Florida’s top public health official in Orlando has been placed on administrative leave after sending an email to his employees noting their lackluster coronavirus vaccination rates and urging them to get the shots.
The official — Dr. Raul Pino, the administrator for the Florida Department of Health’s office in Orange County — sent the email on Jan. 4, in the thick of a surge in cases caused by the Omicron variant.
In the email, Dr. Pino said that he had asked a staff member to pull out the vaccination rates for the office, and that the figures were alarming: Of the office’s 568 employees, only 219 — fewer than half — had completed a full vaccination series, and just 77 of them had received a booster shot, a number he called “SUPER LOW.”
“I am sorry, but in the absence of reasonable and real reasons, it is irresponsible not to be vaccinated,” Dr. Pino wrote in the email, which was first reported by WFTV, the local ABC News affiliate. He called the office’s vaccination rate “pathetic.” (A)
“Pino, 58, declined to discuss the Health Department’s action, but an agency spokesperson confirmed his status.
“As the decision to get vaccinated is a personal medical choice that should be made free from coercion and mandates from employers, the employee in question has been placed on administrative leave, and the Florida Department of Health is conducting an inquiry to determine if any laws were broken in this case,” spokesperson Weesam Khoury said in an email Tuesday. “The Department is committed to upholding all laws, including the ban on vaccine mandates for government employees and will take appropriate action once additional information is known.”..
Since March 2020, when the pandemic erupted in Central Florida, Pino has been a fixture on more than 150 press briefings, appearing beside Orange County Mayor Jerry Demings to discuss COVID-19, best practices, safety protocols, coronavirus infection, testing and vaccination rates, and deaths…
Pino also formerly served as commissioner of the Connecticut Department of Public Health.

“Vilified, threatened with violence and in some cases suffering from burnout, dozens of state and local public health officials around the U.S. have resigned or have been fired amid the coronavirus outbreak, a testament to how politically combustible masks, lockdowns and infection data have become…
A review by the Kaiser Health News service and The Associated Press finds at least 49 state and local public health leaders have resigned, retired or been fired since April across 23 states. The list has grown by more than 20 people since the AP and KHN started keeping track in June.
The departures are making a bad situation worse, at a time when the U.S. needs good public health leadership the most, said Lori Tremmel Freeman, CEO of the National Association of County and City Health Officials.
“We’re moving at breakneck speed here to stop a pandemic, and you can’t afford to hit the pause button and say, ‘We’re going to change the leadership around here and we’ll get back to you after we hire somebody,’” Freeman said…
Many of the firings and resignations have to do with conflicts over mask orders or social distancing shutdowns, Freeman said. Many politicians and ordinary Americans have argued that such measures are not needed, contrary to the scientific evidence and the advice of health experts.
“It’s not a health divide; it’s a political divide,” Freeman said…
West Virginia Gov. Jim Justice forced the resignation of Public Health Commissioner Dr. Cathy Slemp in June over what he said were discrepancies in the data. Slemp said the department’s work had been hurt by outdated technology like fax machines and slow computer networks.
“We are driving a great aunt’s Pinto when what you need is to be driving a Ferrari,” Slemp said.
Tom Inglesby, director of the Center for Health Security at Johns Hopkins, was critical of Slemp’s firing and said it was deeply concerning that public health officials who told “uncomfortable truths” to political leaders had been removed.
“That’s terrible for the national response because what we need for getting through this, first of all, is the truth. We need data and we need people to interpret the data and help political leaders make good judgments,” Inglesby said.” (D)

“The backlash has moved beyond the angry fringe. In the courts, public health powers are being undermined. Lawmakers in at least 24 states have crafted legislation to weaken public health powers, which could make it more difficult for communities to respond to other health emergencies in the future.
“What we’ve taken for granted for 100 years in public health is now very much in doubt,” said Lawrence Gostin, an expert in public health law at Georgetown University in Washington, D.C…
The departures accelerate problems that had already weakened the nation’s public health system. AP and KHN reported that per capita spending for state public health departments had dropped by 16%, and for local health departments by 18%, since 2010. At least 38,000 state and local public health jobs have disappeared since the 2008 recession.
Those diminishing resources were already prompting high turnover. Before the pandemic, nearly half of public health workers said in a survey they planned to retire or leave in the next five years. The top reason given was low pay.
Such reduced staffing in departments that have the power and responsibility to manage everything from water inspections to childhood immunizations left public health workforces ill-equipped when COVID-19 arrived. Then, when pandemic shutdowns reduced tax revenues, some state and local governments cut their public health workforces further.
“Now we’re at this moment where we need this knowledge and leadership the most, everything has come together to cause that brain drain,” said Chrissie Juliano, executive director of the Big Cities Health Coalition, which represents leaders of more than two dozen public health departments…
The politicization has put some local governments at odds with their own health officials.
In California, near Lake Tahoe, the Placer County Board of Supervisors voted to end a local health emergency and declared support for a widely discredited “herd immunity” strategy, which would let the virus spread. The idea is endorsed by many conservatives, including former Trump adviser Dr. Scott Atlas, as a way to keep the economy running, but it has been denounced by public health experts who say millions more people will unnecessarily suffer and die. The supervisors also endorsed a false conspiracy theory claiming many COVID-19 deaths are not actually from COVID-19.
The meeting occurred just days after the county public health officer, Dr. Aimee Sisson, explained to the board the rigorous standards used for counting COVID-19 deaths. Sisson quit the next day…
As public health officials depart, the question of who takes their places has plagued Dr. Oxiris Barbot, who left her job as commissioner of New York City’s health department in August amid a clash with Democratic Mayor Bill de Blasio. During the height of the pandemic, the mayor empowered the city’s hospital system to lead the fight against COVID-19, passing over her highly regarded department.
“I’m concerned about the degree to which they will have the fortitude to tell elected officials what they need to hear instead of what they want to hear,” Barbot said.” (E)

Riverside County, California, was one of the first counties in the country to face Covid-19, when repatriated US citizens were quarantined at March Air Reserve Base on January 29, 2020. Seeing how quickly things moved in China, then Riverside County Health Officer Dr. Cameron Kaiser knew he had to act fast.
“From what we saw was going on in China at the time, it potentially, it could be very big,” he said.
On April 4, 2020, Kaiser took actions more protective than the state’s health orders, including prohibiting gatherings of people from different households in his county, as well as an order for people to mask in public.
He knew none of this would be easy, but he didn’t expect the backlash.
“We never really dealt with a situation where people simply told us to go jump in a lake when we had to make those necessary orders to protect society,” Kaiser said.
A month later, dozens of citizens came before the Riverside County Board of Supervisors — many of them saying that their personal liberties had been infringed upon.
“You are trampling on the Constitution by agreeing to extend Dr. Kaiser’s recommendations,” declared one resident.
Another said: “When we are inside, locked up because of health orders by Dr. Kaiser, we are weak. The American people are a free people.”
A fake Twitter account was created with Kaiser’s picture and a Hitler mustache, calling him Führer Cameron Kaiser.
What’s the science behind CDC’s decision to say fully vaccinated people don’t need masks?
Kaiser says there were many people in the community who supported him, and he did try to acknowledge their concerns. But there were also those who were very vocal and angry. “Listening to the anger, the vitriol, again, they were coming from people who had legitimate concerns, but some of them were also, frankly, simply ways to get at me directly. And that was really hard to listen to. That’s the kind of thing you never want to go through again in your life,” he said.
On May 8, 2020, the Riverside County Board of Supervisors directed Kaiser to rescind his orders, so that the county would be more in line with the state. He said in the months that followed, the threats continued, and he was given fewer and fewer responsibilities at work.
Two months ago, Kaiser received a call from the Riverside County CEO telling him that his services were no longer needed. Kaiser says since his appointment as the county health officer in 2011, he’s had no issues or complaints about his work.
CNN asked the Riverside County Board of Supervisors about Kaiser’s position and they replied by thanking Kaiser for his service and noting the decision to terminate Kaiser’s position had been made by the County CEO, Jeff Van Wagenen. They added that the Board of Supervisors had appointed a new health officer and said they would provide no additional information.” (F)

“Lori Tremmel Freeman, the CEO of the National Association of County and City Health Officials, said more than 500 public health officials have been pushed out or have left their jobs since the early days of the pandemic.
“For us to see this level of turnover is just really tough — tough for the community and tough for our response,” Freeman said. “We don’t have a lot of people in line to take the positions, because they’re difficult. And, of course, the more we talk about how they’re a target, with threats and intimidation and other things, the less appealing those positions sound.”..
The exodus of public health officials is raising concerns with experts like Freeman about the country’s ability to respond to the highly transmissible omicron variant, which is putting further strain on the nation’s health system.
“Our public health workforce has lost over 20 percent of its workers in the last decade due to disinvestment, so these losses are coming on top of losses to the field,” Freeman said. “And as we head into omicron and we’re hearing more and more about the seriousness of transmission, we worry about the capacity of our local health departments to continue to respond.”
Still, some advocates say, the pandemic has presented an opportunity for officials to reconnect with their communities and teach people about their roles.
“We need to make sure people understand what we do and how we protect them,” Dr. Georges Benjamin, the executive director of the American Public Health Association, said in a phone interview.
At the same time, he said, “anyone who thinks we’re suddenly going to wake up two months from now and things are going to be the way they were two years ago is fooling themselves.”
For some public health officials, the backlash to their Covid recommendations can be confounding.
Lisa Macon, a local health director for Granville and Vance counties in North Carolina, said that even though “we are used to having good dialogue across political lines most of the time” in a state with a Democratic governor and a Republican legislature, “it’s still really challenging.”
“It’s hard to understand how people are against the things that are intended to make people safe and save lives and keep people out of the hospital and prevent disease and death,” said Macon, who is also president of the National Association of County and City Health Officials. “I just struggle to understand it other than we know we’re having political and cultural wars right now.”” (G)
“In June, a study published in the Centers for Disease Control and Prevention’s Morbidity and Mortality Weekly Report found that nearly 24% of more than 26,100 state, tribal, local and territorial public health workers surveyed this spring were feeling bullied, threatened or harassed due to work. Almost 12% had received job-related threats.
The problem not only endangers the safety and mental well-being of public health workers; it is pushing out leaders when public health needs them most. It could also make it harder to recruit new workers into governmental public health. According to an analysis from Kaiser Health News and The Associated Press, published in December, at least 181 state and local public health leaders in 38 states had resigned, retired or been fired since April 1.
In August 2020, Joshua Sharfstein, MD, vice dean for public health practice and community engagement at the Johns Hopkins Bloomberg School of Public Health, coauthored an article in the Journal of the American Medical Association highlighting attacks on public health workers. He urged local officials to step up protections, including investigating credible threats, providing security detail when needed and prosecuting those who break the law. A year later, the backlash against local health officials seems to be getting worse, he said, egged on by legislative attempts in more than a dozen states to restrict and undermine the authority of health agencies to act in emergencies.
Sharfstein said many of the factors contributing to the backlash were entrenched well before COVID-19, such as political polarization and social media sites that make it easy to spread misinformation and extremist views. But he said elected officials and community leaders can help by speaking up in support of public health authorities and making sure workers are protected from threats…
Georges Benjamin, MD, executive director of APHA, also urged elected officials to speak out in support of local health authorities and condemn harassment. A former health official in Maryland and Washington, D.C., Benjamin said he has spoken at contentious public hearings during his career, but never experienced the “vitriol and anger” public health workers are now facing.
“People forget that when you run out your local epidemiologist because you’re mad about masking advice, you’re also running out the person protecting you from all kinds of other diseases,” he said. “Elected officials need to control these public meetings and be clear that attacks on public health officials will be prosecuted.” (H)
“A large-scale survey conducted by the federal Centers for Disease Control and Prevention released last month found that more than half of people working in public health at the state, tribal, local and territorial levels during the pandemic reported symptoms of at least one serious mental health condition…
Unless the well-being of these workers is addressed, said Amber Williams, a senior vice president of the Association of State and Territorial Health Officials, “we will continue to get what we are getting now, which is a burned-out workforce and less robust response to a pandemic.”
She noted that the system’s capacity to address the problem is questionable. “There’s very little bandwidth to make the changes that are needed,” she said. It remains an all-hands-on-deck situation that requires long hours and workers assuming unfamiliar roles.
Rao said that while the top-level findings of the report weren’t unexpected, some incident details were.
Nearly 12% of the 26,174 public health workers surveyed said they had received job-related threats since the start of the pandemic. Nearly a quarter said they had felt bullied, threatened or harassed as a result of their work.
“Given the length of the response of pandemic, we had a feeling that public health workers were under immense amount of stress,” said Rao. “The amount of threats, harassment and bullying, that was the surprise.”..
While clinicians treat one patient at a time, the job of public health workers is to protect the health of populations. That means promoting healthy lifestyles and prevention of disease and injury, and detecting and responding to communicable diseases…
Many workers had to shift away from usual jobs and schedules during the pandemic’s early days, and then had to scramble to find personal protective gear, ramp up contact tracing of people infected with the virus or help those experiencing homelessness quarantine after testing positive.” (I)
“Billions of dollars have been made available to public health by the federal government, but most of it has been geared toward stemming the emergency, rather than hiring permanent staff or building long-term capability. Most of the departments that responded to The Times’s survey said they were worried about their funding levels, which in most cases had been decreasing or flat before the pandemic. About three dozen departments said their budgets were the same or smaller than they were at the beginning of the pandemic.
At least 32 states have enacted legislation restricting state or local authority over health and emergency decisions.
There are already signs that the growing shortfalls in public health could have lasting impacts beyond the pandemic.
More than 220 departments told The Times they had to temporarily or permanently abandon other public health functions to respond to the pandemic, leading to a spike in drug overdoses and a disturbing drop in reports of child abuse. Several health officials pointed to runaway infections of sexually transmitted diseases, with gonorrhea cases doubling and syphilis on pace to triple in one county in Pennsylvania. Oswego County, N.Y., recorded a surge in lead poisoning. In Texas, requests for exemptions to the usual suite of required childhood immunizations have risen sharply…
The Times spoke with dozens of lawmakers who have introduced such legislation, most of whom shared a concern that health officials had overstepped their authority and required a check on that power.
“It’s a very dangerous situation when you decide to take away anybody’s rights,” said Bob Rommel, a Republican lawmaker in Florida. He drafted a bill, whose main provisions were incorporated into a law that took effect this summer, allowing the governor to squelch local health orders deemed too restrictive.
Citing that law, Gov. Ron DeSantis’s government, which has been aggressive in slapping down local restrictions, fined Leon County $3.5 million this month for mandating Covid-19 vaccinations for its employees — $5,000 for each person required to get a shot. The state has threatened to levy millions of dollars more in fines for similar county mandates.” (J)
“This has been a major, unprecedented loss in public health leadership across the nation,” Dr. Richard Besser, former acting director of the Centers for Disease Control and Prevention and president and CEO of the Robert Wood Johnson Foundation, told ABC News.
While prepared to handle the relentless strain that comes with managing the COVID-19 crisis, officials told ABC they had not anticipated the cross purposes of scientific response and political backlash — in both red and blue states — that hamstrung the offices in which they’d been tasked to serve — and where some said they found little support from their leadership.
“This past year, you saw public health as a science and as a field being berated and belittled,” Besser said. We saw it being lifted up as the enemy of economic recovery — rather than the path to sustained economic recovery.”
The contentious atmosphere between political and public health risks eroding trust and morale in the system, experts say; that only gets exacerbated as science becomes the target of public ire — and physical threats.”” (K)
“Mr. Biden and his team have gotten much right, including getting at least one dose of a vaccine into nearly 85 percent of Americans 12 and older and rolling out life-saving treatments. Those achievements have put the United States in a far better place to combat the virus than it was a year ago, with most schools and businesses open and the death rate lower because the vaccine significantly reduces the chance of illness or death, even from the highly contagious Omicron variant.
But an examination of Mr. Biden’s first year of fighting the virus — based on interviews with scores of current and former administration officials, public health experts and governors — shows how his effort to confront “one of the most formidable enemies America has ever faced,” as he recently described it, has been marked by setbacks in three key areas:
The White House bet the pandemic would follow a straight line, and was unprepared for the sharp turns it took. The administration did not anticipate the nature and severity of variants, even after clear warning signals from the rest of the world. And it continued to focus almost single-mindedly on vaccinations even after it became clear that the shots could not always prevent the spread of disease.
The administration lacked a sustained focus on testing, not moving to sharply increase the supply of at-home Covid tests until the fall, with Delta tearing through the country and Omicron on its way. The lack of foresight left Americans struggling to find tests that could quickly determine if they were infected.
The president tiptoed around an organized Republican revolt over masks, mandates, vaccine passports and even the vaccine itself, as he worried that pushing certain containment measures would only worsen an already intractable cultural and political divide in the country. The nation’s precarious economic health, and the political blowback that Mr. Biden and members of his party could face if it worsened, made him all the more cautious. So rather than forcing Americans to get shots, he spent months struggling to accomplish it through persuasion….
The episode laid bare a fundamental problem. Some of the administration’s most difficult public health decisions are essentially hammered out by a handful of senior health officials who hold roughly the same status, none of whom are in charge. They are overseen by Mr. Zients, a former economic policy adviser to President Barack Obama who is known for his logistical and planning skills but has no public health expertise. No single public health expert has the role of guiding the response, running interference between various players or standing up to the White House when necessary.

“I would hope that that’s the case. But that would only be the case if we don’t get another variant that eludes the immune response of the prior variant,” he added…
Fauci’s comments come a week after Europe’s top medical product regulation agency said that the COVID-19 omicron variant may be pushing the pandemic into becoming endemic.
Marco Cavaleri, head of vaccine strategy for the European Medicines Agency (EMA), told reporters on Tuesday that the natural immunity conferred by the highly infectious omicron strain may be fast-tracking the progress towards endemicity.
According to the BBC, epidemiologists would consider a disease endemic when “levels are consistent and predictable — unlike the boom and bust waves so far in the pandemic.”…
However, he added that “we don’t want to get into a whack-a-mole for every variant, where you have to make a booster against a particular variant. You’ll be chasing it forever.”” (A)
“Dr. Fauci also said that the world is still in the first of what he considered to be the five phases of the pandemic. The first is the “truly pandemic” phase, “where the whole world is really very negatively impacted,” followed by deceleration, control, elimination and eradication.
He said that only one infectious human disease has ever been eradicated: smallpox.
“That’s not going to happen with this virus,” he said.
However, once countries reach the “control” phase, when the virus becomes a “non-disruptive presence,” then the virus will be considered endemic, he said. The rhinovirus and some upper respiratory infections are examples of endemic diseases.
As Omicron continues to spread at a breakneck speed, some governments seem resigned to the idea that Covid is already a fixture of daily life. In some European countries, the authorities are pushing a “learn to live with it” approach that includes shorter isolation periods and the elimination of pre-departure tests for travel.” (B)
“Dr. Steven Brown is on the front line of the battle against the surge of COVID-19 cases. But for him, the front line is also the home front.
Working overnights, the 66-year-old critical care specialist manages hundreds of intensive care unit patients in hospitals across the Midwest, more than half of them COVID cases, many on ventilators. And he does it from his living room in suburban St. Louis.
Sitting before an array of four large computer screens and two laptops, he compares his work to that of an air traffic controller. He can read charts, scans, X-rays and even look in on patients with the help of sophisticated cameras in the ICUs.
“Each room has its own camera in it and I’m remotely operating it,” he told ABC News.
So despite being distant from his patients, his care is intimate. “I can look in a throat. I can look at how they’re using their muscles of respiration and whether they have disordered breathing. I’m able to do extreme fine-tuning of the ventilator settings for patients.”
And lately, more and more of his cases are COVID patients on ventilators. On his regular 12-hour shifts, he reports that the “amount of disease I am seeing is amplified. While some critical care doctors on a night shift might be managing eight or 10 patients with COVID-19 who are on ventilators for their shift, I’m managing 10 times as many because I’m managing patients in multiple sites….
Working from home, however, does not mean working less. Brown forecasts that the surge in the disease will mean a surge in his already grueling work schedule, from 10 straight days of 7 p.m. to 7 a.m shifts to 12 days in a row or more…
The eICU program, which began in 2008, is a virtual intense care unit where nurses and doctors provide care to patients at a UW Health office building. Care is provided at seven hospitals and eight ICUs across Wisconsin and northern Illinois 24 hours a day, Lynn Jacobs, a nursing supervisor at UW Health said.
“We’ve partnered with hospitals for many years now, and this program has allowed us to play a pivotal role during this terrible pandemic surge helping rural hospitals that are just not designed to handle this severity and volume of illness,” Jacobs said…
The eICU has helped care for about 1,300 COVID-19 ICU patients since the beginning of 2020, Jacobs said. In 2021, around 25% of the eICU admissions with a COVID-19 diagnosis have occurred in the last six weeks.” (C)
“For several years, Houston Methodist had been planning to spin up a virtual intensive care unit to provide support for community hospitals and widen access to care.
“We had chosen the technology, the cameras were hung, and we had signed contracts for our physicians to work,” explained Roberta Schwartz, executive vice president and chief innovation officer at the health system.
The team used Caregility for their cameras and Medical Informatics Corp.’s Sickbay platform for their artificial intelligence tools. Their intensivists were employed through Equum Medical.
In February 2020, the team hung the final camera on the main campus.
“Our first COVID-19 patient arrived in March,” she said…
“We added the community hospitals as we could get carts, and then, eventually, we added their cameras,” she said.
The model allowed the health system to care for the increased volumes of critically ill patients.
Schwartz noted that the project involves “countless” change-management pieces.
“From hiring nurses and contracting physicians, to deciding where buttons go, to deciding when and who should be called on what type of issues – there are so many workflows that need to be identified,” said Schwartz, who also spoke to Healthcare IT News earlier this year about the system’s conversational AI technology.” (D)
“Rural hospitals are getting help from technology during a surge in COVID-19 cases and the need for intensive care treatment.
It allows doctors and nurses to continuously monitor critically ill patients from hundreds of miles away. It’s called eICU, a virtual intensive care unit.
Doctors in a UW Health office building in Madison can provide care to patients in seven hospitals and eight ICUs across the state.
“So we can see all of the patients’ telemetry so their EKG tracings, what their heart rate is, what their blood pressure is doing, how they’re oxygenating. We can also go into all of their health records,” says Lynn Jacobs, Nursing Supervisor, eICU, UW-Health.
The program uses screens and cameras. Doctors can provide expertise to rural hospitals dealing with more critically ill patients due to COVID-19.
“We are seeing a lot more sick patients, a lot of patients that are a lot younger than ones that we’ve cared for, and having the ability to have the intensivists oversight over the care of our patients. give guidance and be a resource has been huge to ensure that the level of care they’re getting here is the same as what they would be getting at UW or at our larger PPS hospital,” says Jessica Faude, interim Vice President of Patient Care Services, Aspirus Medford Hospital.
Faude says eICU is an invaluable tool as rural hospitals lack the number of beds to take care of an influx of critically ill patients and to care for them for a longer period of time.
“We’re keeping vented patients, you know, filling our ICUs with vented patients that we’ve not done for over a while. But we’re comforted in knowing that we’re doing the best care. We have the oversight of UW and their intenseness that are dealing with this every day. So it’s been huge,” says Faude.
Dr. Jeff Wells is the medical director of eICU at UW Health.
“So we see all that information and then information comes into us and we’re able to help them use that to help take care of their patients, to give them the added benefit of experts in critical care that they may not have at the facility or may not have at that time at their facility to help support the care of their patients,” says Wells.
Jacobs explains, “There’s always somebody physically at all of our sites to take care of patients 24/7, but this way, in a small town, physicians are able to get some more rest, and our physicians can kind of take over.”..
While this background assistance helps staff on the ground, it doesn’t give them more hands to get the job done. Through telehealth doctors offsite can make a recommendation, but it still requires that someone on the ground make it happen.
It’s “not quite the same,” Lussier said. But, she added, “without it, we’d have nothing.”” (E)
But expanding telehealth services isn’t easy. Like hospitals providing the direct care, telehealth services also are facing a workforce crunch.
Still, the remote clinicians are doing what they can to help increase capacity at a time when resources are strained.
Lily Powell, director of acute care services for D-H Connected Care, said the program is offering clinicians the flexibility to log on from home for short periods. For example, she said, some of the teleICU nurses have had to be home with their children and aren’t available to work a full shift, but can help out in two-hour increments. That degree of flexibility is uncommon in nursing and it’s been a “big staff satisfier,” she said.
The remote nature of the work also allows clinicians from outside the region to assist, Surgenor said.
In addition to helping other hospitals such as SVMC and Cheshire to care for critical care patients, the teleICU team also has installed hardware necessary to oversee the 60 adult ICU beds at DHMC. Over the holidays, the team installed that equipment on 10 more beds at DHMC in an effort to expand ICU capacity as an increase in demand for the beds is expected due to holiday gatherings and the more transmissible omicron variant of the virus that causes COVID-19.
Without the approximately 30 beds in outside hospitals that D-H Connected Care supports by teleICU, Surgenor said the region’s current bed crunch could be even worse.
“We have built a significant enhanced capacity, which has really paid off in this pandemic,” he said. “We weren’t thinking we’d have a pandemic when we started the program.”” (F)
“With omicron rates soaring, you may find yourself despairingly asking when — or even if — this pandemic is ever going to end.
The good news is that it will end. Experts agree on that. We’re not going to totally eradicate Covid-19, but we will see it move out of the pandemic phase and into the endemic phase.
Endemicity means the virus will keep circulating in parts of the global population for years, but its prevalence and impact will come down to relatively manageable levels, so it ends up more like the flu than a world-stopping disease.
For an infectious disease to be classed in the endemic phase, the rate of infections has to more or less stabilize across years, rather than showing big, unexpected spikes as Covid-19 has been doing. “A disease is endemic if the reproductive number is stably at one,” Boston University epidemiologist Eleanor Murray explained. “That means one infected person, on average, infects one other person.”
We’re nowhere near that right now. The highly contagious omicron variant means each infected person is infecting more than one other person, with the result that cases are exploding across the globe. Nobody can look at the following chart and reasonably conclude that we’re in endemic territory.
Looking at this data might make you wonder about some of the predictions that were floating around before omicron came on the scene. In the fall, some health experts were saying that they thought the delta variant might represent the last big act for this pandemic, and that we could reach endemicity in 2022.
The outlook is more uncertain now. So how should you be thinking about the trajectory and timeline of the pandemic going into the new year? And how should omicron be shaping your everyday decision-making and risk calculus?
Here’s one big question you’d probably like the answer to: Does omicron push endemicity farther off into the future? Or could it actually speed up our path to endemicity by infecting so much of the population so swiftly that we more quickly develop a layer of natural immunity?
“That is really the million-dollar question,” Angela Rasmussen, a virologist at the University of Saskatchewan in Canada, told me. “It’s really hard to say right now.”
That’s partly because endemicity isn’t just about getting the virus’s reproductive number down to one. That’s the bare minimum for earning the endemic classification, but there are other factors that come into play, too: What’s the rate of hospitalizations and deaths? Is the health care system overburdened to the point that there’s a precipitous space or staffing shortage? Are there treatments available to reduce how many people are getting seriously ill?..
…This is why Ramussen says “the key determinant” of when the pandemic ends is how long it will take to make vaccines accessible around the world (and to combat ongoing vaccine hesitancy). Currently, we’re not vaccinating the globe fast enough to starve the virus of opportunities to mutate into something new and serious. “If only a very small proportion of people are getting access to vaccines, we’re just going to keep playing variant whack-a-mole indefinitely,” Rasmussen said.” (G)

The usual division of labor, however, has not worked well during the pandemic. The public has been confused by the lack of clarity around how recommendations are made for vaccines and by the finger-pointing when problems arise. At one low point in February 2020, C.D.C. leaders blocked the access of F.D.A. inspectors to its Covid-19 laboratory — an immediate problem for the review of C.D.C. tests, and perhaps also a reflection of the agencies’ arm’s-length relationship.
It does not have to be this way. Both the C.D.C. and the F.D.A. share the mission of protecting and advancing public health. Greater cooperation would improve the nation’s response to Covid…
In addition to the immediate benefits of better collaboration between the C.D.C. and F.D.A., there is an indirect value: trust. The agencies are full of experts working incredible hours during the pandemic to save lives. The public will be reassured to see them together, connecting the latest information about the pandemic to the most effective use of tests, medications and vaccines.” (H)

Why it matters: Cash prizes and other incentives barely moved the needle on vaccinations. So the government turned from carrots to sticks — but now it has lost its biggest stick.

“The state of Florida is not going to serve as the Biden Administration’s biomedical police,” Christina Pushaw, press secretary for Gov. Ron DeSantis, said in an email to the USA TODAY Network-Florida. “Firing unvaccinated healthcare workers, many of whom have infection-conferred immunity, is unethical and unscientific on its face.”

“A year ago, just two doses of a Covid-19 vaccine — or even one, in the case of Johnson & Johnson’s formulation — were thought to offer sufficient protection against the coronavirus.
Now, faced with the extraordinarily contagious Omicron variant, Israel has begun offering fourth doses to some high-risk groups. On Wednesday, the Centers for Disease Control and Prevention expanded eligibility for boosters to adolescents and backed away from describing anyone as “fully vaccinated” because two shots no longer seem adequate.
Instead, one’s vaccination status will now be “up to date” — or not. It’s no surprise that many Americans are wondering: Where does this end? Are we to roll up our sleeves for booster shots every few months?
Humbled repeatedly by a virus that has defied expectations, scientists are reluctant to predict the future. But in interviews this week, nearly a dozen said that whatever happens, trying to boost the entire population every few months is not realistic. Nor does it make much scientific sense.
“It’s not unheard-of to give vaccines periodically, but I think there are better ways than doing boosters every six months,” said Akiko Iwasaki, an immunologist at Yale University. Other strategies, she said, could “get us out of this forever-boosting kind of a situation.”
For starters, persuading people to line up for shots every few months is probably a losing proposition. About 73 percent of American adults are fully vaccinated, but so far just over a third have opted for a booster….
“We’re going somewhere fast, but we don’t know where we’re going,” said Natalie Dean, a biostatistician at Emory University in Atlanta. “Whatever the future holds, it just needs to be clear what the goal is.”” (F)
“Starting Saturday, new federal rules will require private insurers to cover the at-home coronavirus tests that Americans buy in pharmacies and other stores. The new system could, in theory, allow millions of consumers to pick up tests at thousands of locations without spending any money.
The reality, at least in the short term, is likely to be messier: Some insurers say it will probably take weeks to fully set up the system the White House envisions.
The new process will be hard, the insurers say, because over-the-counter coronavirus tests are different from the doctor’s visits and hospital stays they typically cover.
The tests do not currently have the type of billing codes that insurers use to process claims. Health plans rarely process retail receipts; instead they’ve built systems for digital claims with preset formats and long-established billing codes.
Because of this, some insurers plan to manage the rapid test claims manually at the start…
The new rules require private insurers to cover eight at-home coronavirus tests for each person, every month. The rules will not apply retroactively to at-home tests that Americans have already purchased, and do not cover patients with public insurance such as Medicare and Medicaid.” (G)

But for a select group of employees at some of the country’s largest companies, tests are free and often readily available.
Without an adequate federal system for developing and distributing rapid tests, companies have put their own testing services in place.
Google will send full-time employees in the United States free at-home tests that deliver results within minutes and retail for more than $70 each. BlackRock, an investment firm that manages nearly $10 trillion in assets, offers tele-health supervision as employees self-administer rapid tests for international travel. At JPMorgan Chase, bankers, including those at its retail sites, can order at-home rapid tests from an internal company site.
Some companies are using the tests to call their staff back to the office. For others, at-home Covid testing has become the newest wellness benefit, a perk to keep employees healthy and working — even from their couches — while providing peace of mind.
The testing available to a small number of white-collar professionals underscores the difference between their pandemic experience and that of other Americans, putting them at an advantage over many, including workers at small businesses without the means to procure testing kits for their staffs. Like personal protective equipment and vaccines, tests have become the latest example of how a tool to battle the pandemic can exacerbate social and economic divides.
“We’re the epicenter of the epicenter and I can’t get test kits anywhere,” said Thomas Grech, president of the Queens Chamber of Commerce, which has roughly 1,400 members that employ about 150,000 workers in the borough.” (H)
“The Omicron variant of the coronavirus is causing Covid-19 cases to spike, with an average of more than 747,000 new cases a day, according to the latest numbers from Johns Hopkins University. That’s almost three times the average daily cases from a year ago, when the country was going through its previous peak, and it’s putting an incredible strain on hospitals and emergency rooms.
The good news since the last peak is that the US Food and Drug Administration authorized new antiviral pills. Both Paxlovid and molnupiravir were given emergency use authorization in late December to treat mild to moderate Covid-19. They interfere (through different pathways) with the virus’ ability to replicate — and they can be taken at home, before someone becomes seriously ill.
Molnupiravir reduces the rate of hospitalizations in high-risk patients by about 30% and carries some possible side effects, but Paxlovid reduces the rate of hospitalizations by around 90% and is considered safer. In other words, Paxlovid is a bit of an antiviral superstar…
The bad news? Supply is very limited for both but especially for Paxlovid. Patients and their doctors are struggling to find it.
Just 160,000 courses have been delivered across the country, and with more people now hospitalized with Covid-19 than at any other time during the pandemic, these pills are in short supply.” (I)
“The American Red Cross says the nation is facing its worst blood shortage in more than a decade, citing a drop in blood drives due to the pandemic.
The organization said on Tuesday that the “national blood crisis” is threatening patient care and forcing doctors to make tough choices about who is able to receive blood transfusions, and it’s urging people to donate.
In recent weeks, the Red Cross — which provides some 40% of the nation’s blood — has had less than a one-day supply of critical blood types and has had to limit blood product distributions to hospitals. It says that at times, up to one-quarter of hospital blood needs are not being met.
There has been a significant drop in donations during the pandemic, and weather conditions and staffing limitations have caused ongoing cancelation of planned blood drives. There’s been a 10% overall blood donation decline since March 2020, and a 62% drop in college and high school blood drives during the pandemic, it says.” (J)
“Two of the country’s largest retail pharmacies are temporarily closing stores on weekends due to an increase in the number of COVID-19 infections among staffers.
The two chains said that the rapid spread of the Omicron variant has depleted staffing at several locations nationwide, affecting customer service and delaying key services like medication prescriptions and vaccinations.
Walgreens boasts nearly 9,000 locations across 50 states, the District of Columbia, Puerto Rico, and the US Virgin Islands. CVS operates around 10,000 pharmacies throughout the country.” (K)
“Like many hospitals in New York City, the Brooklyn Hospital Center is straining under the biggest surge of Covid-19 patients since spring 2020, when ambulance sirens filled the air and more than 20,000 New York City residents died.
In the pandemic’s early days, doctors and nurses looked out on emergency rooms filled with patients who were desperate for oxygen. Today, Covid-19 patients’ symptoms are generally milder — stomach aches, fainting, dizziness, nausea, some shortness of breath — and far fewer people are dying.
But the patients keep arriving, and in droves: More than 15,000 people with Covid-19 have been hospitalized in the city in the past four weeks, the most since the initial surge. About half of all patients in the city’s hospitals now have Covid-19.
And there are simply not enough nurses to care for them all. Across New York, hospitals generally employ fewer nurses than they did at the start of the pandemic, according to the New York State Nurses Association, a union.

Doing so would help the country develop an appropriate risk threshold that would establish peak weekly deaths, hospitalizations, and community prevalence of viral respiratory illnesses during high-severity years.
“This risk threshold triggers policy recommendations for emergency implementation of mitigation and other measures. In addition, health systems could rely on this threshold for planning on the bed and workforce capacity they need normally, and when to institute surge measures,” the authors write.
In an interview with CIDRAP News, Osterholm said members of the Biden transition team were having these discussions before the Omicron (B.1.1.529) variant emerged. The federal government has been acting in emergency mode, they said, and needs to start thinking of the pandemic with long-term goals in mind.
“What’s the end game? With the variants, we are never sure what the end game is going to be,” Osterholm said in the interview. “This is not a critique of the current administration, as much as an opportunity to plan.”

The policy, set to remain in place through Feb. 1, is designed to keep many healthcare workers on the job at a time when hospitals are expecting more patients.
Some experts say California’s stance is an unorthodox yet necessary solution to a difficult problem. Yet many healthcare workers and community members say the policy is not only ill-advised, it’s potentially dangerous…
The California Department of Public Health said hospitals are reaching capacity, and the decision was driven in large part by staffing shortages making it difficult to provide essential care.
“Given those conditions, the department is providing temporary flexibility to help hospitals and emergency services providers respond to an unprecedented surge and staffing shortages,” the agency said.
According to the guidelines, hospitals should exhaust all other options before resorting to the new policy, and workers who have tested positive for the virus should “preferably be assigned to work with COVID-19 positive patients.” The workers must always wear N95 masks…
“Is the situation ideal? No,” said Dr. Robert-Kim Farley, an epidemiologist and infectious-diseases expert at the UCLA Fielding School of Public Health. “Is it the lesser of the two evils of having no one to care for patients, versus having staff caring for them that may have COVID? Yes, it’s the lesser of two evils.”
Kim-Farley said the policy is a recognition of the significant strain hospitals are experiencing amid an increased number of patients and decreased number of staff. The chances of transmission from an asymptomatic worker are minimal, he said, particularly since he or she would be practicing precautions, including wearing high-grade medical masks…
One thing that is clear, however, is that understaffing poses a substantial risk to patients. Research has found that short staffing results in more deaths, more morbidity and more accidents and errors, she said.
“Hospitals are really stuck between a rock and a hard place, in that you really do have these staffing shortages and you have volumes of patients increasing,” Spetz said. “What do you do?”” (E)
“The new protocol, announced Saturday by the California Department of Health, means health workers exposed to COVID-19 or those who have the virus with no symptoms may return to work “immediately” – without the need for a test or quarantine – and wear an N95 mask. The move comes in response to a severe staffing shortage in the state, amid an upsurge in the omicron variant.
Asymptomatic health professionals who had tested positive for COVID-19 should “preferably be assigned to work with COVID positive patients”, the Californian health officials said, adding that this might not be possible in areas of “extreme” staff shortages or in places such as the emergency room where it wasn’t possible to know who had COVID-19.
According to the Centers for Disease Control (CDC), health professionals with COVID-19 may work without restrictions if “asymptomatic” or “mildly symptomatic” as part of a staff shortage “crisis” plan.
In “conventional” situations, health workers who had tested positive for COVID-19 and had no symptoms or mild symptoms should isolate for 10 days or seven days with a negative test, the CDC says.
Insider reported Thursday that some nurses with COVID-19 in the US are being forced to work, even with symptoms.
The California Department of Health said the measures were due to “critical staffing shortages”. Sites that implement this change must have made “every attempt” to bring on additional staff and considered “modifications to non-essential procedures”, it said…
Robert Siegel, a professor of microbiology and immunology at Stanford University, told the Chronicle, that there might be situations where “we might want to relax some of the testing requirements because staffing situations are dire, but in general, we should be more cautious.”

“The acting head of the US Food and Drug Administration said on Tuesday that most people in the United States are eventually going to get infected with COVID-19.
“It’s hard to process what’s actually happening with now, which is most people are going to get COVID,” Janet Woodcock, acting commissioner of the FDA, said to a Senate Health, Education, Labor, and Pensions Committee hearing.
The highly transmissible Omicron variant is currently sweeping through the country, and Woodcock said that past approaches to mitigating the spread of the virus don’t reflect the current situation in the US.
“What we need to do is make sure that hospitals can still function, transportation, you know, other essential services are not disrupted while this happens,” she said.” (A)
“In a series of tweets last week, Dr. Ashish Jha, dean of the Brown University School of Public Health, stressed that both patients “with COVID-19” and those admitted “for COVID-19,” have an impact on care, utilization, and can stress the health care system.
Patients, who may have come in for another ailment, such as a broken leg, and are found to be positive for the virus must stay in a COVID-19 isolation room, explained Jha, who added that there are already a limited number of such rooms available.
In addition, every time a health care worker enters a room with a COVID-19 positive patient, they must be wearing full PPE, which in turn, prolongs wait times for many patients.
“Admissions with COVID is still very disruptive to the health care system, at a time when it can’t afford more disruption,” Jha said.” (B)
“The number of hospitalized people with COVID-19 in New Jersey rose by 225 Monday night to 6,036, after an overnight adjustment to Sunday’s numbers by the state Health Department due to a reporting error.
That represents a 28% increase in COVID hospitalizations since Jan. 2. And the number of people seriously ill needing a ventilator rose to 500 Monday — a 71% jump in that period. The number in intensive care also rose, to 892, a 39% increase since Jan. 2…

As CEO of RWJBarnabas Health, it is something that Barry Ostrowsky said his team deals with around the clock.
“It takes literally hourly management and leadership to ensure you have the right level of personnel bedside and supporting the bedside personnel,” he said. “And that has been the case through the pandemic.”
Hospitals are again struggling to find enough staff. The surging wave of cases from the COVID-19 Omicron variant hasn’t helped. Hospitalizations have now exceeded 6,000 — a number the state hasn’t seen since the opening months of the pandemic.
Last week, Health Commissioner Judith Persichilli said hospitals should prepare for a 30% absentee rate for workers…
Mike Maron, the CEO at Holy Name in Teaneck, said the long-term outlook is just as tough. Finding additional personnel to assist has been nearly impossible.
“I’ve never seen this many vacancies in 34 years here,” he said….
Horan said Trinitas has become a community health center, which it is not meant to be during a pandemic.
“Patients are coming in just because they want to be tested, not because of an emergency reason,” he said. “They may need a test just to be able to fly someplace or go visit someplace. The problem is, once they get registered into the emergency department, you can’t just say, ‘Oh, we don’t do the test, go home.’ We have to see them.”” (D)

Guidance to the EMS providers on how to best use their services is needed at this time. The last time the state activated Crisis Standards of Care for Emergency Medical Services was April of 2020.
These crisis standards provide guidance for call centers, dispatch centers, and emergency medical service agencies, and responders regarding how to:
Interact with potentially infectious patients.
Maximize care for multiple patients with limited staff and emergency vehicles.
Determine what kind of treatment to provide, such as whether and where a patient should be transported for further care, if deemed necessary.” (H)
“Citing the “unprecedented strain” from the latest COVID-19 surge, a majority of Delaware hospitals are implementing emergency rules to cope with the surge of COVID-19 patients.
ChristianaCare, Bayhealth, TidalHealth Nanticoke and Saint Francis announced in a joint statement on Monday that they all will be implementing the rules, called Crisis Standards of Care.
This will allow the hospitals to redeploy clinical and non-clinical staff, adjust staffing ratios and change how treatment resources are distributed, according to a news release.
ChristianaCare, the largest health system in Delaware, was the first hospital to make this announcement.
The rules are implemented only during emergencies. Hospitals across the country have made similar decisions due to the number of COVID-19 hospitalizations.
“The current demand for care surpasses the normal resources that we have available,” hospital officials said in a statement. “Each of our organizations is taking steps to ensure that we are able to prioritize care for those with the greatest needs.”
Delaware’s health systems, officials said, are “stretched beyond capacity.” Hospitals are also asking residents to go to hospitals’ emergency rooms for serious health issues only…
The ChristianaCare release defined the emergency rules as a “framework in which health care providers can modify processes or change the way resources are allocated in order to care for patients with the highest need in conditions when it is no longer possible to deliver care according to normal standards.” (I)
“We have more patients,” Dr. Hassan Khouli, who is head of critical care at the Cleveland Clinic, told Snow when comparing the current situation to last year. “Our patients are sicker. Our teams are tired and exhausted, too.”
He said their unit is seeing patients in their 20s, 30s and 40s.
The conversation between Snow and Dr. Hassan then turned to the possibility that overcrowded COVID conditions in the ICU could lead to life-threatening concerns for other patients.
Snow: “Does that mean that somebody that comes in with a heart attack might end up dying because they can’t get into your ICU?”
Dr. Khouli: “That is possible, or they would wait in the emergency room while waiting for a bed in the intensive care unit — or they may be actually crashing on the floor.”
Snow’s report said most of the patients in the Cleveland Clinic’s ICU are currently on ventilators – 90 percent of which are unvaccinated. Approximately 5 percent of the hospital’s staff is also out sick. One nurse, for example, is now covering as many as 17 patients compared to eight on a typical day.” (J)
“Members of the U.S. Air Force will begin arriving in Ohio over the next several days to begin deployment to Cleveland Clinic.
Starting next week, 20 Air Force members will be helping staff the Clinic to deal with the omicron surge.
Those 20 people could have a big impact.
The hope is that adding these service members will allow Clinic hospitals to open more beds and accept more transfers from other hospitals.” (K)
“The Oregon Health Authority (OHA) has provided the state’s hospitals with an interim crisis care tool to help them prioritize treatment if they reach a point when critical care beds, specialized equipment, such as ventilators, and other resources become scarce due to surging COVID-19 admissions.
Oregon hospitals may activate crisis standards of care if their critical care resources are severely limited, the number of patients presenting for critical care exceeds capacity, and there is no option to transfer patients to other critical care facilities, according to a release from the OHA.
Hospitals may implement OHA’s interim crisis care tool – or one of their own that is consistent with Oregon’s Principles in Promoting Health Equity in Resource Constrained Events – if they have taken specific steps to extend their capacity to deliver care. Those steps include stockpiling supplies, delaying non-urgent care, and repurposing existing beds and staff that are not typically used to provide critical care.
Under the interim triage tool, all patients who can potentially benefit from treatment will be offered care, if health care resources are sufficient. If hospital staff, beds and treatment are insufficient, all patients will be individually assessed according to the best available objective medical evidence. According to the tool:
No one will be denied care based on stereotypes, assumptions about any individual’s quality of life, or judgement about an individual’s “worth” based on the presence or absence of disabilities.
Care decisions should be based on the likelihood of survival to hospital discharge.
Under Oregon’s interim crisis care standards, state health officials expect providers to treat all patients with respect, care and compassion. Hospital clinicians may not base care decisions on an individual’s use of past or future medical or social resources. They should apply reasonable modifications to any triage scoring criteria when considering individuals with underlying disabilities or certain underlying health conditions, according to the release.
Triage decisions will be made without regard to morally or scientifically irrelevant considerations such as income, race, ethnicity, gender identity, sexual orientation, immigration status, health insurance coverage or other factors…
At the same time, OHA also is calling for applicants today to serve on a new Oregon Resource Allocation Advisory Committee.
The Oregon Resource Allocation Advisory Committee’s role will be to:
Review and inform updates to OHA’s Principles in Promoting Health Equity During Resource-Constrained Events, which ensures health equity in decision-making when resource shortages occur.
Review and inform future amendments or changes to the interim crisis care tool.
Guide development of any additional necessary resources – including triage tools, guidance, best practices – to ensure these principles can be readily applied in Oregon during a resource-constrained event.
Inform the norms and expectations regarding patient communication and transparency when health system allocation decisions are necessary due to resources constraints.
OHA is seeking applicants representing: the state’s health care delivery system, including hospitals, health care providers and local public health agencies; and organizations and community members who can speak to community needs, especially communities of color, tribal communities and people with disabilities, including people with intellectual and developmental disabilities.” (L)

The phased dispatch of 1,000 military health personnel beginning next week comes as U.S. COVID-19 hospitalizations hit a record high with Omicron overtaking Delta as the dominant variant of the coronavirus and health facilities facing a staffing crunch.
The move is “part of a major deployment of our nation’s armed forces to help hospitals across the country manage this surge of the Omicron virus,” Biden said.
“I know we’re all frustrated as we enter this new year,” Biden said, while reiterating his message that COVID-19 continues to be a “pandemic of the unvaccinated.”
In the first wave of the deployment, teams of military doctors, nurses and other personnel will head to Michigan, New Jersey, New Mexico, New York, Ohio and Rhode Island to support at-capacity emergency rooms and free up overwhelmed hospital staff for non-COVID cases, the White House said.
But with the teams ranging in size from seven to 25, hospitals due to receive the health workers welcomed the assistance but warned it would not be enough to combat the surge.
“There is not a silver bullet solution,” said Bob Riney, president of healthcare operations at Detroit’s Henry Ford Health System, which has already received some federal help and expects to welcome new military medics next week.
“We have systemic challenges (with) incredible volume and very, very tired medical practitioners … and that is true of all health systems that have been in the middle of this surge,” Riney said. The White House’s more aggressive stance follows months of criticism from health experts that the administration was relying too heavily on vaccines alone to stop the spread of the coronavirus, especially given a politically motivated anti-vaccine movement pushed by some Republican officials.
