This, even though Congress has failed to pass “essential” Coronavirus legislation. So who are our “essential” workers?

“Attorney General Letitia James today released a report on her office’s ongoing investigations into nursing homes’ responses to the COVID-19 pandemic. Since March, Attorney General James has been investigating nursing homes throughout New York state based on allegations of patient neglect and other concerning conduct that may have jeopardized the health and safety of residents and employees….

The report includes preliminary findings based on data obtained in investigations conducted to date, recommendations that are based on those findings, related findings in pre-pandemic investigations of nursing homes, and other available data and analysis. Based on this information and subsequent investigation, OAG is currently conducting investigations into more than 20 nursing homes across the state. OAG found that:

A larger number of nursing home residents died from COVID-19 than DOH data reflected;

Lack of compliance with infection control protocols put residents at increased risk of harm;

Nursing homes that entered the pandemic with low U.S. Centers for Medicaid and Medicare Services (CMS) Staffing ratings had higher COVID-19 fatality rates;

Insufficient personal protective equipment (PPE) for nursing home staff put residents at increased risk of harm;

Insufficient COVID-19 testing for residents and staff in the early stages of the pandemic put residents at increased risk of harm;

The current state reimbursement model for nursing homes gives a financial incentive to owners of for-profit nursing homes to transfer funds to related parties (ultimately increasing their own profit) instead of investing in higher levels of staffing and PPE;

Lack of nursing home compliance with the executive order requiring communication with family members caused avoidable pain and distress; and

Government guidance requiring the admission of COVID-19 patients into nursing homes may have put residents at increased risk of harm in some facilities and may have obscured the data available to assess that risk.

Preliminary data obtained by OAG suggests that many nursing home residents died from COVID-19 in hospitals after being transferred from their nursing homes, which is not reflected in DOH’s published total nursing home death data. Preliminary data also reflects apparent underreporting to DOH by some nursing homes of resident deaths occurring in nursing homes. In fact, the OAG found that nursing home resident deaths appear to be undercounted by DOH by approximately 50 percent…

OAG received numerous complaints that some nursing homes failed to implement proper infection controls to prevent or mitigate the transmission of COVID-19 to vulnerable residents. Among those reports were allegations that several nursing homes around the state failed to plan and take proper infection control measures, including:

Failing to properly isolate residents who tested positive for COVID-19;

Failing to adequately screen or test employees for COVID-19;

Demanding that sick employees continue to work and care for residents or face retaliation or termination;

Failing to train employees in infection control protocols; and

Failing to obtain, fit, and train caregivers with PPE….

There are 619 nursing homes in New York, and 401 of these facilities are for-profit, privately owned, and operated entities. Of the state’s 401 for-profit facilities, more than two-thirds — 280 nursing homes — have the lowest possible CMS Staffing ratings. The Staffing rating reflects the number of staffing hours in the nursing department of a facility relative to the number of residents. As of November 16, 3,487 COVID-19 resident deaths (over half of all deaths) occurred in these 280 facilities. Some of these facilities have also been known to transfer facility funds to owners and investors, rather than use them to invest in additional staffing to care for residents.

Pre-existing, insufficient staffing levels put residents and staff at increased risk of harm during the pandemic. As nursing home resident and staff COVID-19 infections rose during the initial wave of the pandemic, staffing absences increased at many nursing homes. As a result, already-low staffing levels decreased even further, to especially dangerous levels in some homes, even as the need for care increased due to the need to comply with COVID-19 infection control protocols and the loss of assistance from family visitors. OAG’s preliminary investigations reflect many examples where for-profit nursing homes’ pre-pandemic low staffing model simply snapped under the stress of the pandemic….

In addition, preliminary investigations indicate that when there were insufficient staff to care for residents, some nursing homes pressured, knowingly permitted, or incentivized existing employees who were ill or met quarantine criteria to report to work and even work multiple consecutive shifts, in violation of infection control protocols. These policies put both residents and staff at great risk.

Despite these disturbing and potentially unlawful findings, due to recent changes in state law, it remains unclear to what extent facilities or individuals can be held accountable if found to have failed to appropriately protect the residents in their care.” (A)

“More than 9,000 recovering coronavirus patients in New York state were released from hospitals into nursing homes early in the pandemic under a controversial directive that was scrapped amid criticism it accelerated outbreaks, according to new records obtained by The Associated Press.

The new number of 9,056 recovering patients sent to hundreds of nursing homes is more than 40% higher than what the state health department previously released. And it raises new questions as to whether a March 25 directive from Gov. Andrew Cuomo’s administration helped spread sickness and death among residents, a charge the state disputes.

“The lack of transparency and the meting out of bits of important data has undermined our ability to both recognize the scope and severity of what’s going on” and address it, said Richard Mollot, the executive director of the Long Term Care Community Coalition, a residents advocacy group.

The new figures come as the Cuomo administration has been forced in recent weeks to acknowledge i t has been underreporting the overall number of COVID-19 deaths among long-term care residents. It is now nearly 15,000 up from the 8,500 previously disclosed.

The Cuomo administration’s March 25 directive barred nursing homes from refusing people just because they had COVID-19. It was intended to free up space in hospitals swamped in the early days of the pandemic. It came under criticism from advocates for nursing home residents and their relatives, who said it had the potential to spread the virus in a state that at the time already had the nation’s highest nursing home death toll.

In its reply to an AP Freedom of Information request from May, the state health department this week released two figures: a previously disclosed count of 6,327 admissions of patients directly from hospitals and a new count of 2,729 “readmissions” of patients sent back from a hospital to the nursing home where they had lived before.

Before the state released any data, the AP conducted its own survey and found at least 4,500 such patients.

Critics have long argued there were many other places those patients could have been sent, including New York City’s Jacob Javits Convention Center, which had been set up as a makeshift hospital, and the USS Comfort military hospital ship. The state contends those facilities were not suitable substitutes for the care of nursing homes.

Cuomo reversed the directive May 10, barring nursing homes from accepting COVID-19 patients without a negative test first.

State health officials contend that asymptomatic nursing home employees, not recovering COVID-19 patients, were the driving factor in nursing home outbreaks. And they have repeatedly noted that by law, nursing homes weren’t supposed to accept anyone they couldn’t adequately care for.

“At least 98% of nursing home facilities in the state had COVID in their facility before their first admission or readmission, and as we’ve seen across the nation, the major driver of infections appears to be from asymptomatic staff through no fault of their own,” said state Health Commissioner Dr. Howard Zucker in a statement to the AP.” (B)

“When New York Gov. Andrew Cuomo was first accused of undercounting nursing home deaths from Covid-19, his administration offered a simple explanation:

The state did not include nursing home residents who died in the hospital in the publicly posted tally of coronavirus deaths linked to long-term care facilities, officials said, because it wanted to avoid a “double count” of those deaths in the statewide total.

But New York stands apart from other states in taking this approach to counting nursing home deaths, research experts said — a decision that made New York’s tally of nursing home deaths appear lower than it was, and that is now under federal investigation.

“It’s tricky to compare state-level data, but New York is the only state that explicitly stated that they were excluding hospital-based deaths,” said Priya Chidambaram, a senior policy analyst at the Kaiser Family Foundation, a nonprofit research organization…

“They’re using the need for maximum accuracy and the difficulty of reconciling the two data sets as a rationale for postponing” the release of public information, he said.” (C)

“Gov. Andrew Cuomo’s top aide privately apologized to Democratic lawmakers for withholding the state’s nursing home death toll from COVID-19 — telling them “we froze” out of fear that the true numbers would “be used against us” by federal prosecutors, The Post has learned.

The stunning admission of a coverup was made by secretary to the governor Melissa DeRosa during a video conference call with state Democratic leaders in which she said the Cuomo administration had rebuffed a legislative request for the tally in August because “right around the same time, [then-President Donald Trump] turns this into a giant political football,” according to an audio recording of the two-hour-plus meeting.

“He starts tweeting that we killed everyone in nursing homes,” DeRosa said. “He starts going after [New Jersey Gov. Phil] Murphy, starts going after [California Gov. Gavin] Newsom, starts going after [Michigan Gov.] Gretchen Whitmer.”

In addition to attacking Cuomo’s fellow Democratic governors, DeRosa said, Trump “directs the Department of Justice to do an investigation into us.”

“And basically, we froze,” she told the lawmakers on the call.

“Because then we were in a position where we weren’t sure if what we were going to give to the Department of Justice, or what we give to you guys, what we start saying, was going to be used against us while we weren’t sure if there was going to be an investigation.”

DeRosa added: “That played a very large role into this.”

After dropping the bombshell, DeRosa asked for “a little bit of appreciation of the context” and offered what appears to be the Cuomo administration’s first apology for its handling of nursing homes amid the pandemic.

But instead of a mea culpa to the grieving family members of more than 13,000 dead seniors or the critics who say the Health Department spread COVID-19 in the care facilities with a March 25 state Health Department directive that nursing homes admit infected patients, DeRosa tried to make amends with the fellow Democrats for the political inconvenience it caused them.

“So we do apologize,” she said. “I do understand the position that you were put in. I know that it is not fair. It was not our intention to put you in that political position with the Republicans.”” (D)

“After insisting his administration had done nothing wrong last week, Governor Andrew Cuomo, on Monday, did not directly apologize for a lack of transparency surrounding the record-keeping of COVID deaths in nursing homes during the pandemic. Instead, he admitted that his administration made a “mistake” by not being transparent about the deaths, which created a “void” that was “filled with skepticism, and cynicism, and conspiracy theories which furthered the confusion.”…

NY1 summarized Friday that the administration “has contended nursing home and long-term care facility data surrounding deaths of residents was a mess and the thousands of fatalities that occurred in different locations has been difficult to accurately reconcile into one number.” Cuomo’s office, responding to this week’s leaked meeting, gave NY1 a timeline of the events DeRosa discussed backing her account. DeRosa herself defended her private comments by reiterating them in a statement:

I was explaining that when we received the DOJ inquiry, we needed to temporarily set aside the Legislature’s request to deal with the federal request first. We informed the houses of this at the time. We were comprehensive and transparent in our responses to the DOJ, and then had to immediately focus our resources on the second wave and vaccine rollout. As I said on a call with legislators, we could not fulfill their request as quickly as anyone would have liked.” (E)

“Cuomo has defended the move as being in line with federal guidelines, adding that, at the time, the top priority was not letting hospitals be overrun. Cuomo insisted on Monday that there has been “much distortion” around the March 25 memo, and said patients were only received at nursing homes if a given home verified they had the capability to accept them, as required by New York law.

Of the 613 nursing homes in New York, Cuomo said 365 received a patient from a hospital. Of the 365, Cuomo said 98% already had COVID-19 in their facilities.

“COVID did not get into the nursing homes from people coming from hospitals,” Cuomo said Monday. “COVID got into nursing homes by staff walking into nursing homes.”” (F)

NEW YORK, Feb. 24, 2021 /PRNewswire/ — As Chief Executive Officers of New York’s major health care systems, we would like to provide facts to clear up confusion in the public and the media regarding decisions to discharge patients to nursing homes during New York’s spring coronavirus surge.

Collectively, our hospitals treated more COVID-19 patients than any group of hospitals in the nation. Since the pandemic began, hospitals in New York State have safely cared for and discharged 135,000 COVID-19 inpatients. Every patient we treat is precious. We are particularly saddened by every patient that has passed away. This disease has taken a significant emotional toll on patients’ families and on our hardworking and dedicated staff.

Starting in late February and early March of 2020, New York hospitals mounted the largest mobilization of health care resources in our nation’s history. We rose to the challenge—but early in the pandemic it was not clear how many patients we would need to admit to our hospitals and not at all clear that we would not become overwhelmed and unable to safely care for our patients. With the Italian experience informing our planning and preparedness efforts, where hospitals were completely overwhelmed, we did everything possible to increase capacity. We cancelled thousands of non-urgent surgeries, procedures, and treatments. We significantly increased our bed capacity within days and were required to develop plans to increase capacity even more.

Part of this effort was to discharge patients to non-hospital care settings where they could be safely cared for, including nursing homes. It is an everyday practice for hospitals to discharge stable, medically recovered patients to nursing homes so long as the nursing home can safely care for the patient. This is true even of infectious patients who are medically stable, if proper precautions are taken.

Federal and NYS guidance make this clear. The U.S. Centers for Medicare and Medicaid Services emphasized this on March 13, 2020—two weeks before the New York State Department of Health issued its March 25th directive—when it stated that nursing homes “can accept a resident diagnosed with COVID-19 and still under Transmission-Based Precautions for COVID-19 [from a hospital] as long as the facility can follow CDC guidance for Transmission-Based Precautions.” The guidance emphasized: “Nursing homes should admit any individuals that they would normally admit to their facility, including individuals from hospitals where a case of COVID-19 was/is present.”1 The March 25th NYS directive closely adheres to this Federal guidance, stating that a resident cannot be denied admission to a nursing home based “solely” on a confirmed or suspected COVID-19 diagnosis, and made clear that precautions must be maintained.2 Nothing in either the Federal or State guidance and directives required nursing homes to accept patients that they could not safely care for. Indeed, the longstanding New York State regulatory requirement that a nursing home “shall accept and retain only those nursing home residents for whom it can provide adequate care” remained in effect.3 In addition, Federal regulations continued to require that nursing homes protect all residents and staff from communicable diseases.4

Experts from across the globe and our own CDC have concluded that COVID-19 patients are only contagious relatively early in their illness. The latest report from the CDC states that most adults with moderate COVID-19 disease “remain infectious no longer than 10 days after symptom onset.”5 The CDC also states that, while there have been reports of potentially infectious virus in some adults with severe disease between 10 and 20 days after symptom onset, it was estimated that 88% and 95% of their specimens were no longer infectious after 10 and 15 days, respectively, following symptom onset.

Many studies have also found that the maximum infectious period is within the first week of illness, with some studies finding maximum infectiousness occurring from 2-3 days before the onset of symptoms and 2-3 days after.6 In many cases, then, including residents in nursing homes, the time period of maximum infectiousness is prior to admission to a hospital, both during an asymptomatic period and 2-3 days after symptoms occur.7

In spring 2020, the average length of hospital stay for COVID-19 hospitalized patients discharged to nursing homes from our hospitals was nearly 10 days. This suggests that a great many recovering COVID-19 patients who were discharged from hospitals to nursing homes in New York were no longer transmitting the virus. Most, if not all, would have been admitted to the hospital once they were highly symptomatic, which would have been several days after first contracting the virus. They then would have spent many days in the hospital.

Given that COVID-19 nursing home residents that were admitted to our hospitals had already, by definition, contracted COVID-19 in their nursing homes, discharging them back to their nursing homes—so long as their nursing homes could properly care for them under Federal and State guidelines and with proper precautions–was a prudent and safe option, both for them and the other residents in their nursing homes.

“Johnson & Johnson’s Covid-19 vaccine is safe and effective, U.S. regulators said, a key milestone on the path toward giving Americans access to the first such shot to work in a single dose.

The vaccine was 72% effective in the U.S.-based porti.on of a global study that enrolled more than 43,000 people, Food and Drug Administration staff wrote in a document that summarized the company’s results and confirmed findings J&J released earlier this month.

The shot was highly effective against severe disease in all regions where it was tested and worked better against mutant coronavirus strains than previously reported, the document showed.

There were no Covid-related deaths among people who received the vaccine in the trial, the FDA staff wrote in their analysis, and no safety concerns that would preclude an emergency-use authorization. The agency lacked data to determine whether the vaccine prevented asymptomatic cases, the report said.” (A)

“President Joe Biden said during a CNN town hall Tuesday that teachers should be moved higher on the list of those who are getting vaccinated against Covid-19…

His comments come amid an ongoing national debate on what the safest way to reopen schools is — and how to best protect educators and staff who are returning to in-person instruction.

So far, 28 states plus Washington, DC, have started allowing all or some teachers and school staff to receive the Covid-19 vaccine. Teachers are still not eligible to receive the vaccine as a specific group in 22 states — although some educators might fall into the current age group a state is vaccinating.

And some experts have been at odds about how big of a priority vaccines are when it comes to returning to class.

In its school reopening guidelines released last week, the Centers for Disease Control and Prevention outlined five key Covid-19 mitigation strategies including the universal and correct wearing of masks; physical distancing; washing hands; cleaning facilities and improving ventilation; and contact tracing, isolation and quarantine.

Vaccines and testing were not among the “key” strategies the agency lays out, but rather “additional layers” of Covid-19 prevention.

CDC Director Dr. Rochelle Walensky told CNN earlier this week that while she’s a “strong advocate of teachers receiving their vaccine,” the agency doesn’t believe it’s a prerequisite for schools to be able to reopen.

But CDC guidance does specify that those who are at a higher risk should have virtual options, the director added.

“We have in the guidance clear language that specifies that teachers that are at higher risk — teachers and students that are higher risk, and their families — should have options for virtual activities, virtual learning, virtual teaching,” Walensky said.

Another expert, emergency physician Dr. Leana Wen, called teacher vaccinations “essential.”

“If we want students to be in school for in-person learning, the least that we can do is to protect the health and well-being of our teachers — especially as in so many parts of the country, teachers are already being made to go back to school in poorly-ventilated, cramped areas, with many students who may not always be masking and practicing physical distancing,” Wen told CNN on Sunday.” (B)

“Vice President Kamala Harris stressed Wednesday that teachers should be given priority for getting Covid-19 vaccinations, but wouldn’t say if she believed that giving them the shots should be a prerequisite for reopening schools…

“Teachers should be a priority,” said Harris, adding that teachers “are critical to our children’s development, they should be able to teach in a safe place and expand the minds and the opportunities of our children. So teachers should be a priority along with other front-line workers.”

Harris said fewer than half the states are prioritizing teachers right now to receive the vaccine….

When asked, Harris wouldn’t say if it a mistake for the CDC to make the recommendation, but only said that it’s merely a recommendation “about how to reopen safely if they’ve been closed, how to stay open if they’ve been opened.”

The vice president also repeated Biden’s statement from Tuesday night at a CNN town hall that his administration’s goal is to get as many K-8 schools reopened within the first 100 days of his presidency, allowing students to attend class five days a week.

“The issue here is not just about statistics — it’s about our kids, it’s about their parents,” Harris said. “It’s about the fact that every day our kids are missing essential, critical days in their educational development.”

“Each day in the life of a child is a very long time,” she continued, “And that’s why we’ve got to collectively do everything in our power to reopen our schools as quickly as possible, as safely as possible.” (C)

“President Joe Biden wants to vaccinate teachers to speed school reopenings, but more than half the states aren’t listening and haven’t made educators a priority — highlighting the limited powers of the federal government, even during a devastating pandemic.

“I can’t set nationally who gets in line, when and first — that’s a decision the states make,” Biden said while touring a Pfizer plant in Michigan on Friday. “I can recommend.”

Under the Constitution, the powers of the federal government are far-reaching but not all-encompassing. States have always retained control over public health and safety, from policing crimes to controlling infectious disease, including distribution of coronavirus vaccines that Washington helped create and whose supply it controls….

The CDC legally can’t force states to roll out Covid-19 vaccinations with any particular priority, said Sarah Gordon, an assistant professor of health law and policy at Boston University.

“They are actually quite limited in what they can do,” Gordon said. “The federalist separation of national versus local public health authority in the U.S. has, repeatedly, hamstrung rapid and effective pandemic response.”

In theory, Biden could cut vaccine supplies to states, which former President Donald Trump threatened to do in response to criticism from New York Gov. Andrew Cuomo, but it would cause an uproar, and the new administration has, instead, chosen a simple formula of allotment based on each state’s adult population. And it can set up its own vaccination centers in regions with eligible populations it’s trying to target.”  (D)

“California is planning to start setting aside 10% of the COVID-19 vaccine the state receives each week to vaccinate teachers, day care workers and other school employees in the hopes of getting more students back in the classroom.

“It must be done, and it must be done much sooner than the current path we are on. And we believe this will advance that cause,” Gov. Gavin Newsom said Friday as he announced the plan at an Oakland vaccination site.

The plan will begin March 1 by setting aside about 75,000 vaccine doses from the state’s current weekly allotment, Newsom said.

The vaccine will be used to inoculate “the ecosystem that is required to reopen our schools for in-person instruction,” including teachers, day care workers and other public school employees, such as bus drivers and cafeteria workers, Newsom said.” (E)

“As another semester gets under way, more than half of U.S. public school students are learning in front of tablets and laptops, according to the organization Burbio. President-elect Joe Biden has pledged to try to open most schools within his first hundred days in office.

And to make opening schools safer, the Centers for Disease Control and Prevention has classed school staff as “frontline essential workers” — meaning they’re among the earliest in line to get the new coronavirus vaccines. But access and timelines vary around the country, and some teachers remain worried about coronavirus continuing to spread in schools, even if they themselves can get the shots.

Per federal vaccination guidelines, school personnel — including custodians, food service workers, and bus drivers — along with child care providers, are in category 1b, just after the elderly in long-term care facilities and health care workers.

Grace Lee, the chief medical officer for practice innovation and pediatric infectious disease physician at Stanford Children’s Health, sat on the committee that drew up those recommendations. She said they put education workers so high up on the list because she’s concerned about the social and academic effects of prolonged school closures. “My worry is that some children are being left behind, and that we need to really be able to make sure that there is the opportunity for everyone to be educated.”…

Clarice Brazas teaches humanities, currently online at The U School, a Philadelphia public high school. Brazas says she’ll be happy to get the vaccine, but she’ll still be worried about her students spreading the virus, especially those who use public transportation.

“I work in a really small high school,” she says, “but we’ve already had several students who have either lost parents or whose parents have been hospitalized. A lot of our students live with their grandparents.”

No vaccine has yet been approved for children under 16, and younger people are at the back of the line according to CDC guidelines. Dr. Lee, at Stanford, says that means masks, handwashing, social distancing and ventilation are here to stay for at least the rest of this school year.

“Vaccines in my mind are another layer of protection,” she said. “It doesn’t mean all the other barriers go away, but it does mean that if we add that layer in, it adds a huge degree of protection that does not always depend on everyone being perfect one hundred percent of the time.” (F)

The news comes as scientists in Europe are debating whether recipients should be given one dose instead of two given the scarcity of the vaccine, the difficulties in fending off a winter surge in infections, and a rapidly mounting death toll.

The problem: While scientists say a single dose may well confer sufficient immunity to stop the spread of the virus, there isn’t sufficient data confirming this because the clinical trials for available vaccines and those nearing authorizations were all designed around a two-dose regimen.

Britain, which is coming under severe strain amid the spread of a new, more infectious variant of the virus, is the first European government to change its policy. It stressed that vaccine recipients would still get a second dose, just three months later than planned.” (B)

The U.S. Food and Drug Administration has suggested Britain’s drug regulator may be gambling with public safety by expanding the time gap between administration of the first and second doses of Pfizer’s coronavirus vaccine beyond that proven safe and effective by trial data. The FDA warned in a statement on Monday that there was a “potential for harm” if people believed they were protected against COVID-19 by a first dose of Pfizer’s vaccine for longer than available data demonstrate.

Facing a dramatic surge in both infections and hospitalizations — fueled, according to U.K. officials, by a more infectious strain of the coronavirus discovered in southeast England — British officials decided last week to change their inoculation strategy…

Oxford scientists have said their trial data supports allowing a window of up to 12 weeks between shots, and that their vaccine will provide protection to the full extent possible (up to 74% effective at preventing symptomatic COVID-19 infection) for that full period.

Pfizer hasn’t said that, however. Along with its German partner BioNTech, the American pharmaceutical maker has made it clear that their trial data only shows efficacy for 21 days after a first dose.

The U.K. regulator appears to be making an assumption based on the available data from the Pfizer-BioNTech Phase 3 human trials…

“Suggesting changes to the FDA-authorized dosing or schedules of these vaccines is premature and not rooted solidly in the available evidence,” the U.S. agency said in its statement. “Without appropriate data supporting such changes in vaccine administration, we run a significant risk of placing public health at risk, undermining the historic vaccination efforts to protect the population from COVID-19.” (C)

The FDA has learned of reports that some health care facilities are trying to stretch the number of limited doses by cutting them in half, extending the length of time between doses, or mixing and matching vaccines in order to immunize more people against COVID-19.

The U.S. Food and Drug Administration is warning health care workers that any changes to the authorized dosing schedules of COVID-19 vaccines currently being administered will significantly place public health at risk and undermine “the historic vaccination effort to protect the population” from the coronavirus pandemic.

The first and second doses of the Pfizer-BioNTech vaccine are approved to be delivered within a 21-day window, while the Moderna injections should be spread over 28 days. When given at those intervals, both vaccines are about 95% effective, according to the respective drugmakers…

Officials added: “If people do not truly know how protective a vaccine is, there is the potential for harm because they may assume that they are fully protected when they are not and, accordingly, alter their behavior to take unnecessary risks.” (D)

Researchers, as well as scientific and medical advisers, should lose no time in monitoring the effects of a change to the dosing schedule so that any benefits or risks can quickly be factored into nations’ dosing strategies. This means following the effect on infections, and studying the duration of immunity in those who have received only one dose of a vaccine. Careful surveillance of coronavirus variants will also be required, to keep an eye out for the emergence of any that might weaken vaccine efficacy.

Scientists need to study any unintended consequences of the new strategy — for example, whether, during a lengthy gap between doses, people are more likely to begin resuming pre-pandemic lifestyles, which they should not be doing. It will also be important to study the effect on public trust in vaccination after a sudden change to regulatory guidance. Regulators and scientific advisers need to be ready to be transparent about those consequences with the rest of the world. At the same time, if the strategy works well, then relevant insights should be passed on so that others can benefit.

Ultimately, there should be enough vaccine supplies to go round, so, in the long term, there will be no need to lengthen the gap between doses. But until there are sufficient supplies, scientific advice must be based on published and easily accessible evidence. It’s an essential principle of the science–government relationship.

“What are the risks of spacing out doses?

They’re medical, theoretical and social. The medical risk is that the first dose almost certainly won’t be as protective as two doses. Spacing out doses extends the period when people — most of whom have been chosen to get vaccines quickly at this point for reasons of vulnerability — will have suboptimal protection until they get their second dose. The theoretical risk is that a weak response to the virus in millions of people who have received just one dose would foster the emergence of new variants that could evade vaccine-induced antibodies. This would undermine a year’s work to develop safe and effective vaccines. The social risk is that mixed messages on the topic — from governments, regulators, professional groups and drugmakers — will make it more difficult to communicate with the public on vaccines that are already controversial, and shunned by some. There is also concern that inoculated patients may change their behavior in a way that increases the risk of spread, negating some of the benefits of vaccination.

What do others say?

Although AstraZeneca has been in favor of spacing out shots, Moderna and Pfizer have been more cautious. Pfizer said the safety and efficacy of its vaccine hasn’t been evaluated on different dosing schedules, and that it’s critical that health authorities carefully monitor the effects of any alternative regimens and ensure each recipient is afforded the maximum possible protection, “which means immunization with two doses of the vaccine.” Anthony Fauci, who heads the U.S. National Institute of Allergy and Infectious Diseases, said he’s worried about the push to stretch dosing schedules. The British Medical Association called for the U.K. to “urgently review” its decision to allow delays of the Pfizer-BioNTech vaccine. The U.K. physicians group supports delays of up to six weeks, but said Britain’s plan goes “well beyond” that.” (F)

“The European Medicines Agency (EMA), no longer responsible for the United Kingdom since Brexit, is skeptical of the decision. A maximum limit for the time interval between doses of the BioNTech-Pfizer vaccine is not explicitly defined, but the proof of efficiency is based on a study in which the doses were administered 19 to 42 days apart, according to EMA. Administration of the second dose after a six-month interval, for instance, would not comply with the regulations and would require an amendment to the authorization and more clinical data, the agency said….

The World Health Organization  (WHO), however, believes that delaying the second dose of the BioNTech-Pfizer vaccine is an option. In exceptional cases, extending the time between the two shots by a couple of weeks would be possible, the chair of the WHO’s Strategic Advisory Group of Experts on Immunization (SAGE), Alejandro Cravioto, told journalists.

Thomas Mertens, chairman of the Permanent Vaccination Commission (STIKO) at Germany’s public health agency, the Robert Koch Institute, also takes a positive view of the British plan. “Since the interval between the two vaccinations can very likely vary within wide limits and protection is already very good after one shot, it is certainly worth considering giving preference to the first injection in the event of a vaccine shortage,” he said.”  (G)

“The Centers for Disease Control and Prevention has quietly changed its recommendations for coronavirus immunizations to give doctors the flexibility to handle “exceptional circumstances,” a spokeswoman said, even though the changes have not been studied in large clinical trials.

In guidelines posted on the agency’s website on Thursday, the C.D.C. said that patients may switch between the two authorized vaccines — one by Pfizer and BioNTech, the other by Moderna — between the first and second doses in “exceptional situations.” The C.D.C. also said patients may extend the interval between doses to six weeks from three or four if giving the second dose sooner was “not feasible.”

With the possibility of vaccine shortages on the horizon and little expectation that supply can be increased before April, the changes may offer a way to vaccinate more people — a high priority for President Biden, who outlined his national coronavirus strategy on Thursday.

Until now, the C.D.C. had cautioned against any dosing changes, saying there was no evidence for it. A C.D.C. spokeswoman, Kristen Nordlund, said the agency’s “intention is not to suggest people do anything different, but provide clinicians with flexibility for exceptional circumstances.”

Dr. Anthony S. Fauci, the president’s special adviser for the coronavirus, had repeatedly advised against delaying the second dose or making any other changes in vaccination protocol without the data to support them. But on Friday, he seemed open to delaying second doses, at least for short periods, telling CNN he took no issue with the agency’s recommendations.

“What the C.D.C. is saying, sometimes, the situation is stressed where it’s very difficult to be exactly on time,” Dr. Fauci said. “So we’re saying, you can probably do it six weeks later, namely, two additional weeks. Quite frankly, immunologically, I don’t think that’s going to make a big difference.”..

The updated C.D.C. guidance still states that the authorized vaccines are “not interchangeable with each other or with other Covid-19 vaccine products.” The agency put the word “not” in bold on its website, and noted that the safety and efficacy of mixing doses has not been studied.

But “in exceptional situations in which the first-dose vaccine product cannot be determined or is no longer available,” the guidelines added, any available mRNA vaccine can be used for the second dose.

With respect to dosing, the guidance says that the second dose should be administered as close as possible to the recommended interval — three weeks for the Pfizer-BioNTech vaccine and four weeks for Moderna. But if that is “not feasible,” the agency wrote, the interval between doses may be extended to six weeks.

The pace of vaccination is critical not just to curbing disease and death, but also to heading off the effect of more infectious forms of the virus. The C.D.C. has warned that one variant, which is believed to be 50 percent more contagious, might become the dominant source of infection in the United States by March.

Although public health experts are optimistic that the existing vaccines will be effective against that variant, known as B.1.1.7, it may drive up the rate of new cases if enough people remain unvaccinated.” (H)

If people are only partially immunized with one dose, could that fuel more dangerous coronavirus variants?

That is a real concern, according to Paul Bieniasz, a retrovirologist at the Rockefeller University. Early in the pandemic, there was little pressure on the novel coronavirus to evolve because nobody’s immune system was primed against infection, and the microbe had easy pickings. But now millions of people have become infected and have developed antibodies, so mutations that give the virus a way to evade those defenses are rising to prominence. “The virus is going to evolve in response to antibodies, irrespective of how we administer vaccines,” Bieniasz says. “The question is: Would we be accelerating that evolution by creating country-sized populations of individuals with partial immunity?”

Just as not finishing your entire course of antibiotics could help to fuel antibiotic-resistant bacteria, not getting fully vaccinated could turn your body into a breeding ground for antibody-resistant viruses. But Trevor Bedford, a computational biologist at the Fred Hutchinson Cancer Research Center who tracks viral mutations, has tweeted that the pace of evolution is not only determined by the weakness or strength of the immune system. It is also affected by the sheer number of viruses circulating in the population, he wrote. Without widespread immunizations, the latter amount—and the number of variants that might beget a more formidable virus—will continue to grow.

Could a longer interval between first and second doses make a COVID vaccine more effective?

That result is possible. All COVID vaccines are not created equal, and the optimal dosing schedule depends on the specific design. Some vaccines are based on fragile strips of genetic material known as mRNA, some rely on hardier DNA, and others use protein fragments. These cores can be carried into a cell sheathed in a tiny lipid droplet or a harmless chimpanzee virus.” (I)

“The possibility that second doses could be delayed has some experts concerned because it might lead to millions of people walking around with only partial immunity to the coronavirus, a condition that could be ripe for harmful mutations of the virus to arise.

Delaying the second shot is a gamble, says Ramón Lorenzo-Redondo, a virologist at Northwestern University Feinberg School of Medicine in Chicago, particularly without a lot of evidence suggesting how well one dose works. Officials “shouldn’t gamble [their] best tools” to fight the pandemic, he says. “We don’t want to fuel [potential viral evolution] by doing suboptimal immunization of the population.”

How that fueling of virus evolution could happen comes down to the immune system. If people have full immunity as a result of vaccination, their immune response is likely to be robust, spawning large numbers of neutralizing antibodies, for example, that stop viruses from getting into cells and heading off harmful mutations before they arise. But if people have partial immunity, that immune response is likely to be weaker.

It’s like when doctors encourage patients to finish a full course of antibiotics, Lorenzo-Redondo says. In that case, eliminating susceptible bacteria with a full course could help lower the chance that stragglers build up resistance.

For the COVID-19 vaccine, if people’s second doses are delayed long enough — akin to not finishing a full complement of antibiotics — it’s possible that low numbers of neutralizing antibodies triggered by only one dose may only partially fight an infection. That might provide more time for variants of the virus with immune-dodging mutations to arise and thrive and be transmitted to other people.”  (J)

With shortages hitting vaccination sites from San Francisco to San Diego, news that Johnson and Johnson’s COVID-19 vaccine could win emergency authorization this week comes as a relief to health officials worried about the spread of dangerous new variants, and raises hopes there could be enough doses this spring to vaccinate anyone who wants a shot.

But is that a realistic timeline?

Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, and other health experts earlier this month said vaccines would be widely available to anyone as soon as April, though Fauci has since moved back his projection and said it could be late summer before most are inoculated. President Joe Biden last week said deals to secure 600 million doses of the Pfizer and Moderna vaccines by the end of July will provide enough for every American to get inoculated.

But some experts warn that it could be months before enough people get shots to have widespread herd immunity. And a closer look at the numbers suggests that, without speedier vaccine approvals or significant changes in how we’re vaccinating, that pessimism may be justified. Manufactured supply doesn’t necessarily track with getting it into people’s arms — there are logistical constraints — including shipping delays due to severe weather this week — and the unwillingness of many to get the shots.

The good news: The U.S. has picked up the vaccination pace, from averaging less than 1 million a day a month ago to about twice that now, according to the U.S. Centers for Disease Control and Prevention. That pace is likely to quicken as vaccines by Johnson and Johnson and other companies become available.

The bad news: Vaccination sites around the state are curtailing hours because they are running low on supply. Even Kaiser Permanente, one of the state’s largest HMOs, cannot provide shots to everyone who is qualified to have one. And emerging virus variants raise concerns that vaccines could be rendered less effective before enough people have had the shots to prevent outbreaks…

Dr. Ashish K. Jha, dean of the Brown University School of Public Health, said earlier this month that “sometime around April and May we will be in a situation where any American who wants a vaccine can get one.”

Doctors Scott Gottlieb of the American Enterprise Institute and Mark McClellan of Duke University’s health policy center, both former Food and Drug Administration commissioners, wrote earlier this month that health officials should prepare for a spring vaccine glut — and prepare a campaign to promote the vaccines’ safety.

“At some point, perhaps in April, supply will start exceeding demand,” they wrote in the Wall Street Journal earlier this month.

McClellan said Friday that nothing has altered his calculus on that.

“It is not that far off,” McClellan said. “It could be pretty soon when we get to the real challenge of making sure all those Americans who are unsure get the information they need to make a good decision.”

In the WSJ article McClellan and Gottlieb note that a Kaiser Family Foundation survey shows just under half of U.S. residents — 47% — say they want the COVID-19 vaccine, with the highest numbers among retirement-aged adults who are most at risk. Two-thirds of those 65 and older say they want to be vaccinated as soon as possible, while just 38% of adults under 30 say that…

Health experts are urging government officials to streamline approvals and get more shots into arms faster in a race against the evolving virus. Pfizer and Moderna last week noted a new study in the New England Journal of Medicine that questioned their vaccines’ effectiveness against the South African variant that has now been discovered in California.

“I am quite distressed about the ‘moving target’ that is happening from our political leaders right now about when enough supply will be available,” said Dr. Monica Gandhi, a medical professor at UC San Francisco.

Gandhi fears the country’s current vaccination pace won’t get shots to all who want them until September. She says government officials should speed emergency authorization of other vaccines in the pipeline and delay second doses of two-shot vaccines to allow more first-dose shots. That would offer at least some protection to more people, she said.

“At the current rate, we will not reach herd immunity until December,” Gandhi said, adding that her recommendations could speed that up by about four months.

“The only big concern I have about the whole thing is whether we’ll have adequate supply over the spring,” Hotez said. He noted that the AstraZeneca vaccine already has been approved for emergency use by the World Health Organization and European Medicines Agency.

“Getting the American people fully vaccinated by the fall, that was a good plan,” Hotez said. “The problem now is the U.K. variant. We need to accelerate that timetable.”” (K)
