants of the virus in the future.” (G)
“Researchers in the United States have conducted a study showing that the Moderna mRNA-1273 vaccine effectively protects against recently emerged variants of severe acute respiratory syndrome coronavirus 2 – the agent that causes coronavirus disease 2019 (COVID-19).
The team compared the neutralization activity of blood taken from infected and vaccinated individuals against a panel of SARS-CoV-2 variants, including the B.1.1.7 that emerged in the UK.
The team – from Emory University School of Medicine in Atlanta, the University of Texas Medical Branch, and the COVID-19 Neutralization Study Group – found that antibodies generated by both infected and vaccinated individuals effectively neutralized all of the variants.
“These findings support the notion that in the context of the UK variant, vaccine-induced immunity can provide protection against COVID-19,” says Mehul Suthar and colleagues.
However, as further variants continue to emerge, it will be essential to assess their impact on the potency of neutralizing responses following both infection and vaccination, warns the team.” (H)

In Iowa, Gov. Kim Reynolds, a Republican, lifted the state’s partial mask mandate on Friday. She also said businesses no longer have to limit the number of customers or enforce social distancing, and no limits remain on the size of public gatherings. The Des Moines Register’s editorial board called the move “inexplicable and irresponsible.”
Reynolds isn’t acting alone. Democratic and Republican governors alike have been loosening restrictions. In New York, Gov. Andrew Cuomo, a Democrat, announced Monday that New York City will allow indoor dining beginning on Friday, a few days earlier than originally planned, and middle schools will reopen for in-person instruction on Feb 25. In Massachusetts, Gov. Charlie Baker, a Republican, said restaurants are allowed to operate at 40 percent capacity as of Monday, citing a decline in hospitalizations as part of his reasoning…
“I have some concern it’s premature” to loosen restrictions, said Dr. Justin Lessler, an infectious disease epidemiologist at the Johns Hopkins Bloomberg School of Public Health.
Lessler said the very restrictions in place are what is allowing Covid-19 infection rates to decline.

“Dr. Michael Osterholm, a University of Minnesota infectious disease expert, warned Minnesota lawmakers Wednesday about the possibility of another ‘major surge’ in COVID-19 cases, as new variants of the virus start to spread in the United States….
He said there is growing concern about the U.K. strain that is now circulating in the U.S….
“I think we have a real possibility of seeing a major surge of cases this fall into early winter here in Minnesota and throughout the United States with this B117 strain, which if we do, I am convinced it’ll be much more severe than anything we’ve seen to date,” Dr. Osterholm said.
Dr. Osterholm told lawmakers it may be time to rethink our vaccine strategy and go with a ‘first-dose only’ approach, so more people over the age of 65 can have at least some protection right away. He said that would require delaying second doses until this summer.
He said the idea is being looked at ‘on the national level.’ He believes the strategy would help lower the baseline of infections in the U.S.
“I can tell you, we are going to have another surge and when it happens, the lower we start from, the better we’re going to be,” Dr. Osterholm said.” (A)
“However, White House chief COVID adviser Anthony Fauci, MD, just warned against this practice, and cautioned people about “the danger” that could come with focusing only on the first dose.
On Feb. 2, Fauci warned against administering just the first dose of the COVID vaccine in an effort to immunize as many people as possible without concern for when the second dose will be available. According to Fauci, there are risks involved with using this tactic. “The danger is that the efficacy following a single dose is not as great as after the second dose,” Fauci explained during a live Twitter event with The Washington Post. “If you have sub-optimum efficacy, you could, in fact, paradoxically be selecting for more mutations. That’s the danger there. … It’s taking a chance.”
While Fauci didn’t elaborate on that last point, some experts have expressed caution that delaying second doses of the vaccine—and therefore delaying the vaccine’s maximum potential protection—could lead to an “escape mutant” that could survive and evade shots when they are finally administered, Business Insider points out.
In The Washington Post interview, Fauci reminded viewers that the science behind the clinical trials showed that the optimal approach is receiving your second Pfizer dose 21 days after your first and your second Moderna dose 28 days after your first. He cautioned against straying from this recommendation seeing as the Pfizer vaccine is 52 percent effective after the first dose and the Moderna vaccine is 80.2 percent effective after just one shot.”(B)
“A (British) government vaccine adviser has defended plans to delay the second dose of the Pfizer coronavirus vaccine from three weeks to 12 weeks after the first jab, as the United States’ top doctor said he disagreed with the approach…
Following the approval of a second vaccine developed by Oxford/AstraZeneca last week, the Department of Health and Social Care said it would prioritise giving the first jab to “as many people in at-risk groups”.
“Everyone will still receive their second dose and this will be within 12 weeks of their first,” it added in a statement. “The second dose completes this course and is important for longer term protection.”…
The British Medical Association also claimed it was “grossly unfair” that patients who have already received their fist dose of the Pfizer/BioNtech vaccine were having appointments cancelled due to the change in guidance.
But speaking to BBC Radio 4’s Today programme, professor Anthony Harnden, the deputy chairman of the Joint Committee on Vaccination and Immunisation (JCVI), stressed that delaying the second dose was the correct approach.
He said that patients he had dealt with accepted the move, adding: “When it was explained to them that the vaccine offers 90 per cent protection for one dose, and the priority was to get as many people vaccinated in the elderly and vulnerable community as possible, they understood.
“I think the country is all in this together. And I think we really, really want to pull together to try to do the best strategy possible”.
He insisted: “It’s clear from looking at the data the Pfizer/BioNtech vaccine after one dose after 14 days is 90 per cent.”
Pressed on Dr Fauci’s comments, he replied: “That’s up to the Americans what they do… we’re not saying that you shouldn’t have a second dose, you do need a second dose, but that it can be temporarily delayed so that we can get many, many more people vaccinated in this vulnerable and elderly group as well as the clinically extremely vulnerable groups so that we get on top of this virus much quicker.
“We are in a dire situation in this country at the moment. The virus is rapidly spreading and the more vaccine we can get into these priority groups that we’ve identified the more deaths and hospitalisations that we will prevent.” (C)
“It is still recommended for people to get their second dose of Covid-19 vaccine on time, Dr. Rochelle Walensky, director of the US Centers for Disease Control and Prevention, said during a virtual White House briefing on Monday.
“Until we have further data,” Walensky said, people should continue to follow the data from trials by continuing the schedule of receiving two doses 21 days apart for the Pfizer/BioNTech vaccine and 28 days apart for the Moderna vaccine. Those are the two vaccines currently authorized for emergency use in the United States…
Delaying second doses would go against the vaccine schedules that have been authorized under the US Food and Drug Administration.
In January, the FDA made clear that it does not plan to change coronavirus vaccine dosing schedules — and for such adjustments to occur, the vaccine manufacturer would have to specifically ask the agency to adjust authorization.
CNN confirmed with the FDA in January that if a manufacturer requests a change to its emergency use authorization, the manufacturer would need to submit data to the FDA supporting the requested change…
“At this point, everything we know about vaccines suggests that actually the protection may be enhanced by giving a booster several months after the first dose,” said Osterholm, who was a coronavirus adviser to the Biden transition team.
Osterholm pointed to a letter-to-the-editor that published in the journal Clinical Infectious Diseases last week, in which Dr. Stanley Plotkin of the University of Pennsylvania and Dr. Neal Halsey of Johns Hopkins University calculated about how many people could be protected against Covid-19 if single doses are given.
The researchers hypothesized, “suppose that 1 million people are to be vaccinated but only 1 million doses are available. If two doses are given to each vaccinee and the efficacy is 95%, 475,000 people will be protected. If single doses are given and the efficacy is 80%, 800,000 people will be protected.”
Osterholm said that getting people vaccinated — even with just one dose for now — remains urgent as more coronavirus variants circulate, including the B.1.1.7 variant first identified in the United Kingdom, B.1.351 first identified in South Africa, P.1 variant first identified in Brazil and the L452R first seen in California.
“We need to have a really comprehensive review and discussion of this as quickly as possible because you know this new variant B.1.1.7 could cause a surge in our cases within weeks,” Osterholm said. He described the emergence of these variants as similar to a hurricane approaching on the horizon.
“I feel like I’m sitting here on a beautiful beach — the light breeze, perfectly blue skies and everybody — and I’m telling them to start to evacuate. People are saying, ‘Are you crazy?’ But I see that 450 miles south, there’s a category five hurricane,” Osterholm said. “That’s the challenge. How do you get people to take action?”… (D)
“What the one-shot, two-shot debate has to teach us about uncertainty and science
The pandemic has repeatedly challenged some assumptions of science communications, where journalists confidently report what is evidence-based and what has “no evidence.” That vocabulary has shaped the conversation about vaccine dosing; for example, Pfizer has said there is “no data” to demonstrate that the immunity from the first dose lasts more than 21 days.
It’s true that length of immunity from the first dose is not what their clinical trials studied. But scientific evidence isn’t all-or-nothing like that. In many cases, scientists (and historians, and researchers in every field) have deeply limited evidence about the topic of interest. Often they’re in a position of trying to make inferences from the results of other similar events, from a few unusual data points that arose due to accidents, or from measured results that were not the primary aim of the trial they were measured in.
Sources of evidence like these are being employed when researchers try to estimate the likely duration of immunity from a single vaccine dose. The UK’s Working Group that made the delayed dosing recommendation “concluded that vaccine efficacy will be maintained with dosing intervals longer than 21 days … based on clinical trial data that showed the vaccine was 90.5 percent effective against preventing Covid-19 after the first dose once the protection that starts at around 12 days kicks in, and there was no evidence to suggest that the effectiveness of the vaccine is declining toward the end of the 21-day period following the first dose,” a spokesperson said.
From one perspective, that study produced “no data” about effectiveness past 21 days. For another research team, it shows there’s “no evidence” of declining effectiveness…
“It’s been a repeated problem throughout the pandemic that we have relied on this ‘we have the evidence/we don’t have the evidence’ binary, meaning that we have moved slowly, waiting for rock-solid confirmation; but moving faster, making decisions on imperfect information, would have saved a lot of lives,” British science journalist Tom Chivers argued in an article Wednesday about the difference between the US and UK approaches to vaccination.
While randomized controlled trials (RCTs) are incredibly valuable for determining how well vaccines work, decisions about vaccination in a pandemic require many judgment calls that we unfortunately cannot assess in advance with an RCT. And to be clear, concerns like whether the change in vaccination schedules will limit public confidence in the vaccine, increase vaccine hesitancy, or prompt individuals to take risks, believing themselves fully immune when they aren’t, are very legitimate and should no doubt feature in our reasoning and planning about vaccination.
But those concerns have also not been rigorously demonstrated in an RCT. No one has conducted a controlled study on how delaying the second dose will affect vaccine hesitancy. Instead, public health officials who raise vaccine hesitancy concerns are looking at lots of factors and making their best informed prediction about what will happen — and public health officials who estimate that delayed second dosing is worth it are doing the same thing.” (E)
“Pfizer, sounded more cautious on the prospect of shifting the schedule of the second dose of the vaccine, noting it had not been evaluated on different dosing schedules.
“There are no data to demonstrate that protection after the first dose is sustained after 21 days,” it said in a statement, adding the implementation of alternative schedules should be closely monitored.
“While decisions on alternative dosing regimens reside with health authorities, Pfizer believes it is critical… to ensure each recipient is afforded the maximum possible protection, which means immunisation with two doses of the vaccine.”
Britain has said the new strategy still means everyone will receive a second dose within 12 weeks of the vaccine.” (F)
“We think more lives would be saved by providing as soon as possible (a) just one dose of vaccine (b) to all people who face the highest risk of dying from Covid-19, whatever the reason (advanced age, other medical conditions, severe obesity), and (c) just forgetting about any boosters for a while, maybe even a very long while. Second doses should be deferred for the time it takes to achieve this primary goal.
The two vaccines’ manufacturers and the F.D.A. have been reluctant to endorse any change to the vaccination schedules that were tested in Phase 3 clinical trials, on grounds that other options weren’t tested and so their efficacy is unknown. On the face of it, this position seems sensible; yet under current circumstances, it is dangerously overcautious.
The C.D.C. has authorized giving a second dose up to six weeks after the first one if sticking to the recommended interval is “not feasible.” But this is an arbitrary extension — it wasn’t tested any more than any other modification — and we think it’s too short to accomplish much good society-wide.
Bear in mind that the clinical trials performed for both the Pfizer-BioNTech and Moderna vaccines weren’t designed to identify the optimal number of doses for inoculation, nor the perfect schedule for administering them. The goal of those trials was only to test whether two doses of vaccine given according to the schedule chosen by the manufacturers did confer sufficient protection against Covid-19.
Note, too, that in the case of both those vaccines the intervals tested (three and four weeks) were unusually short. In standard schedules for childhood vaccines in the United States, for example, the gap between the first shot and the booster typically is no less than two months. The fact that the trials did not test whether administering a second dose much later, or not at all, might work just as well than the recommended schedule, or well enough, also means that it could work…
We must go ahead right now, even without trials, with a one-dose- approach targeting the most vulnerable…
The Biden administration’s by-the-book approach to Covid-19 vaccination — two shots and delivered, as much as is possible, according to the schedule set by manufacturers — may be the safest for policymakers, but it isn’t the safest for the public. It’s overly prudent, and at this stage of the pandemic, the excess of caution is killing people.
More lives could be saved if we simply forgot about boosters and more trials for a while and rushed right now to give just one shot to all the people who are most immediately at risk of dying.” (G)
“There are two ways to resolve this question: We can debate it on social media or we can go get more data. I believe we should do the latter, and propose that academic nonprofit hospitals in the U.S. immediately start a study of their own employees.
The data this study would generate could clarify whether the seemingly intuitive first-shot approach has downsides when compared to the stricter set-aside strategy by focusing on two questions: How long does immunity from the first shot last? And how well does the booster work when it is delayed (no one has yet proposed just a single shot with no booster)?
Here’s how the study would work: Every employee gets a first shot, as many already have. Then, before the second-shot booster, employees — with their consent, of course — are randomized to a second shot according to the FDA authorized schedule or to a placebo shot of saline. (Because vaccines are being drawn up within hospitals, the placebo could be made in the pharmacy and all other levels of blinding can be preserved). Participants would be tested weekly for the infection and cases of clinical Covid-19 would be recorded.
The primary endpoint would be differential rates of clinical Covid-19 beginning a week after the second shot is administered. Secondary endpoints would include rates of asymptomatic carriers. Hospitals would follow a common protocol, but the data could be housed within each hospital and shared only at the time of analyses.
When rates start to diverge, which would answer the first question, all placebo recipients would receive a booster, producing data with varying time between first shot and booster that could help answer the second question to some extent…
Let me put it another way: Hospitals, for the sake of your workers and for all of us, please get your IRBs going.” (H)
“Though rates vary widely, the American Health Care Association, which represents for-profit nursing homes, estimates that about 50 percent of long-term care staff members have been hesitant to get vaccinated. The majority of direct-care workers in nursing homes are people of color, who have generally been more hesitant to get vaccinated, based in part on their distrust of the federal government and the United States’ history of medical racism.
In Utah, 57 percent of long-term care workers have accepted the first dose, compared to 86 percent of residents, according to the state health department. As of early January, only 40 percent of nursing home workers in Ohio had elected to get vaccinated, according to figures cited by Gov. Mike DeWine…” (A)
“While residents of nursing homes and their caregivers have been considered a top priority for COVID-19 vaccination, only 38% of nursing home staff accepted shots when they were offered, new data from the Centers for Disease Control and Prevention revealed Monday…
“These findings show we have a lot of work to do to increase confidence and also really understand the barriers to vaccination amongst this population,” said Dr. Radhika Gharpure, lead author of the study and a member of the CDC’s Vaccine Task Force.
The report cited previous polling data to suggest why employees have been declining vaccines.
Many raised concerns about vaccine side effects. Others said they didn’t want to be among the first to receive the vaccines, which were first authorized in December. Some said they didn’t trust the government, or referenced false claims about the shots.
It’s also possible, Gharpure said, that some people didn’t get vaccinated because they weren’t working when the shots were distributed, or because they work in multiple facilities and were only counted at one…
The Trump administration initiated the Pharmacy Partnership with drugstore chains CVS and Walgreens, who agreed to pay three visits to every participating nursing home, vaccinating as many people as possible the first two times, and providing the second required dose on the later visits.
The vaccines are provided for free with the pharmacies billing private insurers and Medicaid and Medicare for administration fees.
More staff members are signing up for shots on the second and third visits, suggesting the hesitancy may be waning at least somewhat, Link-Gelles said…
Walgreens said it is learning from the reticence of nursing home workers.
“While vaccine hesitancy has been a challenge at some of these facilities, our pharmacists have played a critical role in providing education and information to help residents and staff understand the important role these vaccines will play in helping the nation emerge from this pandemic,” Walgreens President John Standley said in a statement.
Lack of information about the vaccines may explain some of the hesitancy, the report concluded.
The Trump administration promised for months that it would launch a public information campaign about the vaccines but it never materialized.
A nursing-home specific “toolkit” became available late last year, at about the same time vaccinations became available. The Centers for Medicare & Medicaid Services and private groups are also launching communication initiatives.
Link-Gelles said she understands that the vaccine is new, and hopes more people will take it as they see it working well in others.
“Hesitancy, we’ve seen has been a problem not just in this group but across the country,” she said. “Other data has shown that as people have become more comfortable with the vaccines and … obviously not seeing a lot of very serious adverse events, that people will become more comfortable. This population is hopefully no different.”

“This hesitancy is less outright rejection than cautious skepticism. It’s driven by suspicions about the evidence supporting the new vaccines and about the motives of those endorsing them. The astonishing speed of vaccine development has made science a victim of its own success: after being told that it takes years, if not decades, to develop vaccines, many health-care workers are reluctant to accept one that sprinted from conception to injection in less than eleven months. They simply want to wait—to see longer-term safety data, or at least to find out how their colleagues fare after inoculation.
Another major hurdle is mistrust of both the political and the health-care systems. The problem is most acute in historically marginalized communities, which already live with racial disparities in life expectancy, maternal mortality, access to medical care, representation in clinical trials, informed consent, the physician workforce, and covid-19 outcomes. And it’s exacerbated among health-care workers who are underappreciated and poorly paid. “In many cases, vaccine hesitancy is not a lack-of-information problem. It’s a lack-of-trust problem,” David Grabowski, a professor of health-care policy at Harvard, told me. “Staff doesn’t trust leadership. They have a real skepticism of government. “Elaine Ryan, AARP’s vice president for state advocacy and strategy integration, says low participation stems from subpar working conditions that nursing home staff have endured for years. “These workers are poorly paid, get few benefits or sick leave and receive very little information and support when they need it,” she says. “Can you blame them for not rushing to get a shot they know very little about, from [the nursing homes] who have treated them so badly?”…
Many long-term care workers are hourly employees with little to no sick leave or benefits and may not be able to afford to get sick from side effects. Certified nursing assistants, who make up the overwhelming majority of care workers at nursing homes, make less than $15 per hour on average, and getting to work is often their top priority.
Now, the pressure is on the second round of nursing home vaccination clinics to reach those who’ve been missed. Both COVID-19 vaccines authorized in the U.S., from Pfizer and Moderna, require two doses administered up to six weeks apart. Missing a first dose during the second clinic will make it all but impossible for workers to get fully vaccinated through the on-site program. As Ohio Gov. Mike DeWine said in a December address to his state’s nursing home workers — of whom 60 percent were reportedly declining first doses — “The train may not be coming back.”
Rina Shah, group vice president of pharmacy operations and services at Walgreens, said in a press briefing that the pharmacy is developing tools to get long-term care workers vaccinated once the program ends. One possibility, she said, is supplying vaccine vouchers to workers who can then take them to a Walgreens clinic to receive a shot. But doing so may put those workers in large queues with millions of members of the general population who are struggling to secure vaccinations.
Mike Wasserman, past president of the California Association of Long Term Care Medicine — which represents doctors, nurses, pharmacists and others in the industry — says the current program needs to be more accommodating. “Instead of a one-size-fits-all, rigid approach,” he says, what’s needed is a “more flexible plan that might allow the facility to do its own vaccinations over the course of a week.”
That could also prevent staffing shortages that may arise from inoculating too many workers with second doses at one time. In the Pfizer and Moderna vaccine trials, participants reported feeling worse side effects — mainly fatigue, headache, muscle aches, chills, joint pain and possibly some fever — after their second dose, which could lead to lots of staff calling in sick at once, straining a facility’s ability to look after residents at a time of national staffing shortages.
Anecdotal evidence from second-round clinics now taking place suggests that vaccine uptake among staff is improving. The Associated Press reported that last week, in a meeting on vaccine policy, Amanda Cohn, the CDC’s deputy director of immunization services, said more staffers get vaccinated when a second or third clinic is held at a home. And a CVS spokesperson told USA Today that they are seeing a higher uptake by staff members on their second visits.” (D)

Those opposing forces have spawned an unusual situation: In addition to educating their workers about the benefits of the Covid-19 vaccines, a growing number of employers are dangling incentives like cash, extra time off and even Waffle House gift cards for those who get inoculated, while in at least a few cases saying they will fire those who refuse.
Officials at two large long-term care chains, Juniper Communities and Atria Senior Living, said they were requiring their workers, with limited exceptions, to take the vaccine if they wanted to keep their jobs.
“For us, this was not a tough decision,” said Lynne Katzmann, Juniper’s chief executive. “Our goal is to do everything possible to protect our residents and our team members and their families.”
Critics say it is unethical to strong-arm low-paid workers into taking the vaccines, especially when there hasn’t been enough time to gather long-term safety data.
“This is a population of people who have been historically ignored, abused and mistreated,” said Dr. Mike Wasserman, a geriatrician and former president of the California Association of Long Term Care Medicine. “It is laziness on the part of anyone to force these folks to take a vaccine. I believe that we need to be putting all of our energy into respecting, honoring and valuing the work they do and educating them on the benefits to them and the folks they take care of in getting vaccinated.”” (E)
