“The U.S. surgeon general said Sunday that he’s concerned about what lies ahead with cases of COVID-19 increasing in every state, millions still unvaccinated and a highly contagious virus variant spreading rapidly.
Noting that nearly all coronavirus deaths now are among the tens of millions of people who haven’t received shots, despite widespread vaccine availability, Dr. Vivek Murthy painted an unsettling picture of what the future could hold.
“I am worried about what is to come because we are seeing increasing cases among the unvaccinated in particular. And while, if you are vaccinated, you are very well protected against hospitalization and death, unfortunately that is not true if you are not vaccinated,” Murthy said on CNN’s “State of the Union.”
U.S. cases of COVID-19 last week increased by 17,000 nationwide over a 14-day period for the first time since late fall, and an increase in death historically follows a spike in illness. “Of the many questions that continue to circulate about the COVID-19 pandemic, what the future holds with the Delta variant is likely one of the most prevalent.
“If you’re vaccinated, you should not worry about the Delta variant. If you’re not vaccinated, you are really in trouble because it is likely that you will get infected,” Carlos del Rio, MD, said at a media briefing today sponsored by Emory University in Atlanta.
In the past 2 weeks, US COVID-19 cases increased by about 140% and hospitalizations and deaths are up approximately 30%.
“The pandemic is not over,” del Rio said. The global death rate now exceeds 4 million, including 1.8 million deaths last year. “More people died in the first 6 months of 2021 that in the entire year of 2020.”
He attributed the rise in cases, hospitalizations, and deaths to the Delta variant. “I want to emphasize the Delta variant is incredibly infectious, highly transmissible,” said del Rio, professor of medicine, epidemiology, and global health and executive associate dean of Emory University School of Medicine at Grady Health System.
To put the higher transmission in perspective, each person infected with the original SARS-CoV-2 virus was likely to infect 2.5 to 3 others. Someone infected with the Delta variant is likely to infect eight or nine people, who in turn can infect another eight or nine people, and so on, triggering exponential spread…

“Driven by a highly contagious variant of the virus and the significant share of Floridians who remain unvaccinated, the number of new infections reported weekly by Florida’s health department has increased more than fourfold in a month, from 10,095 cases for the week ending June 17 to 45,449 on Friday…
Jackson Health, Miami-Dade’s public hospital system, said the number of COVID-positive patients admitted for overnight care at its three hospitals had ballooned from 66 on July 6 to 139 on Monday, a 111% spike.
At Baptist Health South Florida, there were 303 inpatients with COVID across the system’s 10 hospitals in Monroe, Miami-Dade and Palm Beach counties — nearly three times the number of patients in late June.
And at Memorial Healthcare System, which runs six hospitals in South Broward, administrators reported there were 225 patients with COVID on Sunday, compared to 92 patients with COVID on June 19.

Jason Salemi, an epidemiologist and professor with the University of South Florida in Tampa, has been tracking the pandemic since 2020. Salemi reports findings on his website, and said he is alarmed by the recent rise in cases and hospitalizations…
He said the number of counties reporting positive test rates of more than 10% has jumped dramatically in a very short time.
“Just three weeks ago, 11 of our 67 counties had a positivity over 10%,” he said. “It’s now 53 out of 67 have a positivity over 10%. It is widespread. Everybody is on the rise.”..
On June 19, he said, Florida had 1,764 patients hospitalized with COVID-19. As of July 17, the number had risen to 4,100 statewide. Salemi said Florida ranks fourth highest in per-capita hospitalizations for COVID-19 in the United States, behind Arkansas, Missouri and Nevada.
“It’s the speed with which this is happening that’s quite scary,” he said.
“There’s still 8 million people of vaccine-eligible age in Florida who are not vaccinated,” he said. “Almost one million of those are people 65 and older” — the population at highest risk of getting severely ill and dying from the disease…
“Any surge starts the way this surge is starting,” Napp said. “There’s no way to know how long it will last or how high it will go. But if we do nothing differently, meaning that if this very infectious virus is out in the community and people don’t change behaviors … and don’t get vaccinated, it’s a guarantee it will go up.
“Something has to change to bring that back down,” he said. “It’s not going to do it by itself.”” (D)
“Sarasota Memorial Hospital (SMH) officials said nearly all COVID-19 hospitalizations and deaths have been among people who are unvaccinated or not fully vaccinated.
About a month ago, the number of COVID-19 patients at SMH was in the single digits and no one was in the hospital’s ICU. As of Monday morning, the hospital had 36 patients with the virus. Of those patients, seven are in the ICU.
“Once there is more mobility in a population, interact more, they go out especially indoors, they get into crowded environments and on top of that without masks, that’s a recipe that the virus loves,” said Dr. Manuel Gordillo, Sarasota Memorial Infectious Disease specialist.
Hospital officials said many of the patients are younger than the patients the hospital has previously seen. Some patients are in their 30s.
“Some people that are vaccinated may pick up the virus, but they’re asymptomatic or they have like a flu-like illness and relatively mild. When it comes to severe disease, it’s largely unvaccinated folks,” said Dr. Gordillo.
Sarasota Memorial Hospital said as for breakthrough infections, most cases continue to be mild. Of nearly 100 patients admitted with COVID to SMH during the past 30 days, only 6 have met the criteria for a break-through infection (patients who test positive for COVID at least 14 days after completing their COVID vaccine series).
Officials with Lakeland Regional Health Medical Center said the majority of its hospitalized patients are not vaccinated.
As of Monday, 102 patients are hospitalized with the virus at Lakeland Regional Health. About 27 people are waiting on test results. About 95% of people are not vaccinated.” (E)

“It used to be around 10 patients, then it went up to 30 and now we are up to 56 patients today,” said Dr. Seetha Lakshmi, Medical Director for GEDI at Tampa General Hospital.
More than 90% of the patients are unvaccinated and most of them range in age from 20 to 50-years-old.
“It’s heartbreaking to see we have a 17-year-old, 20-year-old, 30-year-old, 40-year-old. It’s heartbreaking to see them actually be admitted to the hospital,” added Dr. Lakshmi.
The Department of Health in Hillsborough County says 46% of residents 12-years and older have at least one dose of the vaccine, while 38% are fully vaccinated.
“Our cases, hospitalizations, even deaths are all going up while our vaccination numbers are going down; these things are very directly correlated,” said Dr. Douglas Holt, Medical Director, Department of Health Hillsborough County.
“The way the Delta variant is panning out, if you’re are vaccinated you might get a head cold, if you’re not vaccinated, you go through the whole thing of lung disease, ventilator, it’s really heartbreaking,” said Dr. Lakshmi.
Dr. Lakshmi is urging those who have not been vaccinated yet to get one. She said if people are nervous, they should talk with their doctor or a trusted medical professional and not take advice from social media.” (F)

“UF Health Jacksonville broke its record for most hospitalized COVID-19 patients July 19, according to NBC News. At one point on July 19, the hospital reported 126 COVID-19 patients, up 40 percent in one day.
“We’re gaining cases at such a rapid rate we don’t really know where it’s going to stop,” Chad Neilsen, UF Health Jacksonville director of infection prevention, told NBC News. “We aren’t even thinking a couple of months. We’re thinking what’s going to immediately happen in the next week.”” (H)
“Hospitals in Missouri and Florida discussed how they were going to care for overflow patients with COVID-19 as infections across the U.S. increase, the Associated Press reported.
Springfield, Missouri is one of the most overwhelmed areas of the nation. Public health officials have asked the state to convert dormitories, hotels or other large spaces for the care of less seriously ill COVID-19 patients so that the city’s two hospitals can focus on the sickest.
UF Health Jacksonville in Florida is thinking about setting up tents in their parking lot to help with overflow after the cumber of their COVID-19 patients doubled to 77 over the past couple of weeks. The hospital’s director of infection prevention, Chad Neilsen, said the hospital expects to surpass its January high of 125 COVID-19 inpatients within the next few weeks.
With the highly contagious delta variant spreading rapidly, cases in the U.S. are up around 70 percent over the last week, hospital admissions have climbed about 36 percent and deaths rose by 26 percent, the Centers for Disease Control and Prevention said Friday.
Some hospitals are reporting record or near-record patient volumes. But even for those that aren’t, this round of the pandemic is proving tougher in some ways, hospital and health officials said. Staff members are worn out, and finding traveling nurses to boost their ranks can be tough.
“I really think of it as a war and how long can you stay on the front line,” said Dr. Mark Rosenberg, president of the American College of Emergency Physicians. “And how many times do you want to go back for another tour of duty. Eventually, you just don’t want to do it.”
Also, many hospitals were busy even before the surge began, dealing with a backlog of cancer screenings, operations and other procedures that were put off during the winter surge to free up space and staff members, according to health care leaders.
“Eventually you have to pay the piper, and those things have now built up,” said Dr. James Lawler of the Global Center for Health Security at the University of Nebraska Medical Center in Omaha.

“The latest city to offer a helping hand as Springfield hospitals deal with an overflow of COVID-19 patients is Columbia, Missouri.
CoxHealth sites in southwest have sent 10 COVID-19 patients over to MU Health Care in Columbia. Eric Maze with MU Health Care says their hospitals in Columbia have helped more than 20 patients transferred from Springfield and Branson since the start of June.
“During this pandemic, we are partnering with other health care institutions across the state to ensure Missouri patients are provided the best care possible,” said Maze.
The help comes as the Missouri Department of Health and Senior Services declares Greene County is a hot spot for new and growing COVID-19 cases, largely due to the spread of the Delta variant and low vaccination rates.
More than 228 COVID-19 patients are getting treatment at Greene County hospitals as of Sunday, according to the health department. The Springfield-Greene County Health Department just announced Thursday that 40% of Greene County residents, who qualify based on age, have been fully vaccinated against COVID-19.
Even though Columbia is taking patients from outside its metropolitan region, their health leaders are still are making sure they manage room to provide proper care and service to their own residents in need, including their primary county in Boone County.” (J)
“Ten patients and four employees at Vancouver, Wash.-based PeaceHealth Southwest Medical Center have tested positive for COVID-19, the hospital said July 18.
Five of the 14 infections involved people who were fully vaccinated, and only unvaccinated patients are experiencing symptoms, according to PeaceHealth.
The infected patients all had links to the same patient floor. PeaceHealth Southwest Medical Center has temporarily stopped new admissions to the floor and restricted hospital visitors out of an abundance of caution. “A major Texas hospital system has reported its first case of the lambda COVID-19 variant, as the state reels from the rampant delta variant.
Houston Methodist Hospital, which operates eight hospitals in its network, said the first lambda case was confirmed Monday.
The lambda variant was first detected in Peru in December 2020, according to the World Health Organization and makes up 81% of COVID-19 cases sequenced in the country since April 2021, according to a June WHO report. Currently, WHO designates lambda as a “variant of interest.”…
“The lambda is the dominant variant in Peru and Peru has had a very difficult time with COVID-19. “The Johnson & Johnson COVID-19 vaccine may be less effective in battling coronavirus variants than other shots, a new study suggests.
The results, published by bioRxiv but not yet peer reviewed or published in a journal, suggest that the mRNA vaccines made by Pfizer-BioNTech or Moderna could better protect against the delta and lambda strains than the Johnson & Johnson vaccine.
The effectiveness of the vaccine in neutralizing the disease “significantly decreased” with the Johnson & Johnson vaccine, the study found.

CDC Director Rochelle Walensky said that cases, hospitalizations and deaths from the coronavirus are increasing nationwide, adding that over 97% of new hospitalizations are in patients who are unvaccinated.
“There is a clear message that is coming through,” Walensky said at a press briefing. “This is becoming a pandemic of the unvaccinated. We are seeing outbreaks of cases in parts of the country that have low vaccination coverage because unvaccinated people are at risk, and communities that are fully vaccinated are generally faring well.”
The five states with the highest infection rates are Arkansas, Florida, Louisiana, Missouri and Nevada. Florida alone accounted for one in five of all new cases in the past week. But those states are also seeing a higher rate of new vaccinations compared to the national average, according to Jeff Zients, the White House COVID-19 response coordinator/

Thursday’s announcement by the nation’s largest county sent a chill through the state just one month into California’s long-awaited reopening. And it could prompt other local health officials to follow suit, complicating the post-pandemic recovery narrative of Gov. Gavin Newsom as he faces a September recall election.
Los Angeles County just over two weeks ago recommended that people wear masks indoors, a step that Sacramento County health officials also took on Thursday and that Yolo County initiated a day earlier.
The rise of the Delta variant has caused case rates to double nationwide over the past three weeks. On Thursday the state reported 3,622 new cases as part of a startling trend in the wake of the state’s June 15 reopening.
While California started from a relatively low level, the trajectory and speed at which the more transmissible Delta variant has spread is sounding alarms and causing public health officials to rethink their infection-control strategies.
“Across the state we are seeing increases in cases,” said Marin County Public Health Officer Matt Willis, who is considering a recommendation for indoor masking, but not a mandate. “All of us right now — public health officers — are sharing the same challenge: What is the next step for us to reduce transmission?”
A steep rise has pushed Los Angeles County’s case rate to 7.1 per 100,000 residents, which puts the region in the “substantial” category of spread, according to the U.S. Centers for Disease Control and Prevention. That would also have put the county in the second-most restrictive tier under the state’s now-retired reopening blueprint.
The new order will go into effect late Saturday, just before midnight.
“It is clear that the Delta variant is here and spreading rapidly, overwhelmingly in our unvaccinated communities, and we need to take action now before we see uncontrollable spread,” Los Angeles County Board of Supervisors Chair Hilda Solis said on Twitter.

However, Los Angeles has been among the more cautious jurisdictions throughout its response to the pandemic, and California’s guidance gives counties the option to impose tighter restrictions locally.
Officials in Los Angeles stressed that they were acting out of an abundance of caution, in an effort to pre-empt the sort of case numbers that have rapidly increased in other parts of the country. Every state has reported an increase in the number of new virus cases in recent days.
California’s figures have nearly tripled over the past month, largely because of San Bernardino and Los Angeles, but the current rate of 3,000 new cases a day is a blip compared to the winter peak, when there were more than 44,000. California is doing slightly better than the national per capita average and far better than in hot spots around the country. “Fueled by the Delta variant, daily coronavirus case counts in New York City have begun climbing in recent days, even as the city seems determined to turn the page on the pandemic.
Just a few weeks ago, there were only 200 new cases a day across the city on average, the lowest level since the early days of the pandemic. But the city has now had a streak of days with 400 or more cases. And the test positivity rate has doubled: from below 0.6 percent on average to about 1.3 percent.
Those numbers are still low, but the increase has been swift, surprising some epidemiologists and public health officials who had not expected to see cases jump so quickly after remaining level throughout June.
Still, with some 64 percent of adults in the city fully vaccinated, epidemiologists said it was unlikely that the Delta variant would create conditions anywhere near as devastating as the past two waves of Covid-19.
“Alarming is not the right word” to describe the recent uptick in cases, said Denis Nash, an epidemiologist at the CUNY Graduate School of Public Health and Health Policy. “I would say concerning.”….

In interviews in recent weeks, some epidemiologists said they were expecting a calm summer followed by an uptick of cases around the time students returned to school and Manhattan office workers migrated back to the office.

So far, the Delta variant has not led the city to drastically change its public health guidance or virus-related restrictions. Nor has it affected the plans of many large companies to get workers back to their desks in Manhattan, according to Kathryn Wylde, president of the Partnership for New York City, a leading business association.
“It would have been prudent for our elected officials to have specified in advance if, when and under what circumstances they would walk back certain components of opening up,” Professor Nash said.

“Dr. Sergio Segarra says when Covid-19 patient numbers in the Miami hospital he works in dipped below 20 last month, he began feeling optimistic.
“I remember seeing articles in the news about hospitals in California with empty Covid units and I longed for that experience,” Segarra, the chief medical officer with Baptist Health’s Baptist Hospital, said. “It’s an experience we were working our way towards that unfortunately has taken a rather sad turn.”
In the weeks since, the hospital’s Covid-19 patient numbers have more than tripled, and staff are now treating more than 70 people, an “exponential growth,” he said, that they were not expecting. To accommodate the climbing patient numbers, the hospital recently reopened two units that were previously shut down.
The overwhelming majority of those coming in sick with Covid are unvaccinated, Segarra said. Many are young — people in their 20s and 30s who are getting “extremely, extremely sick” and some of whom are dying.
Roughly 45% of people in Florida are fully vaccinated, according to state data. The low vaccination rates, along with a dangerous coronavirus variant that’s now the dominant strain in the United States and the relaxed Covid-19 guidelines, are what Segarra said he thinks have led to the increase.
“It’s very sad to see as a health care professional, to see that this is generally an avoidable and preventable disease and to see so many people dying from a preventable illness,” he said. “It’s heartbreaking.”…
“We haven’t made the progress that we wish in terms of treating Covid, we don’t have that magic bullet in terms of treating Covid, but boy, we’ve come pretty close to having that magic bullet to avoid it — and to see it not being used is very frustrating,” he said.” (G)
“Dr. Alan Jones, associate vice chancellor for clinical affairs at the University of Mississippi Medical Center in Jackson, told ABC News Wednesday that his facility was seeing a “significant increase” in pediatric patients over the last several weeks.
The medical center is currently treating five children with confirmed COVID-19 cases, two of whom are in the ICU. Two other children at the hospital are being investigated for potential COVID-19 infections, he said.
The children, who are in stable condition, range from infants to teenagers, Jones said. He could not share any further details about the cases but said that many patients within his hospital who are currently in the ICU do have some chronic, underlying comorbidity diseases that would put them at a higher risk of having a more severe illness associated with COVID-19.
The state has been seeing a “pretty alarming” increase in the number of COVID-19 cases and hospitalizations, Dobbs said during a press briefing last Friday, as the highly contagious delta variant, which was initially identified in India, has quickly become the dominant variant in the state.
“We have seen pretty much an entire takeover of the delta variant for our transmission,” Dobbs said during the briefing, noting that the current cases in the state are “pretty much all delta.”
Between June 15 and July 9, the delta variant accounted for 80% of all specimens sequenced in the state, according to Dobbs.
Hospital systems are not currently overwhelmed, but Dobbs said there are “concerns about it going forward, as has been seen in other states” due to delta, which has become the dominant variant nationwide.
Mississippi’s daily COVID-19 case average has more than doubled in the last three weeks. Less than three weeks ago, there were under 100 patients receiving care for COVID-19 in Mississippi. As of July 11, there were nearly 200. Hospital admissions have also increased by 26.7% in the last week.
The state has seen an increase in COVID-19 outbreaks as well, particularly among youth, in summer activities and nursing homes, Dobbs said.
Jones said his hospital’s pediatric patients seem to be more symptomatic than the children who became ill earlier in the pandemic, which he attributed to the delta variant.
“These seem to be more classic COVID symptoms — fever, cough, respiratory illness,” he said. “I suspect that’s probably because this delta variant is importing a little more severe illness in the pediatric population than those earlier strains that were circulating.”
The delta variant is surging as Mississippi has the second-lowest vaccination rate in the country, with approximately one-third of the state’s total population fully vaccinated, according to the CDC. The “vast majority” of cases, hospitalizations and deaths in the state now are in unvaccinated people, Dobbs said.
Due to the low vaccination rate, the state health department advised Friday that those who are ages 65 and older or have chronic medical conditions avoid mass indoor gatherings for several weeks.
“Our collective under-vaccination in the state has put us all at risk, especially the most vulnerable,” Dobbs said.” (H)
“Northwest Arkansas and River Valley doctors and healthcare leaders said hospitals are feeling the strain associated with the more virulent delta variant, as active COVID-19 cases and hospitalizations increase in Arkansas.
“Our COVID ICU unit is full and the majority of the patients are on ventilators right now. Our COVID unit upstairs, which is our med surge COVID unit, is also full right now,” said Debbie Hewett, R.N. and director of critical care services at Mercy Fort Smith. “Our patients here in the ICU are just so incredibly ill and that’s COVID and non-COVID patients.”
Hewett said the hospital is not yet seeing record numbers of COVID-19 patients, but they are much more sick than in previous months and when combined with non-COVID-19 patients and limited nurses, it is a stress for the health care system.
“They’re deteriorating much faster. We could predict last go around how long from the time they came in and hit a certain amount of oxygen before we had to intubate them and that would be, on average three to four days, now it’s hours,” she said.
“We’re very concerned of what could be coming,” said Dr. Jason McKinney, ICU medical director for Mercy NWA in Rogers.
Dr. McKinney said the COVID-19 unit is currently full and that hospital staff are moving to open a second unit to help care for patients. He’s worried that the current surge of sickness could surpass previous spikes in active COVID-19 cases and hospitalizations.
“What we’re seeing in south Missouri is that it tends to be worse and that’s kind of where we’re heading now,” McKinney said. “Common sense tells you that when you have this huge population of super sick patients in a hospital, that’s going to strain the system and it’s going to push it again. It was strained last year and it’s going to be strained again.”
“Almost 100% of the cases we are seeing in the hospital are unvaccinated individuals,” said Dr. Raed Khairy, infectious disease consultant for Baptist Health Fort Smith.
Khairy said the hospital has also seen increases in COVID-19 patients, but said the hospital has not yet had to expand capacity.
“Certainly, this may create a stress on any health care system. If we have more cases, then we will have less beds to admit other regular cases and other regular admissions. But, at Baptist Health we are ready for that kind of situation,” Khairy said.” (I)
“The Springfield, Missouri, health department is requesting funding for an alternate COVID-19 care site in response to a spike in infections and hospitalizations in recent weeks, health officials announced Wednesday.
Several local health facilities and hospitals jointly requested the alternative care site funding, which would include money for more beds, staff and antibody testing. One of those facilities was Springfield-based Mercy Hospital, which had so many hospitalized COVID patients last week that it had to call in backup ventilators from other hospitals in its network when it ran out.
The increase in severe illness is taxing the health system and sick patients are expected to outpace hospital capacity, according to the Springfield-Greene County Health Department.
Of the 231 patients currently being treated in Greene County hospitals, 104 are in critical care and 61 are on ventilators, the health department said.

On Thursday, Dr. Vivek Murthy released the first surgeon general’s advisory of his time serving in the Biden administration, describing the “urgent threat” posed by the rise of false information around COVID-19 — one that continues to put “lives at risk” and prolong the pandemic.
Murthy says Americans must do their part to fight misinformation.
“COVID has really brought into sharp focus the full extent of damage that health misinformation is doing,” Murthy told NPR in an exclusive interview ahead of the advisory’s release. Surgeon general’s advisories are reserved for significant public health challenges that demand immediate attention…
Rates of COVID-19 are rising nationwide, driven in large part by the spread of the highly transmissible delta variant. A recent analysis by NPR shows that cases are highest in places where vaccination rates lag. Multiple factors, including inadequate access to vaccines, can keep vaccination rates low in some communities, but Murthy says fear about possible side effects or extremely rare adverse events are also a powerful driver of vaccine hesitancy.
In many cases, false information about the vaccines feeds that hesitancy. According to polling from the Kaiser Family Foundation, two-thirds of unvaccinated adults either believe vaccine myths or are unsure about whether they are true. Murthy says that means misinformation is literally putting lives at risk…

Individuals who reject public health measures, including vaccines, in the name of liberty will soon face an ongoing risk of fatal disease as those who are vaccinated assert their own rights to gather and travel. The only difference is that, unlike with masks or social distancing, the unvaccinated will be able to protect themselves fully with a shot that is both safe and free. In fact, the willfully unvaccinated may continue to threaten the health of the vaccinated by requiring hospital space for COVID-19 care, as well as health care resources that might be spent otherwise….” (L)
ETHICAL CONSIDERATIONS RELEVANT TO VACCINATIONREQUIREMENTS (CONTINUED)
Childress et al. suggest that three of these moral considerations—benefiting others, preventing and removing harms, and utility—are critical to the goal of public health while three others—justice, respect for autonomy, and privacy—are most likely to limit public health activities.4
In addition to defining the ethical issues relevant to public health, we must also acknowledge that there are often instances in the practice of public health decision-making and program implementation where the above noted considerations may come into conflict, provoking a need to choose or assign a greater weight to one of these moral considerations over another. Childress et al. proposed five “justificatory conditions” to help determine when the moral considerations critical to public health (benefiting others, preventing and removing harms, and utility) can take precedence over the other goals during specific public health activities.4
These “conditions” are: effectiveness of the activity, proportionality of the activity (the probable public health benefits outweigh the “infringed” other moral considerations), necessity of the activity, the extent to which the activity represents the least infringement of the other moral considerations, and lastly, the ability to publicly justify the activity in a transparent manner. Because vaccination activities are a key component of many public health programs, we suggest that they fall within this ethical framework. Diekema and Marcuse, however, have put forth a more direct approach for evaluating and resolving ethical issues around mandatory vaccination programs.5
Their approach is based upon the often cited medical maxim commonly translated as: “first, do no harm.” When applied to vaccination activities, this maxim has the following implications: the vaccination should be of benefit to the subject being vaccinated; care should be taken to prevent any harm that might accrue from the vaccination; compared to other procedures for addressing the same issue, the vaccination should be the best opportunity for successfully preventing disease as compared to the risk for harm; and if harm does result from the vaccination, the benefit of vaccination to the subject should at least compensate for the harm incurred.5
Because vaccination provides not only a direct benefit (immunity to disease) to the person being vaccinated but also provides a benefit to others in the community via herd immunity, Diekema and Marcuse remind us that unvaccinated persons can be viewed as “harming” the community.5 It, therefore, follows that for serious and highly communicable diseases, there is a role for compulsory vaccination programs. Diekema and Marcuse cite the utilitarian philosopher John Stuart Mill who held that: “The only purpose for which power can rightfully be exercised over any member of a civilized community, against his will, is to prevent harm to others. His own good, either physical or moral, is not sufficient warrant.”5
This principle, known as the “harm principle,” can be used to expand the application of the maxim: first, do no harm” to the community interests that result from vaccination programs.5
It, therefore, follows that the “harm principle” (“first, do no harm.”) can be used to justify compulsory vaccination programs in specific instances where the community interests or benefits are deemed to be significant. Often, the issue is in determining what is considered significant. Finally, the “precautionary principle” has been put forth by Gostin and colleagues as being critical in public health’s response to potential infectious disease emergencies.6
This principle was initially defined as being relevant to environmental health emergencies and asserts that public health is obligated “…to protect populations against reasonably foreseeable threats, even under conditions of uncertainty… Given the potential costs of inaction, it is the failure to implement preventive measures that requires justification…”6” (O)

“Dr. Michelle Fiscus was caught up in a controversy after she passed along legal guidance to health providers saying teenagers do not need parents’ consent to receive a COVID-19 vaccine shot — a position established by decades of state law.
“Specifically, it was MY job to provide evidence-based education and vaccine access so that Tennesseans could protect themselves against COVID-19,” Fiscus said in a scathing statement about her firing. “I have now been terminated for doing exactly that.”
Tennessee’s leaders have betrayed the public trust, Fiscus says, accusing them of putting their own political gains ahead of the people’s well-being. She defended her colleagues in the health sector who have been fighting the pandemic — and she notably took umbrage that a lawmaker had called the state health department’s actions “reprehensible.”
Fiscus said that “the ‘leaders’ of this state who have put their heads in the sand and denied the existence of COVID-19 or who thought they knew better than the scientists who have spent their lives working to prevent disease… they are what is ‘reprehensible.’ I am ashamed of them. I am afraid for my state.”
Because of the pushback from lawmakers, Fiscus says, Tennessee is halting all of its vaccination outreach efforts for teens and children – not only for COVID-19 but also for measles and other illnesses…
It all started in the spring, when Fiscus says several health care providers asked her office for clarity about how to handle the then-looming authorization of the Pfizer COVID-19 vaccine for minors as young as 12. As she prepared a memo on the subject, she turned to the Tennessee Department of Health’s general counsel, which replied with a doctrine based on a 1987 Tennessee Supreme Court ruling.
The doctrine, which says teens from ages 14-17 don’t need to get their parents’ or guardians’ consent before getting the vaccine, was posted online “and is blessed by the Governor’s office on the subject,” the legal office said, according to Fiscus. The office reportedly added, “This is forward facing so feel free to distribute to anyone.”
But when Fiscus sent a memo sharing that guidance, critics seized on the message and called it a governmental overreach, threatening to disband the state Department of Health, despite the COVID-19 pandemic. At a hearing in Tennessee’s legislature, a Republican legislator said the health department’s ad campaign encouraging teens to get vaccinated amounted to an attempt “to target children.”…
“I was told that I should have been more ‘politically aware’ and that I ‘poked the bear’ when I sent a memo to medical providers clarifying a 34-year-old Tennessee Supreme Court ruling,” Fiscus said in her statement.
“I am not a political operative, I am a physician who was, until today, charged with protecting the people of Tennessee, including its children, against preventable diseases like COVID-19.” (A)
“Exhausted, frustrated, facing personal threats and political pressure, public health officials nationwide are departing their posts at a rate never seen before, and in the middle of a pandemic.
More than 180 state and local public health leaders — high level health department staffers in at least 38 states — have resigned, retired or been fired since April 1, according to an analysis by Kaiser Health News and The Associated Press. It is the largest exodus of public health officials in American history, experts say.
One in eight Americans — roughly 40 million people — lives in a community that has lost its local public health department leader during the coronavirus pandemic, the analysis found.
“This has been a major, unprecedented loss in public health leadership across the nation,” Dr. Richard Besser, former acting director of the Centers for Disease Control and Prevention and president and CEO of the Robert Wood Johnson Foundation, told ABC News.
While prepared to handle the relentless strain that comes with managing the COVID-19 crisis, officials told ABC they had not anticipated the cross purposes of scientific response and political backlash — in both red and blue states — that hamstrung the offices in which they’d been tasked to serve — and where some said they found little support from their leadership.
“This past year, you saw public health as a science and as a field being berated and belittled,” Besser said. We saw it being lifted up as the enemy of economic recovery — rather than the path to sustained economic recovery.”
The contentious atmosphere between political and public health risks eroding trust and morale in the system, experts say; that only gets exacerbated as science becomes the target of public ire — and physical threats…
In California, Dr. Aimee Sisson is among several health officers throughout the state who have resigned in the wake of friction with elected officials, departing from her role as Placer County public health director after a unanimous vote by the board of supervisors in September to end the local COVID-19 health emergency declaration, in defiance of her guidance…
“It was the last straw,” Sisson told ABC News. “Ultimately, it came down to feeling like I wasn’t able to do the job that I was hired to do.””
“Where I get concerned is when public health is not one of the key voices — or doesn’t get a set of keys at all to what’s driving our response, and they’re left in the backseat,” Sisson said.
“I hope what comes out of all this is increased understanding and investment in what public health does. We always say — when we’re doing our job well, nobody knows that we exist — because we’re preventing outbreaks before they happen,” Sisson said. “This is bigger than the pandemic.” (B)
“Nearly a quarter of public health workers report feeling bullied, harassed or threatened due to their work as the pandemic was unfolding, with 1 in 8 saying they had received job-related threats. That’s according to a new study from the Centers for Disease Control and Prevention.
The agency found that almost three-quarters of more than 26,000 health care workers surveyed during a three-week period in late March and early April of 2020 felt overwhelmed by work. More than half reported symptoms of mental health conditions, including depression among nearly a third.
The troubling symptoms were more acute among workers who spent most of their time with COVID-19 patients. Of those who spent three-quarters of their work life responding to pandemic-related cases, nearly half reported symptoms of post-traumatic stress disorder…
Stressful events like the coronavirus pandemic impair health care workers when they’re needed most, the CDC researchers stated.
“Increases in adverse mental health symptoms among workers have been linked to increased absenteeism, high turnover, lower productivity and lower morale, which could influence the effectiveness of public health organizations during emergencies,” they wrote.
The findings echo the experiences of public health officers, dozens of whom have resigned or been fired since the start of the pandemic, noted researchers in an article published last year by the Journal of the American Medical Association.
“Today’s increasingly routine harassment and threats against public officials have much in common with growing resistance to childhood vaccination,” they stated. “The environment deteriorates further when elected leaders attack their own public health officials..” (C)
“Riverside County, California, was one of the first counties in the country to face Covid-19, when repatriated US citizens were quarantined at March Air Reserve Base on January 29, 2020. Seeing how quickly things moved in China, then Riverside County Health Officer Dr. Cameron Kaiser knew he had to act fast.
“From what we saw was going on in China at the time, it potentially, it could be very big,” he said.
On April 4, 2020, Kaiser took actions more protective than the state’s health orders, including prohibiting gatherings of people from different households in his county, as well as an order for people to mask in public.
He knew none of this would be easy, but he didn’t expect the backlash.
“We never really dealt with a situation where people simply told us to go jump in a lake when we had to make those necessary orders to protect society,” Kaiser said.
A month later, dozens of citizens came before the Riverside County Board of Supervisors — many of them saying that their personal liberties had been infringed upon.
“You are trampling on the Constitution by agreeing to extend Dr. Kaiser’s recommendations,” declared one resident.
Another said: “When we are inside, locked up because of health orders by Dr. Kaiser, we are weak. The American people are a free people.”…
On May 8, 2020, the Riverside County Board of Supervisors directed Kaiser to rescind his orders, so that the county would be more in line with the state. He said in the months that followed, the threats continued, and he was given fewer and fewer responsibilities at work.
Two months ago, Kaiser received a call from the Riverside County CEO telling him that his services were no longer needed. Kaiser says since his appointment as the county health officer in 2011, he’s had no issues or complaints about his work…
Kaiser’s experience is far from unique, said Lori Freeman, CEO of the National Association of County and City Health Officials (NACCHO). Since the beginning of the pandemic, she said the group has tracked more than 250 public health officials leaving the field. While some of it is part of the normal ebb and flow of the workforce leaving or retiring, Freeman says much of it is “due to the pressures of this pandemic.”…
Despite what he has been through, Kaiser said he would make the same decisions again and wants to work in the field in the future.
“It’s more than just smoking and cholesterol and communicable diseases. There are so many issues that affect people’s health and the way that they live their lives, and you get a chance to be part of it,” he said. “When the job’s at its best, there’s no better job in the world.”” (D)
“Former President Trump and his GOP allies have stepped up attacks on Anthony Fauci, seizing on portions of his emails and a renewed interest in the origins of the coronavirus pandemic to demonize the nation’s top infectious disease doctor.
The attacks, which are largely based on out-of-context comments and draw on unsubstantiated conclusions, gloss over the Trump administration’s role in the nation’s early failures to respond to the pandemic.
Instead, conservative lawmakers and media personalities are lionizing the former president as someone betrayed by his advisers. Fauci is painted as a liar who misled both Trump and the American people, and is now facing calls for his resignation, prosecution, or both.
