“Like the pandemic itself, the attention given to the origin of Covid-19 has come in waves. When SARS-CoV-2 first started to spread, there were immediate questions about how this virus infected humans for the first time.
Based on existing knowledge of other coronaviruses, most scientists backed the hypothesis that SARS-CoV-2 was spread through “natural spillover” — meaning that a virus was able to overcome the usual hurdles that prevent it from transmitting from one species to another (in this case, humans). The first confirmed cases of what we now know as Covid-19 were initially linked to markets in the Chinese city of Wuhan, where animals are sold both dead and alive.
There was also a competing theory: That the virus spread following an accidental release from a lab — specifically, the Wuhan Institute of Virology, where researchers study multiple strains of coronaviruses. Proponents of this explanation, including Trump, argue that it’s too much of a coincidence that the virus was first detected in markets in the same city that hosts a lab that studied the same type of virus. But, as the New York Times reports, “Wuhan is an urban center larger than New York City” with animal-market traffic from throughout China and the rest of the world — so it’s entirely possible that it is a coincidence.” (I)
The 120-page (WHO) report, released on March 30, 2021, was inconclusive about the origins of the virus. But of four scenarios, it said it is “likely to very likely” the virus spread from an animal to an intermediate host to humans. The report said a lab leak was “extremely unlikely” although it did not study this possibility enough. WHO Director General Tedros Adhanom Ghenreyesus said “as far as WHO is concerned, all hypotheses remain on the table”. Although the team concluded that a lab leak is the least likely hypothesis, he said “this requires further investigation, potentially with additional missions involving specialist experts, which I am prepared to deploy”.
The report caused concerns among most countries. The US and 13 other countries issued a statement at the World Health Assembly “expressing shared concerns regarding the recent WHO-convened study in China, while at the same time reinforcing the importance of working together toward the development and use of a swift, effective, transparent ,science-based, and independent process for in evaluations of such outbreaks of unknown origins in the future”. EU member states said there was a “need to further work to study the origins of the virus and its route of introduction to the human population”.
The 18 scientists who wrote to Science also criticised the WHO report for weighing on the side of a zoonotic spillover from an intermediate host without having studied the lab leak possibility adequately.” (J)
“In a statement, Mr. Biden made it clear that the C.I.A. and other intelligence agencies had not yet reached a consensus on how the virus, which prompted a pandemic and has killed almost 600,000 Americans, originated in China. He directed them to report back to him in 90 days….
“I have now asked the intelligence community to redouble their efforts to collect and analyze information that could bring us closer to a definitive conclusion,” the president said.
The president had asked in March for an internal assessment of the virus’s origins, which was delivered to him two weeks ago in his Presidential Daily Brief of intelligence, according to a senior White House official and an administration official. That started a discussion, the officials said, about declassifying some of the findings and having intelligence officials issue a public statement.
The statement — which described the lack of consensus among intelligence agencies — was ready this week, but Mr. Biden felt it would not help clarify the issue for the public, according to the senior official. And on Tuesday, China took a hard line against cooperating with the W.H.O. on further inquiries, which prompted Mr. Biden to press for a more robust United States investigation, the official said…
There was clear evidence for the coronavirus having emerged naturally through the recombination of genetic material from different bat coronaviruses, said Dr. Robert Garry, a virologist at the Tulane University School of Medicine. The data so far bears the hallmarks of natural recombination, he said, and no signs of human intervention.
“The pieces are out there,” Dr. Garry said. “These viruses mix bits and pieces all the time.”..
“It is most likely that this is a virus that arose naturally, but we cannot exclude the possibility of some kind of a lab accident,” Dr. Francis Collins, the director of the National Institutes of Health, told senators on Wednesday…
But the March report by the group of W.H.O.-chosen experts in collaboration with Chinese scientists, dismissing the possibility of a lab leak as “extremely unlikely,” compelled some scientists to speak out.
“When I read that, I was very frustrated,” said Akiko Iwasaki, an immunologist at Yale University. Along with Professor Lipsitch, she signed a letter published in the journal Science this month saying that there was not enough evidence to decide whether a natural origin or an accidental laboratory leak caused the coronavirus pandemic.
“I think it’s really an unanswered question that really needs more rigorous investigation,” Dr. Iwasaki added.
Uncertainty remains about the origins of the virus, which continues to cause havoc in countries like India. India’s official coronavirus statistics report about 27 million cases and over 300,000 deaths as of Tuesday.
From the earliest weeks of the outbreak, the Chinese government has worked to delay, deflect or block independent investigation of the virus’s origins.” (C)
“A US intelligence report found that several researchers at China’s Wuhan Institute of Virology fell ill in November 2019 and had to be hospitalized, a new detail about the severity of their symptoms. It’s not clear the researchers contracted Covid-19 and the lab strongly denied the report, calling it a lie to push the so-called lab-leak theory for the disease origin.

On Capitol Hill Tuesday, Dr. Anthony Fauci, the director of the National Institute of Allergy and Infectious Diseases, said it would have been a “dereliction of duty” not to fund earlier coronavirus research in bats in China.
“You don’t want to study bats in Fairfax County, Virginia, to find out what the animal-human interface is that might lead to a jumping of species,” Fauci said, adding the US had to go “where the action is.”
Separately, at the White House, Fauci said many scientists still believe the disease occurred naturally, but it’s also imperative to get to the bottom of it with more investigation.
An adviser for the World Health Organization, Jamie Metzl, said the lab-leak theory is possible while scientists were “poking and prodding and studying” viruses with the good intention of developing vaccines.
“Then I believe what possibly happened was there was an accidental leak followed by a criminal cover-up,” said Metzl, who served in the Clinton administration in the US Department of State and is a senior fellow at the Atlantic Council.
Fauci, the top infectious disease expert in the US, also said this week he is not convinced the disease occurred naturally and pushed for more investigation…
Dr. Paul Offit, an infectious disease expert at the University of Pennsylvania Perelman School of Medicine, told CNN’s Wolf Blitzer it was unlikely the Wuhan lab had manipulated the virus to make it more contagious using controversial gain of function research — but we’ve got to find out.
While Chinese authorities have stayed unwilling to allow such an open investigation, Offit said the world needs it in order to guard against another pandemic.
“What I do know is they have to allow this,” Offit said. “This is now the third pandemic strain that has raised its head in the last 20 years. The first was SARS 1, the second was MERS. I think that we can assume that we’re not done with this.”” (D)
“Consensus among top officials in the Biden administration is that the pandemic originated in one of two ways: the virus emerged from human contact with an infected animal; or, from a laboratory accident — diverging from the WHO-led team’s report this spring, which ruled the lab-leak theory “extremely unlikely.”..
“We know way too little about the who, what, where, when, how and why surrounding COVID-19 origins,” David Asher, a former State Department official who led the department’s investigation into the pandemic’s origins under the Trump administration, said at a private briefing to congressional offices this week obtained by ABC News. “I compare it to a Dali painting where the clocks have melted but time has not stood still.”
The long-awaited March WHO-led report came to no firm conclusions on COVID-19’s origins. Critics argued the Chinese government’s resistance to transparency muzzled a fair and independent investigation.
A WHO spokesperson said the group is “reviewing the recommendations” from the report, and preparing for the next phase of study.
But the WHO has a limited mandate, with no multilateral regulatory or subpoena power — leaving the global community to strategize.
“If China blocks that then the rest of the world will have no choice but to take an alternative route,” Metzl said.
There’s a delicate balance in the forensic work ahead, experts say.
“The availability of certain data sources might go away, telling antibodies for the virus can wane,” said Dr. John Brownstein, the Harvard Medical School professor who led the satellite research.
“But,” Brownstein, an ABC News contributor, added, “we’re now empowered with a new understanding of the virus — and we can leverage that context as we look back.”” (F)
“The intelligence community’s push to uncover the origins of the Covid-19 pandemic has largely relied on traditional intelligence-gathering tools up until now, but President Joe Biden’s 90-day “redoubled” effort is intended in part to elevate scientific analysis.
The effort will bring in the National Labs, a collection of 17 elite research facilities under the Energy Department, “because of their ability to crunch massive amounts of data,” a White House official told CNN.
What those “massive amounts of data” contain remains unclear, although multiple sources tell CNN it is almost certainly not a new tranche of intelligence information.
“There is really no new collection that the intelligence community can do [on the origin question] that I’m aware of,” said one US official.
But officials might not need to collect anything new to achieve a breakthrough. They may just need the right tools to more thoroughly analyze the mountains of intercepted communications and other signals intelligence they already have. The intelligence community routinely picks up far more raw intelligence than its analysts can comb through — “it is a perennial challenge that we collect more than we have analysts to process,” said one person familiar with the intelligence — suggesting that the National Labs’ supercomputers may be used to help sift through that data.
Even so, current and former intelligence officials have long been suspicious that traditional intelligence-gathering tools were going to uncover the origins of the pandemic. The answer, many suggest, is more likely to come from the academic community — within or without government — which has spent the last year carefully studying the structure of the virus to determine if it escaped from a lab or originated naturally in the wild.
Now, as the lab leak theory has begun to gain more mainstream acceptance in the academic community and pressure has risen on Capitol Hill to explore that possibility, the Biden administration has indicated that it wants to lean more heavily on traditional science to help come up with an answer.
“We want the science to be a big part of this,” the White House official said. “We are going to use the full resources of our intelligence and scientific community to try to get to the bottom of this.”
That may mean simply leveraging sophisticated data science tools to sift through reams of intercepted intelligence. But it may also mean empowering the traditional lab scientists within the intelligence community…
Up until now, one source familiar with the matter said, the intelligence community has been relying very heavily on intercepts to try to figure out the origins of the pandemic that to date has killed more than 3.5 million people worldwide.” (G)
“Pathogens escape from institutions working on them with depressing frequency. The last known death from smallpox was the result of a laboratory leak in Britain in 1978. sars-cov-1, the virus which causes sars, escaped from labs twice as it spread round the world in 2003, once in Singapore and once in Taiwan; it leaked out of a Beijing lab on two separate occasions in 2004. In December 2019 more than 100 students and staff at two agricultural research centres in Lanzhou were struck with an outbreak of brucellosis, a bacterial disease usually caught from livestock.
Most alarmingly, the h1n1 strain of influenza which started spreading around the world in 1977 is now known to have been released from a north-east Asian lab—possibly in China, possibly in Russia. Some Western observers suspected this at the time, but they made little fuss about it, perhaps afraid that doing so would lead to China and/or Russia pulling out of international flu-surveillance efforts, or spark a backlash against virology…
What of evidence from the spread of the disease? According to the Guardian, a British newspaper, when the who sent scientist Peter Ben Embarek to China in July 2020 his subsequent report to the agency stated that the Chinese had done “little…in terms of epidemiological investigations around Wuhan since January 2020”. Some infer that China is not looking because it knows, or perhaps just fears, the answer…
The evidence to date shows that the circumstantial assumptions on which the idea is based—that there was coronavirus research and that it could have leaked—are true; it does not provide direct insight into the outbreak proper. As Ralph Baric, an American researcher who helped set up the wiv’s coronavirus work, told the Wall Street Journal, “more investigation and transparency are needed to define the origin”; he himself continues to see zoonotic spillover as the more likely possibility.
Ideally, China would help such investigations unearth new evidence. That can hardly be counted on. It is possible that the dogged work of America’s intelligence services may turn up compelling arguments for or against regardless, or that the many scientists poring over details of the virus’s genome and structure may come up with something. But there is no guarantee that the question will be solved soon.
For observers such as Filippa Lentzos, a biosecurity expert at King’s College London, the uncertainty underlines the need for more discussion about the risks that the world is willing to take in the name of science. More facilities for pathogen research are being built around the world, and even the most sophisticated biosecurity measures may sometimes leak.
That means the research needs to be carried out in ways that allow scrutiny and accountability, that the knowledge sought needs to be worth the risks, and that that knowledge, once gained, should be used and made useful. There is no compelling evidence that the presence of the wiv in the city where the covid-19 pandemic began was anything other than a coincidence. But neither is there evidence that the wiv’s coronavirus research, justified in the name of pandemic preparedness, did anything to lessen this pandemic’s toll.” (H)
“At the end of this process, I would not be surprised if we did not know much more than we know now,” said W. Ian Lipkin, a virologist at the Mailman School of Public Health at Columbia University who was one of the first U.S. scientists to visit China in early 2020 and consult with public health authorities there…
“There was no evidence to suggest this had been weaponized,” Dr. Lipkin said. “I haven’t changed my view on that.”..
Robert Garry, a virologist at Tulane Medical School and a co-author of Dr. Lipkin’s letter, observed that Chinese scientists would have to have collected SARS-CoV-2 and then grown it in a colony of cells, but somehow never publish any details of it even as they published reports on other coronaviruses for years.
“It makes no sense to me. Why did they hold onto the virus?” Dr. Garry said…
Speaking recently to the former New York Times journalist Donald McNeil Jr., Dr. Lipkin said he was dismayed to learn of two coronavirus studies from the Wuhan Institute of Virology that had been carried out with only a modest level of safety measures, known as BSL-2.
In an interview with The Times, Dr. Lipkin said this fact wasn’t proof in itself that SARS-CoV-2 spread from the lab. “But it certainly does raise the possibility that must be considered,” he said.
A BSL-2 level of research would also add to the difficulty researchers will face trying to pin down clear evidence that a coronavirus infected the staff. At higher levels of security, staff regularly give blood samples that can be studied later for genetic material from viruses and antibodies against them. There may be no such record for SARS-CoV-2.
A number of researchers remain unpersuaded by the possibility of a lab leak. “This pathway remains the least likely of all for the origin of COVID-19,” said Peter Daszak, a virologist at EcoHealth Alliance who has worked frequently with the Wuhan virologists.
“On the other hand, there is a great deal of evidence that these viruses originate in wildlife, and have previously and repeatedly infected people who are highly exposed to these animals, work in the wildlife trade or in wildlife farms and markets,” Dr. Daszak said.
Earlier this month, Dr. Garry of Tulane argued that the genetic variations in early cases of Covid-19 in Wuhan could be explained by wild animals being brought to animal markets in the city. “If you suppose that the viruses came in through the wildlife trade, then it’s pretty simple and straightforward,” he said.
Even if SARS-CoV-2 jumped from bats or other animals to humans outside of a lab, as Dr. Garry suspects, it will be hard to find airtight proof for that pathway. When animals die, they take their viruses with them.
While scientists have fairly good evidence for how two coronaviruses — the cause of SARS and MERS — jumped from bats to humans, the origins of the other four coronaviruses that infect humans remain a mystery.
“Sometimes you get lucky, sometimes you don’t,” Dr. Garry said. “It can take decades.” (E)
“Like most scientists, Dr. Peter Hotez, co-director of the Center for Vaccine Development at Texas Children’s Hospital and dean of the National School of Tropical Medicine at Baylor College of Medicine, continues to “favor the natural-origins hypothesis,” based on the evidence that is currently publicly available. “There’s no smoking gun saying that it either was synthetically made in the laboratory in any way, or leaked — and it’s certainly possible — but I don’t see anything that’s really compelling that makes me think that,” he explains.
In fact, Hotez, author of Preventing the Next Pandemic: Vaccine Diplomacy in a Time of Anti-science, says that the only way we’ll have any answers to our questions about the origins of Covid-19 “is through a pretty extensive on-the-ground investigation.” Along the same lines, Adalja — who has supported doing a thorough and independent review of the lab-leak hypothesis since the pandemic began — says that it would have a significant value from a public health standpoint.
“We still don’t understand how [SARS-CoV-2] made its way from bats into some intermediate animal, or into humans, and I think unraveling that is really important,” he explains “Once people realized the first SARS came from palm civet cats, it basically disappeared, because they started handling those animals very differently. So, from that sense, I think that there is a public health importance to understanding the origin of the virus and understanding viral emergence.” (I)
“A 41-year-old man in China’s eastern province of Jiangsu has been confirmed as the first human case of infection with a rare strain of bird flu known as H10N3, Beijing’s National Health Commission (NHC) said on Tuesday.
Many different strains of bird flu are present in China and some sporadically infect people, usually those working with poultry. There is no indication that H10N3 can spread easily in humans.
The man, a resident of the city of Zhenjiang, was hospitalized on April 28 and diagnosed with H10N3 on May 28, the health commission said. It did not give details on how the man was infected….
The World Health Organization (WHO), in a reply to Reuters in Geneva, said: “The source of the patient’s exposure to the H10N3 virus is not known at this time, and no other cases were found in emergency surveillance among the local population. At this time, there is no indication of human-to-human transmission.
“As long as avian influenza viruses circulate in poultry, sporadic infection of avian influenza in humans is not surprising, which is a vivid reminder that the threat of an influenza pandemic is persistent,” the WHO added.” (A)

As variants of the novel coronavirus continue to spread and mutate, researchers are monitoring how the vaccines perform and whether booster shots will be needed to maintain meaningful immunity.
Right now, experts say it’s too early to speculate whether we’ll need booster shots like some routine vaccines.
Dr. Amesh Adalja, a senior scholar at Johns Hopkins Center for Health Security in Baltimore, says it’s premature to predict whether COVID-19 boosters will be needed and, if so, at what intervals.
“To me, the threshold for boosters would be to see fully vaccinated individuals getting breakthrough infection severe enough to land them in the hospital,” Adalja told Healthline. “We have not crossed that threshold.”…(L)
“Leading infectious disease expert Anthony Fauci on Wednesday said that he believes recipients of the coronavirus vaccine will need a booster shot, but he declined to say when.
“I don’t anticipate that the durability of the vaccine protection is going to be infinite,” Fauci told a Senate subcommittee. “It’s just not. So I would imagine we will need, at some time, a booster.”
He said the interval of protection from the vaccines is unknown but that it lasts at least six months and likely a year.
Fauci said that he is “not exactly sure when” a booster would be needed but that research into vaccine durability is ongoing.” (A)
“The U.S. government is planning for the potential need for Covid-19 vaccine booster shots “just in case,” the head of the Centers for Disease Control and Prevention told CNBC.
CDC Director Dr. Rochelle Walensky didn’t say what those plans were. However, should Americans require booster shots, the U.S. would likely need to make arrangements with drugmakers to supply additional doses and make plans for vaccine distribution.
Walensky told CNBC that U.S. health officials are “thinking ahead” in the event that vaccine-induced immunity against the coronavirus begins to wane. Officials still don’t know if that will happen or if booster shots will be needed, “but we are planning for it just in case,” she said.
“Right now, if you have two doses of the mRNA vaccines, you are protected,” Walensky said in comments that aired Tuesday during CNBC’s Healthy Returns Summit.
“What we’re talking about is thinking ahead,” she said in the interview with CNBC’s Meg Tirrell. “What happens if in a year from now or 18 months from now your immunity wanes? That’s really our job is to hope for the best and plan for what might happen if we need further boosters in the future, the way we get flu vaccine boosters every year.”
Walensky said scientists are currently looking at the immunity of a small population of people who were vaccinated months ago. However, she said, she doesn’t expect scientists will do mass testing looking for reduced immunity against the virus….
Walensky told CNBC scientists still don’t know whether the coronavirus is seasonal with new infections slowing down during warm summer months and speeding up in the winter.
Some scientists predict states will see repeated outbreaks when the U.S. enters its colder months.
“We have other coronaviruses be seasonal viruses and yet this coronavirus has not proven to be seasonal,” she said, adding that the U.S. witnessed a surge in cases last summer.” (B)
“Pfizer announced Monday morning that it had started testing a COVID-19 booster shot — a third dose the company’s CEO said fully vaccinated people will likely need in another 12 months.
The goal of the study is to see whether another vaccine can be taken along with a booster dose of Pfizer’s COVID-19 vaccine. In the study, participants will get both the booster shot and a pneumonia vaccine.
“To see if they’re able to tolerate it, see if that in any way adversely affects their development of antibodies against COVID-19 and see how the immune reaction is, you know, the side effect profile of the combination,” Joshi explained. “And if that looks good, maybe then consider combining it with seasonal flu vaccine as well.” (C)
“A large body of research on the coronavirus now indicates that both natural infection and vaccines stimulate the production of these long-lasting memory cells, which means that boosters should not be needed any time soon to conquer covid-19.
We already knew mRNA vaccines — like the Moderna and Pfizer vaccines being widely administered against the coronavirus — form memory B cells. That finding was bolstered in a big way Monday, when researchers reported finding memory B cells in the bone marrow of people who had experienced even mild cases of covid-19. Another study of recovered covid-19 patients found the development of memory T cells comparable to those generated by yellow fever vaccination, which usually needs just one dose over a person’s lifetime. And a 2020 study showed that T cells generated among patients who survived the SARS pandemic of 2003, which is caused by a similar virus, were still going strong 17 years later, indicating long-lasting immunity…
The talk of boosters is not only distracting from the brighter and brighter light we are seeing at the end of our covid-19 nightmare in the United States and the start of normal life, it also distracts us from the most pressing problem of our day, which is to achieve global vaccine equity. Unfortunately, pharmaceutical company executives have suggested that boosters could be necessary for coronavirus vaccines as soon as this fall. Pouring effort into developing boosters — Pfizer has begun studying a modified third booster shot to follow its initial two-shot regiment — threatens to distract from production of adequate supplies of vaccine for the world. Moreover, such messaging of perpetual boosters could also dissuade some people from getting the vaccine altogether.
The pandemic is not over until it is over for all of us. We also have a moral and ethical obligation based on our interconnectedness to vaccinate the planet. Let’s cease talk that boosters will be needed in the near future and get vaccines out to the world.” (D)

That means people who have not had Covid-19 and have been immunized may eventually need a booster shot, Dr. Nussenzweig said. “That’s the kind of thing that we will know very, very soon,” he said….
The results of Dr. Nussenzweig’s study suggest that people who have recovered from Covid-19 and who have later been vaccinated will continue to have extremely high levels of protection against emerging variants, even without receiving a vaccine booster down the line.
“It kind of looks exactly like what we would hope a good memory B cell response would look like,” said Marion Pepper, an immunologist at the University of Washington in Seattle who was not involved in the new research.
The experts all agreed that immunity is likely to play out very differently in people who have never had Covid-19. Fighting a live virus is different from responding to a single viral protein introduced by a vaccine. And in those who had Covid-19, the initial immune response had time to mature over six to 12 months before being challenged by the vaccine.
“Those kinetics are different than someone who got immunized and then gets immunized again three weeks later,” Dr. Pepper said. “That’s not to say that they might not have as broad a response, but it could be very different.” (E)

Vaccine supplies need to be diverted now to where the crisis is the worst, if necessary away from the wealthy countries that have purchased most of the supply. It is, of course, understandable that every nation wants to vaccinate its own first, but a country with high levels of vaccination, especially among its more vulnerable populations, can hold things off, especially if they also had big outbreaks before. In addition, excess stockpiles can go where they are needed without even slowing down existing vaccination programs…
The kind of catastrophic outbreak like the one in India can cause many more needless deaths simply by overwhelming our resources. Already, there are reports that countries ranging from Nepal to the Philippines to South Africa to Nigeria may face supplemental oxygen shortages of the kind seen in India. This pathogen has one fatality rate when oxygen is available as a therapy and one when it is not, and it would be an unspeakable tragedy to suffer the latter in the second year of the pandemic.
Like all pandemics, this one will end either with millions — maybe billions — being infected or being vaccinated. This time, world leaders have a choice, but little time to make that choice before it is made for them.” (G)

Many LMICs do not have an established platform for vaccinating their adult populations.10 Although it is feasible to deliver COVID-19 vaccines to health-care and other front-line essential workers, in some LMICs it will be difficult to effectively reach and vaccinate with two doses all elderly populations and individuals with co-morbidities, given insufficient mechanisms to identify such groups. Governments and technical leaders will need to use transparent, accountable, and unbiased processes when they make and explain evidence-based vaccine prioritisation decisions, while also building confidence in COVID-19 vaccines and engaging with all the stakeholders.
The ultracold chain requirements of mRNA COVID-19 vaccines are likely to be an insurmountable hurdle in LMICs, outside of major cities. COVID-19 vaccine delivery will require considerable investment of resources, health-care staff, and careful planning to avoid opportunity costs, including a disruption of routine health services and a decline in essential childhood vaccination coverage, which could result in outbreaks of measles and other vaccine-preventable diseases…
There are further challenges. Governments in LMICs with strong private health sectors, as those in high-income countries, will need to manage the inherent potential for inequity, whereby the rich could access COVID-19 vaccines before individuals with less access to private care who may be at increased risk of severe disease and death, such as older people and those with comorbidities. LMICs affected by war, civil conflict, economic crises, or natural disasters, or with large refugee populations or populations with special needs or vulnerabilities need additional support for vaccines and vaccination under extremely difficult operational conditions.” (H)
“In January, the director-general of the World Health Organization (WHO), Tedros Adhanom Ghebreyesus, issued a blunt warning. The world was “on the brink of a catastrophic moral failure,” he said. Wealthy countries were buying up available COVID-19 vaccines, leaving tiny amounts for others—a replay of what happened during the 2009 influenza pandemic. “The price of this failure will be paid with lives and livelihoods in the world’s poorest countries,” Tedros said.
He was right. Today, some rich countries are vaccinating children as young as 12 years old, who are at extremely low risk of developing severe COVID-19, while poorer countries don’t even have enough shots for health care workers. Nearly 85% of the COVID-19 vaccine doses administered to date have gone to people in high-income and upper middle–income countries. The countries with the lowest gross domestic product per capita only have 0.3%.
Tedros lambasted the “scandalous inequity” again in his opening speech at the World Health Assembly on 24 May. By September, at least 10% of the population in every country should be vaccinated, he said.
Disparities in global health are nothing new. Lifesaving therapies such as monoclonal antibodies are unavailable in large parts of the world. Even vaccines and drugs that cost pennies to make don’t reach millions of people who need them. But the COVID-19 crisis has exposed the inequities in a distinct, acute way. As normality is returning to vaccine front-runners such as Israel, the United Kingdom, and the United States, India’s health system is buckling under soaring case numbers—and the world is still recording almost 5 million cases and more than 80,000 deaths every week.
The moral argument aside, there’s a very practical reason to try to distribute vaccines more equitably: No part of the world can feel safe if the pandemic rages on elsewhere, posing the risk of reintroduction and spawning potentially more dangerous viral mutants.” (I)

Using publicly available figures from Our World in Data, The Washington Post found that 45 percent of all vaccine doses administered so far have gone to just 16 percent of the world’s population in what the World Bank considers high-income countries.
Through the summer and fall of last year, wealthy nations cut deals directly with vaccine-makers, buying up a disproportionate share of early doses — and undermining a World Health Organization-backed effort, called Covax, to equitably distribute shots.
So now, in a small number of relatively wealthy nations, including the United States, doses are relatively plentiful and mass immunization campaigns are progressing apace. But much of the world is still struggling to secure enough supply. For many, herd immunity is many months — if not years — away, which could extend the crisis.
A team at Duke University’s Global Health Innovation Center recently found that high-income countries locked up 53 percent of near-term vaccine supply. They estimate that the world’s poorest 92 countries will not be able to reach a vaccination rate of 60 percent of their populations until 2023 or later.” (J)

“U.S. health officials and the State Department on Monday warned Americans against travel to Japan because of a surge in coronavirus cases in the country, which is preparing to host the Olympics in just two months.
The twin alerts don’t ban U.S. citizens from visiting the country, but they could have an impact on insurance rates for travelers and may factor into decisions by Olympic athletes and spectators on whether to compete in or attend the games, which are due to start in July. There was no immediate indication as to what effect the warnings might have on would-be Olympic-goers.
“Travelers should avoid all travel to Japan,” the Atlanta-based Centers for Disease Control and Prevention said in a new Covid-19 update. “Because of the current situation in Japan even fully vaccinated travelers may be at risk for getting and spreading COVID-19 variants and should avoid all travel to Japan.”
The State Department’s warning, which followed the CDC alert, was more blunt. “Do not travel to Japan due to Covid-19,” it said in the announcement, which raised the department’s travel alert from Level 3 — Reconsider travel — to Level 4 — Do not travel. The previous alert was issued on April 21.
The United States Olympic & Paralympic Committee said it still anticipates that American athletes will be able to safely compete at the Tokyo Games.
“We feel confident that the current mitigation practices in place for athletes and staff by both the USOPC and the Tokyo Organizing Committee, coupled with the testing before travel, on arrival in Japan, and during Games time, will allow for safe participation of Team USA athletes this summer,” the committee said in a statement Monday.” (A)
“The Japanese government Tuesday was quick to deny a U.S. warning for Americans to avoid traveling to Japan would have an impact on Olympians wanting to compete in the postponed Tokyo Games…
Most metro areas in Japan are under a state of emergency and expected to remain so through mid-June because of rising serious COVID-19 cases that are putting pressure on the country’s medical care systems. That raises concern about how the country could cope with the arrival of tens of thousands of Olympic participants if its hospitals remain stressed and little of its population is vaccinated.
Chief Cabinet Secretary Katsunobu Kato said at a news conference Tuesday that the warning does not prohibit essential travel and Japan believes the U.S. support for Tokyo’s effort to hold the Olympics is unchanged.
“We believe there is no change to the U.S. position supporting the Japanese government’s determination to achieve the games,” Kato said, adding that Washington has told Tokyo the travel warning is not related to the participation of the U.S. Olympic team…
Fans coming from abroad were banned from the Tokyo Olympics months ago, but athletes, families, sporting officials from around the world and other stakeholders still amount to a mass influx of international travelers. “The International Olympic Committee this week unveiled updated plans for holding the Summer Games in Japan, meticulous “playbooks” that promise to ensure the safety not only of the athletes taking part but also of the Japanese public during a global pandemic.
They represent “very hard work” and “determination, a lot of determination” to stage the Games safely and successfully, the IOC’s sports director, Christophe Dubi, said at a news conference, and they are based on the latest scientific expertise as well as considerable experience with holding global sporting events during the pandemic. The meticulous planning, meanwhile, has one potentially fatal flaw: Japan’s already overloaded health system can’t cope with the additional demands the Games will bring without putting more lives at risk, doctors and nurses say. The Olympics would involve more than 11,000 athletes plus tens of thousands of officials, coaches, media members and support staffers converging on Tokyo, which is closed to most foreign visitors.
“Most health workers say even thinking about the Olympics is just ridiculous,” said Kentaro Iwata, an infectious-disease expert and doctor at Kobe University Hospital, which is in a city in western Japan where 1,700 people with covid-19 need hospitalization but can’t get a bed.

The daily rate of new infections approached 6,000, the highest level in three months. On the day the Olympics were postponed, March 24, 2020, Japan recorded just 65 new infections…
On Wednesday, health minister Norihisa Tamura warned Olympic organizers they would have to “secure their own” hospital beds for anyone falling ill at the Games, explaining the government would not release beds set aside for Japanese covid-19 patients…
Vaccines will not be mandatory, but most athletes and officials probably will be vaccinated. Yet within Japan, just 3.2 million vaccine doses have been delivered, the equivalent of two shots for just 1.3 percent of the population, reflecting a combination of the country’s slow efforts to develop its own vaccine or source one from others and skepticism among many citizens about foreign drugs.
The Games organizers have asked the Japanese Nursing Association for 500 nurses to help out during the 17-day event. On social media, nurses soon began tweeting under the hashtag “nurses opposed to being dispatched to the Olympics,” arguing they had to prioritize existing patients. The hashtag attracted nearly 200,000 retweets within days.
“We are not disposable pawns,” one nurse wrote on a placard that she held up in a tweet.
Others have taken to the streets, asking why IOC officials will get regular testing for the coronavirus while many health workers still can’t access tests.
Nurse Konoka Shibata, 25, said it was “unreasonable” to expect nurses to volunteer for the Games, putting themselves at additional risk when many hospitals are short-staffed and struggling to cope with the pandemic. Kobe University’s Iwata said he also is concerned that delegations from Brazil and India could bring more dangerous variants to Japan.” (C)
“A senior member of the International Olympic Committee has said the Tokyo 2020 Games can go ahead even if the host city is under a state of emergency because of the coronavirus.
“All the measures we are undertaking will ensure a safe Games regardless of whether there is a state of emergency or not,” John Coates, an IOC vice-president who is in charge of preparations, told reporters after a virtual meeting with organisers on Friday.
“Provided that we can protect the Japanese public, the most important thing is giving athletes a chance to compete.”
The IOC and Tokyo 2020 organisers have insisted the Games will go ahead, despite widespread public opposition in Japan and warnings from health experts that the arrival of tens of thousands of athletes, officials, journalists and support staff this summer risked spreading Covid-19.
Japanese doctors and medical workers have called for the Games to be cancelled to relieve pressure on overwhelmed health services, while a recent poll found that 83% of the public want the event to be either called off or postponed for a second time.
While most athletes are expected to be fully vaccinated by the time the Games open in two months’ time, only 4.1% of Japan’s population has received at least one jab – the lowest rate of any advanced economy – and only 30% of medical workers in Tokyo are fully protected, the Nikkei business newspaper said.
Seiko Hashimoto, the president of Tokyo 2020, said up to 230 doctors and 310 nurses would be needed each day during the Olympics and the Paralympics. The number of officials, journalists and other Games-related staff due to arrive in Tokyo has been cut from 180,000 to 78,000, she added.
“I can say it’s now clearer than ever that these Games would be safe for everyone participating and, importantly, safe for the people of Japan,” Coates said, adding that the IOC was working with organisers to send medical personnel to Tokyo. “After eight years of hard work and planning, the finish line is within touching distance.”…
On Friday, Shigeru Omi, who heads the government’s coronavirus advisory panel, told MPs that organisers must consider the impact the Games could have on Japan’s medical infrastructure before deciding whether to go ahead.” (D)

The snap decisions don’t always make sense and by their nature discriminate against someone.
The International Olympic Committee has asked all accredited personnel heading to the games to sign a waiver removing all liability from the IOC, the International Paralympic Committee, and the Tokyo organisers, despite them not yet being able to provide definitive countermeasures as the existing precautions may still change.
The Olympic and Paralympic Games Tokyo 2020 Addendum distributed this week states:
“To the fullest extent admissible under applicable laws, the Responsible Organisation agrees to irrevocably release Tokyo 2020, the IOC, the IPC (and their respective members, directors, officers, employees, volunteers, contractors or agents) from any liability for any kind of loss, injury or damage that the Responsible Organisation, including any Accredited Person under its responsibility, may suffer or be exposed to in connection with their participation in the Games.”
On top of this athletes must sign a hard copy of a ‘written pledge’ and present it to immigration officials on arrival guaranteeing they will follow all the countermeasures despite not knowing the final requirements…

“Organizers are relying on a series of six playbooks of rules that detail how participants of the Olympics and Paralympics can compete, move around and socialize, in order to manage the risks of what will be the world’s biggest COVID-19 bubble. While those involved in the games will be somewhat isolated from the Japanese public, Tokyo has ruled out using two core tenets of containment: quarantines and vaccinations. Without those, experts say infections could spread.
If it does, not only could the Olympics become the site of a sizable outbreak that spreads into Japan, it could become a cauldron of novel variants gathered from around the world. The risk is that athletes could bring them home, potentially fueling the pandemic.
While Japan has seen much lower transmission rates compared with other rich nations, its vaccination campaign is only just starting, months behind places like the United Kingdom, the United States and even other parts of Asia. There are still many unknowns, including exactly how many people will be coming into Japan, and organizers haven’t decided yet on how many domestic spectators will be allowed into venues.
Depending on how infection rates develop, there’s still a possibility that more stringent measures will be adopted, with the final version of the playbooks due in June.
“The situation surrounding the coronavirus is constantly changing, and it’s our hope that the efforts of the government, the city of Tokyo and other stakeholders will help to mitigate spread of infections,” Tokyo 2020 said in an emailed statement.
Sporting events have gone on around the world this past year — with both success stories and cautionary tales.
The Olympic organizers are set to emulate the NBA, which saw no infections during its three-month run in the summer and fall of 2020. But the NBA bubble saw just a few hundred athletes cordoned off together at the Walt Disney World Resort in Orlando, Florida. Including staff and coaches, there were fewer than 1,000 people involved…
The extent of the risk also varies depending on the sport. A study of the NFL’s latest season in the U.S. found that players didn’t transmit the virus during game play, while matches for a high school wrestling tournament turned into deadly superspreading events. The Olympics will feature 33 sports across 42 venues across Japan.
Further complicating the task is the social nature of the Olympics. Places such as the Athlete’s Village were designed to have people meet and socialize. Although long conversations and collective meals will be off limits, how those rules will be enforced is unclear. Some of the athletes are teenagers and the average age of an Olympian is usually in the 20s — groups where virus spread has been harder to control…
It’s not just the athletes. The games will require thousands of volunteers to help the events run smoothly, as well as local staff who will need to go in and out of the Olympic bubble regularly to do things such as cook meals, clean facilities and run the proceedings. It’s not clear how such staffers — which the Olympic Organising Committee and Tokyo Metropolitan Government say will probably number more than 150,000 — will be handled, and the playbooks don’t offer explicit instructions.
Even in normal times, disease outbreaks are common at the Olympics. During the Winter Games in Pyeongchang in 2018, around 200 athletes caught norovirus. Two years earlier, the Rio Summer Olympics were held amid the specter of Zika. More than 300 athletes caught respiratory illnesses out of 10,568 competing in the London Olympics in 2012.” (G)
“Is there a level playing field for all nations expected to attend? To date, more than 458 million vaccine doses have been administrated worldwide. And while this is good news, there is a lack of global equitable access to vaccines, and even within countries, not everyone is being included in national vaccination plans.
COVAX is currently the only initiative working to deliver vaccines equitably to 92 lower-income countries. The World Health Organization argues that there will be “no end to the pandemic without equitable distribution.”
According to the IOC playbooks, being vaccinated is not a requirement for anyone attending the Games. IOC member Dick Pound suggested athletes must be given priority access to the coronavirus vaccine. IOC President Thomas Bach, however, argued that the IOC is not in favour of athletes jumping the queue and it was to be determined by governments.
Some countries — like Lithuania, Hungary, Serbia, Israel, Mexico and Russia — have already announced commitments to allowing their athletes priority or voluntary access to the vaccine. Other countries — like Canada, Germany, Britain and Italy — have announced vaccines will be given to the most vulnerable populations first.
The Chinese Olympic Committee has even gone so far as to offering to vaccinate Tokyo 2020 and Beijing 2022 athletes in territories that have approved the Chinese vaccine for use. So access for athletes is unevenly distributed…
Money over health?
But Japan is currently behind on vaccinations compared to other countries since it only started in February 2021. According to survey by Kyodo News, 80 per cent of Japanese residents would rather see the Games cancelled or further delayed.
Last April, the IOC argued that the Games could not be delayed to 2022 as the Japanese partners and the organizing committee could not manage it. This was due to the availability of the Olympic Village, sports venues and the need for people to carry on working.
The decision to continue with the Games during a global pandemic makes it clear that finances are driving decisions, not the health of those involved.
The Olympic Charter proclaims that the Games are about “promoting a peaceful society concerned with the preservation of human dignity.” The question becomes: who will be the voice of reason?” (H)
Japan wants to try to create a sort of athlete “bubble.” Is that enough?
Compared to last year when the Tokyo Games were postponed, the world knows a lot more about how to mitigate the spread of Covid-19. Large sporting events have since gone forward, from the NBA to the Australian Open to the Super Bowl.
But the Olympics are another feat entirely. As Lee Igel, a clinical professor at the NYU Tisch Institute for Global Sport, said, they’re really more a festival than an athletic competition — which means they have different challenges, and a lot more people.
Mass gatherings like the Olympics often present public health challenges (see: free condoms in the Olympic Villages) simply because you’re bringing a lot of people from around the world and packing them all in together.
Some of the festival atmosphere isn’t going to exist with Covid-19. But even without the influx of foreign fans this year, thousands of athletes and all their coaches and support staff, and global media and all their crews, will be arriving in Tokyo. They’ll be flying in from all over the world, some from places experiencing severe outbreaks. And they’ll arrive in Japan where, right now, cases are close to their January peak and the country’s fully vaccinated rate is around 1 percent.
“Is that a set of circumstances where you open a country to travelers bringing new variants from, well, everywhere?” Amir Attaran, professor of law, epidemiology, and public health at the University of Ottawa, wrote in an email.
Once the games are over, all those people will be flying back home, too. If there’s a coronavirus outbreak among athletes, or exposure, they could bring those cases back home. In other words, it could become a superspreader event….
The NBA bubble was successful, with zero positive Covid-19 tests. But the Olympics bubble can’t replicate that experience exactly. For one, NBA players and coaches were basically full-time quarantined in the Walt Disney World Resort in Orlando.
According to Bloomberg, just about 1,000 people were involved in that bubble, compared to thousands and thousands more — from all over the world — expected to attend the Olympics. Plus, the Olympics are not going to mandate that everyone quarantine, and though athletes will be discouraged from leaving the campus, they won’t be walled off from the rest of Tokyo.
The Olympics also won’t mandate that athletes be vaccinated to participate in the games, which raises the issue of whether athletes can or should get a vaccine before the Olympics.
Athletes, coaches, and media in places like the United States, which is approaching a vaccine glut, will likely have no trouble getting a shot. The US Olympic and Paralympic Committee is encouraging all of its athletes to get vaccinated, though it is not mandating it.
Other countries, like Australia and Italy, are also vaccinating their Olympic teams. Pfizer and BioNTech announced earlier this month that they reached an agreement with the IOC to distribute vaccine doses to athletes and delegates from participating countries, starting in late May to allow the two-dose regimen to be completed before the games.
But it’s unlikely everyone making the journey will be vaccinated. And there is a global shortage of vaccines, with lower-income countries lacking enough doses to inoculate their health care workers or their vulnerable older populations.
“We’re still in a global pandemic with cases higher than they’ve ever been,” said Krutika Kuppalli, an assistant professor with the division of infectious diseases at the Medical University of South Carolina. “Is it appropriate for people from Brazil to go and compete in the Olympics when you have thousands of people dying?”
Kuppalli added that she understood that so many of these athletes have a life-long dream at stake, but that the world is still very much facing a global emergency. Putting resources and investment into an athletic competition, as exciting and important as it is, seems a bit misplaced when countries in need don’t have enough vaccines or oxygen concentrators.
It makes the Olympics, a celebration of international sport and cooperation, seem a bit hollow, whether or not they go forward.” (H)
“The time has come to press pause and reimagine the Olympics. It might even be time, I’ve come to believe, for the entire endeavor to close down for good.
What say you?
First, consider the near term.
In July, yet another wildly overbudget Summer Games, originally slated for 2020 but postponed because of the pandemic, will begin in Tokyo.
The timing remains awful.
Japan has worked hard to tamp down the coronavirus, but now cases are creeping up, and the nation’s vaccination rate is lagging. Organizers just rerouted the torch relay planned this week to reach the streets of Osaka, where one health official said the spread of new variants had pushed the medical system to “the verge of collapse.”
Into this troubled environment, 11,000 athletes from all corners of the globe will descend, along with coaches, officials, Olympic support staff, media workers and more. The Tokyo Games could end up being a three-week superspreader event that leads to death and illness across Japan and far beyond.
The Japanese public has grown wise to the health risk. It is also aware of the estimates that show the cost for the Games has swollen to a record $15.4 billion, up $3 billion in the last year alone. Recent surveys show close to 80 percent of Japanese say the Games should be postponed again or canceled.” (I)
“One of the latest warnings came Tuesday in an article published by The New England Journal of Medicine, in which a group of public health specialists harshly criticized the International Olympic Committee’s so-called playbooks — the colorfully illustrated packets detailing countermeasures designed to keep athletes, other Olympic visitors and the broader Japanese population safe from the virus.
In the article, the specialists suggested the playbooks — created by the I.O.C. in consultation with the World Health Organization — lacked transparency, were insufficiently detailed, and focused at times on science that was outdated or irrelevant to ensuring the event could be safe. The authors stopped short of arguing for a cancellation of the Games, but said the risk was too high to ignore.
“All along the way there’s been an ignorance of science,” Dr. Annie Sparrow, a global health specialist and the lead author of the article, said of the I.O.C. in an interview. “It’s not rocket science to hold a safe Olympics. It’s basic medical science. But that’s what the I.O.C. has ignored, and I don’t know if they’re going to start paying attention now.”…
The I.O.C., which will publish the third and final iteration of its playbook next month, has trumpeted the partnership it established last year with the W.H.O. in an attempt to build confidence in the viability of its health protocols.
But Sparrow, a former adviser to Dr. Tedros Adhanom Ghebreyesus, the director-general of the W.H.O., and her three co-authors urged the health organization’s membership to convene emergency committee meetings before the Tokyo Games, just as it did before the 2016 Rio Olympics amid widespread outbreaks of the Zika virus…
Among other things, they criticized the playbooks’ focus on countermeasures like disinfecting surfaces and temperature checks, which they called ineffective; the lack of specialized guidelines for different competition venues, common spaces and sports; the use of contact-tracing and health-reporting apps, which they called inferior to wearable trackers; and the possibility that athletes were at a higher risk of infection by sharing rooms in the athletes’ village.
The authors also laid out in stark terms how little the landscape had improved in the past year.
“When the I.O.C. postponed the Tokyo Olympics in March 2020, Japan had 865 active cases of Covid-19 against a global backdrop of 385,000 active cases,” the article said, noting that many people had assumed the pandemic would be under control by 2021. “Fourteen months later, Japan is in a state of emergency, with 70,000 active cases. Globally, there are 19 million active cases.”
Such troubling numbers were behind the State Department’s decision on Monday to warn Americans not to travel to Japan. (The recommendation was mostly symbolic; nonresident foreigners have largely been banned from entering Japan since the middle of last year.)
Still, the alarming advisory — Level 4 is the highest possible warning — did not immediately change the United States Olympic and Paralympic Committee’s outlook on attending the Games.
“We feel confident that the current mitigation practices in place for athletes and staff by both the U.S.O.P.C. and the Tokyo Organizing Committee, coupled with the testing before travel, on arrival in Japan, and during Games time, will allow for safe participation of Team USA athletes this summer,” a spokesman for the U.S.O.P.C. said in a statement…
