The chief medical officers of England, Scotland, Wales and Northern Ireland said the emergence of the highly transmissible new strain “adds additional and rapidly increasing risk to the public and health care services” at a time when Covid-19 is already widespread. They recommended raising the alert level from 3 to 4 on a 5-point scale. The top level, 5, indicates authorities think the health-care system is about to be overwhelmed.
The doctors said early evidence shows omicron is spreading much faster than the currently dominant delta variant, and that vaccines offer less protection against it. British officials say omicron is likely to replace delta as the dominant strain in the U.K. within days.

“BERGEN: You predicted in April 2020 that there could be 800,000 deaths in the United States in 18 months, and we’re now at 790,000-plus deaths right around that 18-month time frame. How did you make that prediction?
OSTERHOLM: I based my estimates at the time on historic data from previous pandemics.
What is troubling to me is our fascination with modeling. I think modeling, particularly when it’s erroneous, can be very detrimental. I’ve watched so many different estimates of case numbers from these models taken literally by policymakers and the public and particularly the media.
The reality is you can’t model beyond 30 days out. Just look at what is happening right now. We can’t even predict why these surges occur or when they occur. Who, 30 days ago, could have developed a model that would accurately predict what we’re seeing right now with Omicron? Who could have predicted that?…
BERGEN: Two percent of the population of low-income countries has had one shot versus 65% for high income countries. does that portend for the future?
OSTERHOLM: Two things: One is that this pandemic has really provided a window into our global vaccine capacity in a way nothing else has ever done before.
I think that there’s been some red herrings in terms of what the issues are. For example, we keep hearing about technology transfer and giving these countries the ability to make their own vaccines, and yet the expertise needed to make these vaccines is really at a premium. It’s very difficult to find people who know how to do this. So, it’s not enough to transfer technology to a low-income country if you don’t provide the expertise to make these vaccines. It’s not as simple as making chicken soup.
Also, our focus has been almost solely on getting vaccines to people around the world, which is surely important. But we haven’t been thinking nearly enough about what it would take to turn a vaccine into a vaccination, that needle into the arm. We have seen the challenges in this country with administering vaccinations, and those challenges also exist around the world.
So, just shipping a couple of pallets of vaccines to a low-income country may be a useless effort if, in fact, they don’t have the infrastructure to deliver the vaccine and they don’t have a means for helping the population understand how and why they should want to be vaccinated. What this whole situation has highlighted, is the fact that we have a lot more work to do to understand not just how to make vaccines, but also how to turn vaccines into vaccinations.” (C)
“A MEMBER OF the Scientific Advisory Group for Emergencies (Sage) has said Omicron is “much more infectious” than Delta and all previous strains.
Professor Andrew Hayward told LBC: “Maybe somewhere between twice and possibly three times as infectious.”
An additional 633 confirmed cases of the Omicron variant were reported across the UK yesterday, bringing the total number of confirmed cases to 1,898.
“What we can also see is that the vaccine, two doses of the vaccine, has relatively little impact on stopping that transmission.
“So putting those two together, and the fact that it’s already increasing, doubling every two or three days, what we can be pretty sure of is a very, very large wave of infections, bigger than the waves of infections that we’ve had before, so really the uncertainty is in how that’s going to translate into hospitalisations and deaths.”
Hayward said the hospital system is already “on its knees” and there is a “big problem” ahead.
It was put to him that people may think ministers feel they have to put extra restrictions in place “based on modelling and theories”. He said: “There’s a very, very strong theoretical possibility here. It’s not just an off-chance that we’re talking about.
“And certainly we’re talking about a hospital system that is already pretty much on its knees with the current level of activity, and that level of activity is going to go up dramatically and much worse than a normal winter.
“So you know, there is going to be a big problem. The question is how much we’re prepared to do to stop that as a society.”” (D)
“Doctors in Minnesota took the unusual step of pleading in a full-page newspaper ad for people to get vaccinated against Covid-19 and take other precautions to curb the pandemic.
“We’re heartbroken. We’re overwhelmed,” the top of the Sunday ad in the Minneapolis Star Tribune said in large type.
“Our emergency departments are overfilled, and we have patients in every bed in our hospitals,” the ad said. “This pandemic has strained our operations and demoralized many people on our teams.”
The ad was signed by nine executives of health care systems. Seven doctors, including the president and CEO of the Mayo Clinic, Gianricco Farrugia, and Marc Gorelick, president and CEO of Children’s Minnesota, signed it.
According to data compiled by Johns Hopkins University, the number of hospitalizations in Minnesota for the past 30 days is 14.5% higher than the previous 30-day period. The number of Covid-19 deaths has increased by 36.4%.
Minnesota Hospital Association President and CEO Rahul Koranne told CNN in a statement “the full-page ad that some of Minnesota’s hospital and health system leaders signed is a dire yet accurate summary of the situation here in Minnesota. “The omicron variant appears to cause less severe disease than previous versions of the coronavirus, and the Pfizer vaccine seems to offer less defense against infection from it but still good protection from hospitalization, according to an analysis of data from South Africa, where the new variant is driving a surge in infections.
While the findings released Tuesday are preliminary and have not been peer-reviewed — the gold standard in scientific research — they line up with other early data about omicron’s behavior, including that it seems to be more easily transmitted.
A two-dose Pfizer-BioNTech vaccination appeared to provide just 33 percent protection against infection during South Africa’s current omicron wave, but 70 percent protection against hospitalization, according to the analysis conducted by Discovery Health, South Africa’s largest private health insurer, and the South African Medical Research Council.
The data were gathered from Nov. 15 to Dec. 7, during which time omicron was first spotted by scientists in South Africa and Botswana, and may change as time passes. Experts now say that omicron accounts for more than 90 percent of all new infections in South Africa, according to Discovery Health chief executive Dr. Ryan Noach.

“Omicron is spreading at a rate we have not seen with any previous variant,” said Dr. Tedros Adhanom Ghebreyesus, director-general of the World Health Organization.
“We underestimate this virus at our peril,” he said in a news conference Tuesday from Geneva. “Even if omicron does cause less severe disease, the sheer number of cases could once again overwhelm unprepared health systems.”..

“Scientists have moved quickly since the World Health Organization designated Omicron a variant of concern in late November. Observations arrive in rapid succession, often posted straight to social media without peer review or the caveats and uncertainties that would be noted in published papers. Watching science unfold in real time can be confusing and misleading.
“We are going to see a couple of weeks of every day, new information, new studies,” said Dr. Soumya Swaminathan, chief scientist at the W.H.O. “One study is not going to really prove anything.”
So what do researchers need to do in the weeks and months ahead to better understand Omicron — and any future variants — and how should we gauge their progress? Here’s a guide for following along.

“When scientists discovered the highly mutated Omicron variant of the coronavirus last month, it set off an eerily familiar chain of events.
Health experts held somber news conferences that raised more questions than answers. Officials imposed travel bans that very likely came too late. Virus trackers filled in their maps as the variant was reported in country after country. And the rest of us waited, with increasing unease, to learn more about the threat we were facing.
The same sequence unfolded nearly two years ago when the novel coronavirus, SARS-CoV-2, was first discovered. In those early weeks of 2020, the United States proved to be woefully unprepared for the challenges ahead, starting with the most fundamental of tasks: detecting the virus.
“We had a delay of one to two months before we were even able to identify the presence of the virus,” said Dr. Charles Chiu, an infectious disease specialist and microbiologist at the University of California, San Francisco. “And by that time, it had already circulated widely between multiple states and from coast to coast.”
These failures have been well-chronicled, and Omicron is one more sign that the current pandemic, which has now claimed the lives of nearly 800,000 Americans, is not over.
But Omicron is also a dress rehearsal for the next pandemic. The work before us now — detecting, tracking and slowing the spread of a health threat we do not fully understand — is the same work that will be required to stop a future outbreak in its tracks…
Many of these problems can be traced to the nation’s failure to invest in testing early in the pandemic. The Trump administration created Operation Warp Speed to turbocharge vaccine development. The country needed a similar effort for diagnostic tests, experts said.
Diagnostic testing may not be as tantalizing as vaccines, but in any future pandemic, they said, it should be a priority from the start.
The next vital link in the surveillance chain is routine, widespread genomic sequencing. This kind of surveillance helps experts keep tabs on how a pathogen is mutating and how new variants are spreading.
In the United States, this effort got off to a very slow start. “Many of the public health labs were, frankly, just overwhelmed by the initial testing volume and competing obligations,” said Duncan MacCannell, the chief science officer at the C.D.C.’s office of advanced molecular detection….
Some of the progress the country has made could leave us better positioned for the next pandemic. For instance, public health laboratories have new equipment and expertise, which they can now use to track the flu, food-borne illnesses and whatever the next great global health threat turns out to be.
But solidifying these gains will require a continuing commitment and funding after the immediate crisis has passed. “The historical pattern is, we surge and then we forget and we neglect,” Dr. Bright said.
Many exhausted health officials have already left their jobs, and legislators have passed more than 100 laws limiting the public health powers of state and local authorities, a New York Times review found.
Some of the problems that the pandemic has highlighted are deeply rooted. For example, a highly coordinated genomic surveillance program like Britain’s, which is frequently held up as an exemplar, was always going to be a heavy lift in the United States.
“We have this Balkanized health care system, and the system is a giant mess,” said Jeremy Kamil, a virologist at Louisiana State University Health Sciences Center Shreveport.
And pandemic preparedness does not happen in a vacuum. Just as a more equitable distribution of vaccines might help squelch the next variant of concern, preventing the next big global outbreak will require ensuring that every country has the resources to detect and respond to emerging pathogens.
The United States is a large and fractured country — politically polarized and burdened with glaring inequities, rampant misinformation and disinformation, and a considerable distrust of public officials. These are enormous, thorny problems and are much harder to address than ensuring that labs have the capacity to detect Omicron or any new pathogen.
“I’m confident in our ability to detect the variant,” Dr. Fauver said. “What I’m not confident in is our ability to do anything about it. We’re detecting the Delta variant every single day, every time we sequence.”
Even before the Omicron news broke, another Delta-driven surge had already begun. Scientists are finding more Omicron cases every day, and the variant could soon overtake Delta. What comes next — what we should aim for, even — is less clear. Should we spend the winter trying to stop every infection? Protecting the highest risk people from severe disease and death? Ensuring that hospitals are not overrun?
“One thing that we’ve lacked continuously through the pandemic is a goal,” said Emily Gurley, an epidemiologist at the Johns Hopkins Bloomberg School of Public Health. “We still don’t have that. Certainly, we don’t have that for Omicron.” (L)

“Geisinger, one of Pennsylvania’s largest health systems has run out of beds due to the COVID surge, causing patients to wait 10 to 20 hours in the emergency department, officials said Wednesday.
Officials said doctors and nurses are having to perform “waiting room medicine” on waiting patients.
Geisinger is operating at 110 percent capacity in its nine hospitals located in central and northeastern Pennsylvania. A quarter to over half of admissions are unvaccinated COVID patients, Dr. Jaewon Ryu, Geisinger’s president and chief executive officer, said. “Citing a temporary federal injunction, Bayhealth and Beebe Healthcare will no longer require employees to get their COVID-19 vaccine as a condition of employment.
A federal judge on Tuesday blocked President Joe Biden’s mandate that all health care workers must receive the vaccine, which was set to go into place next week. The original requirement would have forced workers to get the first dose by Dec. 6 and the second dose by Jan. 4.
In a Dec. 1 email to employees, Shana Ross, vice president of human resources, wrote that Bayhealth will no longer fire employees who have not received their first COVID-19 vaccine dose by Dec. 6, according to an email obtained by Delaware Online/The News Journal.
Bayhealth will continue to “strongly urge all employees to get vaccinated consistent with our commitment to maintaining a safe environment for all patients, employees, physicians and visitors,” according to the email.
The health system will still follow Delaware’s directive of health workers either receiving the vaccine or undergoing regular testing, according to the email. A spokesperson could not be reached for comment.” (C)
“Moderna CEO Stephane Bancel on Monday said it will take months to develop and ship a vaccine that specifically targets the omicron variant of the virus that causes Covid-19.
However, a higher 100-microgram dose of the company’s booster shot could be ready much sooner.
“The higher dose could be done right away but it will be months before the omicron specific variant is ready to ship in massive quantities,” Bancel told CNBC’s “Squawk Box.”
Bancel said Moderna believes the omicron variant is highly infectious, but it will take at least two weeks to determine how much the mutations have impacted the efficacy of the vaccines currently on the market.
“Depending on how much it dropped, we might decide on the one hand to give a higher dose of the current vaccine around the world to protect people, maybe people at very high risk, the immunocompromised, and the elderly should need a fourth dose” he said.” (D)
“Because the Delta variant of coronavirus spreads easier than the original virus, the proportion of the population that needs to be vaccinated to reach “herd immunity” protection could be upwards of 80% or more, experts say.
Also, it could be time to consider wearing an N95 mask in public indoor spaces, regardless of vaccination status, according to a media briefing today sponsored by the Infectious Diseases Society of America.
Furthermore, getting booster shots to the fully vaccinated is not the top public health priority now. Instead, third immunizations should be reserved for more vulnerable populations – and efforts should focus on getting first vaccinations to unvaccinated people in the U.S. and around the world.
“The problem here is that the Delta variant is … more transmissible than the original virus. That pushes the overall population herd immunity threshold much higher,” Ricardo Franco, MD, assistant professor of medicine at the University of Alabama at Birmingham, said during the briefing.
“For Delta, those threshold estimates go well over 80% and may be approaching 90%,” he said.
To put that figure in context, the original coronavirus required an estimated 67% of the population to be vaccinated to achieve herd immunity. Also, measles has one of the highest herd immunity thresholds at 95%, Franco added.” (E)

“As new COVID infections, hospitalizations and the rate of transmission (r/t) continues to rise in New Jersey, there are growing concerns about staffing shortages at hospitals across the Garden State.
During Wednesday’s virtual coronavirus response press conference, New Jersey Department of Health Commissioner Judith Persichilli said she has been speaking to a number of hospital CEOs about the shortages they face, adding that there are multiple factors responsible for the shortages, including exhaustion.
Why are hospitals short-staffed?
Burnout is one challenge facing health care workers amid the pandemic.
“Those that have worked over the past 22 months taking care of patients that are suffering from COVID, the nurses are tired and they’re burned out,” Persichilli said.
She said besides burnout, another factor is “age out,” where many nurses who were near retirement have now decided it’s time to pack things in and stop working.
Persichilli said there is also the issue of “buyout,” where nurses are being offered “very attractive packages from staffing agencies, for individuals to leave and work as an agency nurse or sometimes a traveling nurse.”
She noted, “In some cases they’re not only bought out, they’re then sold back to the hospitals that they had just left.” (G)
“The first cases of influenza have been detected in Marin County pointing to the start of flu season which typically kills thousands of people each year.
On average influenza kills between 20,000 – 50,000 Americans each year, and now experts are concerned we might be entering a “twindemic” with cases of the COVID-19 variant omicron likely to rise.
“We don’t know whether Omicron will add substantially to that or not. But, Delta is enough of a problem as it is. We don’t want people ending up at the hospital with Delta and people winding in the hospital with the influenza and hospital beds getting filled and the ICU beds getting with both of these patients,” said Dr. John Swartzberg, UC Berkeley Clinical Professor Emeritus & Infectious Disease Expert.
Dr. Swartzberg says we are about two weeks from seeing how fast influenza will spread this year.

Because the mandate itself has a slim chance of becoming law, the measure to overturn it will have little practical effect. A federal court has already halted the administration’s Covid vaccination and testing requirements for private businesses with 100 or more employees.
Even so, the vote underscores resistance to the Biden policy even among Democrats who represent red states. It reflects the White House’s struggle to increase U.S. vaccinations and booster shots as the highly mutated omicron variant — which has shown the potential to evade protection offered by a two-dose vaccine regimen — starts to gain a foothold around the country.
The Senate approved the measure in a 52-48 vote. Sens. Joe Manchin, D-W.V., and Jon Tester, D-Mont., joined every Republican in supporting it. It needed only a simple majority to pass under the Congressional Review Act, a process that allows Congress to overturn rules made by federal agencies.
Republicans who introduced the plan to overturn the mandate argued it would damage small businesses as they try to navigate the pandemic.

“Nearly two years into the Covid-19 pandemic, the world remains “dangerously unprepared” for the next major outbreak, according to a new report.
The 2021 Global Health Security Index, released on Wednesday, ranks 195 countries according to their capacity to respond to epidemics and pandemics. The inaugural version of the index, published just months before the first Covid-19 cases were detected, concluded that no nation was ready for such a crisis.
Overall, the world is not any better prepared today, according to the 2021 index, which was created by the Nuclear Threat Initiative, a global security nonprofit group, and the Johns Hopkins Center for Health Security at the Johns Hopkins University Bloomberg School of Public Health.
“I would call this a damning report,” said Dr. Rick Bright, the chief executive of the Rockefeller Foundation’s Pandemic Prevention Institute, who was not involved in creating the index. “The world is not ready.”” (J)
“I am disappointed, and I am angry, not just with my friend but with all the people who are choosing not to get vaccinated.
There was a point, earlier on in the pandemic, when vaccines were still scarce, when I tried to be tolerant with the holdouts, tried not to shame them, tried not to be angry with them, tried to allow them time to educate themselves about the benefits of getting vaccinated.

This virus is deadly and unrelenting. The only way out of this situation, for our country and the world, is through the vaccines. We must dramatically shrink the number of people vulnerable to the virus — or else we risk allowing our population to act as a petri dish for the growth of variants.
I have heard all the reasons for resistance. There are the people who have politicized the virus and see getting vaccinated through a partisan lens. There are the people who view government pressure, and especially mandates, to put something in your body as overreach and anathema to the American ideal of independence and freedom. There are people who don’t trust the government, sometimes with good reason…
All others have a choice to either be part of the solution or part of the problem. The unvaccinated are choosing to be part of the problem.” (K)
“The governors of Maine and New York deployed the National Guard in response to dangerously low capacity at statewide medical facilities due to the pandemic.
The New York National Guard announced Wednesday that it had deployed 120 medics and medical technicians to a dozen long-term care facilities statewide. The deployment came at the behest of Gov. Kathy Hochul, who issued the order last week in response to staffing shortages.
Service members deployed to facilities in Syracuse, Rochester, Albany, Buffalo, Utica, Plattsburgh, Uniondale, Liberty, Vestal, Olean, Lyons and Goshen, the Guard said in a statement.
Hochul has indicated that she may deploy the Guard to hospitals as well — as of last week, some 50 hospitals in northern New York had less than 10% bed capacity in large part due to lack of staff.
In Maine, Gov. Janet Mills activated the National Guard following a spike in Covid-19 cases.
“I do not take this action lightly, but we must take steps to alleviate the strain on our health care system and ensure care for all those who need it,” Mills said in a statement.
The state reported a record-high of 379 people hospitalized with Covid-19 as of Wednesday, 60 of whom were on ventilators.
Mills’ office said in a statement Wednesday that Maine had only 42 ICU beds available statewide.
Some 73% of Maine residents have received a full initial dose of Covid-19 vaccine, according to state data, with 16% of the population receiving a booster shot.
New Hampshire governor calls on FEMA and National Guard to prepare for winter Covid-19 surge
Epidemiologist Michael Osterholm said Wednesday he thinks the whole country will see Covid-19 case numbers go up in coming weeks. Already 23 states have had more than a 20% increase in case numbers in just the last two weeks.
There have been surges in the Northeast and the surges in the upper Midwest have been “dire,” said Osterholm.
“But we expect to see other areas of the country also light up in the next several weeks,” Osterholm, the director of the Center for Infectious Disease Research and Policy at the University of Minnesota, told CNN’s Erin Burnett.
The biggest problem are the unvaccinated. “We have tried everything from public relations to incentive offers, to just having people see what’s happening in our intensive care units and we still have that reluctant group of individuals that just won’t get vaccinated,” said Osterholm. (L)

Researchers worldwide are racing to understand the threat that the variant — now confirmed in more than 20 countries — poses to the world. Yet it might take scientists weeks to paint a more complete picture of Omicron, and to gain an understanding of its transmissibility and severity, as well as its potential to evade vaccines and cause reinfections…
Omicron’s rapid rise in South Africa is what worries researchers most, because it suggests the variant could spark explosive increases in COVID-19 cases elsewhere. On 1 December, South Africa recorded 8,561 cases, up from the 3,402 reported on 26 November and several hundred per day in mid-November, with much of the growth occurring in Gauteng Province, home to Johannesburg.
Epidemiologists measure an epidemic’s growth using R, the average number of new cases spawned by each infection. In late November, South Africa’s National Institute for Communicable Diseases (NICD) in Johannesburg determined that R was above 2 in Gauteng. That level of growth was last observed in the early days of the pandemic, Richard Lessels, an infectious-disease physician at KwaZulu-Natal University in Durban, South Africa, told a press briefing last week.
Gauteng’s R value was well below 1 in September — when Delta was the predominant variant and cases were falling — suggesting that Omicron has the potential to spread much faster and infect vastly more people than Delta, says Tom Wenseleers, an evolutionary biologist at the Catholic University of Leuven in Belgium. Based on the rise in COVID-19 cases and on sequencing data, Wenseleers estimates that Omicron can infect three to six times as many people as Delta, over the same time period. “That’s a huge advantage for the virus — but not for us,” he adds.
Researchers will be watching how Omicron spreads in other parts of South Africa and globally to get a better read on its transmissibility, says Christian Althaus, a computational epidemiologist at the University of Bern, Switzerland. Heightened surveillance in South Africa could cause researchers to overestimate Omicron’s fast growth. But if this pattern is repeated in other countries, it would be very strong evidence that Omicron has a transmission advantage, adds Althaus. “If it doesn’t happen, for example, in European countries, it means things are a bit more complex and strongly depend on the immunological landscape. So we have to wait.”
The threat of Omicron has prompted some rich countries, such as the United Kingdom, to accelerate and broaden the roll-out of COVID vaccine booster doses. But it’s not yet clear how effective these doses will be against this variant…
Early reports linked Omicron with mild disease, raising hopes that the variant might be less severe than some of its predecessors. But these reports — which are often based on anecdotes or scant scraps of data — can be misleading, cautions Müge Çevik, an infectious-disease specialist at the University of St Andrews, UK. “Everyone is trying to find some data that could guide us,” she says. “But it’s very difficult at the moment.” (A)
“Is omicron worse than delta?
Right now, it’s too early to tell. Omicron has some similar mutations in its spike protein to the delta variant, according to the Republic of South Africa’s Department of Health, as well as the alpha, gamma and beta variants — all classified as variants of concern by the WHO. This means omicron will also be very transmissible, likely enabling the virus to more easily get around someone’s antibodies, lowering the vaccine’s efficacy against symptomatic disease (as in the delta variant’s case, scientists expect the vaccines to remain protective against severe COVID-19). Scientists from South Africa also believe omicron may lead to more cases of reinfection in people who’ve already had COVID-19.
Omicron has more mutations on its spike protein than the delta variant does, but whether that means anything remains to be seen.
“What all those changes in the aggregate are going to do for the things that matter for this virus, we don’t really know yet,” Robert Garry, a virologist at Tulane University, told CNN.
At a briefing Wednesday, Fauci emphasized that within the next two or three weeks, there will be more information about omicron and its efficacy with the vaccines, monoclonal antibodies and more.” (B)
“When I’m reading about the new omicron variant, I feel like I need a medical degree. What does “immune evasion” mean, exactly? Virus neutralization assay?! Help!
We get it: COVID-19 news happens fast, and those of us without a medical degree can feel a little lost. So we turned to three COVID-wise experts — Dr. Jill Weatherhead, assistant professor of adult and pediatric infectious diseases at Baylor College of Medicine; Dr. Greg Poland, professor of medicine and infectious diseases at the Mayo Clinic in Rochester, Minn.; and Matthew Binnicker, Ph.D., vice chair of practice in the Department of Laboratory Medicine and Pathology at the Mayo Clinic — to help us translate some of the terms you’re likely to run across.
So let’s start with a quote from Weatherhead:
“The omicron variant has over 50 genetic mutations in the viral genome. Many of these mutations are found in the spike protein region,” Weatherhead says. “Some of the individual mutations suggest that the omicron variant may be more transmissible or evade immune responses. Despite the association of these mutations with transmissibility and immune evasion, the clinical impact of omicron remains unknown because of the phenomenon of epistasis.”
If you didn’t quite catch all of that, here’s a guide to help.
Mutation: A mutation is a change in a virus’s genetic code. Mutations are a normal part of viral replication. Viruses make copies of themselves as they spread within a human. Mutations happen when a mistake is made during copying. In rare instances, mutations change the behavior of the virus. Most of the time, they don’t.
Variant: In essence, a variant is a version of the virus with a slightly different genetic sequence because of the appearance of mutations. Many mutations quickly disappear as the virus spreads. But sometimes, mutations stick around and get passed down to future generations of the virus. Over time, the virus can accumulate a unique set of mutations that make it distinct from other versions of the virus. These distinct versions are called variants.
Variant of concern (VOC): The World Health Organization and the Centers for Disease Control and Prevention monitor variants and alert the public to variants of concern that could pose added risk because of “increased transmissibility, increased severity of disease or decreased effectiveness of public health interventions [diagnostic tests, preventive measures like vaccines or therapeutic measures like monoclonal antibodies],” Weatherhead explains.
Transmissibility: The ability of a virus to spread from infected people to others. The delta variant is more transmissible than previous variants because of mutations on its spike protein, which make it better at attaching to and infecting cells. There is concern that omicron could be even more transmissible than the highly transmissible delta variant, but we don’t know yet whether that is the case.
Immune evasion: After an infection or a vaccine, your body produces antibodies that can detect and kill the virus upon a subsequent exposure. Sometimes a virus can mutate in a way that helps it evade or escape detection by those antibodies. That’s what scientists call “immune evasion.” There’s preliminary evidence that omicron could be much better at immune evasion than the delta variant or any other variant before it because of the specific mutations on its spike protein.
Outcompete: In sports, if you outcompete other athletes by getting to the finish line first or scoring more points, you’re No. 1. In the world of viruses, winning is more akin to the reality show Survivor; viruses can evolve to infect more people — or to infect them faster — than previous variants and become No. 1. “It’s basically survival of the fittest,” Binnicker explains. “Delta showed us it was king of the hill. We’ll have to see over the next two to four weeks if omicron is better at transmitting than delta.” If it is, it will likely push delta out of many regions of the world and could become the dominate variant.
Epistasis: The Greek roots of this word mean “standing upon.” So what does that have to do with viruses? Well, in genetics, the term describes how mutations depend on each other. The way an individual mutation makes a virus behave often depends on what other mutations are present. That’s why scientists can’t look at a single mutation and know exactly how it will change a virus — mutations need to be assessed together. Think of it as cheerleaders standing on each other: Any movement by one of them often affects the entire formation.
Virus neutralization assay (VNA): OK, let’s take this apart. An assay is a test. Neutralization is a term familiar to those who love science fiction. If you neutralize an attack from an alien ship, you’ve won! In the world of viruses, we want to know if antibodies can stop — or neutralize — a virus particle. And the way to find out is a VNA: a lab test that measures the capability and magnitude of a human’s antibodies to stop infection. This test can be used to determine how well one part of the immune system is working to prevent or clear an infection. Scientists can use it to estimate how well a vaccine may work.
Hybrid immunity: Scientists say a person has “hybrid immunity” if the person was infected with SARS-CoV-2 and then, at least six months later, received a COVID-19 vaccine. This scenario triggers high levels of antibodies and strong protection against variants. Some scientists have called it “super-immunity.”
Spike protein: You’ve probably seen more than enough pictures of the SARS-CoV-2 virus. The spike protein creates the bumps on the virus particle. It’s a critical region of the virus for several reasons. Spike proteins bind to human cells and help the virus gain entry inside cells. And these spike proteins are the target of many critical antibodies. A major way the immune system detects the virus is by its spike proteins.
Preventive measures: OK, this isn’t a hard one to figure out. And for now, the fact remains that not enough is known to take much action to forestall infections besides getting vaccinated and boosted and following the prevention strategies of masking and physical distancing that we’ve relied on throughout the coronavirus pandemic.” (C)
“How the Omicron Variant Got So Many Scary Mutations So Quickly
Mutations develop spontaneously as a virus replicates and spreads, but scientists are now trying to understand how so many mutations arose in Omicron in such a seemingly short space of time.
“The question is how this [rapid evolution] occurred, where it occurred, and which were the conditions that fueled [it],” says Gonzalo Bello, a virologist at the Oswaldo Cruz Institute in Rio de Janeiro. Bello was part of the team that tracked the rise of the Gamma variant in Brazil in November 2020, which fueled outbreaks in that country. With Gamma, “we realized that mutations did not appear in a single step in a single individual,” Bello explains. Instead, some mutations occurred in certain individuals but not in others. The rise of Gamma “was a process that occurred in a population of individuals, not in a single [person],” he says.
One possibility for how a heavily mutated variant, such as Omicron, could have arisen is that the virus began circulating and mutating in an isolated group of people, where it would have had an opportunity to change dramatically compared with variants outside of that bubble. It could then have gotten introduced, with its numerous mutations, into the larger population, where it was able to travel into different groups and countries, Bello says.
Alternatively, the virus may have mutated significantly in a single individual before finding a new host. “This could happen in an immune-suppressed person,” such as someone with HIV, Anna-Lise Williamson, chair in vaccinology at the University of Cape Town, and Ed Rybicki, director of the university’s biopharming research unit, wrote in response to e-mailed questions. South Africa has the world’s largest HIV epidemic, with more than seven million people infected with the virus. Neighboring countries also have widespread HIV infections. This has led some scientists to hypothesize that Omicron arose in Southern Africa because it was first identified there, but older cases have since been identified in European countries and the U.S…
To fully understand where Omicron arose, it helps to determine the earliest human patient or community in which it spread. But it is difficult to identify the patient zero for a variant, says Akiko Iwasaki, a professor of immunobiology and molecular, cellular and developmental biology at Yale University. “That is because [virus] surveillance efforts only capture a fraction of infected people,” she says. As more people are tested, and their viral genomes are sequenced, it may be possible to pinpoint a general geographic area and an approximate time when the virus mutated into Omicron, Iwasaki says.
For Michael Head, a senior research fellow in global health at the University of Southampton in the U.K., Omicron is a wake-up call about vaccine inequity and the need for greater access. According to Oxford University’s Our World in Data platform, as of November 30, about eight billion vaccinations had been administered globally, and only 6 percent of people in low-income countries had received at least one dose. African countries have administered more than 235 million vaccines, according to the African Centers for Disease Control and Prevention, but the continent’s population exceeds 1.2 billion people.
“The main variants that have caused concern so far—in terms of Alpha to Delta, basically—have emerged from uncontrolled outbreaks in unvaccinated populations,” Head says. “That’s where COVID thrives best and that’s where the virus has the greater chance to mutate. If [the vaccine] can keep outbreaks under control, you reduce those opportunities.”
The World Health Organization’s Director-General Tedros Adhanom Ghebreyesus echoed these sentiments on November 28: “The Omicron variant reflects the threat of prolonged vaccine injustice,” he wrote in a tweet. “The longer we take to deliver #VaccinEquity, the more we allow the #COVID19 virus to circulate, mutate and become potentially more dangerous.”” (D)
“As the world waits for studies that give a clear picture of the Omicron variant, early clinical data emerging from South Africa hint at a virus that may cause less severe cases of Covid-19.
The South African Medical Research Council posted a report Saturday of the early experiences at several hospitals in Gauteng Province, where Omicron was first spotted in the country. Strikingly, most hospitalized patients who tested positive for Covid did not need supplemental oxygen. Few developed Covid pneumonia, few required high-level care, and fewer still were admitted to intensive care.
Experts caution against reading too much into these early reports, which are based on small numbers of patients. They suggest it will take time for the true profile of the Omicron variant to come into focus. But several note that while early discussions about previous variants of concern have hinged on trying to figure out whether they caused more severe disease, with Omicron the questions relate to whether it is associated with milder infections.
The report included an analysis of 42 Covid patients in the hospital on Dec. 2 which showed that most were actually hospitalized for other medical reasons; their infections were only detected because hospitals are testing all incoming patients for Covid. Many did not have respiratory symptoms. And the average length of hospital stay was 2.8 days, far shorter than the average of 8.5 days recorded in the region over the past 18 months, the report said.
“The relatively low number of Covid-19 pneumonia hospitalizations in the general, high care and ICU wards constitutes a very different picture compared to the beginning of previous waves,” said the report, authored by Fareed Abdullah, director of the SAMRC’s office of AIDS and TB research.
Michael Osterholm, director of the University of Minnesota’s Center for Infectious Diseases Research and Policy, told STAT he has “really been impressed by the relative lack of severe illness” seen with Omicron so far. “We’re just not seeing the number of patients that have been seen in previous surges who are seriously ill, even this soon into the surge.”
Amesh Adalja, an expert on emerging infectious disease and pandemic preparedness at Johns Hopkins University’s Center for Health Security, said more data are needed but the early indications are “very intriguing.”
“It’s part of a trend of anecdotal reports that we’re hearing that the clinical spectrum seems to be more mild, especially in vaccinated people,” he said.
It appears that the Omicron variant may be more transmissible than past variants of the virus, potentially even the highly transmissible Delta variant. But it was only identified within the past couple of weeks and still makes up only a tiny fraction of cases worldwide, so drawing conclusions at this point is a risky business…
Osterholm and others said it will be important, too, to watch how Omicron fares in places where the Delta variant is currently causing high levels of illness, as is the case in many parts of the United States and Europe presently.
And Peter Hotez, dean of Baylor College of Medicine’s national school of tropical medicine, said we’ll have to see what kind of illness Omicron triggers in other places before drawing conclusions.
“Potentially it means that the severity of illness is less with Omicron,” he acknowledged. “But I think we have to be very cautious about making any definitive statement. Right now, it’s an interesting anecdote as much as anything else. And we’ll see as Omicron accelerates in the United States whether if falls along a similar pattern.”” (E)

Scientists do not yet know whether omicron is more infectious than other variants, whether it causes more severe disease or whether it will overtake Delta as the most prevalent variant. It may take several weeks to get answers to these questions.” (F)
“The arrival of the newest coronavirus variant, first identified in Botswana and South Africa and now present in the United States, might be bad news, or it might be terrible news—or maybe it’s just a temporary distraction from Delta. Ultimately, Omicron’s effect on the course of the pandemic will be determined by three factors: its transmissibility; the degree to which it evades our existing immune defenses; and its virulence, or the severity of the disease that it causes. If Omicron turns out to jump between hosts with ease, blow past our neutralizing antibodies, and cause unusually dangerous complications, we’ll all be in deep trouble. But it could also turn out to do a lot of other things, with more subtle implications. If Omicron ends up being super contagious, for example, but mild in its symptoms, that might even be a good thing—a perfect variant, just in time for Christmas…
If Omicron continues to show signs of being milder than Delta, that’s good news, of course. But if it also turns out to spread more quickly than Delta, that could be great news. When two variants are circulating, the one that infects more people more quickly will tend to dominate, said Samuel Scarpino, of the Rockefeller Foundation’s Pandemic Prevention Institute. That variant could win out either because it replicates more quickly in its human hosts and spreads more efficiently between them—that is, it’s more transmissible—or because it more deftly evades the immunity we already have….
Omicron could, of course, turn out to be pretty much anything. Maybe it’s somewhat more transmissible than Delta, but no more virulent; that, Kirk Sell said, could send us back to stricter masking and social-distancing programs for a while. Or else it might be less transmissible than Delta, and no better at slipping past our immune barriers, in which case “it’s going to be a blip on the horizon” regardless of how virulent it is, according to Halloran. If that happens, we’ll effectively be back where we were two weeks ago: stuck with Delta and waiting for the next shoe to drop.
There are worse fates. “In some ways, Delta is the ideal variant,” Scarpino said: It’s transmissible enough to dominate more dangerous variants, and its virulence can be controlled through vaccination. In the next few weeks, we’ll find out whether Omicron will have its own silver lining—or whether it’ll be catastrophically worse.” (G)
“On Thursday night, New York Governor Kathy Hochul announced that five cases of the new variant had been confirmed in the state. One of the cases involves a 67-year-old woman from Suffolk county “with some vaccination history” who had recently been in South Africa. The patient reported mild symptoms after testing negative upon her return on November 25. She tested positive on November 30. Two more cases were confirmed in Queens, in addition to one in Brooklyn, and a “suspected traveler case” in “one of the five boroughs,” according to public-health officials. The vaccination status and potential exposure of the four New York City cases were unknown…
Here are some things to know about New York and the Omicron variant:
This could be the fourth variant to hit the city in 2021
New York City has already weathered rising caseloads from three coronavirus variants this year. First there was a homegrown variant, called B.1.526 or Iota, which spread rapidly from January to March. That was followed this spring by the highly transmissible Alpha variant, first identified in Britain. Then by summer came the even more highly transmissible Delta variant, which caused a small wave of infections and then subsided in October before rising again in November.
Each new variant provoked worry. But so far, the coronavirus has been diminishing as a threat, after the devastating first wave last year. The long second wave that stretched from the autumn of 2020 to this spring saw far fewer infections and deaths than the first, even as several strains of Covid-19 spread.
The pattern of diminishing waves is different than what has been seen in much of the country and owes something to New York City’s relatively high rate of vaccination — about 77 percent of residents of all ages have received at least one shot.
There are other factors: Considerable levels of natural immunity from the devastation of the first wave in spring 2020. A willingness to wear masks. Widely available testing and a robust public health apparatus.
“I think we are potentially more prepared than most,” said Dr. Bernard Camins, an infectious diseases specialist and medical director of infection prevention for the Mount Sinai Health System. “The question is whether at this point people are more fatigued from all those mitigation strategies, and they may not listen.”
Mayor Bill de Blasio says that a mask mandate could prove a distraction from the city’s main goal: getting vaccines and booster shots into more people.
What happens next turns mainly on the variant
Initial reports suggest this variant might be especially contagious. But little is known about Omicron’s virulence, or what protection existing coronavirus vaccines offer.
“We do expect the vaccines to retain some degree of effectiveness, but precisely how much will take a few weeks to clarify,” Dr. Chokshi said on Monday. “We’re counseling, you know, a bit of patience as the science sorts itself out.”
But Dr. Denis Nash, an epidemiology professor at CUNY School of Public Health, warns that “if this is more transmissible than Delta or more virulent, or more evasive of the immune protection that the vaccine provides,” then the city “could be susceptible to a surge.”
Amid these unknowns, Mayor Bill de Blasio had some blunt advice: Get vaccinated, or get a booster shot, and “get those masks back on now,” he said on Monday. Fewer than 20 percent of adult residents have gotten their booster shot.
In particular, he urged parents to get their children vaccinated. Just 17 percent of 5- to 11-year-olds have received a first dose since they became eligible a month ago. “It’s time,” he said.
Since Nov. 1, daily case counts have risen more than 75 percent, reaching 1,500 newly identified cases a day last week. The rise, driven by the Delta variant, has been steepest in Queens, a borough with vaccination rates well above the citywide average.
The increase, epidemiologists say, is likely caused by the changing of the seasons, with colder weather pushing people indoors and prompting them to close windows, raising the risk of transmission. Cold, drier air may also benefit the coronavirus by helping viral particles to survive and linger in the air for longer.
Holiday travel and gatherings could also accelerate transmission.
One disease model from a team at New York University Grossman School of Medicine, which predated Omicron, predicted about 10 deaths a day from Delta in the weeks ahead, according to a member of the team, Dr. Ronald Scott Braithwaite, a professor at N.Y.U. and an adviser to the city. At an earlier point this year, during the height of last winter’s wave, there were as many as 98 Covid-19 deaths in a single day.
In New York City, the Delta variant is expected to drive only a minor surge in cases. But it is creating a potentially dire situation in Western New York, where last week, the number of coronavirus patients admitted to hospitals on a single day — 120 on Nov. 26 — was the highest in the region since the start of the pandemic.
Surveillance and testing systems are strong
As new forms of the virus began to emerge across the globe early this year, New York City began cobbling together its own surveillance system. Turning to universities, hospitals and other laboratories, the city set out to analyze at least 10 percent of coronavirus cases in New York for mutations.
When the coronavirus reached New York early last year, New York’s officials, including then-Gov. Andrew M. Cuomo and Mr. de Blasio, were slow to act.
Citing this history, a reporter asked Mr. de Blasio on Monday why he was merely recommending mask wearing, rather than imposing interventions like a mask mandate until more about Omicron was understood.
Mr. de Blasio said that he worried that a mask mandate would prove a distraction from the city’s main goal: getting vaccines and booster shots into more people.
“What we do not want to do is mix messages about what’s the thing that actually has the most profound impact,” Mr. de Blasio said. “The thing that we need to do with urgency is get people vaccinated.”
Dr. Jay Varma, an epidemiologist who has helped guide New York City’s pandemic response as a mayoral adviser, says that like Delta, Omicron could take months to spread widely in New York, giving the city time to nudge more residents to get vaccinations or boosters, and convince more companies to impose vaccine requirements.
Nearly 89 percent of adults in New York City have gotten at least one dose — six percentage points higher than the national average. In recent months, the city raised the vaccination rate through mandates for municipal employees and by imposing restrictions on people who are not vaccinated.
Yet the rate is uneven. Though the gap has shrunk, Black residents are less likely to be vaccinated than other groups, particularly among younger people; just 57 percent of Black New Yorkers between 18 and 44 have gotten at least one dose of a vaccine. And children younger than 5 still remain ineligible for vaccination.
Public health experts also worry that nursing homes are underprotected.
As of last week, only 51.4 percent of the nearly 92,000 nursing home residents statewide had received a booster shot, according to the state Department of Health. Across the city, there are more than 30 nursing homes where fewer than 25 percent of residents have received booster shots.
“I find it highly, highly distressing,” said Dr. Nash, the epidemiology professor. “I’m afraid it will be a travesty with a lot of avoidable deaths.” (H)

Today, President Biden will announce new actions to combat COVID-19 as the United States heads into the winter months and with the emergence of a new variant, Omicron.
The United States has come far in its fight against the virus and is more prepared than ever to deal with the challenges of COVID-19. We have the public health tools we need to continue to fight this virus without shutting down our schools and businesses. As we head into winter, today, the President will announce actions to provide additional protection to Americans and fight the Omicron and Delta variants, while keeping our economy growing.

Last week, after the World Health Organization (WHO) named the Omicron variant as a Variant of Concern, the President took immediate steps to restrict travel from the most impacted countries in order to give the U.S. time to learn more about the variant and prepare. We have more tools today to fight the Omicron variant than we have had to fight previous variants, including Delta. Nearly 60 percent of Americans are fully vaccinated, booster shots are authorized for all adults, and a vaccine is authorized for kids aged 5 and older. The U.S. is leading the world in vaccinating children, and millions of Americans have already gotten their boosters. And, the Food and Drug Administration (FDA) is reviewing additional antiviral treatments for when people do get sick.
Today’s actions will ensure we are using these tools as effectively as possible to protect the American people against this variant and to continue to battle the Delta variant during the winter months when viruses tend to thrive. These actions will help keep our economy growing and keep Americans safe from severe COVID-19.
Today, President Biden will announce the following actions:
Boosters for All Adults: President Biden will announce new steps to ensure that the nearly 100 million eligible Americans who have not yet gotten their booster shot, get one as soon as possible. As we face the Omicron variant, boosters are more important than ever. Boosters increase the strength of your antibody response, so when the virus mutates, a booster makes it more likely that your antibodies can protect you against the new variant. This week, the Centers for Disease Control and Prevention (CDC) issued updated guidance recommending that every adult get a booster. All adults who completed a primary vaccination series with an mRNA vaccine at least six months ago and those who received a Johnson & Johnson shot at least two months ago are eligible for a booster. The Biden Administration has made booster shots free and convenient at over 80,000 locations nationwide. To date, over 41 million Americans have already received a booster shot, including half of eligible seniors. The President will announce additional steps to help in this effort.
Expanding pharmacy availability through December and reaching out to all eligible customers to get their booster: As demand for boosters increases, the largest federal pharmacy partners will be ready to help meet that demand. Pharmacy partners are launching broad-based outreach campaigns to encourage the public to get their boosters, and are sending millions of texts, calls, and emails to eligible customers with information on how to schedule an appointment or walk-in for their booster shot. In addition, pharmacy partners will continue to offer call center services to support members of the public who need assistance, including in multiple languages. The President will also announce that the pharmacy program will continue to take steps to expand availability, so that adults can continue to receive their booster shots at trusted, convenient pharmacies near them. Nationwide, more than two in three COVID-19 vaccinations are already happening at local pharmacies. Federal pharmacy partners will continue to expand access by adding capacity across their network to meet the needs of their local communities. They will also work to ensure equitable access to boosters by offering vaccinations at convenient hours, including evenings and over weekends, so that adults can get boosted at a time that works best for them.
Launching a new public education campaign to encourage adults to get boosters, with a special focus on seniors: The Department of Health and Human Services (HHS) will launch a new public education campaign to ensure every adult American is getting their booster as soon as they are eligible. This effort will put a special focus on seniors who are the most vulnerable, including seniors from communities of color and seniors from underserved communities. The campaign will feature paid advertising across multiple channels, engagement with community organizations, robust stakeholder outreach, and earned media campaigns.
Collaborating with AARP on an education campaign focused on getting seniors boosted: AARP has been engaged in a robust education effort around COVID-19 and primary vaccinations throughout the pandemic. As we shift toward getting more seniors boosted, AARP will build on these efforts, and in collaboration with the Administration, will serve seniors through:
Town Halls: The Administration will participate in AARP-sponsored tele-town halls to reach thousands of seniors over the coming months and educate older Americans about the importance of boosters.
Rides to Booster Shots: AARP has committed to delivering rides through volunteers and partnerships with other organizations to help seniors get boosted at local pharmacies, clinics, events, churches, or other trusted locations.
Events and Call Center: AARP and the Administration will participate in local events and media opportunities across the country in the weeks ahead. In addition, the Administration has provided new training to help CDC’s National COVID-19 Vaccine Assistance hotline answer AARP members’ and all seniors’ questions about boosters or find an appointment at 1-800-232-0233.
Targeting outreach to Medicare beneficiaries: As part of a comprehensive plan to get older adults the extra protection they need through a booster, the Centers for Medicare & Medicaid Services (CMS) is launching an education and outreach initiative to get Medicare beneficiaries boosted. This will include sending a notice from the CMS Administrator to all Medicare beneficiaries encouraging them to get boosted and providing information about how to easily access a booster shot in their community. Approximately 63 million people are enrolled in Medicare, and this is the first time in more than 4 years that Medicare has sent all Medicare beneficiaries a notice of this kind. CMS will also send emails, add messages to the 1-800-MEDICARE call center and incorporate messaging into advertising campaigns highly targeted to high-risk audiences with lower booster uptake.
Calling on employers to follow the federal government’s lead and provide paid time off to their employees to get boosted: All federal employees currently receive paid time off to get booster shots. The President will call on employers throughout the country to remove a barrier to vaccination access by providing the same paid time off for their employees if they are not doing so already, including paid time off for family members getting their first, second, or booster shots. No one should have to choose between their pay check and getting the additional protection of a booster shot or a child vaccination. Currently, about one-third of workers report not receiving paid time off for vaccinations, and thirty-five percent of parents report being concerned about having to take time off work to get their child vaccinated or care for them if they experience side effects. Over the course of our pandemic response, these concerns have been even more pressing in our underserved communities; earlier this year, 64 percent of unvaccinated Hispanic/Latino adults and 55 percent of unvaccinated Black adults reported concern about missing work to get vaccinated.
Vaccinations to Protect Our Kids and Keep Our Schools Open: The President will announce new actions to get more kids ages 5 and older vaccinated and to keep our schools open. When the President came into office, more than half the schools in our country were closed. Today, 99 percent of schools across the country are fully open and in person. The steps the President is announcing today will ensure that remains the case. As we face the Omicron variant, we now have an important new tool: vaccines for kids ages 5-11. The U.S. leads the world in vaccinating children in this age group. To date, we have already vaccinated over 4 million 5- to 11-year-olds and 15 million adolescents. Vaccinating our kids protects them, keeps schools open, and protects everyone around them. The Biden Administration has made it easy for parents to get their kids vaccinated with over 35,000 sites that parents know and trust, including pharmacies, pediatricians’ offices, children’s hospitals and school-based clinics. The President will announce new actions to get kids vaccinated and ensure that schools stay open.
Launching hundreds of family vaccination clinics to get the whole family vaccinated or boosted in one trusted and convenient location: The President will announce a new effort to launch hundreds of family vaccination clinics across the country. Together, these clinics will offer vaccinations for the whole family – with first shots for parents, teens, and kids, and boosters for those eligible. This model builds on the Administration’s ongoing efforts – in coordination with states, localities, providers and community leaders – to meet people where they are and make vaccinations accessible and convenient in communities across the country. HRSA will launch Family Vaccination Days – with hundreds of community health centers across the country hosting family vaccination clinics throughout December. Over two in three shots at community health centers are administered to people of color. FEMA will launch Family Mobile Vaccination Clinics, deploying sites, staff, and support to states across the country that need help – beginning with its first deployments to Washington and New Mexico. States and localities will pioneer this model nationwide – with full federal funding and support, and receive a new playbook to provide all partners with the information they need to stand up these sites and increase equitable access to vaccination. And, at thousands of pharmacies nationwide, federal pharmacy partners – including CVS and Rite Aid – will make available family-based scheduling over the coming months so that parents have a one-stop-shop to get their family their vaccination appointments all at once.
Requiring Medicaid to pay health care providers to talk to families about getting their kids vaccinated: Increasing COVID-19 vaccinations and centering equity in COVID-19 vaccinations requires meeting our hardest-hit and highest-risk communities where they are with information from trusted sources. To help parents get their questions answered and make informed decisions about COVID-19 vaccination for their children, all Medicaid programs will pay health care providers to talk to parents about the importance of kids’ vaccination. Medicaid will cover COVID-19 vaccine counseling visits for most children and youth up to age 21 with 100% federal funding throughout the public health emergency and the following year. Medicaid and the Children’s Health Insurance Program provide health insurance coverage to over 40 percent of all children in the United States and are a significant source of coverage for Black and brown children. Today’s action will help expand access to individualized medical advice in all of our communities and give families the support they need to engage with trusted community providers.
Reviewing school COVID-19 prevention policies to avoid closures of entire classrooms or schools when there is a positive case: Thanks to efforts from the Biden Administration, 99 percent of schools are now open for full-time in-person learning – up from 46 percent at the beginning of the Administration. This progress has been crucial to making sure all students can safely be back where they belong – learning alongside their peers – and to help them accelerate through any learning loss they may have experienced in the last year and a half. Schools can stay open safely by implementing layered prevention strategies, and with the tools, guidance, and resources the Administration has provided, schools should not have to close due to COVID-19. To make sure schools can remain safely open:
CDC will release findings on quarantine and testing policies in schools: Today, close contacts who are not fully vaccinated should be referred for COVID-19 testing and quarantine at home for up to 14 days after exposure. While it is crucial to take proper measures to contain spread of the virus following COVID-19 exposure, this period of quarantine can significantly interrupt student’s learning, and make it challenging for parents to work. States and districts around the country have been pioneering alternative approaches to quarantine, including “test to stay” policies – where exposed students remain in school, wear masks, and test repeatedly in the days following exposure to identify and contain infection. CDC has been studying approaches to quarantine and testing, including looking at the science and data of how they may keep school communities safe. CDC will release their findings on these approaches in the coming weeks.
The Administration will issue a new “Safe School Checklist” to give schools a clear game plan for how to get as many of their staff and students vaccinated as possible: The best way to avoid outbreaks in schools is to stop transmission before it happens – and the best tool we have to stop transmission and keep schools open is vaccinating everyone who is eligible. Now, all students ages 5 and up are eligible for COVID-19 vaccination. Today, the Administration will issue a new “Safe Schools Checklist” to all K-12 schools, detailing a set of actions that every school can take to get their staff and students vaccinated – including hosting school-located vaccination clinics, hosting community-based and family vaccination clinics and events, implementing vaccination requirements for school staff, and getting eligible vaccinated school staff booster shots. The Checklist will also link to resources schools can use to set up these initiatives and talk to families about the importance of vaccination.
Providing every resource to the FDA to support timely review of applications for vaccines for individuals under the age of 5: The President will announce that, as he did for a vaccine for kids ages 5-11, he supports the independent scientific review of a vaccine for those individuals under the age of 5 and will provide the FDA with any needed resources to do this safely and as quickly as possible once data is submitted to the agency.
Expanding Free At-Home Testing for Americans: Today, the President will announce new steps to ensure that Americans has access to free at-home testing. First, the more than 150 million Americans with private insurance – who now are able to get tests covered in physician offices, pharmacies, and clinics with no cost sharing – will also be able to get at-home tests reimbursed by their insurance. Second, for those not covered by private insurance, in addition to more than 20,000 federally-supported free testing sites across the U.S., at-home tests will be distributed through key community sites, such as health centers and rural clinics. The Biden Administration has taken significant steps to increase testing in the country since January. We are on track to quadruple the supply of rapid at-home tests that we had in late-Summer. Today’s actions will help Americans access the tests they need to help them stop the spread of COVID-19 to others.
Providing health plan coverage of no-cost rapid, over-the-counter (OTC) COVID-19 tests: To expand access and affordability of at-home COVID-19 tests, the Departments of Health and Human Services, Labor and the Treasury will issue guidance by January 15th to clarify that individuals who purchase OTC COVID-19 diagnostic tests will be able to seek reimbursement from their group health plan or health insurance issuer and have insurance cover the cost during the public health emergency. Workplace screening would remain consistent with current guidance. Today’s announcement follows the President’s September action directing more than $2 billion to accelerate the production of rapid tests and an additional $1 billion investment in procuring at-home tests. Over the same time period, FDA authorized five additional over-the-counter tests. A total of 8 tests are on the market today; no test was on the market when the President took office.
Expanding community distribution of free at-home tests through neighborhood sites such as health centers and rural clinics: To ensure equitable access to free at-home tests for our uninsured and underserved communities, the President will double the commitment from September to distribute 25 million free tests to community sites to 50 million tests and will add rural clinics to the program. Partnerships with trusted community providers will aid in getting these important testing supplies into the homes of our hardest-hit communities.
Stronger Public Health Protocols for Safe International Travel: Last month, the Administration implemented stronger international travel protocols, including requirements for foreign travelers to be fully vaccinated. The very day the WHO identified the new Omicron variant, the Biden Administration took immediate steps to restrict travel from the countries in the region where it was confirmed to be spreading quickly. The President will announce additional steps to strengthen the safety of international travel as we face this new threat – just as we have faced those that have come before it.
Strengthening global pre-departure testing protocols: Early next week, the United States will tighten pre-departure testing protocols by requiring all inbound international travelers to test within one day of departure globally, regardless of nationality or vaccination status. This tighter testing timeline provides an added degree of public health protection as scientists continue to assess the Omicron variant.
Extending the requirement to wear a mask on airplanes, rail travel, and public transportation: The Administration will continue to require masking during international or other public travel – as well as in transportation hubs such as airports or indoor bus terminals – through March 18 as we continue to battle COVID-19 this winter. The Transportation Security Administration will extend its implementing orders to maintain these requirements through March 18. Fines will continue to be doubled from their initial levels for noncompliance with the masking requirements – with a minimum fine of $500 and fines of up to $3,000 for repeat offenders.
Protecting Workplaces to Keep Businesses Open: Today, the President will announce additional progress we’re making in protecting workers and keeping our economy growing and businesses open. Since President Biden took office, the economy has added 5.6 million jobs, new unemployment claims have fallen by 70 percent, and applications for new businesses have risen 30 percent above the pre-pandemic average. To protect this progress and to ensure workers stay safe and on the job, we have to slow the spread of COVID-19 in our workplaces and places of businesses. Vaccination requirements do just that. The President is calling on businesses to continue to take steps to ensure workers are protected as we head into the winter.
Calling on businesses to move forward with vaccination or testing programs: The President will call on businesses to move forward expeditiously with requiring their workers to get vaccinated or tested weekly. This is especially important given the Omicron variant. No business should shut down this winter because of COVID-19. The Department of Labor has provided a clear roadmap to help businesses keep workers safe and their doors open. Already, 60 percent of businesses report they are moving forward with implementing a program to ensure their workers are either vaccinated or tested on a weekly basis, and the U.S. Chamber of Commerce and more than 100 leading public health experts have encouraged businesses to not delay in implementing these protective measures. The President will urge businesses to take steps now to protect workers, customers, and the economy.
Rapid Response Teams to Help Battle Rising Cases: Today, the President will announce new actions to help states battle any potential COVID-19 outbreaks this winter, including of the Omicron variant. As we worked to bring down the Delta surge throughout this summer and fall, we successfully deployed thousands of federal personnel to help 27 states and two territories. These COVID-19 Surge Response Teams mobilized to address critical needs on the ground, including personnel, therapeutics, and technical expertise. To date, we have deployed over 2,000 personnel, including 1,300 clinical providers; surged over 3,200 ventilators, ambulances and other critical supplies; and shipped over 2.3 million courses of lifesaving monoclonal antibody treatments. As we face the potential of a new variant and rising cases during the winter months, today, the President will make clear that federal government will once again be prepared to help.
Making 60+ Winter COVID emergency response team deployments available to states: To ensure states have the help they need as they battle rising cases, the President will announce new emergency response teams that will be available to help supplement state efforts. These teams include:
20+ Department of Defense Medical Response Team deployments to support clinical staffing at strained hospitals.
10 National Disaster Medical System team deployments to provide clinical support at strained hospitals.
20+ monoclonal antibody strike team deployments to support the administration of these lifesaving treatments.
15+ CDC expert deployments to conduct outbreak investigations and provide epidemiological or technical support whenever needed.
Strengthening our national volunteer emergency medical response corps to support communities in need: To help local municipalities strengthen their health preparedness and response, today the President will announce $20 million in funding from the American Rescue Plan to strengthen the Medical Reserve Corps (MRC), a network of medical and public health volunteers organized locally to improve the health of their communities. This funding will provide additional resources needed to support the roughly 300,000 MRC medical and public health professionals who have already volunteered more than 2 million hours toward local COVID-19 response. HHS will also lead a national effort to mobilize volunteers, including retired doctors and nurses, in areas with rising COVID cases.
Supplying Treatment Pills to Help Prevent Hospitalizations and Death: As we head into winter, new COVID-19 treatments may be on the horizon that could help prevent hospitalization and death. As these treatments continue to be developed and reviewed, today, the President will announce that we are ensuring that if and when any new COVID-19 treatment pills have been found to meet FDA’s scientific standards, they are equitably accessible to all Americans, regardless of their income or their zip code.
Securing enough supply and ensuring pills are widely available in the hardest-hit, highest-risk communities: The Administration is taking steps to secure 13 million doses of antiviral courses to ensure we have ample supply for Americans who need treatment. That number is six times the number of COVID-19 hospitalizations reported throughout this entire year. As more and more treatments may become available in the coming months, the Administration will ensure our underserved communities, often at highest risk from the virus, will be able to conveniently access these potentially lifesaving treatments.
Continued Commitment to Global Vaccination Efforts: Today, the President will reaffirm his commitment to help vaccinate the globe, and call on other countries to do the same to combat this pandemic globally and help prevent the development of new variants.
Donating 1.2 billion doses to the world: To date, the U.S. has committed to donate 1.2 billion doses to the world. For every one shot the Biden Administration has administered in the U.S., we are donating about three doses to people around the world. The U.S. is also the first country to give up its place in line for vaccines, allowing the African Union to immediately start receiving up to 110 million doses of Moderna at a reduced rate negotiated by the United States. And, we are working with partners to expand supply of critical ancillary needs like syringes.
Accelerating the delivery of more vaccines to countries in need by pledging to deliver 200 million more doses in the next 100 days. To build on our donations of over 275 million doses that have been shared with 110 countries, including 94 million doses to Africa, we are pledging to deliver 200 million more doses in the next 100 days – accelerating the delivery of vaccines to countries in need. These doses are being delivered for free with no strings attached. The U.S. is also the first country to negotiate a deal with J&J and the COVAX facility to send vaccines directly to humanitarian settings and conflict zones to vaccinate displaced people.
Taking steps to ramp up manufacturing here and abroad, building out a sustainable supply chain and increasing capacity globally to make vaccines: We are ramping up vaccine manufacturing, building a sustainable supply chain and increasing global capacity to make additional vaccines. We will continue our work to invest in companies that have experience manufacturing mRNA vaccines to help them expand capacity by an additional 1 billion doses per year, with production starting by the second half of 2022.
Turning vaccines into vaccinations: We are working with country partners to get doses into arms with plans tailored to specific country needs. With lines of effort ranging from communications campaigns to build vaccine confidence, to funding for vaccinators on the front line – the U.S. government is committed to getting people vaccinated around the world.
Steps to Ensure We Are Prepared for All Scenarios: Health and medical experts believe that the current vaccines authorized in the U.S. provide at least some protection against the Omicron variant and that boosters strengthen that protection significantly. However, to ensure that we are prepared for all scenarios, the Administration is taking steps now to be able to quickly act if updated vaccinations or boosters are needed to respond to the Omicron variant.
Accelerating the development and deployment of new vaccines and boosters if needed for the Omicron variant: The President is committed to using every resource and tool available to the U.S. government to ensure that we can quickly get updated vaccines and boosters to the American people in the unlikely event they are needed to battle the Omicron variant. The Administration is working closely with executives at Pfizer, Moderna, and Johnson & Johnson to develop contingency plans for other vaccinations or boosters if needed. The President will also use every resource available to help support the FDA and CDC to quickly review new vaccines, if needed, while ensuring that the rigorous safety review process is upheld. The President is also committed to doing the same for any COVID-19 treatment that may be needed for this variant.
