On the campaign trail, Joe Biden proposed a $125 billion investment in prevention of substance abuse, treatment, and recovery, to be paid over 10 years with taxes on the pharmaceutical industry…
Biden called on Congress last month to set aside $4 billion for HHS to expand drug treatment access during the pandemic. And within his first 30 days, he hired six senior staffers for the Office of National Drug Control Policy (ONDCP), which Trump had gutted.
Advocates want to see the office funded at least at pre-Trump levels. Just as critical is how that money will be spent. The drug czar, whom Biden is expected to appoint soon, has budgetary authority over more than a dozen federal agencies in the National Drug Control Program.
“Less than 50% of ONDCP’s annual $25 billion budget is allocated to treatment and prevention,” said Oliva. “The majority of that budget is spent on law enforcement and interdiction. I would urge the new drug czar to reverse these priorities.”
Treatment will be an emphasis, but enforcement will not go away, LaBelle, the acting drug czar, says. “Drug interdiction, international drug trafficking and precursor chemicals, and the future of drug trafficking and the shift toward synthetics is another issue that has to be taken on.”
Frontline workers around the country say they need funding for less visible long-term recovery support systems—such things as housing, therapy, job placement, and peer support. For any of it to work, mental health care and addiction treatment will need to be widely available and cheap or free.

Overdose deaths increased in almost every state; 24 states and the District of Columbia had an estimated increase of at least 30 percent, and the overall U.S. total increased by 33 percent.
States like West Virginia and Kentucky have long been at the heart of the opioid epidemic, and that region is still reporting some of the largest proportional increases. Recent research has also highlighted the growing impact of fentanyl and overdose deaths within states farther West. Arizona, California, Colorado, Illinois, Texas, and Washington all experienced increases above 35 percent during the first eight months of 2020; Colorado recently reported record overdose deaths during full year 2020…
One policy tool that can address multiple objectives is Medicaid expansion. Data continue to show the positive impact of expansion on coverage, MAT access, and mortality outcomes for substance-use patients.
By simply expanding Medicaid, nonexpansion states like Florida, Georgia, Tennessee, and South Carolina could access significant federal financing in their push to help an ever-growing number of people in need. These four states all experienced overdose death increases above 30 percent during the first eight months of 2020.
Utilizing Medicaid also decreases the reliance on annual discretionary funding to support siloed treatment programs, which has proven to be unsustainable in the fight to reduce drug overdoses. Policy experts recently argued for restructured financing of substance-use treatment through “mainstream public and private insurance programs” like Medicaid that allow states to reliably “pull” down funding as their needs increase.
But in the absence of further financing reform, federal discretionary funding has quickly increased to meet the growing crisis. The December 2020 funding package included $4.25 billion in mental health and substance-use emergency funding; the recently passed American Rescue Plan (ARP) will provide an additional $3.5 billion for block grants in these same areas. President Biden recently announced $2.5 billion to further support states.
Combined with ARP’s significant financial assistance for state and local governments, the targeted substance-use funding will likely be critical for struggling addiction-treatment providers and government agencies that account for a significant percentage of overall substance-use treatment funding. Many have had to contend with tighter budgets related to the pandemic’s economic impact.” (B)
“Some people are calling them twin pandemics that have collided,” Harvard researcher Michael Barnett, Ph.D., said on CBSN Monday.
The Centers for Disease Control and Prevention estimates that 90,237 people in the U.S. died of opioid overdoses between October 2019 and September 2020. The figure is the highest ever recorded since the opioid crisis began in the late 1990s.
“This is an incredibly important public health crisis that has come along with COVID,” Barnett said. “Before 2020, we went into the COVID pandemic with an out-of-control public health crisis of addiction.”
Barnett, who serves as an assistant professor of health policy and management at the Harvard T. H. Chan School of Public Health, pointed out that 2019 had been the worst year on record in terms of opioid-related deaths before this latest figure and said 2020 “would have been terrible as well” with or without the pandemic.

The number of American adults who reported symptoms of anxiety or depression between April 2020 and February 2021 rose by 27% over the previous year, also according to CDC data. Emergency room visits for drug overdoses increased by 36% in the same period.
Overall stress due to the pandemic’s impact on health and the economy as well as increased isolation have been described as the main drivers behind the spike in mental health problems.

“Pennsylvania has been among the states hardest hit by the opioid epidemic. It had one of the highest rates of death due to drug overdose in 2018, with 65%, a total of 2,866 fatalities, involving opioids.
The state’s stay-at-home order, implemented on April 1, 2020, mandated that residents stay within their homes whenever possible, practice social distancing and wear masks when outside the home. All schools shifted to remote learning, and most businesses were required to operate remotely or close. Only essential services were allowed to continue operating in person.
In the following months, the public’s overall cooperation with these mandates contributed to measurable declines in coronavirus infection rates. To learn how these mandates also affected people’s use of opioids, we assessed data from the Pennsylvania Overdose Information Network for changes in monthly incidents of opioid-related overdose before and after April 1, 2020. We also examined the change by gender, age, race, drug class and doses of naloxone administered. (Naloxone is a drug widely used to reverse the effects of overdose.)
Our analysis of both fatal and nonfatal cases of opioid-related overdose from January 2019 through July 2020 revealed statistically significant increases in overdose incidents for both men and women, among whites and Blacks, and across several age groups, most notably the 30-39 and 40-49 groups, following April 1. This means there was an acceleration of overdoses within some of the populations most affected by opioids prior to the COVID-19 pandemic. “The COVID-19 pandemic has exacerbated the many challenges people with any substance use disorder face, not just opioids. Most distressing is that overdose deaths have skyrocketed…
Vulnerable and marginalized patients experiencing homelessness were particularly hit hard and are those I worry most about. Early on, shelters and social service agencies shut down or limited their hours and numbers of people they could serve. Also, public bathrooms to wash hands closed, and it’s almost impossible to maintain social distancing and good hygiene if you’re surviving on the street with other people in the same situation.
Infections related to injection use increased because people could not access harm-reduction supplies, like needles or pipes. Before the pandemic we didn’t have adequate harm-reduction measures, and we still don’t. The pandemic further highlighted the dire need to incorporate and offer harm reduction, as well as welcoming safe spaces throughout the continuum of care for people who use drugs, not just in clinics like mine or needle exchanges.
If I have patients who are injecting and are unable to access safer supplies, their drug use doesn’t just stop. They’re going to use and share needles with other people, which puts them more at risk not only of contracting and spreading COVID, but also other blood-borne infections like hepatitis and HIV.
Patients who were taking buprenorphine, life-saving medication for opioid use disorder, had trouble getting their medication. Buprenorphine is a partial opioid agonist medication, which means it activates the same receptors in the brain as other opioids, but only partially. It prevents and treats painful withdrawal, relieves cravings and prevents overdose death. As it restores altered neuropathways that drive ongoing compulsive opioid use, it helps people with opioid misuse disorder regain normal function.
Many patients had a hard time reaching their prescribers when clinics shut down, so we accepted them at our clinic. We’d get calls from the New England area, not just Massachusetts but places like New Hampshire and Maine. People would say, ‘I can’t reach my provider, what should I do?’” (E)

“But the opioid epidemic has not unfolded unchecked. Over the past decade, successful state-level policies have included prescription drug monitoring programmes and increased availability of naloxone to prevent overdose. Endorsement of medication-assisted therapy for treatment of substance abuse and overdose prevention has also increased. In April, 2021, the so-called X-waiver was rolled back by the DHHS, enabling more health-care providers to prescribe buprenorphine for opioid use disorder. But stigma around addiction, and restrictive regulation and persistent reluctance by medical professionals to prescribe medication-assisted therapy have hampered progress.
It is promising, then, that the Biden Administration has nominated Rahul Gupta as head of the Office of National Drug Control Policy. If confirmed, Gupta—former health commissioner of West Virginia—will be the first physician to take the role, signifying less focus on legal and law enforcement approaches to drug policy and an increased emphasis on addiction treatment and expanded health-care services.
His appointment comes at a vital time. The 2020 data suggest that the COVID-19 pandemic has been a potent accelerant of opioid-related overdose deaths. The mechanisms are unclear, but it is likely that disruptions in available treatment services and reduced access to harm reduction practices, such as closures of safe injection sites, will have played a role. The data also highlight important demographic points. West Virginia, the epicentre of the crisis, continues to have the highest number of overdose deaths, but urban areas have overtaken rural areas for age-adjusted death rates. The number of 45–64-year-old non-Hispanic Black people in urban areas dying from synthetic opioid overdose has been rising swiftly, a population who are also at greater risk of dying from COVID-19. Although white, rural, middle-aged people are still severely affected by the opioid crisis, it would be unwise and counterproductive to consider overdose deaths their sole preserve. A major test for Gupta will be how to increase financing of addiction prevention and treatment services within the often fragmented infrastructure funded by public health insurance. He will need to reinforce resources in the regions that have been hardest hit by COVID-19, and equitably target emerging regional and group vulnerabilities to opioid use.” (F)

“In the last week of January 2020, the Seaside Recovery Center, a clinic that uses methadone and other medication to treat people with opioid addiction, opened in the city’s south end…
So the feds began to grant clinics like Seaside Recovery Center the freedom to determine whether patients could be trusted to take medicine home with them — to not sell, abuse or otherwise mishandle it — and how often that person should have to visit the clinic. A patient who had been dropping by daily could now do so weekly.
“We actually got to make those decisions based on what we knew about the patient, and not necessarily just based on what these very old rules told us we had to do,” Noice said.
One upshot is that Seaside patients missed out on a key part of their treatment: group counseling. The clinic tried to hold electronic sessions, but individual phone calls between counselors and patients proved more successful.
Addiction treatment relies on peer groups; recovery involves building a network that supports a person’s sobriety, Noice said. For about a month and a half, the few Seaside patients met in the group therapy room, but a strong cohort — the desired number is between 8 and 12, Noice said — couldn’t get established before in-person meetings were discontinued.
The clinic’s doors remained open, medication went to everyone who needed it, new people signed up and care never ceased. Patients still underwent urine analyses, met with counselors — if only briefly — and scheduled longer therapy sessions to take place later by phone or video.
“But we didn’t build a community the way that you normally see a community develop in a clinic like that,” Noice said.
The pandemic has so defined the experience of Seaside’s staff and patients that it is hard to tell whether unforeseen challenges — for instance, the patient population has not increased as fast as CODA had expected — are due to COVID or the newness of the clinic.” (I)

“The COVID-19 pandemic forced many of us to spend months or longer isolated in our homes. Throughout 2020, experts warned that the side effects of this necessary but difficult public health emergency would be especially hard for those struggling with behavioral health conditions or a substance use disorder.
Unfortunately, they were right.
Throughout the country, an estimated 93,000 Americans were killed by an accidental overdose last year — a 30% increase from 2019. While we avoided such a spike, and actually saw a reduction, in Camden County, 288 of our friends, family, and neighbors were still lost to this debilitating disease.
While the COVID-19 pandemic still represents the most significant public health crisis of our lifetimes, it took just one year to develop free, safe and effective vaccines for the public that protect against the virus and dramatically reduce the risk of death from contracting COVID-19.
There is no vaccine for overdoses, and none is on the way.
Our best tool in fighting the disease of addiction is openness, empathy and information. If we work together, we can eliminate the scourge of opioid-related deaths from our community, but it is not easy.” (G)

“Pennsylvania’s opioid disaster declaration is set to expire at the end of Wednesday after state lawmakers, newly empowered to help manage statewide emergencies, declined the governor’s request for another extension.
Democratic Gov. Tom Wolf first declared a public health emergency in January 2018 after Pennsylvania set a record for opioid deaths, then renewed it more than a dozen times as the state battled an overdose epidemic that has worsened during the coronavirus pandemic.
Wolf had to seek legislative approval for another extension because of a newly approved constitutional amendment limiting a governor’s emergency powers. But the GOP-led General Assembly declined to go along.
“Our fight is not over,” Wolf said in a written statement. “We have an obligation to support individuals desperately in need of substance use disorder services and supports. With or without a disaster declaration, this will remain a top priority of my administration.”
The disaster declaration made it easier for people to get treatment, expanded the state’s prescription drug monitoring program and established an inter-agency opioid “command center” to coordinate the state’s efforts, among other things.
State officials cited progress, with opioid prescribing down by more than 40% and overdose deaths falling by nearly 20% after a record 5,403 people statewide died in 2017.
But overdose deaths have climbed again during the pandemic. More than 5,000 people died of drug overdoses in 2020, according the Wolf administration.” “ (H)

“In the wake of the pandemic, the overdose crisis has reached new heights that call for rethinking our response, both nationally and in Florida. In many parts of the country, the year-over-year overdose death rate jumped by more than 50% between 2019 and 2020. The Centers for Disease Control (CDC) calculates the average nationally to reach rates as high as 40%, bringing us over 93,000 overdose deaths nationally in 2020.
We are quite literally at an unheard-of rate of increase — on top of annual increases that had long ago put us at record numbers. The drug overdose death rate suggests a public-health threat of a magnitude that we have never seen in American history. Can you imagine the outcry if we saw that kind of increase in vehicular-accident deaths or gun-related deaths? But drug overdoses? Not so much…
Beyond focusing attention on the crisis, our response needs to be attentive to the fast-moving ways in which the overdose crisis, still mostly fueled by opioids, is shifting. The people distributing and seeking drugs make for an efficient market that responded to the prescription pain medicine crackdown by shifting to illicit fentanyl on the street.
Pain prescribing is reaching historic lows. As measured by prescription of morphine milligram equivalents (MMEs), the amount of opioid prescriptions in Florida are down 67% since 2018. The battle to save lives is no longer just in the doctor’s office; it is on our streets and online via apps like OfferUp (the successor to Craigslist) that have taken drug dealing digital.
The crisis is changing quickly in other ways. Though in earlier phases of the opioid crisis, the face of the crisis was rural working-class Americans, today, the most dramatic change has been the rate of overdose increases among Black and Latino Americans. Just as COVID-19 had disproportionate impacts for Black and Latino Americans, so too the opioid crisis is wreaking havoc in communities of color…
While the delta variant has highlighted the extent to which the pandemic demands our attention to preventing unnecessary deaths, it is finally time for leaders to acknowledge that we are simultaneously fighting a second health crisis that also requires urgent attention.
I encourage you to ask the tough question: why are we not doing more to prioritize and address this tragedy? We have the ability to address the pandemic and still meet the deep needs of a society battling an unprecedented opioid crisis that has changed before our eyes. This will only happen if and when leaders begin prioritizing this epidemic inside the pandemic to stem the tide of another generation lost to the modern drug crisis.” (J)

“Methadone has long been a gold-standard treatment for opioid addiction. But government regulations mean that many patients have to organize their lives around getting and taking it, no matter how well they are doing, a new study has found.
The pandemic made it unsafe for people to queue up daily at methadone clinics, so rules were relaxed to allow patients to take doses home just like other prescriptions. And, advocates say, the change helped people maintain recovery and ought to become permanent.
The new study, published in the Journal of Harm Reduction and conducted by researchers at New York University, Icahn School of Medicine, and the City University of New York, interviewed current and former methadone users, people who used illegal opioids but never methadone, and treatment providers.
“It’s like liquid handcuffs,” one woman, a former methadone patient, told the study authors, using a common euphemism for the treatment. “Say you want to go somewhere for a few days, you need take-homes and if they won’t give them to you, there’s nothing you can do. Everyone has trouble with [take-homes], whether it’s losing their job, or they can’t go out of town, or they’re just late, or sick.”
Methadone staves off the painful withdrawal symptoms that otherwise might send people who use opioids in search of street drugs. Medication-assisted treatments (MAT) such as methadone have repeatedly been shown to produce more durable recovery than 12-step programs alone. It’s possible to take home other types of MAT, but methadone is much cheaper.
Federal regulations require patients to take their methadone under daily supervision at a clinic, largely to prevent street sales and overdoses. Guidelines allow for patients to receive up to two weeks, and, in some cases, a month’s worth of doses to take at home once they’re deemed stable enough. Yet many clinics maintain even stricter rules around take-home doses, and most patients on methadone rarely get more than a few days’ worth of take-home doses, the study authors wrote.
The requirement to show up each day for a single dose of medication makes it hard to find and keep a job, travel, or return to the rhythms of “normal life,” patients said. Several described missing family gatherings, funerals and weddings because they couldn’t get take-home doses, or were forced to juggle work responsibilities with clinic appointments.” (L)

“New Jersey experienced a record number of overdose deaths during the first half of this year, according to state data, and could surpass the all-time high number of drug-related fatalities recorded in 2020 if the current pattern holds.
Between January and the end of June, 1,626 New Jerseyans lost their lives to an overdose, the state Attorney General’s office reports, nearly three dozen more deaths than were recorded by that point last year. If this pace continues through the fall, more than 3,250 people would die in this manner during 2021 — an increase of more than 6% over the 2020 total, an NJ Spotlight News analysis found.
Overdose deaths have been on the rise for more than a decade in New Jersey before they appeared to peak in 2018, when some 3,118 residents lost their lives in this way. The tally declined to 3,021 in 2019, before creeping up again to 3,046 last year.
Experts say the recent increase reflects the impact of the ongoing pandemic, which has led a growing number of people to turn to drugs and alcohol to cope with the isolation, fear and economic- and health-related stress. Gov. Phil Murphy’s administration has continued to invest tens of millions of dollars annually in efforts to expand access to treatment and other social services for those with substance use disorders.
But support for these programs is not universal. In July, Atlantic City voted to close a harm-reduction center because of concerns about its impact on the resort’s tourist district. It was one of six facilities statewide that offered free clean needles and other assistance to IV drug users, a proven way to combat overdoses and the spread of infection. State officials later offered their support for a bill by Sen. Joe Vitale (D-Middlesex) that would give the state Department of Health power in deciding where harm-reduction programs should be located, instead of local governments.” (K)

According to Yashar Eshraghi, MD, the medical director of pain research at Ochsner Health in New Orleans, who led the study, current data on the risk for chronic opioid use after short-term use is both unclear and nuanced. Some research does suggest that under certain circumstances, short-term opioid use does not increase a patient’s risk for chronic use or abuse, while other studies have found the opposite to be true….
According to Eshraghi, the new research addresses the nuances needed to properly assess the cost versus benefit of treating a patient’s pain with opioids.” (N)
But the agreement includes a much-disputed condition: It largely absolves the Sacklers of Purdue’s opioid-related liability. And as such, they will remain among the richest families in the country.
Judge Robert Drain of the U.S. Bankruptcy Court in White Plains, N.Y., approved the settlement, saying he wanted modest adjustments. The painstakingly negotiated plan will end thousands of lawsuits brought by state and local governments, tribes, hospitals and individuals to address a public health crisis that led to the deaths of more than 500,000 people nationwide.
The settlement terms have been harshly criticized for shielding the Sacklers. They are receiving protections that are typically given to companies that emerge from bankruptcy, but not necessarily to owners who, like the Sacklers, do not themselves file for bankruptcy.
Several states, including Connecticut and Washington State, have already said they intend to appeal the judge’s ruling.
In exchange for the protections, the Sacklers agreed to turn over $4.5 billion, including federal settlement fees, paid in installments over roughly nine years. Those payments, and the profits of a new drug company rising from Purdue’s ashes with no ties to the Sackler family, will mainly go to addiction treatment and prevention programs across the country.
Judge Drain delivered his ruling orally from the bench in a marathon session that ran to six hours, meticulously working through his reasoning in a case he called the most complex he had ever faced. “This is a bitter result,” he said. “B-I-T-T-E-R,” he spelled out, explaining that he was frustrated that so much Sackler money was parked in offshore accounts. He said he had expected and wished for a higher settlement.
But the costs of further delay, he said, and the benefits of an agreement he described as “remarkable” in its ability to help abate the epidemic, tilted toward approval.
While the settlement serves as a benchmark in the nationwide opioid litigation aimed at covering governments’ costs and compensating families, it also means that a full accounting of Purdue’s role in the epidemic will never unfold in open court. Purdue pleaded guilty to federal criminal charges for drastically downplaying OxyContin’s addictive properties and, years later, for soliciting high-volume prescribers.” (M)

“According to hospital executives and nursing administrators in several states, the struggle to find enough workers to care for people sick with covid-19 has emerged as a critical problem as other daunting shortages, widespread early in the pandemic, have eased. Once-scarce supplies of protective gear, ventilators and coronavirus tests are now plentiful, hospital officials consistently say.
Finding ways to hire and keep nurses is the main problem, officials say, although some hospitals also are short on respiratory therapists and workers for nutritional services and housekeeping. Some hospitals could enlarge their bed capacity if they had more nurses to tend to the coronavirus patients who would fill them….
Ochsner, with 24 hospitals in Louisiana and into Mississippi, has 300 travel nurses from all over the country.
“Frankly we could use more — probably another 100 to 150 would help us,” said Moffatt, the chief nursing officer.
During the initial surge that hit New Orleans after Mardi Gras last year, Ochsner expanded the role of a hub that coordinates nurses who float among different hospitals in the system. The hub also became a staffing agency to hire contract nurses from elsewhere and was able to recruit about 500 at first.
“In this fourth wave, it’s become a lot harder to recruit nurses,” Moffatt said. She suspects that some travel nurses who worked through earlier pandemic surges are burned out — or have made enough income that they are taking time off.
With the pool of available travel nurses smaller and coronavirus cases flourishing, she said, hospitals in hot spots “are all at this time drinking from the same pool of people.”
Bidding wars occasionally break out. “We will sometimes see an agency nurse accept at Ochsner,” she said, “and . . . two hours later they pull out and [have] accepted a higher rate.”” (A)
“Before the coronavirus pandemic, Ivette Palomeque made $45 an hour on a flexible schedule as a staff intensive-care nurse at Memorial Hermann Health System in Houston.
Today, she earns $120 an hour working in an ICU in McAllen, Texas, the latest in a string of “travel nurse” jobs she has held over the past 16 months. The journey has taken her from Miami to New York City and back to Texas.” (B)
“During the first wave of the pandemic, traveling nurses descended on New York City and other hotspots, then moved on as the virus did. This time around, much of the country is a hotspot. And adding traveling nurses can be costly.
“Travelers are expensive,” Ross said. “We have our nurses begging for them to get extra help. Some states I’m told that are hardest hit right now are finally looking to other states and asking for help, and asking for travelers.”
But even if hospitals have the budget, Ross added, securing travelers only gets harder as demand skyrockets “country-wide, even worldwide.”” (C)
“Travel and resident nurses who spoke to 11Alive Thursday said that if hospitals decide to mandate vaccines, it’ll only drive the number of workers needed in the industry higher.
“I really honestly feel as though it shouldn’t be forced on me now as a nurse,” said travel nurse Shy Warren. “I know a lot of people are so against us nurses not wanting to get the vaccine. But we have also done our research.”..
Ashley Blackmon, president-elect for United Advanced Practice Registered Nurses of Georgia, said the nurse to patient ratio right now proves how bad the shortage is.
“I’ve never seen anything like this. I have friends who work in urgent care and usually at this time of the year, they see 30 patients a day now they’re seeing 130 and one nurse practitioner running an urgent care that’s next to impossible to do,” she said.
“You have bedside nurses and in ERs that, who are doing everything they can do, but there are still 60 people in the waiting room, all coughing in each other’s faces and waiting for 10 hours. In our outreach hospitals in more rural areas of of Georgia that’s never been heard of. So it’s been quite a culture shock.”
Blackmon added that not only are nurses experiencing burnout because of the shortage, but also because they are seeing more deaths now than previously in the pandemic.
Horton said keeping experienced nurses on the job needs to be a priority, along with finding the next generation of nurses. She said developing future nurses is also a challenge because of issues on the academic level.
“We also have the challenge with our nursing faculty,” Horton said. “Many of them are at a point of retirement and because nurses can make more in a clinical setting than they can as a nursing faculty member, it is much more difficult to retain faculty and to recruit faculty in order for us to expand our growth as far as bringing in more nursing students.”” (D)
“But burnout isn’t the only thing compounding California’s nursing shortage: The state’s new vaccine mandate for health care workers is already causing headaches for understaffed hospitals before it is even implemented. Some traveling nurses — who are in high demand nationwide — are turning down California assignments because they don’t want to get vaccinated.

In March 2020, the California Department of Public Health contracted with Aya Health — one of the nation’s largest traveling nurse providers — to pay up to $1 billion over six months to help hospitals meet nursing and other clinical staff shortages…
Last week, Gov. Gavin Newsom signed an executive order reinstating emergency provisions aimed at ensuring adequate staffing. In part, the order allows health care workers from out of state to work in California….
“Out of ratio” means that a nurse is assigned too many patients. California is the only state that caps the number of patients that can be assigned to a single nurse. Under state requirements, for instance, an ICU nurse can have no more than two patients and an emergency room nurse, no more than four.
In December, during the winter surge, emergency waivers for the ratios were granted to hospitals throughout the state, allowing ICU nurses to see three patients, for instance. Critics say the conditions threatened patient safety.
Newsom’s order last week did not reinstate the waivers of the nurse-patient ratios, which expired on Feb. 8. Individual facilities, however, can still apply for them…
Hospital administrators worry that the state’s vaccine mandate for health care workers, which goes into effect Sept. 30, could drive some of their workers out. Already, some report resistance among employees.
“One hospital told us they had 474 unvaccinated employees. They did a big education and incentive push. Only 12 people signed up,” said Richardson, the hospital association’s attorney.
Administrators are particularly concerned about low vaccination rates among support staff like janitors and food service workers. However, some nurses also are wary of the COVID-19 vaccine. Some nurses with large social media followings have participated in protests in Southern California, arguing that the mandates violate their personal freedom….
To contend with local shortages, hospitals are increasingly turning to hiring temporary, traveling nurses from around the country.” (E)
“Duke hired 1,800 nurses in the past year, and lost 1,200, for a net gain of 600, Fuchs said. But it still has 650 positions that it’s trying to fill, she said.
Nursing shortages are baked into the system in North Carolina, said Taylor, president of the nurses association. Nursing programs at the state’s four-year and community colleges produce 5,000 to 6,000 nursing graduates a year, Taylor said, but the health care industry needs between 7,000 and 8,000 a year.
“So when you look at it, even if we filled every seat that we could in the nursing programs in North Carolina, we’re still short,” he said….
One complicating factor in the staffing at hospitals is the role of travel nurse agencies, which recruit nurses and temporarily hire them out to hospitals where and when they’re needed. As turnover increases, hospitals are leaning more on traveling nurses, said Jeronica Goodwin, WakeMed’s senior vice president of human resources.
“We have increased our nurse agency use like everyone across the Triangle has,” Goodwin said. “I don’t think any hospital across the nation could function right now without travelers.”
But traveling nurse agencies also help increase hospital turnover, because they pay so well. Fuchs says Duke lost 100 nurses in three months last winter to travel agencies.
“Many of our nurses, our younger nurses especially, they choose travel nursing, and it’s because they can make $4,000, $5,000, $6,000, $7,000 a week for being able to care for patients,” she said. “Now what that’s doing for hospitals is putting them in a really tough situation financially.”
As the name implies, nurses who sign on to travel agencies usually go to another part of the country, where the need is greatest. But with everyone in need, nurses find they can work for a travel agency without leaving home.
Taylor says he knows of at least 10 nurses who have left their regular full-time jobs in recent months to become travel nurses.
“I’m talking about still working here in North Carolina, not having to relocate anywhere,” he said. “They’re signing some of these short-term contracts that sometimes can be up to $6,000 or $7,000 a week that they’re being paid, which is sometimes significantly more than what they would be making if they remained in their current full-time position.”
Hospitals are trying all sorts of strategies to bring on more nurses and other health care workers. WakeMed has hired more recruiters and streamlined the hiring process and is holding more job fairs, Goodwin said.
Duke and UNC are offering up to $25,000 to pay off student loans for new hires, as well as relocation expenses and hiring bonuses. Fuchs said Duke is offering a $15,000 signing bonus for experienced nurses for the first time in 25 years.
Hospitals are also working to hang on to the workers they have. They’re offering more flexible hours and assignments and providing more opportunities and bonuses for continuing education and training. Duke stresses its supportive work environment, including adequate staffing levels of nurses and assistants so people don’t feel as stressed.” (F)

Staffing shortages are an old problem made worse by COVID-19
Staffing didn’t just become a problem when coronavirus hit. Wisconsin’s health care workforce is aging, and older employees are retiring faster than they can be replaced, according to a 2020 report from the Wisconsin Hospital Association.
Nurses, perhaps more than other health care positions, are always in short supply. Wisconsin health systems said ICU, critical care and medical-surgical nurse staff were among their most urgently needed.
The main problem is in the pipeline, said Linda Young, dean of the College of Nursing and Health Sciences at the University of Wisconsin-Eau Claire.
Low faculty numbers and not enough sites for clinical rotations mean nursing schools must cap their admittance far lower than the number of qualified students actually applying. In Eau Claire, the school admits 43 students per semester, turning away at least as many more, Young said.
More than 80,000 qualified applicants to U.S. nursing schools were turned away in 2019 for those reasons, according to the American Association of Colleges of Nursing. The country is projected to be short more than 500,000 registered nurses by 2030, found a 2018 forecast from the United States Registered Nurse Workforce Report Card.
Young chairs the legislative committee of the Administrators of Nursing Education of Wisconsin, which in the past budget cycle secured $5 million to help nurses obtain teaching degrees if they commit to teaching for at least three years. Nurses working in academia earn less than those working in clinical settings, she said.
The pandemic has only highlighted the consequences of the nursing shortage, Young said.
“Demand is even greater for more new graduates,” she said. “We’re just not able to get them out.”
More nurses retiring also means there are fewer nurses to help new ones learn the ropes in the hospital, said Connie Bradley, senior vice president and chief nurse executive at Aspirus Health.
Newer nurses are thrown into more complex environments faster than in the past because there aren’t as many experienced nurses to fill those roles, she said.
“Years ago, you wouldn’t bring novices into the (emergency department) or the ICU. Well, now you don’t have other options,” Bradley said. “You have to bring novice nurses in, and yet you have less people who are experienced to train them, so you have new nurses being trained by novice nurses.”
Nurses now juggle patients with other health needs — and they’re burning out
Brusko said feelings of burnout have built up over the past year and a half.
“It would be shortsighted for us to think that our nursing colleagues, and frankly (health care) as a whole, are going to somehow fairly rapidly put this pandemic in their rearview mirror once we get through this,” Brusko said.
It’s especially difficult for them in this phase of the pandemic, where the majority of people who are ill in hospitals could have prevented it by getting vaccinated, he said.
For people at the beginning or end of their careers, the pandemic’s chaos may be the impetus for a career change or retirement, said Dr. Sarah Lulloff, medical director of infection prevention at Hospital Sisters Health System’s St. Vincent Hospital in Green Bay, in a Wednesday press conference.
Those who remain are struggling to balance an increase in COVID-19 patients with their already demanding workload of people with other health needs.” (G)

A total of 101,433 COVID patients were hospitalized, according to data published on Friday morning.
U.S. COVID-19 hospitalizations have more than doubled in the past month. Over the past week, more than 500 people with COVID were admitted to hospitals each hour on average, according to data from the Centers for Disease Control and Prevention.
The United States reached its all-time peak for hospitalizations on Jan. 14 when there were over 142,000 coronavirus-infected patients in hospital beds, according to HHS.” (A)
“Southern Louisiana’s hospitals, already packed with coronavirus patients from a fourth surge of the virus, were dealing Sunday with another challenge — the howling Category 4 hurricane pounding the coast.
Lady of the Sea General Hospital in Lafourche Parish, near where Ida made landfall, reported extensive roof damage. “All patients and staff are fine at this time without injury; although, our hospital has sustained significant damage,” hospital CEO Karen Collins said in a message relayed via Facebook. The hospital’s phone system was down.
“Once it is safe to do so they will evacuate their small number of patients,” state health department spokeswoman Aly Neel said in an email. Details on the number of patients involved were not immediately available.
Another Lafourche Parish hospital, Thibodaux Regional Medical Center, reported a partial generator failure to the state. Christina Stephens, a spokesperson for Gov. John Bel Edwards, said the facility “had not lost all critical power.” She said some patients were moved to another part of the facility and the state health department was working with the hospital.
Ida struck as hospitals and their intensive care units were filled with patients from the fourth surge of the COVID-19 pandemic, sparked by the highly contagious delta variant and low vaccination rates across Louisiana.
The governor also said 22 nursing homes and 18 assisted living facilities have been evacuated, though evacuating the largest hospitals was not an option because there simply aren’t other places to send them. Anticipating that power could be out for weeks in places, Edwards said a big focus will be on making sure there is enough generator power and water at hospitals so they can keep up with vital patient needs such as providing oxygen or powering ventilators.
“I hate to say it this way, but we have a lot of people on ventilators today and they don’t work without electricity,” he said.” (B)
“Hospitals in New Orleans are bracing for public health emergencies on two fronts as Hurricane Ida threatens to strike at the same time as Louisiana is experiencing a surge in coronavirus cases.
New Orleans is in the midst of a “severe outbreak” with a seven-day average of 220 new infections, according to the city’s Covid-19 dashboard. Across the state, more than 3,400 new cases were confirmed as of Friday, according to the Louisiana Department of Health. At least 2,684 people are hospitalized in Louisiana with Covid-19.
Last month, Louisiana hit the biggest single-day increase of Covid-19 hospitalizations since the pandemic’s start with 6,800 new cases in a single day, the second-highest single-day case count since Jan. 6, 2021…
Health care systems across New Orleans are already putting contingency plans in place, including beefing up staffing, ordering a surplus of supplies and minimizing the amount of patients who are admitted to hospitals with non-emergencies.
“We definitely were in full disaster mode and getting ready for this storm,” said Warner Thomas, president and CEO of Ochsner Health.
Ochsner has ordered 10 days worth of food, medication and medical supplies, Thomas said…
At LCMC Health, the hospital will enter a “code gray lockdown” early Sunday morning and bring in additional staff to care for patients. Staff and patients will shelter in place but have not been ordered to evacuate yet, said Dr. Jeffrey Elder, medical director for emergency management.
Additionally, doctors and nurses have been told to prepare for a high volume of patients amid the Covid-19 surge.

“As Louisiana reels from a fourth COVID-19 wave — with the highest single-day cases since the pandemic began — hospitals in the state are preparing for yet another public health crisis with Hurricane Ida battering the coast. The Louisiana governor said this hurricane will be one of the strongest to hit the state in more than 150 years.
Gov. John Bel Edwards said evacuation of hospitals in threatened areas — something that would normally be considered — is impractical with COVID-19 patients.
“That isn’t possible. We don’t have any place to bring those patients. Not in state, not out of state,” Edwards said.
More than 2,600 COVID-19 patients are hospitalized across Louisiana, according to state data. The seven-day average has begun to decline in recent days, having reached nearly 2,700 hospitalizations — the peak from April 2020.
“We have been talking to hospitals to make sure that their generators are working, that they have way more water on hand than normal, that they have PPE on hand,” Edwards said.
Officials decided against evacuating New Orleans hospitals. There’s little room for their patients elsewhere, with hospitals from Texas to Florida already dealing with a spike in coronavirus patients, according to Dr. Jennifer Avengo, the city’s health director…
Some hospitals appeared to have evacuated their most critical patients ahead of the storm, as they prepared to lose power. According to The Advocate, the Ochsner Health System evacuated 17 of its most critically ill patients from three hospitals, with 100 patients remaining at those locations.” (D)

“Several hospitals in Florida, South Carolina, Texas and Louisiana are struggling with oxygen scarcity. Some are at risk of having to use their reserve supply or risk running out of oxygen imminently, according to state health officials and hospital consultants.
With the continued uptick in Covid-19 cases, there has been more demand on the oxygen supply, and hospitals cannot keep up the pace to meet those needs, Donna Cross, senior director of facilities and construction at Premier — a health care performance improvement company — told CNN.

“Normally, an oxygen tank would be about 90% full, and the suppliers would let them get down to a refill level of 30-40% left in their tank, giving them a three- to five-day cushion of supply,” said Cross. “What’s happening now is that hospitals are running down to about 10-20%, which is a one- to two-day supply on hand, before they’re getting backfilled.”
Even when they’re getting backfill, it’s only a partial supply of about 50%, Cross said. “It is very critical situation.”” (F)
“The Florida story is a cautionary tale for dealing with the current incarnation of the coronavirus. The United States has used the vaccines as its primary pandemic weapon. But Florida shows that even a state that made a major push for vaccinations — Florida ranks 21st among states and Washington, D.C., in giving people of all ages at least one shot — can be crushed by the Delta variant, reaching frightening levels of hospitalizations and deaths…
The best explanation of what has happened is that Florida’s vaccination rates were good, but not good enough for its demographics. It has so many older people that even vaccinating a vast majority of them left more than 800,000 unprotected. Vaccination rates among younger people were uneven, so clusters of people remained at risk. Previous virus waves, which were milder than in some other states, conferred only some natural immunity.

“On the ground floor of a parking garage at the University of Mississippi Medical Center, there are coronavirus patients where the cars should be — about 20 of them on any given day, laid up in air-conditioned tents and cared for by a team of medical personnel from a Christian charity group. Another garage nearby has been transformed into a staging area for a monoclonal antibody clinic for Covid-19 patients.
These scenes, unfolding in the heart of Mississippi’s capital city, are a clear indication that the health care system in the nation’s poorest state is close to buckling under the latest avalanche of cases triggered by the highly contagious Delta variant of the virus.
“We have reached a failure point,” LouAnn Woodward, the medical center’s top executive, said late last week. “The demand has exceeded our resources.”
The current coronavirus spike has hit the South hard, but a combination of poverty and politics made Mississippi uniquely unprepared to handle what is now the worst coronavirus outbreak in the nation. The state has fewer active physicians per capita than any other. Five rural hospitals have closed in the past decade, and 35 more are at imminent risk of closing, according to an assessment from a nonprofit health care quality agency. There are 2,000 fewer nurses in Mississippi today than there were at the beginning of the year, according to the state hospital association.
“If you look around, the state’s hospitals were in bad shape before there was such a thing as Covid,” said Marty Wiseman, an emeritus political science professor at Mississippi State University. “It was not a good time to layer a pandemic on top of that.” (H)

“At Children’s Hospital New Orleans, where the intensive care unit has been jammed with Covid-19 patients, scenes like this have played out unrelentingly over the past month. Nurses raced around monitoring one gut-wrenching case after another. One child was getting a complicated breathing treatment known as ECMO, a last resort after ventilators fail, which nurses said was almost unheard-of for pediatric cases. About a half-dozen others were in various stages of distress.
“We all thought, ‘Well, thankfully it’s not happening to the kids; none of us would be able to stomach that,’” said Mark Melancon, a longtime nurse at the hospital, recalling previous stages of the coronavirus pandemic. “Fast forward to now, and it’s happening with the kids.”
Mr. Melancon said that until this summer, the hospital had treated only one or two Covid-19 patients at a time. Now, a procession of patients require constant attention, their oxygen levels often dipping precariously low without maximum intervention.
As children’s hospitals in many parts of the United States admit more Covid-19 patients, a result of the highly contagious Delta variant, federal and state health officials are grappling with a sharp new concern: children not yet eligible for vaccination in places with substantial viral spread, now at higher risk of being infected than at any other time in the pandemic. Nowhere is that worry greater than in Louisiana, which has among the highest new daily case rates in the country and only 40 percent of people are fully vaccinated, putting children at particular risk as they return to school.
Most children with Covid-19 have only mild symptoms, and there is not enough evidence to conclude that Delta makes some of them sicker than other variants do, scientists say. Doctors and nurses at Children’s Hospital agreed with that assessment.
Theresa Sokol, Louisiana’s top epidemiologist, said that people younger than 18 had among the highest test positivity rates in the state and were responsible for a significant share of transmission, with many cases most likely undetected.
“So many days are filled with this puzzle of: We don’t have enough beds for this patient who wants to come, so how are we going to shuffle our children around to accommodate one more?” said Devon H. Relle, a pediatric nurse practitioner at Children’s Hospital New Orleans, where she worked the front desk of the 17-bed I.C.U. The hospital was also seeing an early, worrisome wave of respiratory syncytial virus, known as R.S.V., which can cause some of the same symptoms and was contributing to the overflow conditions.
The crush of Covid-19 at Children’s Hospital grew so intense this month that the state called in a federal “surge team” of emergency responders from the Department of Health and Human Services’s National Disaster Medical System. The group of about 14 included a physician, a nurse practitioner, nurses, paramedics, a respiratory therapist and a pharmacist.
The team was the first assigned to a children’s hospital during the pandemic.
“Covid-19 right now down here is so endemic that you don’t have to have a specific exposure, because it’s just out there,” said Anne Barylick, a nurse practitioner on the surge team who handled patient intake in the emergency department and Covid-19 units. “Statistically, you’re going to run into it.”” (J)

When I was appointed President and CEO of Jersey City Medical Center in1989 in the peak of the HIV/ AIDS crisis, we had a dedicated, and always full, dedicated 60 bed AIDS unit, staffed by one full time nurse and numerous part-time and per diem nurses. Today, right now, hospitals caught in the Covid-19 surge, are forced to redeploy nursing staff, respiratory therapists and other clinicians, most without prior infectious diseases experience. Why are we tolerating the unvaccinated putting our hospital staffs at risk?
